Colonic perforation secondary to post endoscopic retrograde cholangio-pancreatography severe pancreatitis: a case report on the lethal potential of neglected cholelithiasis with review of literature.

A 40-year-old lady with a known history of gallstones presented with acute abdominal pain. Diagnostic imaging confirmed cholelithiasis with concomitant choledocholithiasis. She underwent a therapeutic endoscopic retrograde cholangiopancreatography for stone extraction. Her post-procedural course was complicated by the development of severe acute pancreatitis. Despite aggressive medical management, her clinical condition deteriorated rapidly. Subsequent computed tomography scans revealed extensive pancreatic necrosis and a perforation of the splenic flexure of the colon. The patient was taken f

[Pneumatosis cystoides intestinalis with extensive intraperitoneal free air, retroperitoneal emphysema, and pneumomediastinum:a case report].

Pneumatosis cystoides intestinalis is characterized by the presence of several gas-filled cysts within the intestinal wall. Here, we report an exceptionally rare case, wherein pneumatosis cystoides intestinalis extended from the ileum through the right colon to the splenic flexure, accompanied by intraperitoneal free air, retroperitoneal emphysema, and mediastinal emphysema. The patient reported a complex medical history, including interstitial lung disease and bronchial asthma, and received steroid treatment alongside α-glucosidase inhibitors for diabetes mellitus. Conservative management was

Minimally invasive surgery for diverticulitis: a single-center retrospective study in the United States focusing on splenic flexure mobilization in elective sigmoid colectomy.

PURPOSE: Splenic flexure mobilization (SFM) is occasionally utilized during sigmoidectomy to facilitate a tension-free colorectal anastomosis. Present literature regarding its use, safety, and efficacy predominantly evaluates traditional laparoscopic vs. open techniques, thus not adequately representing the current minimally invasive surgical landscape. METHODS: This retrospective cohort analysis evaluated SFM during sigmoidectomy for diverticular disease via traditional laparoscopic, single-incision laparoscopic, and robotic techniques at a United States academic institution from 2019 to 202

Splenic flexure cancer shows poorer survival than descending colon cancer: an integrated Surveillance, Epidemiology, and End Results and The Cancer Genome Atlas analysis.

BACKGROUND: The survival differences between splenic flexure cancer (SFC) and descending colon cancer (DCC) are unclear owing to their distinct anatomic and molecular features. This study compares their survival outcomes and genetic differences using data from the Surveillance, Epidemiology, and End Results (SEER) and The Cancer Genome Atlas (TCGA) databases. METHODS: This study used SEER data (2000-2022) to compare postoperative patients with SFC and DCC. Propensity score matching (PSM) was performed to balance baseline characteristics. Overall survival (OS) and cancer-specific survival (CSS

Phase-specific operative times in robotic distal pancreatectomy: impact of body mass index and learning experience.

BACKGROUND: Robotic distal pancreatectomy (RDP) in obese patients is technically demanding, but the phase-specific impact of body mass index (BMI) remains unclear. METHODS: We analyzed 164 patients who underwent RDP with splenectomy between October 2018 and September 2025. Twelve surgical phases were predefined, and all recorded frames were assigned to one phase. Based on the lead surgeon's cumulative sum (CUSUM) curve, the learning period was defined as the first 34 cases. Multivariable linear regression identified determinants of total operative time, and associations between BMI and phase

Efficacy and prognosis evaluation of left colic artery preservation in laparoscopic NOSES for rectal cancer: retrospective survival analysis.

This retrospective cohort study aimed to clarify the impact of preserving the left colic artery (LCA) during laparoscopic Natural Orifice Specimen Extraction Surgery (lapNOSES) for rectal cancer (RC) on perioperative outcomes and 3-year survival, and to examine treatment-by-covariate interactions on recurrence-free survival (RFS) and overall survival (OS). No significant interactions were identified for RFS, whereas significant interactions for OS were observed among patients with larger maximum tumor diameter and elevated CEA levels. RC patients who underwent lapNOSES between January 2018 and

Contrasting responses to neoadjuvant immunotherapy in synchronous transverse colon cancers with discordant mismatch repair status.

Mismatch repair (MMR) status is a critical predictive biomarker for immune checkpoint inhibitor (ICI) efficacy in colorectal cancer (CRC); however, direct in vivo comparisons of ICI responsiveness between MMR-deficient (dMMR) and MMR-proficient (pMMR) tumors within the same patient are exceedingly rare. We report an exceptional case of synchronous transverse colon cancer with discordant MMR status that demonstrated strikingly divergent responses to neoadjuvant immunotherapy. A 75-year-old woman with a history of dyslipidemia presented with anemia. She was diagnosed with two synchronous transve

A Case of Granulomatosis with Polyangiitis Predominantly Presenting with Gastrointestinal Involvement: A Diagnosis Made by a Colonic Biopsy and Successful Endoscopic Balloon Dilation for Cicatricial Stricture.

A 73-year-old woman predominantly presented with diarrhea, accompanied by abdominal pain and hematochezia. Colonoscopy revealed multiple colonic ulcers with skip lesions, and a biopsy displayed small-vessel vasculitis, leading to a diagnosis of granulomatosis with polyangiitis (GPA). Corticosteroid therapy improved the ulcers; however, cicatricial stricture developed at the splenic flexure, which was successfully managed with endoscopic balloon dilation (EBD). This rare case of GPA, predominantly presenting with gastrointestinal symptoms, underscores the significance of considering vasculitis

Performance Under Pressure: A Retrospective Review of the Pneumatic Reduction of Ileocolic Intussusception in a Paediatric Hospital Over 17 Years.

PURPOSE: Ileocolic intussusception is a paediatric emergency requiring rapid reduction of the telescoped bowel. Often initial attempts at reduction are conservative, using gas or liquid enema. Our aim is to examine the success of pneumatic reduction over the past 17 years at our centre, compare our rates of success and complication to the published literature, and evaluate the use of delayed repeat reduction attempts (DRRAs). METHODS: A retrospective single centre review of all patients with attempted pneumatic reduction of ileocolic intussusception, between January 2006 and December 2023. Pa

Staged Surgical Management of a Colosplenic Fistula Caused by Locally Advanced Splenic-Flexure Adenocarcinoma.

Colosplenic fistula is a rare complication most commonly associated with colonic lymphoma, adenocarcinoma, and Crohn's disease. In unstable and septic patients, a staged strategy consisting of initial laparoscopic source control and diversion followed by definitive multivisceral resection can balance urgent management of sepsis with adherence to oncological therapy.

Comparison of short-term outcomes among hand-sewn, stapled, and two-stage Turnbull-Cutait pull-through anastomoses in TaTME for low rectal cancer.

BACKGROUND: Although transanal total mesorectal excision (TaTME) has been recognized as an important surgical method for low rectal cancer, optimal anastomotic techniques remain debated. This study compared short-term outcomes of hand-sewn coloanal anastomosis (CAA), single-stapled anastomosis (SSA), and two-stage Turnbull-Cutait pull-through coloanal anastomosis (TCA). METHODS: A retrospective analysis of 147 patients who underwent TaTME for low rectal cancer (tumor distal margin ≤ 5 cm from anal verge) was conducted between July 2020 and July 2023. The patients were categorized into CAA, SS

A Mapping of Operative Heterogeneity in Robotic Splenic Flexure Cancer Surgery, Focusing on Vascular Ligation and Reconstructive Strategy.

Background: Splenic flexure cancers represent a small but anatomically complex subset of colorectal malignancies, characterised by variable vascular supply and lymphatic drainage. This has led to ongoing uncertainty regarding the optimal extent of resection and level of vascular ligation. With increasing adoption of robotic surgery, a range of operative approaches has been described, although these remain inconsistently reported and poorly synthesised. Aim: We aimed to map operative heterogeneity in robotic splenic flexure cancer surgery, focusing on resection extent, vascular ligation, recons

Synchronous Primary Adenocarcinomas of the Hepatic and Splenic Flexures Presenting as Acute Large Bowel Obstruction in a 38-Year-Old Man: A Case Report.

Synchronous primary colorectal cancers (CRCs) are uncommon and are more frequently identified in older patients or in association with hereditary cancer syndromes. Although the incidence of early-onset CRC (EOCRC) is increasing, synchronous primary tumors in young individuals without identifiable risk factors remain rare. Presentation as acute large bowel obstruction further complicates diagnosis because complete preoperative colonoscopic evaluation is often not feasible. We report the case of a 38-year-old man with no significant medical or family history who presented with a two-day history

External validation of predictive models for prolonged postoperative ileus following colorectal resection.

INTRODUCTION: Prolonged postoperative ileus (PPOI) is a frequent complication after colorectal surgery. Several prediction models have been proposed to estimate PPOI risk, but few have undergone external validation, limiting their generalizability and clinical usefulness. The primary aim of this study was to externally validate two predictive models for PPOI after colorectal resection; a secondary aim was to explore factors associated with PPOI in our cohort. METHODS: Data from all consecutive patients who underwent elective colorectal resection in our department between 2019 and 2022 were re
