Monoclonal Antibodies Targeting Bacillus Calmette-Guérin (BCG) Outer Membrane Proteins Confer Protection Against Tuberculosis.

Tuberculosis (TB), caused by the Mycobacterium tuberculosis complex (MTBC), is a persistent global health crisis, remaining the world's leading infectious killer. There were abundant outer membrane proteins (OMPs) distributed in the bacterial cell wall, while fewer proteins were identified and characterized as antigens that could induce protective antibodies against TB. The OMPs of BCG were extracted with the detergent N-octyl-β-D-glucopyranoside, and the extracted proteins were used to immunize mice to generate an antibody library. Subsequently, the library underwent HAT/HT screening, and fiv

Immunogenicity and protective efficacy of recombinant BCG expressing a multi-epitope vaccine against mycobacterium tuberculosis.

INTRODUCTION: Mycobacterium tuberculosis (Mtb) infects approximately one-quarter of the global population, of whom 5%-10% develop active tuberculosis (TB). Although Bacillus Calmette-Guérin (BCG) remains the cornerstone vaccine against severe infant TB, its efficacy in adults is limited, underscoring the need for novel TB vaccines. This study aimed to design a multi-epitope vaccine, tMEV, and validate its protective potential through bioinformatic prediction and in vivo experiments. METHODS: tMEV was constructed from dominant T- and B-cell epitopes derived from Mtb ESAT-6, PE35, PPE38, and PE

Case report: Maternal engraftment of T-cells in a patient with Severe Combined Immunodeficiency due to DCLRE1C pathogenic variant - diagnostic and treatment challenges.

BACKGROUND: Severe combined immunodeficiency (SCID) is a rare but serious congenital condition caused by genetic defects that severely impair the development and function of T cells, B cells, and natural killer (NK) cells. Without early diagnosis and immune system reconstitution, SCID is usually fatal in infancy. The condition requires prompt recognition and hematopoietic stem cell transplantation (HSCT) to restore immune function. This report describes the first genetically confirmed case of SCID in Latvia - caused by a DCLRE1C gene pathogenic variant and diagnosed prior to the implementation

Construction, expression, and immunogenicity analysis of a fusion gene based on the PE/PPE family in combination with adjuvants.

Tuberculosis (TB) remains the leading global cause of death from a single infectious agent and a major fatal infectious disease worldwide. However, the Bacillus Calmette-Guérin (BCG) vaccine-currently the only licensed TB vaccine in clinical practice-has critical limitations in preventing latent infection and disease reactivation, failing to deliver comprehensive protective immunity. Given these gaps, targeting Mycobacterium tuberculosis (M.tb) antigens with strong immunogenic potential is a key strategy for next-generation TB vaccine development. The pathogenic M.tb harbors the Pro-Glu (PE) a

Recent advances in antituberculosis drugs and therapeutics: from mechanistic breakthroughs to clinical translation.

Tuberculosis (TB), caused by Mycobacterium tuberculosis, reclaimed its status as the deadliest single-pathogen infectious disease globally in 2023, accounting for 10.8 million new cases and 1.25 million deaths in 2023. The escalating crisis of drug-resistant TB, suboptimal BCG vaccine efficacy in adults, and persistent treatment gaps for vulnerable populations collectively drive an urgent shift toward mechanism-driven, precision-oriented therapeutic strategies. This review presents a critically integrated appraisal of anti-TB research advances spanning 2020 to 2025, drawing on structural biolog

Functional divergence of regulatory and conventional bovine neutrophils following Mycobacterium bovis infection.

INTRODUCTION: Bovine tuberculosis (bTB), due to Mycobacterium bovis (Mb) infection, is a chronic cattle disease and neglected cause of zoonotic tuberculosis. The role of neutrophils in bTB is overlooked. We recently identified a new neutrophil subset in cattle and mice with a similar morphology to conventional inflammatory neutrophils (Nconv). However, unlike Nconv, these regulatory neutrophils (Nreg) express MHC-II at their surface and can suppress lymphocyte proliferation. METHODS: In this study, we compared the responses of bovine Nconv and Nreg to infection with a virulent Mb strain circu

Epigenetic reprogramming by BCG (Bacillus Calmette-Guérin) vaccination: A novel strategy to reverse immunodeficiency in chronic kidney disease.

Chronic kidney disease (CKD) is associated with reflecting innate immune impairment. Chronic inflammation, oxidative stress, metabolic dysregulation, and prolonged uraemia are the primary causes of this condition. Monocytes, macrophages, and their progenitors may undergo drastic epigenetic changes in response to these systemic stresses. This has lowered the activation of histone marks, such as H3K4me3, and destabilised repressive marks, like H3K9me3. Critical immune response genes are silenced by chromatin closure. This epigenetic change exhausts the immune system and impairs its ability to cl

Impact of intravesical BCG Therapy on quality of life in non-muscle-invasive bladder cancer: a prospective evaluation using the FACT-Bl questionnaire.

INTRODUCTION AND OBJECTIVE: Intravesical bacillus Calmette-Guérin(BCG) remains the standard treatment for high-risk non-muscle-invasive bladder cancer(NMIBC) and is also considered an option for intermediate and very high-risk groups. Despite over five decades of clinical use, limited information is available regarding patients' quality of life(QoL) during therapy. The objective was to evaluate QoL patients receiving BCG therapy using the Functional Assessment of Cancer Therapy-Bladder Cancer(FACT-Bl) questionnaire. MATERIALS AND METHODS: Prospective multicentre observational study from Febru

Automated differentiation of parkinsonian disorders: an ROI-based analysis of subcortical shape and cortical surface features.

UNLABELLED: The clinical manifestations of early-stage parkinsonian syndromes overlap, making accurate differential diagnosis crucial yet challenging. This study aimed to develop a system for automated differentiation of idiopathic Parkinson's disease (IPD) from progressive supranuclear palsy (PSP) and corticobasal syndrome (CBS). Our sample included clinical data and T1-weighted magnetic resonance imaging from 50 IPD, 47 PSP, and 38 CBS patients. We introduced an atlas-based approach to extract shape features from subcortical regions in each subject's native coordinate image space. The surfac

Exploring the ethics of tuberculosis human challenge models.

We extend recent conversation about the ethics of human challenge trials to tuberculosis (TB). TB challenge studies could accelerate vaccine development, but ethical concerns regarding risks to trial participants and third parties have been a limiting factor. We analyse the expected social value and risks of different challenge models, concluding that if a TB challenge trial has between a 10% and a 50% chance of leading to the authorisation and near-universal delivery of a more effective vaccine 3-5 years earlier, then the trial would save between 26 400 and 1 100 000 lives over the next 10 ye

Enhanced immunogenicity of Staphylococcus aureus IsdE vaccine with SeNPs and BCG adjuvants in mice.

Staphylococcus aureus is a major cause of nosocomial infections and antimicrobial resistance, emphasizing the urgent need for an effective vaccine. The IsdE protein, a heme-iron transporter, has been identified as a promising antigen target. In this study, we evaluated the immunogenicity and protective efficacy of recombinant IsdE (r-IsdE) formulated with the MF59 adjuvant and co-adjuvanted with Bacillus Calmette–Guérin (BCG) and synthetic SeNPs (SeNPs) in BALB/c mice. Mice were immunized three times at two-week intervals, and immune responses were analyzed through cytokine assays, antibody is

Integrative profiling of transcriptome and H3K27ac modification reveals changes associated with BCG-induced trained immunity in bovine immune cells.

We previously demonstrated that Bacillus Calmette Guérin (BCG) vaccination induces trained immunity in bovine monocytes and gamma delta (γδ) T cells, enhancing IL-1β and IL-6 production. To define the underlying molecular mechanisms, we applied a multi-omic approach to circulating monocytes and alveolar macrophages (amac). Twenty Holstein-Angus heifers were randomly separated into two groups and immunized subcutaneously with 6 million CFUs of BCG Danish strain (n = 10), or saline (n = 10), and boosted after 2 weeks. Monocytes (4 weeks post-boost) and amac (6 weeks post-boost) were analyzed for

Inflammatory models of depression in rodents and humans.

The precise pathophysiological mechanisms underlying major depressive disorder (MDD) remain poorly understood. Substantial evidence implicates immune-mediated mechanisms in the pathogenesis of this clinically heterogeneous and multifactorial disease. This review provides a comprehensive synthesis of current knowledge regarding the association between inflammation and depression, critically evaluates established approaches for modeling inflammation-induced depressive states in both rodents and humans, and assesses these models against standard validity criteria. The empirical link between depre

Malathion-induced neurological alterations associated with dysregulation of RORγt/STAT3/IL-17 and FOXP3/STAT5/IL-10 pathways: potential neuroprotective effects of BCG.

Despite their effectiveness in agriculture to control a variety of pests, organophosphorus compounds (OPC) such as malathion were linked with neurological dysfunctions and possibly death. The present study aimed to investigate the impact of OPC exposure on neuroinflammation via disrupting the equilibrium between pro-inflammatory (RORγt/STAT3/IL-17/IL-22) and anti-inflammatory (FOXP3/STAT5/IL-10) pathways. The study extended to evaluate the potential of BCG vaccination in alleviating neuroinflammation. Rats were distributed into four groups: control, malathion-intoxicated group, BCG-treated gro

A randomized prospective study of neonatal hepatitis B vaccine immunogenicity in The Gambia and Papua New Guinea.

BACKGROUND: Protection of newborns from infection can be achieved through maternal or vaccine-induced antibodies, but the factors influencing vaccine protection (correlate of protection) and subsequent infant immunity remain insufficiently understood. Further investigation is essential to optimize early-life vaccination strategies. OBJECTIVE: We sought to evaluate the impact of timing and sequence of hepatitis B vaccine (HBV) and Bacille Calmette-Guérin (BCG) vaccine within the first week of life on infant HBV immunogenicity and its relationship with maternal antibodies. METHODS: This was an

Engineered BCG selectively triggers trained immunity in tumor-associated macrophages and sensitizes glioblastoma to radiotherapy in mice.

Radiotherapy plays a crucial role in antitumor immunity in glioblastoma, yet its efficacy is often limited, resulting in tumor recurrence. Here, we engineer a macrophage membrane-camouflaged Bacillus Calmette-Guérin (BCG) via bioorthogonal chemistry to enhance radiotherapy against glioblastoma. This engineered BCG penetrates the blood-brain barrier, targets tumors, and alleviates hypoxia through intrinsic catalase activity, exerting antitumor effects in both murine orthotopic glioblastoma and humanized mouse models. Notably, it also initiates trained immunity in tumor-associated macrophages. D

A versatile plasmid platform for auxotrophic complementation in attenuated Mycobacterium bovis BCG.

BACKGROUND: Mycobacterium bovis BCG is widely used as a vaccine vector due to its safety, genetic stability, and immunomodulatory properties. However, current expression systems for BCG transformation rely on antibiotic resistance genes, raising biosafety concerns. Therefore, alternative strategies—such as the use of auxotrophic strains—are highly desirable. Our group previously employed BioBrick™ technology to construct the pUP500 plasmid series containing various mycobacterial promoters. We engineered these plasmids for the auxotrophic complementation of BCG ΔLeuD, a leucine-auxotrophic stra

Incidence of Bacille Calmette-Guérin associated lymphadenitis in healthy children: A systematic review and meta-analysis.

Few systematic reviews or meta-analyses have addressed on the incidence of Bacille Calmette-Guérin (BCG) lymphadenitis in healthy children. A systematic literature search was independently conducted by two researchers using four English and Korean databases (PubMed, Scopus, KMbase, RISS). The risk of bias was assessed. A total of 52 peer-reviewed journal articles (about 1.8 million children) were included in meta-analyses. The incidence proportion of BCG lymphadenitis ranged from 0.0% to 25.6%. The pooled incidence proportion was 2.5% (95% CI, 1.6-3.6; I2 = 99%). Studies employing active (vs.

Trained Immunity Induced by Vaccines: A Shifting Paradigm for Infant and Adult Immunity.

In addition to inducing pathogen-specific adaptive immune responses, vaccines can train the innate immune system, thereby providing broader host protection. This concept of trained immunity (TRIM) is well-established in benchtop laboratory science. This review aims to evaluate the current evidence of vaccine-induced TRIM and translate these findings into a clinical context. Various laboratory methods are used to assess TRIM; however, inconsistent results have been reported across non-BCG vaccine studies. Existing analyses lack exploration of the mechanistic basis of vaccine-induced TRIM, parti
