BACKGROUND: The default pulse width setting on a commonly used electroconvulsive therapy (ECT) device, 0.5 ms (ms), is untested against the proven effectiveness of conventional 1.0-1.5 ms brief pulse widths.
METHODS: We examined the association between pulse width and clinical response using registry data from 87 centres in the United Kingdom and Republic of Ireland in patients (N = 1956) receiving consecutive acute courses of ECT ending during 2023 for any clinical indication. The outcome was clinical response, defined as an end-of-treatment Clinical Global Impression‒Improvement score of 1

Vocabulary learning requires learners to establish stable mappings between symbols and meanings. Reinforcement learning (RL) principles, through feedback-based value computation, provide a compelling framework supporting such mappings, with the ventromedial prefrontal cortex (vmPFC) playing a key role in value-based learning and decision-making. In this study, we combined artificial pseudowords with a probabilistic reversal learning paradigm and applied high-definition transcranial direct current stimulation (HD-tDCS) in the vmPFC to investigate its role in vocabulary acquisition. Computationa

Electroconvulsive therapy (ECT) remains the most effective intervention for treatment-resistant depression (TRD). We investigated whether cortico-limbic structural connectivity is associated with ECT response. Twenty-nine TRD patients underwent bifrontal ECT and baseline probabilistic tractography assessing connectivity between amygdala, anterior insula (aINS), orbitofrontal cortex (OFC), posterior cingulate cortex (PCC), posterior ventrolateral (VLPFC) and dorsolateral prefrontal cortex (DLPFC), subgenual cingulate cortex (SGCC) and thalamus. Stronger connectivity within a subnetwork comprisi

BACKGROUND: The reading adaptation effect in stuttering refers to decreased stuttering frequency during repeated readings. This study investigated whether anodal transcranial direct current stimulation (tDCS) over the left primary motor cortex (M1) enhances reading adaptation in adults who stutter.
METHODS: Two consecutive studies were conducted. Study 1 was a randomized, double-blind, sham-controlled trial with twenty adults who stutter (14 males, 6 females) assigned to either an anodal tDCS (2 mA, 20 min) or a sham stimulation group. Participants read a passage five times during stimulation

BACKGROUND: Electroconvulsive therapy (ECT) is effective in depression, but associated with postictal agitation. Mechanisms of postictal symptoms are largely unclear. We studied whether postictal symptoms are related to immediate postictal diffusivity changes.
METHODS: In a randomized cross-over clinical trial, we acquired diffusion tensor imaging (DTI) scans before the ECT-course and three times immediately postictally in 22 patients. In this secondary analysis, mixed effects models and Generalized Estimating Equations were used to assess postictal changes in mean fractional anisotropy (FA)

A recent study by Hausdorf et al. reported that combining closed-loop acoustic stimulation with transcranial direct current stimulation during sleep does not improve memory consolidation. This contrasts with past studies that reported a beneficial effect of each method individually. In this commentary, I discuss the relevance of the study in the current literature and offer potential explanations for this absence of effect.

BACKGROUND: The amyloid-β peptide 42 (Aβ42) forms fibrillar aggregates that are a hallmark of Alzheimer's disease. While recent therapeutic strategies targeting Aβ42 fibrils and oligomers have shown promise, safer and more effective interventions are still needed. Noninvasive brain stimulation (NIBS) techniques such as repetitive transcranial magnetic stimulation (rTMS) and transcranial direct current stimulation (tDCS) have emerged as potential complementary approaches, yet the molecular mechanisms by which electric fields influence amyloid aggregation remain poorly understood.

BACKGROUND: Transcranial direct current stimulation (tDCS) has gained attention as a potential intervention to improve social cognition in children with autism spectrum disorder (ASD). However, its effects across social domains and the influence of stimulation parameters remain unclear. This systematic review and meta-analysis evaluate the effectiveness of tDCS in enhancing social functioning in children with ASD, focusing on emotion recognition, theory of mind (ToM), and social responsiveness. METHODS: A comprehensive search identified randomized controlled trials (RCTs) investigating tDCS ef

AIMS: Emerging evidence suggests that indole-3-propionic acid (IPA), a gut microbiota-derived tryptophan metabolite, confers metabolic benefits in type 2 diabetes (T2D). However, its direct role in preserving pancreatic β-cell function and the underlying molecular mechanisms remains largely undefined. This study aimed to investigate the role and underlying mechanisms of IPA in preserving β-cell function and alleviating endoplasmic reticulum (ER) stress under diabetogenic conditions.
MATERIALS AND METHODS: High-fat diet-fed and db/db mice were treated with IPA via oral gavage. Glucose homeosta

INTRODUCTION: Current predictors of electroconvulsive therapy (ECT) effects rely on static measures. The prognostic value of anxiety evolution during ECT remains unestablished, which impedes personalized treatment. This study aims to investigate the dynamic evolution of anxiety during ECT and its predictive role in treatment effects.
METHODS: We collected 1,053 data points from 117 patients with depression who were undergoing ECT, measuring both the Hamilton Anxiety Rating Scale (HAMA) and the 17-item Hamilton Depression Rating Scale (HAMD-17) at baseline and at each of the first eight ECT se

Frontal midline theta oscillations are key neural markers for learning, set-shifting, and adaptive behavior, signaling cognitive control and the reorganization of neural representations. The present study explored how these oscillations mediate the extraction and updating of statistical regularities. We delivered 6-Hz in-phase or sham transcranial alternating current stimulation, synchronizing frontal midline theta during an eye-tracking probabilistic sequence learning task designed to test cognitive flexibility and assess changes in pre-stimulus gaze direction. A novel probabilistic sequence

Stroke is a leading cause of long-term disability, and many patients fail to achieve complete recovery following cerebral injury. Therefore, post-stroke rehabilitation is essential to restore impaired function. Transcranial electrical stimulation (tES), transcranial direct current stimulation (tDCS), and transcranial alternating current stimulation (tACS) have emerged as promising neuromodulation approaches to enhance post-stroke recovery. These treatments have therapeutic effects to restore impaired function by modulating cortical excitability and reorganizing brain tissue through electrical

BACKGROUND: Transcranial direct current stimulation (tDCS) has been proposed as a potential non-pharmacological intervention for Attention-Deficit/Hyperactivity Disorder (ADHD). While evidence from pediatric populations is growing, the effects of tDCS in adults with ADHD remain insufficiently characterized, increasing recognition of ADHD as a lifelong neurodevelopmental condition with persistent functional impairments.
OBJECTIVE: This systematic review aimed to evaluate the effects of tDCS in adults with ADHD across clinical, cognitive, and neurophysiological outcome domains.

Interventions for major depressive disorder (MDD) using noninvasive brain stimulation often target the left lateral prefrontal cortex to reduce symptoms of anhedonia. However, the electrophysiological mechanism by which symptoms of anhedonia arise from dysfunction of the lateral prefrontal cortex is poorly understood. Furthermore, multiple constructs related to reward processing within the positive valence systems have been found to be blunted with anhedonia. To disentangle components of the positive valence systems, human participants with an active episode of MDD performed an adaptive versio
