Chronic Fasciola hepatica infection in a patient with cutaneous visceral loxoscelism: paediatric case report.

We present the case of a 9-year-old girl from the Peruvian Andean Region (Ancash), diagnosed with fascioliasis, who was also suffering from severe malnutrition. Initially, she was referred for treatment of skin necrosis caused by cutaneous visceral loxoscelism, but during hospitalisation she developed persistent vomiting and pancreatitis. Magnetic resonance cholangiography showed dilation of the intrahepatic and common bile ducts. Endoscopic retrograde cholangiopancreatography identified an adult Fasciola spp. specimen. She also expelled adult worms of Ascaris lumbricoides during vomiting. Del

Sigmoid volvulus and incidental enterobiasis in a young adult: a case report.

BACKGROUND: Sigmoid volvulus is a disease that typically occurs in elderly patients with chronic constipation and an elongated sigmoid mesentery. Its occurrence in previously healthy young adults is uncommon, often with delayed diagnosis, and may coexist with under-recognized predisposing factors. In this case, family clustering of vague gastrointestinal symptoms and a coexisting parasitic infestation are most likely incidental rather than causative. CASE PRESENTATION: A previously healthy young Saudi woman presented with 2 weeks of intermittent colicky abdominal pain, low-volume diarrhea, an

Molecular detection and diagnostic evaluation of Spirocerca lupi infection in dogs from Reunion Island.

Spirocerca lupi is a parasitic nematode responsible for spirocercosis, a potentially severe disease in domestic dogs. Clinical manifestations are variable and nonspecific, often leading to confusion with other gastrointestinal, respiratory, or neoplastic disorders and making diagnosis challenging. Although canine spirocercosis has previously been reported on Reunion Island, the molecular epidemiology and data on its occurrence remain limited. This study aimed to investigate the circulation of S. lupi on Reunion Island, using endoscopy, coproscopy, and molecular assays. Over a 75-day period, 88

Molecular, parasitological and clinical diagnosis of female genital schistosomiasis: A diagnostic yield and comparative accuracy study using latent class analysis.

BACKGROUND: Female genital schistosomiasis (FGS) is a neglected manifestation of Schistosoma haematobium infection whose diagnosis is restricted by the absence of a flawless "gold standard" diagnostic tool. This study compared the yield, accuracy, performance of polymerase chain reaction (PCR), urine egg microscopy, and colposcopy. METHODS: A cross-sectional study was conducted among 143 female participants from S. haematobium-endemic communities in the West Region of Cameroon between July and August 2024. Participants underwent urine microscopy, genital swab PCR, and colposcopy, and a two-cl

Concerted changes in the pediatric single-cell intestinal ecosystem before and after anti-TNF blockade.

Crohn's disease is an inflammatory bowel disease (IBD) commonly treated through anti-TNF blockade. However, most patients still relapse and inevitably progress. Comprehensive single-cell RNA-sequencing (scRNA-seq) atlases have largely sampled patients with established treatment-refractory IBD, limiting our understanding of which cell types, subsets, and states at diagnosis anticipate disease severity and response to treatment. Here, through combining clinical, flow cytometry, histology, and scRNA-seq methods, we profile diagnostic human biopsies from the terminal ileum of treatment-naive pedia

Baseline HDV-RNA Levels Could Stratify Clinical Risk Before, But Not After, Cirrhosis in Chronic Hepatitis D: A Multicentre European Study.

BACKGROUND AND AIMS: Chronic hepatitis D (CHD) is associated with rapid progression to liver cirrhosis. However, the prognostic relevance of quantitative serum HDV-RNA remains incompletely defined. We aimed to evaluate whether baseline HDV-RNA levels are associated with the risk of liver-related events in untreated patients with CHD and to validate these findings in two independent European cohorts. METHODS: We conducted a multicentre retrospective-prospective cohort study including 260 untreated adults with CHD and detectable HDV-RNA. Patients were derived from a Spanish derivation cohort (n

Regulation of immune tolerance in hepatocellular carcinoma by liver diseases: a review.

The clinical benefit of immunotherapy for hepatocellular carcinoma (HCC) is often limited by innate immune tolerance. Traditional research focuses on the intrinsic escape mechanism of tumor cells or the generalized immunosuppression of the tumor microenvironment; however, it often ignores the chronic liver disease background of HCC patients with different etiologies. The aim of this review was to systematically discuss the mechanisms by which different underlying liver diseases, particularly metabolic dysfunction associated steatohepatitis, viral hepatitis (hepatitis B virus/hepatitis C virus)

Applying the Baveno VII Criteria for Variceal Screening to Reduce Endoscopy Costs and Risks to Patients in an Australian Referral Centre.

Baveno VII was introduced to help identify patients with compensated advanced chronic liver disease (ACLD) who should undergo screening gastroscopy to detect varices needing treatment (VNT), with a positive predictive value exceeding 90%. We performed a study at our centre to evaluate the impact of implementing this strategy, which has the potential to lower both healthcare costs and the risks associated with invasive procedures. We carried out a retrospective, single-centre analysis between 2021 and 2024, reviewing screening gastroscopies for oesophageal varices. Data reviewed included gastro

Predictors of Incident High-Risk Varices in Patients with Hepatitis B-Related Cirrhosis Receiving Nucleos(t)ide Analog Therapy.

INTRODUCTION: In patients receiving nucleos(t)ide analog (NA) therapy for chronic hepatitis B (CHB)-related cirrhosis, routine endoscopic surveillance may be deferred in the absence of high-risk varices (HRV). We aimed to identify predictors of incident HRV in this population. METHODS: This retrospective cohort study included patients with CHB-related cirrhosis who initiated NA therapy between March 2008 and June 2020 and were followed through May 2024. Eligible patients underwent baseline and on-treatment upper endoscopy. Patients with baseline HRV, prior variceal ligation, use of nonselecti

Characteristics and Outcomes of Patients With Imaging-Defined MASLD and Histological Steatosis Grade S0.

INTRODUCTION: Metabolic dysfunction-associated steatotic liver disease (MASLD) is a prevalent chronic liver disease, and liver biopsy remains the gold standard for diagnosis. However, a subset of patients diagnosed with MASLD by imaging paradoxically exhibit steatosis grade S0 on liver biopsy, posing a diagnostic challenge. This study investigated the outcomes and causes of steatosis 0. METHODS: Steatosis 0 was defined as imaging-detected MASLD with a biopsy-defined steatosis grade S0. We used global cohorts to evaluate outcomes, histologic progression by paired biopsies, and possible causes.

Embolization of spontaneous portosystemic shunts for refractory hepatic encephalopathy in cirrhosis patients:A meta-analysis.

BACKGROUND AND AIM: Hepatic encephalopathy is an important cause of morbidity in cirrhosis patients. The presence of spontaneous portosystemic shunts (SPSS) is associated with an increased risk of recurrent/refractory hepatic encephalopathy (HE). Embolization of SPSS has been shown to improve HE symptoms, but it may worsen portal hypertension and related complications. The aim of this study was to determine the efficacy of SPSS embolization for recurrent/refractory HE. MATERIALS AND METHODS: Five databases were screened to identify studies assessing the efficacy of SPSS embolization for HE. T

Life-Threatening Gastrointestinal Bleeding in a Child with Acute Hepatitis A.

BACKGROUND: Hepatitis A virus (HAV) infection is a common cause of acute viral hepatitis in children. Severe complications are rare but may occur, particularly in older children or in the presence of concomitant conditions. CASE PRESENTATION: We report the case of an 11-year-old girl with acute hepatitis A with severe hepatic derangements who developed life-threatening upper gastrointestinal bleeding due to a previously undiagnosed duodenal ulcer. Emergency endoscopy confirmed active bleeding from a duodenal ulcer, and the patient survived the complications with treatment with a proton pump i

Splenic Elastography as a Predictor of Gastroesophageal Varices in Cirrhosis.

INTRODUCTION: Gastroesophageal varices are a major complication of portal hypertension in cirrhosis, associated with high morbidity and mortality. Noninvasive predictors are needed to reduce reliance on endoscopy. Splenic elastography has emerged as a potential tool to assess portal hypertension severity. METHODS: We conducted prospective, descriptive cross-sectional study at Hospital Roosevelt, Guatemala, in 2024, including 78 patients with recent diagnosis of cirrhosis confirmed by ultrasonography. Patients with hepatocellular carcinoma, acute decompensation, or conditions interfering with

Hepatitis D in the Middle East: A Review of Disease Burden, Health-System Barriers, and Strategic Directions for Management and Elimination.

Hepatitis delta virus (HDV) is the most aggressive form of viral hepatitis and worsens outcomes in people with chronic hepatitis B (HBV). It speeds up fibrosis, causes early cirrhosis, hepatic decompensation, and increases hepatocellular carcinoma risk. In the Middle East, presumed high HBV prevalence, uneven surveillance, and significant diagnostic and treatment barriers lead to under-recognition of HDV and uncertainty about its true prevalence. This review summarizes current regional evidence on HDV distribution, diagnostic gaps, emerging treatments, and health policy shortcomings. The resul

Spontaneous Pneumomediastinum, Subcutaneous Emphysema, and Pneumoperitoneum in RT-PCR-Confirmed Measles: A Pediatric Case Report.

Measles remains a major global public health challenge as declining vaccination coverage fuels outbreaks worldwide. Although pneumonia is the most recognized respiratory complication, spontaneous air leak syndrome-including pneumomediastinum, subcutaneous emphysema, and pneumoperitoneum-is rarely documented. We report the case of a 9-year-old previously healthy girl with no documented measles-rubella vaccination who presented with fever, maculopapular exanthem, Koplik spots, and persistent cough. Measles was confirmed by both immunoglobulin M enzyme-linked immunosorbent assay and real-time rev

Mpox Presenting as an Anorectal Disease: A Case Report.

Mpox (monkeypox) is an emerging viral infection with a broad and evolving clinical spectrum that increasingly spans beyond its characteristic cutaneous manifestations. Anorectal manifestations are well recognized in patients with Mpox in recent outbreaks. However, the gastrointestinal (GI) or anorectal symptoms may precede the typical rash, creating significant diagnostic challenges and delaying appropriate infection control measures. We describe a quinquagenarian man with poorly controlled diabetes mellitus who presented with fever, diarrhea, tenesmus, and severe perianal pain. Cross-sectiona
