Mentorship as metadata: clinical annotation and the limits of real-world data in oculoplastic surgery.

In oculoplastic surgery, real-world databases capture diagnosis and procedure codes but rarely the clinical variables that define surgical meaning: laterality, levator function, margin-reflex distance, inflammatory activity, operative intent, or the reason for revision. We propose that clinical supervision be recorded as documented clinical interpretive annotation: an explicit, written layer that defines the limits of routinely collected data before they are analyzed. "Mentorship as metadata" names the educational relationship that produces this annotation and is used throughout as a labelled

Patient-Reported Outcomes Across Treatment Strategies in Papillary Thyroid Microcarcinoma: A Meta-Analysis.

IMPORTANCE: Given the favorable prognosis of papillary thyroid microcarcinoma (PTMC), treatment decisions increasingly rely on patient-reported outcomes (PROs), such as health-related quality of life (HRQOL). Comparative HRQOL data between active surveillance, thermal ablation, and surgery remain limited. OBJECTIVE: To compare HRQOL among adults with PTMC undergoing active surveillance, thermal ablation, or surgery to inform shared decision-making. DATA SOURCES: Embase, MEDLINE via Ovid, Web of Science Core Collection, Cochrane Central Register of Controlled Trials, and the top 100 reference

Device malfunctions and patient harm in robotic-assisted prostatectomy: a 17-year review of the FDA MAUDE database.

Robot-assisted radical prostatectomy (RARP) is the predominant surgical treatment for localized prostate cancer. However, postmarket surveillance of device-related adverse events is critical, as robotic systems entered the market through pathways requiring less extensive premarket clinical testing. This study analyzes device malfunctions and patient harm associated with RARP reported to the FDA’s MAUDE database over a 17-year period. We conducted a retrospective analysis of the MAUDE database from 2009 to March 2026. All reports related to prostatectomy using the Da Vinci surgical system were

Combination of gelatin sponge and tranexamic acid for hemostasis after tooth extraction in anticoagulated patients: a scoping review.

Post-extraction bleeding management in patients receiving anticoagulant therapy remains a clinical challenge requiring a balance between hemorrhagic and thromboembolic risks. Gelatin sponge and tranexamic acid (TXA) are widely used local hemostatic agents, but their combined use remains inconsistently reported and poorly standardized. This scoping review aimed to map existing evidence, characterize intervention variability, and identify research gaps regarding their combined use. This scoping review followed the Arksey and O'Malley framework and was reported according to PRISMA-ScR. PubMed, Sc

Increased Primary Cesarean Births Among People with Epilepsy: A Population-Based Cohort Study.

OBJECTIVE: Pregnant people with epilepsy are at increased risk of severe maternal morbidity (SMM). Cesarean birth is a known contributor to SMM; however, the factors driving cesarean rates in this population remain poorly understood. We evaluated whether primary cesarean births were increased among nulliparous, term, singleton, vertex (NTSV) live births in people with epilepsy and whether differences were explained by sociodemographic, clinical, or hospital factors. MATERIALS AND METHODS: This cohort study used linked data from NTSV live births in California from 2007 to 2018. International C

Letter to the editor regarding "association between sarcopenia on residual back pain after percutaneous kyphoplasty for osteoporotic vertebral compression fractures".

We commend Chengming Li et al. for examining the link between sarcopenia and residual back pain after PKP. However, their retrospective single‑center study (N = 231) has notable limitations. Sarcopenia was diagnosed solely via SMI at T12 on CT, without functional measures (e.g., grip strength, gait speed), and T12 is not a standard spinal muscle evaluation level, risking diagnostic bias. Pain was assessed only by VAS, lacking detail on characteristics or frequency, hindering mechanistic insight. Causal inference is weak due to unaddressed reverse causality-pre‑operative pain may cause disuse a
