Pre-hospital time and risk of perforation in patients with acute appendicitis in 112 countries (AlliGatOr): an international, prospective, observational cohort study.

BACKGROUND: Acute appendicitis is the most common surgical emergency worldwide. Concern that delayed treatment might increase perforation can lead to unnecessary surgery and high rates of negative appendicectomy, where a normal, non-inflamed appendix is removed. However, whether perforation is related to pre-hospital time (symptom onset to first surgical assessment) has not been explored in contemporary global data. METHODS: We conducted an international, multicentre, prospective, observational cohort study (AlliGatOr) of patients undergoing emergency appendicectomy for suspected acute append

Accelerating the clinical development of treatments for dengue: WHO dengue therapeutics landscape analysis and target product profiles.

Dengue, an Aedes mosquito-borne viral infection, is on the rise with climate and demographic change. In 2023, WHO declared dengue its highest grade of emergency, following the largest number of cases and deaths in recorded history. This emergency highlighted the absence of safe and effective dengue-specific treatments as a gap in the product landscape for care for people with dengue. To accelerate development of dengue-specific treatments for wide implementation and access in all dengue-endemic countries, WHO conducted a landscape analysis of the dengue therapeutics pipeline and convened a glo

Standard vs lower pressure pneumoperitoneum in laparoscopic gynecologic surgery: a randomized controlled trial.

BACKGROUND: Postoperative pain following laparoscopic surgery can delay recovery and increase opioid use. Lower insufflation pressures have been consistently shown to reduce pain in general surgery procedures, but their impact in gynecologic surgery is not well established. OBJECTIVE: To investigate the effect of varying insufflation pressures on postoperative pain and operative field visualization among patients undergoing laparoscopic gynecologic surgery. STUDY DESIGN: We conducted a randomized controlled trial of patients undergoing conventional laparoscopy within a minimally invasive gyn

Geography, Policy, and System Strain in U.S. Obstetric and Gynecologic Care.

OBJECTIVE: To examine how abortion policy environments and geographic context are associated with reproductive health outcomes, and to characterize implications for obstetrics and gynecology (OBGYN) care. METHODS: We conducted a cross-sectional analysis exploring associations between outcomes of health care quality and access ranked in the 2024 Commonwealth Fund State Scorecard to Guttmacher abortion policy categories (restrictive, middle-ground, and protective) and U.S. Census regions. Kruskal-Wallis rank-sum tests with Dunn post hoc comparisons and Spearman correlations assessed the relatio

Relationship extraction for adverse drug events in clinical notes using large language models: a retrospective cross-sectional study.

BACKGROUND: Adverse drug events (ADEs) are a critical indicator of patient safety but are often documented only in free-text clinical notes. The potential of recent advances in natural language processing (NLP), particularly generative large language models (LLMs), to identify ADEs remains understudied. This study aimed to compare the performance of multiple LLMs in identifying ADE-Drug relationships in inpatient and ambulatory clinical notes. METHODS: We used clinical notes from the 2018 National NLP Clinical Challenge (n2c2) ADE dataset (inpatient; n = 505) and from outpatient encounters (n

The effect of time-varying left ventricular ejection fraction on clinical outcome in critically ill COVID-19 patients.

BACKGROUND: Although right ventricular (RV) dysfunction and acute cor pulmonale are associated with adverse outcomes in critically ill patients with COVID-19, the prognostic significance of left ventricular (LV) systolic dysfunction remains insufficiently characterized. In particular, dynamic changes in cardiac function during intensive care has seldom been studied. We therefore investigated the association between time-varying LV systolic function, assessed by LV ejection fraction (LVEF) and intensive care unit (ICU) mortality in patients with COVID-19, taking into account right ventricular f

Implementation and sustainability of surgical randomized trials in gynecologic oncology: Adoption beyond publication.

Surgical randomized trials have transformed gynecologic oncology by redefining the extent and route of surgery, nodal assessment, and multimodality care. However, the effect of a surgical trial does not end at publication. Unlike medical therapies, surgical interventions depend on operator expertise, team coordination, pathology support and perioperative infrastructure. Beyond controlled trial settings, these conditions may change, creating technical drift, indication drift, and inequitable adoption. Implementation should therefore be considered a distinct, structured phase of surgical researc

Evaluating [18F]-AlF-FAPI-74 PET/CT for diagnosis of the unfavourable CMS4 colorectal cancer subtype (FoCus study): protocol for a prospective, observational, proof-of-concept clinical study.

INTRODUCTION: Consensus molecular subtype 4 (CMS4) colorectal cancer (CRC) is characterised by stromal activation, therapy resistance and poor prognosis. Current CMS4 assessment relies on tissue sampling, which is limited by sampling bias due to inter-lesion and intra-lesion heterogeneity. Fibroblast activation protein (FAP) gene-expression is strongly associated with CMS4 status. [18F]-AlF-FAPI-74 positron emission tomography (PET)/ computed tomography (CT) enables non-invasive, whole-body assessment of FAP expression and may therefore provide an imaging-based biomarker for CMS4 status in CRC

Postoperative anaesthesia follow-up via telemedicine after outpatient surgery: a proof-of-concept study.

BACKGROUND: Postoperative anaesthesia visits after inpatient procedures guarantee a comprehensive assessment of the patient's status. As the number of outpatient surgical cases in hospitals increases, more patients are recovering at home. Telemedicine may provide an efficient and effective link between anaesthesiologists and patients at home without restricting their freedom and self-determination. METHODS: After an initial acceptance survey, a single-arm, observational pilot study piloted an optional postoperative anaesthesia visit for patients undergoing outpatient surgery under general ana

Primary Intraparenchymal Solitary Malignant Melanoma of the Brain Without Dural Attachment: A Rare Case With Local Recurrence.

Malignant melanoma is a neoplasm arising from the malignant transformation of melanocytes. These cells are derived from the neural crest and are found predominantly in the skin, but also in mucous membranes, the uveal tract of the eye, the leptomeninges, and other sites. A 39-year-old previously healthy male presented with a holocranial headache for two months and a single episode of a focal seizure involving his left arm. Neurological exam revealed left-sided pronator drift and gross papilledema. Contrast-enhanced magnetic resonance imaging (CE-MRI) of the brain revealed a large (4.8 x 3.9 cm

Bridging the gap: Are OSA patients ready for Telemedicine? An Italian perspective.

Different telemedicine approaches for managing sleep disorders have been extensively researched worldwide, yet data on patients' individual willingness to adopt these protocols remain limited. This study aims to fill this gap, focusing specifically on the Italian population. A 19-item anonymous questionnaire was administered to 255 consecutive obstructive sleep apnoea (OSA) patients. Our findings indicate a substantial willingness to use telemedicine: 56.5% were unreservedly in favour of remote follow-up and 67.4% preferred comprehensive telemonitoring to PAP-only monitoring, although only 48.

Plastic surgery access deserts in the United States: A GIS-based national assessment.

BACKGROUND: Timely access to plastic and reconstructive surgery is critical for trauma, burns, congenital anomalies, hand conditions, and oncologic reconstruction. Prior workforce studies often rely on county- or state-level surgeon counts, which fail to capture patient travel burden. Network-based geographic analyzes using travel time may better reflect true access. METHODS: Using the 2023 Centers for Medicare & Medicaid Services Physician and Other Practitioners Public Use File, all Medicare-participating plastic surgeons with valid U.S. practice addresses were identified. Locations were ge

Synoptic reporting of cytoreductive surgical procedures for patients with peritoneal malignancy based on the PSOGI-ESGO-ISSPP consensus.

Cytoreductive surgery (CRS), for peritoneal malignancies (PM) is performed with the goal of achieving a CC-0 resection. However, the extent of peritonectomy and type of visceral resections (oncological versus non-oncological) performed during CRS vary among surgeons and lack standardization. The classification of peritonectomy procedures was revised by a consensus among some of the most experienced CRS surgeons in the world, defining the subdivisions and boundaries of each procedure. A format of documenting the extent of peritoneal resection was created which is an important research tool. To
