Equivalent incidences of paediatric graft-versus-host disease regardless of donor-recipient matching in the era of modern prophylaxis agents.

Recipients of haploidentical and unrelated haematopoietic cell transplants (HCTs) historically had a higher risk for graft-versus-host disease (GvHD). However, with improved GvHD prophylaxis, recent data suggest adult patients undergoing transplantation from unrelated or mismatched donors now experience the rates of GvHD similar to those receiving matched related donor grafts. This finding is not yet explored in paediatric patients. The objectives of this study were to: (1) compare acute and chronic GvHD incidence, mortality and GvHD-free relapse-free survival (GRFS) among matched related dono

Antibody-mediated rejection of transplanted kidney associated with anti-Jka alloantibodies: An interesting case report with review of literature.

There is increasing evidence for renal expression of non-ABO minor blood group antigens on the renal endothelium of the vasa recta and tubular epithelial cells. The role of antibodies to minor blood group antigens posing as an immunological risk to the transplanted allograft is under-recognized. We present one interesting case of a 40-year-old male patient who developed antibody-mediated rejection (AMR), leading to graft loss despite multiple interventions. Interestingly, he was found to have antibodies to the Kidd red-cell antigen, Jka, which later was found to be also present in his kidney d

Donor-specific anti-HLA antibodies (DSAs) in patients undergoing allogeneic hematopoietic stem cell transplantation from mismatched donors on behalf of GITMO and AIBT.

BACKGROUND: Antibodies directed against donor-specific HLA allele(s)/antigen(s) (DSAs) represent a known risk factor for hematopoietic stem cell transplantation (HSCT) engraftment. Still, the overall management needs to be standardized. MATERIAL AND METHODS: GITMO and AIBT ran a survey on DSAs in Italian Transplant Programs including mismatched HSCT performed between January 2014 and June 2017. RESULTS: One-thousand-thirty-three patients were proposed for the study, 804 were evaluable. Overall, 355 (44%) were screened: 91/355 (25.6%) showed anti-HLA antibodies, 23 DSAs (6.5%). Female gender

The association of donor and recipient sex on sepsis rates and hemoglobin increment among critically ill patients receiving red cell transfusions in a retrospective study.

BACKGROUND: Existing research presents conflicting results on the influence of blood donor sex on hemoglobin (Hb) change and transfusion-associated infection and mortality in transfusion recipients. AIM: This retrospective study explored the association between donor and recipient sex on hospital-onset sepsis (HO-sepsis) and Hb changes in critically ill patients receiving red blood cell (RBC) transfusions. METHODS: Data from 2010-2020 were extracted from an academic hospital's clinical database and a blood supplier's donor database. HO-sepsis was determined based on the International Classif

When Blood Remembers Its Sex: Toward Truly Personalized Transfusion Medicine.

Background: Biological sex differences are well-recognized as non-negligible factors in implementing precision medicine practice. Sex chromosomes influence protein expression and signaling, and thus cellular pathways are often regulated differently. Additionally, the importance of sex as a biological variable has gained significant traction in biomedical research, including transfusion medicine. Regarding transfusion medicine, several studies reveal the role of gender in blood transfusion, blood donors' behavior towards donation, blood products' composition and storage, transfusion therapy, an

Association analysis of T and B-cell epitopes with humoral alloimmunisation in kidney transplantation: A Tunisian cohort study.

INTRODUCTION: HLA matching is critical for successful kidney transplantation. This study aimed to investigate the impact of eplet mismatches and Predicted Indirectly Recognizable HLA Epitopes (PIRCHE-II) scores on the development of de novo donor-specific antibodies (dnDSA) and graft survival in a Tunisian cohort, characterized by a high prevalence of living donors and significant genetic diversity in HLA profiles. METHODS: This retrospective study included 112 adult kidney transplant recipients who underwent transplantation between 2012 and 2018. Donor and recipient HLA typing was performed

Evaluating the Antigen and Eplet Accuracy of DQA1 Imputations With the HaploSFHI Two-Field HLA Typing Inference Tool.

Donor/recipient mismatched HLA antigens can lead to the production of Donor-Specific Antibodies by the recipient, which are deleterious to organ transplants. The HLA-DQ locus is the most frequent target, with both the DQ beta and alpha chains involved. For deceased donors in particular, while HLA-DQB1 has been typed in emergencies for a long time, HLA-DQA1 has only recently been included. No imputation algorithmic tool was available to impute HLA-DQA1 until the development of HaploSFHI, trained on 61,393 two-field typings by NGS methods. We evaluated the accuracy of two-field HLA-DQA1 imputati

The impact of ABO compatibility/incompatibility between donor and recipient of allogeneic bone marrow transplant on transplant outcomes.

BACKGROUND: ABO blood group mismatch between donor and recipients is not considered as a major contraindication to allogeneic hematopoietic stem cell transplantation (allo-HSCT). Nevertheless, there is still conflicting reports on the impact of ABO incompatibility on allo-HSCT outcomes, including the risk of graft-versus-host disease (GVHD), relapse of underlying disease, and patient survivals. METHODS: We conducted a retrospective cohort study in 157 patients who underwent HSCT from October 1st 2019, to September 30th 2023, to determine the effect of ABO compatibility on allo-HSCT outcomes,

Transfusion strategy and allo-immunization to red blood cell antigens after allogeneic hematopoietic stem cell transplantation with partial donor/recipient Rh matching.

BACKGROUND: After allogeneic hematopoietic stem cell transplantation (alloHSCT), the HAS recommends transfusing red blood cells (RBCs) matched for Rh ("Rhesus") phenotypes, and the EFS recommends respecting both donor and recipient Rh phenotypes. We previously reported a 10.3% allo-immunization rate in 58 Rh-incompatible alloHSCT recipients transfused in the donor phenotype: none was directed against the mismatched donor Rh antigens. STUDY DESIGN AND METHODS: In this single-center retrospective study, we investigated the incidence of allo-immunization to RBC antigens during the first year aft

ABO blood group discrepancy - An unexpected news for a known blood group O positive regular blood donor.

ABO discrepancies occur when cell and serum blood grouping results do not match, posing a significant risk during transfusions. One rare cause of such discrepancies is the presence of B subgroups, including B 3 , B x , B m , and B el , which show weak or absent B antigen expression due to genetic variations. These subgroups can lead to misclassification, requiring specialized serological tests such as adsorption-elution or molecular genotyping for accurate identification. Proper recognition and documentation are essential to prevent transfusion reactions. We present a case of a 27-year-old reg

GALNT7 Stratifies dMMR/MSI Colorectal Cancer into Distinct Molecular Subsets Associated with Prognosis and PD-L1 Expression.

UNLABELLED: Colorectal cancer with deficient mismatch repair (dMMR)/microsatellite instability (MSI) constitutes a distinct clinicopathologic and immunologic subtype, characterized by high sensitivity to immune checkpoint inhibitors. However, prognosis and therapeutic response vary considerably among dMMR/MSI colorectal cancers, underscoring the need for molecular markers to refine patient stratification. In this study, we systematically investigated cancer cell-intrinsic expression profiles of 188 glycosyltransferase genes by integrating single-cell, bulk, and cell line RNA sequencing dataset

A novel and integrated software solution for providing compatible platelets to HLA-sensitized patients.

Transfusion support for sensitized patients is an important function of blood centers and HLA laboratories. However, access to widely available software to facilitate matching patients with compatible donor units is limited. We developed vendor-supported and integrated software that interfaces with the laboratory informatics system, sourcing typing data from our platelet donor registry and retrieving patient HLA typing and unacceptable antibody specificities. The application filters antigen-negative donors and ranks them using an algorithm that incorporates HLA match, donor homozygosity, and c

ABO matching for platelet transfusions for prevention or treatment of bleeding: A systematic review with meta-analysis.

Transfusion of ABO identical platelets is recommended in national guidelines, though transfusion of ABO non-identical platelets has been widely adopted to ensure availability and reduce wastage. When ABO non-identical platelets are necessitated, there is a lack of consensus on prioritisation of major or minor compatibility. We conducted a systematic review and meta-analysis (PROSPERO CRD42023450792) of randomised and non-randomised studies to assess whether there is a difference when comparing ABO-identical and non-identical (major, minor, bi-directional mismatch) platelet transfusions. Hematopoietic stem cell transplantation across ABO compatibility barrier in HLA genoidentical related donor: Is it an unfavorable factor for overall survival (OS) in Tunisian patients?

Benchmarking of survival following Hematopoietic Stem Cell Transplantation (HSCT) is very useful in the field of transplant medicine. It has been reported that decreased survival rates after HSCT may be affected by the ABO mismatch in some cases. In this study, we aim to assess this relationship in a Tunisian cohort of adult patients who have received HSCT from HLA genoidentical related donors. A total of 147 patients and their 147 respective related donors, were retrospectively analyzed in this study. HLA and ABO antigens typing was performed according to established routine methods. Overall

Human leukocyte antigen sensitization from red cell transfusions: Rate and risk factors in renal and nonrenal patients requiring transfusion.

De novo blood donor-specific human leukocyte antigen (HLA) antibodies (bDSA) from red cell transfusions can complicate solid organ transplantation, but the incidence rate is uncertain. We conducted a prospective observational study to assess the cumulative incidence rate of de novo bDSA in a cohort of patients with chronic kidney disease and no prior transplant, patients with prior kidney transplants (with or without continuing immunosuppression), and a general patient population. HLA antibody testing was performed before and 6 to 8 weeks after transfusion, with patient and blood donor typing.

Kidney transplant rejection despite zero eplet DR / DQ mismatch: A case report.

Eplet mismatch (MM) quantification is a valuable tool for refining histocompatibility in kidney transplantation, especially at HLA-DR/DQ loci. However, zero MM at these loci does not ensure complete immunological safety. We report a 17-year-old male who received a living donor kidney transplant via the kidney paired donation (KPD) program. The donor had 0 eplet MMs at HLA-DR/DQ, with negative crossmatch and no pre-transplant donor-specific antibodies (DSAs). Despite this, the patient developed early mixed rejection-T cell-mediated (TCMR) and antibody-mediated (ABMR), and further analysis revea

Unrelated donor transplantation offers a real curative opportunity for patients with transfusion-dependent thalassaemia. A multicenter retrospective study on behalf of the Spanish group for hematopoietic transplantation and cellular therapy (GETH-TC).

INTRODUCTION: Patients with transfusion-dependent b-thalassaemia (TDT) suffer from severe anemia, both primary and secondary iron overload and end-organ damage. Although new disease-modifying and curative therapies are emerging, hematopoietic stem cell transplantation (HSCT) remains the most widely available curative option for TDT patients. This study aims to compare HSCT outcomes according to the type of donor used, considering not only survival, but also graft-versus-host disease (GVHD) and other transplant-related complications. METHODS: A multicentre retrospective study was conducted, in

Daratumumab for PRCA after HCT: study and practical considerations from the EBMT Transplant Complications Working Party.

Pure red cell aplasia (PRCA) is a relevant complication after ABO-mismatched allogeneic hematopoietic cell transplantation (HCT). No standard treatment exists, and practice is heterogenous. In this study, we took advantage of an international collaboration to describe characteristics and outcomes of patients receiving daratumumab for PRCA following first allogeneic HCT. We identified 45 patients meeting these criteria (median patient age, 56 years). The median time from HCT to PRCA was 55 days (IQR, 36-116) and all patients were transfusion-dependent at time of daratumumab start. Daratumumab w

Assessment of the trends of never events across nhs england: a 6-year analysis.

BACKGROUND: Never events (NEs) are serious clinical incidents. Assessing the trends of these incidents over the years will help direct efforts towards specific areas for improvement. The NHS has made significant efforts in this direction by introducing the National Safety Standards for Invasive Procedures (NatSSIPs) in 2015 and updating them in 2023. This study aims to assess the trends of the common themes of NEs and highlight potential solutions in the available literature and NatSSIPs. METHODS: A taxonomy development and thematic analysis of the NHS NEs annual reports from 2018 to 2024 to

[Campylobacter lari bacteremia during treatment of pure red cell aplasia following major ABO-mismatched hematopoietic stem cell transplantation].

Campylobacter lari is a rare cause of bacteremia in Campylobacter spp. Clinical course and symptoms vary by report, and no standard approaches to antimicrobial selection and duration of treatment have been established. A 68-year-old man who underwent haploidentical hematopoietic stem cell transplantation (HSCT) from a major ABO-incompatible donor with post-transplant cyclophosphamide for myelodysplastic syndromes developed pure red cell anemia and post-transfusion iron overload. On day 360, the patient developed fever and spiral-shaped gram-negative bacteria were detected by blood culture. The
