Extracellular vesicle signature is associated with cardio-metabolic improvement after bariatric surgery.

AIMS: Bariatric surgery (BS) reduces obesity-associated systemic inflammation leading to multiple cardiovascular (CV) and metabolic benefits. Here, we tested whether measuring vaso-inflammatory cytokines, gut hormones, and circulating extracellular vesicles (EV) provide vaso-inflammatory-metabolic signatures that better correlate to CV-metabolic outcomes after BS, compared to a standard clinical assessment including body weight (BW) loss and traditional CV risk factors. METHODS AND RESULTS: In 111 patients with severe obesity, conventional clinical-biochemical parameters and non-conventional

A WeChat-based, expert knowledge-constrained artificial intelligence health education intervention for patients undergoing gastrointestinal surgery: protocol for a randomized controlled trial.

BACKGROUND: Perioperative health education is essential for patients undergoing gastrointestinal surgery, but conventional approaches are often limited by time, workload, continuity, and personalization. Artificial intelligence (AI) may offer a more accessible and interactive approach to patient education. This trial aims to evaluate whether a WeChat-based, expert knowledge-constrained AI health education system can improve anxiety and other patient-reported outcomes in patients undergoing gastrointestinal surgery. METHODS: This is a single-center, open-label, parallel-group randomized contro

The durability of different anchoring points and anchoring types in the left ventricle in an ex vivo porcine model.

OBJECTIVE: Surgical mitral valve repair, including anchoring in different parts of the mitral apparatus, is the standard of care for severe mitral regurgitation. Various percutaneous approaches have been developed and are currently under development, but are struggling with durable results. This study investigates the durability of different anatomic anchoring points and anchoring types in the left ventricle. MATERIAL AND METHODS: Anchoring was tested with PTFE sutures or a 5 mm helix at three locations: the head of the papillary muscle, the lateral wall of the ventricle, and the apex of the

Fertility preservation in girls and adolescents with Turner syndrome: lessons for reproductive care in differences of sex development - a mini-review.

Current knowledge regarding fertility preservation (FP) in girls is derived predominantly from pediatric oncology cohorts; however, FP is increasingly being applied to non-oncological conditions associated with impaired fertility, including differences of sex development (DSD). Turner syndrome (TS), characterized by accelerated germ cell depletion beginning during fetal life, represents the DSD condition with the greatest clinical experience in FP. This mini-review summarizes current evidence on ovarian tissue cryopreservation and oocyte cryopreservation in TS. Available data support the techn

The effectiveness of serious games in healthcare education: An umbrella review.

BACKGROUND: Serious games (SGs) use game-based principles seen in entertaining games but for an educational purpose. The integration of SGs into health profession education has gained increasing attention to enhance learning outcomes and prepare students for clinical practice. SGs provide students with a realistic clinical experience in assessing data, making clinical decisions and experiencing the consequences of their decisions in a safe environment. However, studies with health professions students tend to focus primarily on short-term knowledge outcomes, with limited attention to other ess

Association of Irreversible Electroporation Treatment with Overall Survival in Stage III Pancreatic Ductal Adenocarcinoma: DIRECT Registry Results.

BACKGROUND: Irreversible electroporation (IRE) is used in selected patients with Stage III pancreatic ductal adenocarcinoma (PDAC), but its association with survival relative to standard-of-care (SOC) therapy remains uncertain. METHODS: This multicenter observational study included 137 patients from the DIRECT registry: 99 treated within an IRE-based strategy and 38 receiving SOC without IRE. Overall survival (OS) from diagnosis was evaluated using propensity-score overlap weighting as the principal adjusted analysis. Conventional inverse probability of treatment weighting (IPTW), propensity-

Survival benefit of recurrence-focused treatment after surgery for pancreatic ductal adenocarcinoma: A propensity score matched analysis of patients from a Dutch National and US institutional cohort.

BACKGROUND: It is unclear whether recurrence-focused treatment (RFT) improves survival in all patients with recurrent pancreatic ductal adenocarcinoma (PDAC). This study aimed (1) to evaluate post-recurrence survival (PRS) in patients treated with RFT versus best supportive care (BSC)-only, and (2) to assess potential effect modification by prognosis at recurrence. METHODS: This observational cohort study included patients with local or distant recurrence after PDAC resection (2014-2022) at all Dutch pancreatic surgery centers and NYU Langone Health. Patients with unknown RFT or survival stat

Identifying disparities at the intersection of race and HIV in patients with sepsis and pneumonia.

BACKGROUND: Pneumonia and sepsis remain leading causes of hospitalization and death among people with HIV (PWH) despite widespread antiretroviral therapy. Although disparities in critical illness have been described, it remains unclear whether the association between HIV status and mortality differs across racial groups. SETTING: Retrospective cohort study of adults hospitalized with pneumonia and/or sepsis using the Healthcare Cost and Utilization Project State Inpatient Databases from seven U.S. states (2018-2021). METHODS: We compared crude in-hospital mortality by HIV status and race. Mu

Anesthetic Management of a Patient With Myasthenia Gravis Undergoing Thoracoscopic Thymectomy: A Case Report and Literature Review.

Myasthenia gravis is an autoimmune neuromuscular disorder characterized by fluctuating skeletal muscle weakness and fatigability. Perioperative management is challenging because patients may have bulbar or respiratory muscle involvement and altered sensitivity to neuromuscular blocking agents, increasing the risk of postoperative respiratory failure and myasthenic crisis. Thymectomy is an established therapeutic option in selected patients with myasthenia gravis and may be performed using minimally invasive thoracoscopic techniques. We report the anesthetic management of a 48-year-old man with

Percutaneous Transaxillary Access for Large-Bore Cardiovascular Interventions: Evidence, Technique, and Complication Management.

Percutaneous transaxillary access is an established alternative to transfemoral large-bore arterial access for transcatheter structural heart interventions and mechanical circulatory support (MCS). Although useful for overcoming hostile iliofemoral anatomy and providing prolonged support and patient mobilization in case of MCS, the axillary/subclavian territory poses distinct technical and anatomical challenges that demand meticulous pre-procedural imaging, patient selection, accurate puncture under ultrasound and angiographic guidance, preservation of bailout access, and familiarity with sutu

Transaxillary access for ascending aortic and root surgery: a single-center comparison with full and partial sternotomy.

BACKGROUND: Minimally invasive approaches for ascending aortic and root surgery remain controversial regarding operative complexity and safety. The transaxillary (TAX) approach avoids sternotomy entirely but has not been compared with both full sternotomy (FS) and partial upper sternotomy [minimally invasive cardiac surgery (MIC)]. This study evaluates perioperative outcomes across these three access strategies. METHODS: Between January 2022 and December 2025, 194 consecutive patients undergoing elective isolated ascending aortic or root surgery were retrospectively analyzed. Surgical access
