Electroconvulsive therapy shifting brain dynamics toward more adaptive and integrated states in treatment-resistant depression based on a hidden markov model.

Treatment-resistant depression (TRD) is characterized by persistent symptoms and poor response to conventional therapies, yet its dynamic neural mechanisms and treatment-related changes remain incompletely understood. This study applied a hidden Markov model (HMM) combined with neurotransmitter atlas mapping to investigate whole-brain spatiotemporal dynamics in TRD and their modulation by electroconvulsive therapy (ECT). Resting-state fMRI data were collected from 72 TRD patients (52 completing follow-up) and 63 healthy controls. HMM identified six recurring brain states defined by distinct pa

Neural substrates of knowledge integration: differential effects of high-definition transcranial direct current stimulation (HD-tDCS) on angular gyrus and ventrolateral prefrontal cortex.

BACKGROUND: Humans develop extensive knowledge systems throughout their lives, which are continuously updated in response to new information. One way in which prior knowledge is updated is through semantic integration, where related semantic facts are integrated to support derivation of new information. METHODS: This study investigates how anodal high-definition transcranial direct current stimulation (HD-tDCS) applied to the left angular gyrus (AG; Experiment 1) or to the left ventrolateral prefrontal cortex (vlPFC; Experiment 2) influences semantic integration and the derivation of novel in

Distinct antidepressant therapies act on a common brain network.

Depression disorder is a global health crisis, due to its high prevalence. Yet clinical treatments are inadequate. Uncovering the neural correlates of depression remission is a vital step toward developing effective therapies. Using a method termed therapeutic network mapping, we performed secondary analyses on the data of 16 longitudinal experiments and found that the network of the amygdala was commonly modulated by three antidepressant therapies (pharmacotherapy, electroconvulsive therapy, and magnetic seizure therapy). This therapeutic network was i) validated by two independent treatment

Science or hype? Non-invasive brain stimulation and sport-specific performance: Evidence from a systematic review and three-level meta-analysis.

Enhancing athletic performance is a core objective in competitive sports. Non-invasive brain stimulation (NIBS), a set of neuromodulatory techniques, aims to modulate cortical excitability and thereby influence motor and cognitive functions. Despite its commercial appeal and growing application in sports contexts, the empirical evidence supporting its efficacy remains inconclusive. A three-level meta-analysis was conducted to evaluate the effects of NIBS on sport-specific performance and to identify potential moderating factors. The present study included 25 eligible studies and 64 independent

Left Frontal HD-tACS Enhances Prefrontal Theta Activity During Action Verbal Fluency in Parkinson's Disease.

While invasive brain stimulation of subcortical structures improves motor symptoms in Parkinson's disease (PD), its effects on speech and language impairments remain inconsistent. Accumulating evidence supports the application of transcranial alternating current stimulation (tACS) to enhance both motor and non-motor functions in the disorder. We conducted a sham-controlled, multisite 4 Hz HD-tACS over the left central and frontal regions in 17 non-demented PD patients. Following stimulation, EEG data were recorded while participants performed action fluency (AF), phonemic fluency (PF), and sem

Transcranial direct current stimulation and cognitive training in the treatment of cognitive deficit in schizophrenia: a randomized controlled trial.

BACKGROUND: Cognitive impairment is a core feature of schizophrenia spectrum disorders and a major contributor to functional impairment in patients with the disorder. Given the limited effects of pharmacological treatments on cognition, there is an increasing interest in the use of alternative therapeutic approaches, including cognitive training and non-invasive brain stimulation. Our study aimed to evaluate the effectiveness of the combined treatment of tDCS and cognitive training on improving cognitive functions in patients with schizophrenia. METHODS: This one-week, randomized, double-blind

Clinical impact of transcranial unilateral motor evoked potentials monitoring in cerebral aneurysm endovascular treatment.

OBJECTIVE: Neurosurgery commonly employs motor evoked potential (MEP) monitoring to assess intraoperative motor function. However, its utility in neuroendovascular procedures has been sporadically reported, widespread adoption remains limited, largely due to challenges associated with stimulus-induced patient movement and subsequent image misregistration. This study aimed to address these limitations by evaluating the feasibility of an optimized transcranial electrical stimulation MEP (TES-MEP) protocol designed to minimize body movement while preserving monitoring reliability. METHODS: We per

Neuromodulation of heart rate variability: A systematic review.

BACKGROUND: Autonomic nervous system (ANS) dysfunction contributes to an increased risk of cardiovascular events. Non-invasive neuromodulation methods, such as transcranial direct current stimulation (tDCS), transcranial magnetic stimulation (TMS), and transcutaneous vagus nerve stimulation (tVNS), have been used to modulate the ANS. However, their comparative efficacy and optimal protocols remain uncertain due to study heterogeneity. This study aimed to verify the level of evidence for the effect of these neuromodulation methods on heart rate variability (HRV) and sympathovagal balance. METH

The Integrated Application and Future Trends of Multimodal Neuromodulation Techniques in Spinal Cord Injury Rehabilitation.

Spinal cord injury (SCI) remains a severe condition that leads to permanent motor and sensory impairments, significantly affecting patients' quality of life. In recent years, neuromodulation techniques such as spinal cord stimulation (SCS), transcranial magnetic stimulation (TMS), and transcranial direct current stimulation (tDCS) have shown promising results in promoting neural plasticity and functional recovery. However, the limitations of single-modality approaches have spurred the development of multimodal neuromodulation strategies. This review systematically analyzes the integrated appli

Transcranial direct current stimulation eliminates the own-age bias as indexed by the face inversion effect for own- versus other-age faces.

We investigate individuals' reduced ability to recognize faces of other ages, a phenomenon known as the Own-Age Bias (OAB). This study utilized a double-blind, between-subjects design in which anodal tDCS (10 minutes duration, 1.5 mA intensity, targeting the Fp3 location) was applied to disrupt the face inversion effect (FIE), which reflects higher recognition performance for upright compared to upside-down faces. Young adults participated in an old/new recognition task, where upright and inverted 'own-age' faces (19-30 years) and 'other-age' faces (older, 69-80 years) were presented. In the s

Effects of personalized vs. non-personalized neurostimulation protocols in improving speech and limb reaction times.

Abnormal activity within supplementary motor area (SMA) has been associated with impaired speech and limb movement in neurological conditions. Normalizing aberrant neural activity through non-invasive neuromodulation techniques over SMA has demonstrated promising effects in ameliorating motor and non-motor functions. However, there is limited research on the application of transcranial electrical stimulation (tES) over the left SMA as a potential non-invasive protocol to improve speech production. In this study, we examined the effects of several tES protocols, including high-definition transc

High definition tDCS of the left dorsolateral prefrontal cortex modulates nonlinear HRV in high test anxiety individuals.

Prior neurostimulation research has underscored the importance of the left dorsolateral prefrontal cortex (DLPFC) in the modulation of autonomic nervous system responses, with activation of this region significantly enhancing heart rate variability (HRV) - a key indicator of autonomic flexibility. High test anxiety (HTA) individuals often demonstrate diminished HRV in evaluative scenarios, signalling reduced physiological adaptability to stress. This study investigates the effects of offline high-definition transcranial direct current stimulation (HD-tDCS) targeting the left DLPFC on nonlinear

Transcranial Direct Current Stimulation Does Not Enhance Perceptual Learning of Chinese Character Reading in Adults With Macular Degeneration.

PURPOSE: Macular degeneration impairs central vision, compelling patients to use their peripheral vision for reading, which is difficult due to reduced spatial resolution and crowding. Although perceptual learning improves reading, single-session anodal transcranial direct current stimulation (a-tDCS) over the visual cortex has shown inconsistent outcomes, with transient improvements observed in English reading but no benefit for Chinese reading in macular degeneration patients. This randomized controlled trial investigated whether combining multi-session a-tDCS with perceptual learning enhanc

Magnetic seizure therapy preserves delayed memory and 40-Hz auditory steady-state responses compared with electroconvulsive therapy in schizophrenia.

BACKGROUND: Electroconvulsive therapy (ECT) is effective for schizophrenia but often impairs delayed memory, whereas magnetic seizure therapy (MST) typically spares cognition. We investigated whether these modalities differentially affect 40-Hz auditory steady-state responses (ASSR)-a marker of gamma synchrony critical for memory-and if these changes predict cognitive outcomes. METHODS: This study is a secondary analysis of a randomized clinical trial for schizophrenia comparing ECT (n = 16) and MST (n = 17). Clinical symptoms and cognition were assessed using Positive and Negative Syndrome S
