Trunk Postural Control During Unstable Sitting Does Not Explain Whole-Body Angular Momentum Control During Walking Among Individuals With Unilateral Lower Limb Amputation.

During walking, uninjured individuals maintain whole-body angular momentum (WBAM) within a narrow range. Individuals with lower limb amputation (LLA) have demonstrated larger WBAM ranges during walking relative to uninjured controls, suggesting disrupted WBAM control. Given that the trunk is a principal contributor to WBAM and trunk control may be altered after LLA, we related trunk postural control isolated during unstable sitting and WBAM control during walking across 9 individuals with unilateral transfemoral amputation, 24 individuals with unilateral transtibial amputation, and 16 uninjure

Decompressive fasciotomy as a limb-sparing intervention for gas-forming soft tissue infection of the lower limb in diabetic patients: A prospective cohort study of 20 cases from Dakar, Senegal.

Major amputation remains the default response to gas-forming soft tissue infection of the lower limb in diabetic patients across Sub-Saharan Africa, despite its devastating functional and economic consequences in settings with minimal prosthetic services. We report a prospective cohort study of 20 such patients who underwent primary decompressive fasciotomy at a tertiary centre in Dakar, Senegal. Overall limb salvage was achieved in 75% (15/20), and in-hospital mortality was zero. Among nine patients ultimately requiring amputation, the planned level was downstaged in 78%, including one patien

The case study of prosthetic foot alignment on amputee gait kinematics using IMU-based motion capture system.

The determination of the optimal prosthesis alignment for maximum patient comfort is an important rehabilitation goal, especially for lower limb amputees, where the assessment of biomechanics is complex. Despite this, quantitative gait analysis is rarely used in clinical practice, and the influence of prosthesis alignment on gait kinematics remains unclear. This study explores the use of an IMU-based motion capture system to assess the impact of prosthesis alignment on gait biomechanics. A transtibial amputee's gait was analyzed with various prosthetic foot alignments in the sagittal and front

Analysis of Circulating MicroRNAs in Patients with Diabetic Foot Ulcers and Lower Limb Amputation.

Non-healing diabetic foot ulcers substantially increase the risk of infection and lower limb amputation, contributing to elevated morbidity and mortality among individuals with diabetes. MicroRNAs (miRNAs) have been widely studied as potential early biomarkers for various diseases. This pilot study assessed circulating miRNA expression profiles in adults with diabetic ulcers (n = 31), individuals who underwent limb amputation due to foot ulcers (n = 20), and 50 control subjects using RT-qPCR. Patients with diabetic foot ulcers and amputations exhibited significantly higher plasma levels of miR

A Device for Characterizing Skin Physiological Response to Mechanical Loading in Transtibial Prosthesis Users.

The skin's physiological response to repetitive stress is not well understood in prosthesis users. Improving this understanding could facilitate the design of a diagnostic tool to determine if the skin is adapting to tolerate stress from a prosthetic socket. The objective of this research was to develop a physical system that mechanically stresses the skin in a controlled manner and then implements the imaging modalities of infrared (IR) imaging and optical coherence tomography (OCT). IR imaging characterizes the skin's temperature response, while OCT characterizes vessel diameter changes over

Reinnervation of Muscle Targets Enhances the Separability of Motor Unit Signals Following Peripheral Nerve Transfers.

After amputation, advanced prosthetic limbs offer a promising means of restoring motor function. However, state-of-the-art prostheses often rely on aggregate electromyogram (EMG) signals to decode motor intention, which limits their ability to replicate natural limb movements. Decomposing EMG signals into individual motor unit components has shown potential for more natural control, but distinguishing between individual units can be challenging when nearby signals overlap. This study demonstrates that muscle target reinnervation surgeries can naturally increase physical separation between moto

[Technological advances in amputation medicine].

Amputation medicine has evolved into a specialized, technology-driven discipline. With increasing global amputation rates, advanced surgical techniques, innovative prosthetic solutions and digital rehabilitation are gaining importance. Transcutaneous osseointegrated prosthetic systems (TOPS) can improve mobility, proprioception and quality of life but carry risks such as stomal infections or periprosthetic fractures. Neuroreconstructive procedures including targeted muscle reinnervation (TMR), targeted sensory reinnervation (TSR), regenerative peripheral nerve interface (RPNI), agonist-antagon

Limb-specific mediolateral ground reaction force impulses during cross-slope walking in unilateral lower-limb prosthesis users.

Walking on coronally inclined (cross-slope) surfaces imposes downhill-directed gravitational impulses that must be countered by uphill-directed mediolateral (ML) ground reaction force (GRF) impulses to sustain steady-state straight-ahead walking. We investigated how transfemoral (TFPUs) and transtibial (TTPUs) prosthesis users regulate limb-specific ML GRF impulses when walking on cross-slope. Participants included 12 TFPUs (12 males), 12 TTPUs (11 males, 1 female), and 14 able-bodied controls (6 males, 8 females) who walked at a self-selected speed on an instrumented inclined walkway. The GRF

A clear cell sarcoma tumor of the finger with an unusual presentation misdiagnosed as a glomus tumor in an adolescent patient: A case report.

Clear cell sarcoma (CCS) is an uncommon malignancy originating from neural crest cells; this type of cancer can mimic a number of other tumors and frequently presents in the lower limbs, but is occasionally found in atypical locations. The present study describes a rare case of a 14-year-old girl presenting with a subungual swelling on the right index finger, with the initial impression of a glomus tumor, who underwent unplanned surgical resection at another institution. The histopathological analysis revealed CCS with positive margins, and the patient was therefore referred to an orthopedic o

Trametinib for the treatment of advanced stage patients with arterio-venous malformations.

Background: Arterio-venous malformations (AVMs) may lead to vessel aneurysms and thus to life-threatening bleeding or malperfusion of the extremities, causing recurrent ulceration. Possible therapies are embolisation and/or resection. This study investigates treatment options for patients with AVM Schobinger stage III and IV for whom embolisation and resection are no longer possible and medical treatment with trametinib, a mitogen-activated protein kinase inhibitor (MEK) was performed. Patients and methods: As part of the German guideline development process for the diagnosis and treatment of

Economic burden of extremities amputations: A 10-year retrospective analysis of direct surgical and hospitalization costs.

Limb amputation is a major public health concern, with profound physical, psychological, and socioeconomic consequences. Although largely preventable, the prevalence of amputation continues to rise, particularly in regions with high rates of diabetes and vascular diseases. Despite this, national-level data evaluating demographic trends, clinical characteristics, and the associated costs of amputations remain limited. This study aimed to provide a comprehensive decade-long retrospective analysis to assess the epidemiological patterns and economic burden of amputations. A retrospective review wa

Comparing Real-Time and Recalled Ratings of Phantom Limb Pain Intensity.

INTRODUCTION: Evidence-based practice relies on accurate measurement of healthcare conditions. However, measurement of phantom limb pain after amputation can be challenging due to the varying intensity, duration, and quality of pain episodes. Standard measurement of phantom limb pain involves patient recall during clinic appointments and therefore may provide biased information. The purpose of this study was to compare recalled average and maximum ratings of phantom limb pain intensity with real-time ratings. METHODS: Participants (n = 17, 59% male, age 32-76) with lower limb amputation compl

Design and Initial Evaluation of a Low-Cost Microprocessor-Controlled Ankle Prosthesis.

Lower-limb amputation remains a significant clinical and socio-economic challenge, while the high cost of microprocessor-controlled prostheses (MPKs) limits their widespread accessibility. This paper presents the design and preliminary laboratory-scale evaluation of a low-cost microprocessor-controlled ankle prosthesis intended as a feasibility-oriented alternative platform for future active prosthetic system development. Building upon the previously developed V1 mechanical architecture, an updated CAD model was created in the SolidWorks 2024 environment, and the kinematic configuration was re

Biomechanical Problems Related to Lesser Toes Dysfunction and Amputation.

Should we care about the biomechanics of the foot or the full locomotor system? Anatomic changes are the main concerns of an orthopedic surgeon; however, the main objectives of treatment for lesser toe dysfunctions are to relieve the patient's symptoms to preserve or improve function. It is imperative to understand that the foot is part a three-dimensional biokinetic system that needs to function combined so that loads are harmoniously distributed throughout the body without harmful effects, especially for one of the most frequent voluntary neuromotor tasks in humans: walking.

Unilateral Forearm Ulceration in a Newborn: Thinking Outside the Box (Compartment).

This case details a neonate with unilateral forearm ulceration and motor weakness diagnosed with neonatal limb compartment syndrome (NLCS) following interdisciplinary collaboration. NLCS is a rare cause of congenital extremity ulceration. Due to low clinical suspicion, it is easily misdiagnosed, leading to delayed treatment and preventable complications including motor deficits, or in severe cases, amputation. NLCS is sparsely recognized in dermatologic literature, although dermatologists may be the first point of contact as consultants.

Cortical Activity Associated With Phantom Leg Movements.

We tested the feasibility for amputees to control artificial limbs using non-invasive electroencephalography (EEG). Thirteen participants engaged in attempts of isometric ankle plantar-flexions using their phantom or intact limb at slow or ballistic speeds. EEG data were analyzed for movement-related cortical potentials (MRCPs), the slow negative potentials related to the planning and execution of movements. We focused on temporal profiles and single-trial classification at electrode location Cz where MRCPs are most prominent. Distinctly different MRCP morphologies were observed for both movem

A preliminary technique development for quantitative analysis of transhumeral bone, post-amputation, using a computed tomography BMD phantom.

Bone anchored prostheses (BAP) provide an alternative to socket prostheses for many amputees. However, progressive loss of bone mineral density (BMD) after amputation can decrease bone strength and prosthesis support around these implants. Loading of bone following implantation has allowed for successful adoption of lower limb BAPs; however, less is known about BAPs in upper limb amputees. Herein, methods are described to analyze BMD in individuals with transhumeral amputation (THA). Seventeen participants with THA received CT scans with a quantitative computed tomography (QCT) phantom. Howeve
