BACKGROUND: Elevated parathyroid hormone (PTH) levels are associated with cardiovascular events, bone disease, and mortality in patients undergoing maintenance hemodialysis. Although PTH levels vary widely in patients undergoing hemodialysis, whether this affects mortality is unclear.
METHODS: A total of 315 maintenance hemodialysis patients who underwent PTH measurements at least twice a year were enrolled. The association between all-cause mortality, cardiovascular events, and fractures was evaluated in patients with PTH concentrations of 60-240 pg/mL (time-in-target range (TTR) 100%) and t

Fluid overload accompanies critical illness, contributing to morbidity and mortality. The mechanism is multifactorial, involving microvascular and endothelial glycocalyx damage. Felines are prompt to fluid overload. Studies in other species suggest that IV fluid administration could damage the endothelial glycocalyx, contributing to fluid overload. Therefore, assessing the effects of IV fluids on the feline microcirculation is crucial to avoid such complications. This study evaluated whether a large-volume-short-duration IV fluid (fluid challenge) results in endothelial glycocalyx changes in c

BACKGROUND: In patients with renal hyperparathyroidism the choice of surgery varies between total parathyroidectomy with autotransplantation (TPTx + AT) and subtotal parathyroidectomy (SPT) with variable outcomes. The present study investigated the risk of extracervical recurrences in patients who underwent total parathyroidectomy with autotransplantation for dialysis patients with renal hyperparathyroidism.
METHODS: A retrospective cohort analysis of patients who underwent TPTx + AT for renal hyperparathyroidism between the years 2000 and 2023 at a tertiary referral hospital. Patient demogra

BACKGROUND: Hyperkalemia is a life-threatening electrolyte complication reported sporadically in adults on chronic dialysis and secondary hyperparathyroidism (SHPT) after parathyroidectomy (PTX), but has never been recorded in pediatric patients. This study aimed to estimate the incidence, explore potential risk factors and early predictive markers associated with its occurrence in children.
METHODS: We conducted a retrospective cohort study that included 21 pediatric patients on chronic dialysis and SHPT who underwent subtotal PTX at a single pediatric center. Patients were divided into two

Hemodialysis is an intensive treatment with a substantial time burden for patients. Generally, a standard hemodialysis schedule consists of three treatments per week, each lasting 4 hours. However, it can be debated whether this schedule is appropriate for all patients. Incremental hemodialysis refers to a treatment approach where hemodialysis is initiated with a less intensive schedule ( i.e ., fewer sessions per week) and gradually increased as residual kidney function declines. The effects of incremental hemodialysis on key outcomes-health-related quality of life, vascular access-related co

BACKGROUND: Vitamin D deficiency is common in chronic kidney disease (CKD). Vitamin D/vitamin D receptor (VDR) signaling intersects inflammation, oxidative stress/mitochondrial injury, fibrogenic pathways, the renin-angiotensin-aldosterone system (RAAS), and the gut-kidney axis, providing a biologic rationale for renoprotection.
METHODS: Narrative review; literature identified from PubMed/MEDLINE, Embase, Web of Science, and Cochrane Library (January 2000-August 2025). Adult CKD populations (non-dialysis, dialysis, transplant) were included. Outcomes covered biologic/surrogate (e.g., proteinu

Sodium imbalance is a common concern in children with kidney diseases, presenting as either sodium excess or sodium deficit, each with significant clinical implications. Sodium excess contributes to fluid overload and hypertension, while increased sodium losses, particularly via urine or peritoneal fluid, can predispose patients to hypotension and growth failure. Effective sodium management is thus a critical component of care in pediatric kidney diseases, with dietary sodium intake playing a pivotal role in maintaining homeostasis. The Pediatric Renal Nutrition Taskforce (PRNT) has developed

OBJECTIVE: Intramural hematoma (IMH) carries significant risk of morbidity and mortality, and although thoracic endovascular aortic repair (TEVAR) is increasingly used as a treatment, the optimal timing for its intervention remains uncertain. This study aimed to compare outcomes between patients undergoing urgent TEVAR (within 24 hours of admission) and those undergoing elective TEVAR (beyond 24 hours from admission).
METHODS: Patients who underwent TEVAR between aortic zones 2 and 5 for IMH from 2013 to 2023 were included, excluding those with rupture. Propensity scores were generated using

BACKGROUND: Sublingual video microscopy is increasingly used to study the microcirculation in acute illness. However, the sublingual microcirculation in patients with chronic cardiovascular diseases (CVD) is understudied. Our objective was to quantify sublingual microcirculatory parameters in a large cohort of patients with CVD.
METHODS: One hundred and thirteen patients with CVD were enrolled. Incident dark-field handheld video microscopy (IDF-HVM) was used to quantify microvascular flow index (MFI), microvascular heterogeneity index (MHI), proportion of perfused vessels (PPV), perfused vess

Perinatal hypoxia causes hypoxic-ischemic brain damage (HIBD), which can result in neurological sequelae of varying severity. This syndrome is a major cause of neonatal mortality and pediatric impairment. Although research is ongoing to better understand its origins and develop targeted treatments, endoplasmic reticulum stress (ERS)-mediated apoptosis via the unfolded protein response (UPR) is crucial for the progression of cerebral ischemia injury. Parthenolide (PN), a sesquiterpene lactone derived from the Asteraceae family, exhibits anti-inflammatory, anti-apoptotic, and antioxidant propert

BACKGROUND: High-dose methotrexate (HDMTX) is central to treating primary central nervous system lymphoma but carries a risk of acute kidney injury (AKI), which can delay methotrexate (MTX) clearance and increase toxicity. Glucarpidase is the treatment of choice for MTX toxicity, but limited access in many countries may necessitate alternatives. We present the first reported adult case of combined high-flux hemodialysis (HFHD) and HA230 hemoadsorption for MTX clearance.
CASE SUMMARY: A 66-year-old woman with newly diagnosed primary central nervous system lymphoma began induction chemotherapy

Chronic wounds (CWs) represent a major clinical challenge, characterized by persistent inflammation and failed repair. While proresolving pathways are known to regulate inflammatory responses, their potential dysfunction in CWs remains unexplored. Here, we identify dysregulation of the pro-opiomelanocortin-melanocortin 1 receptor (POMC-MC1R) axis as a common feature across pressure ulcers, venous ulcers, and diabetic ulcers. Using MC1Re/e mice lacking functional MC1R, we demonstrate impaired wound healing marked by delayed reepithelialization and increased neutrophil extracellular traps-pathol

BACKGROUND: Selenium is an essential trace element, and its deficiency is associated with thyroid dysfunction, which is known as a cardiovascular risk factor. Hemodialysis (HD) patients are particularly prone to selenium deficiency, and this nutritional factor may increase the risk of thyroid dysfunction and subsequently cardiovascular disease (CVD). Therefore, we investigated selenium deficiency in HD patients and tried to analyze the effect of selenium deficiency on thyroid dysfunction and its correlation with CVD.
METHODS: A cross-sectional study was conducted with 183 chronic HD patients.

BACKGROUND: The Commando procedure, which involves replacement of both the aortic and mitral valves along with reconstruction of the intervalvular fibrous body, is technically demanding. Commando procedure typically performed via an incision extending from the ascending aorta to the roof of the left atrium. However, in patients with extensive adhesions due to prior cardiac surgery, adding a superior transseptal incision can provide good exposure and reduce the risk of surrounding tissue injury.
CASE PRESENTATION: A 48-year-old woman with end-stage kidney disease on dialysis, diabetes mellitus

Hypophosphatemia, defined as serum phosphate levels below 2.5 mg/dL, is a common yet underrecognized electrolyte disturbance in critically ill patients, with prevalence estimates reaching up to 80%. This review explores the intricate bidirectional relationship between hypophosphatemia and hyperventilation, emphasizing its profound implications for respiratory function and critical care management. Hypophosphatemia impairs oxygen delivery by depleting 2,3-diphosphoglycerate (2,3-DPG), disrupts central respiratory drive, and weakens respiratory muscles, leading to hyperventilation, ventilatory f
