BACKGROUND: Mixed venous oxygen saturation (SvO2), measured with right heart catheterization, is a crucial prognostic tool in patients with heart failure. The prognostic significance of SvO2 estimated noninvasively using cardiovascular magnetic resonance (CMR) from the T2 of intracardiac blood pools remains unknown.
OBJECTIVES: The objective of the study was to develop a CMR model of mixed venous saturation (imaging-derived SvO2 [iSvO2]), and establish if it is associated with future adverse events in heart failure.
METHODS: The iSvO2 was modeled in the discovery cohort (N = 30), who underwe

Estimating the causal treatment effects by subgroups is important in observational studies when the treatment effect heterogeneity is present. Existing propensity score methods rely on a correctly specified propensity score model. Model misspecification results in biased treatment effect estimation and covariate imbalance. We proposed a method for the propensity score analysis with controlled subgroup balance (G-SBPS) to achieve covariate mean balance in all subgroups. We further incorporated nonparametric kernel regression for the propensity scores and developed a kernelized G-SBPS (kG-SBPS)

AIMS: Post-capillary pulmonary hypertension (pcPH) is a frequent complication of heart failure (HF), associated with poor outcomes. While right heart catheterization (RHC) is the diagnostic gold standard, echocardiographic indices such as left atrial volume index (LAVI) and the TAPSE/PASP ratio may offer non-invasive prognostic value.
OBJECTIVES: To assess the prognostic utility of LAVI and TAPSE/PASP compared with invasive haemodynamic parameters in patients with HF and pcPH undergoing RHC.
METHODS AND RESULTS: The PH-HF study is a prospective multicentre cohort of adults with chronic HF an

Traditional echocardiographic metrics of right ventricular (RV) function, including tricuspid annular plane systolic excursion and two-dimensional (2-D) strain, are limited to the description of longitudinal systolic function. These metrics, however, fail to account for the complex, three-dimensional (3-D) deformation of the RV. 3-D echocardiograms (3DE) were obtained simultaneously during clinically indicated right heart catheterization (RHC). We determined the maximum principal surface strain (PSMax) and angle (ϴMax) of RV surface deformation in pulmonary arterial hypertension (PAH) and cont

Transcatheter heart interventions are expanding, and structural procedures are becoming more complex. This makes detailed visualisation and characterisation of cardiac anatomy and pathology increasingly important. As a result, there is a growing interest in interventional imaging for procedural guidance. Specifically, there is an increasing interest in using intracardiac echocardiography (ICE) as a complementary or alternative tool to transoesophageal echocardiography. Furthermore, new-generation three-dimensional matrix array ICE probes provide the possibility of obtaining multiplanar reconst

BACKGROUND: International guidelines recommend percutaneous coronary intervention within 120 min for high-risk acute coronary syndrome. In Indonesia, a sprawling archipelago with a rising cardiovascular burden, the ministry of health is expanding catheterisation laboratory (cath lab) infrastructure. This study aims to evaluate the current distribution, population coverage and 'effective access' to cath labs to inform equitable infrastructure planning.
METHODS: We conducted a cross-sectional geospatial analysis using a primary hospital survey (January-June 2024) to identify 335 functional cath

Guidelines recommend fasting before catheterization laboratory (cath lab) procedures. However, recent studies question the necessity of fasting, suggesting that fasting may not provide significant clinical benefits. This study aims to evaluate the existing evidence between outcomes for patients with fasting and nonfasting regimens. We conducted a comprehensive search for randomized controlled trials comparing fasting and nonfasting protocols before cath lab procedures. Outcomes included hypoglycemia, aspiration pneumonia, contrast nephropathy, all-cause mortality, and cardiovascular mortality.

AIMS: Concomitant pulsed field ablation (PFA) for atrial fibrillation (AF) with left atrial appendage closure (LAAC) offers a single-procedure approach for arrhythmia control and thromboembolic risk reduction. This study assessed the workflow, safety, and feasibility of combined PFA and LAAC in routine practice.
METHODS AND RESULTS: We prospectively analysed patients undergoing zero-fluoroscopy PFA, with low fluoroscopy for LAAC. Pre-procedural planning used CT imaging and AI-based models for device selection and landing-zone assessment. A single transseptal puncture facilitated intracardiac

PURPOSE OF REVIEW: Right ventricular (RV) function is a key determinant in selecting and managing patients undergoing transcatheter tricuspid valve replacement (TTVR) or transcatheter tricuspid edge-to-edge repair (T-TEER). This review aims to highlight the role of multimodality imaging in assessing RV function, with a focus on its impact on patient selection, procedural planning, and prognostic evaluation. RECENT FINDINGS: Advances in echocardiography, cardiac magnetic resonance imaging, cardiac computed tomography, and right heart catheterization have improved the ability to evaluate RV morp

BACKGROUND: Although cancer and atherosclerosis are based on shared risk factors, the impact of the acute coronary syndrome (ACS) on long-term survival in cancer patients is still unclear. METHODS: We retrospectively analyzed 441 patients with initial diagnosis of cancer prior to ACS who were subjected to cardiac catheterization (CC) with a follow-up of at median 4.78 years (IQR: 0.58–9.33). These patients were compared to non-cancer ACS patients via propensity score matching according to the cardiac risk factors age, sex, arterial hypertension and diabetes. RESULTS: We found ACS, and overall

INTRODUCTION: Most multipolar mapping catheters use a proximal magnetic sensor, risking distal positional error and slower point acquisition. We tested whether adding distal sensors improves speed and geometric fidelity vs. a conventional design.
METHODS: In a preclinical swine model, maps were acquired on EnSite X. For each region-RA, RV, LA, LV, coronary sinus (CS), and epicardium (Epi)-two maps were created in sequence (HD Grid X first, then HD Grid) and mapping time was recorded. To evaluate geometric accuracy, we sampled 12 predefined sites per chamber, arranged as 3 longitudinal levels

Guide catheter extensions (GCE) represent a useful tool in percutaneous coronary interventions (PCI), allowing to increase support and successfully deliver material through calcific lesions or tortuous anatomy. We present the first case of fracture of the metallic collar braid of a Guidezilla TM II (Boston Scientific, Natick, MA, USA) GCE, remaining trapped in a severely calcified left anterior descending artery of a patient presenting with ST-segment elevation myocardial infarction. The fragment was retrieved from the coronary artery with snaring technique, and the PCI was competed with conve

INTRODUCTION: Evaluating pulmonary circulation parameters (PCP) with cardiovascular magnetic resonance (CMR) is a relatively new approach with the potential for complex evaluation of the cardio-pulmonary system. Its impact might complement clinical assessment through right heart catheterization (RHC), the gold standard in evaluating pulmonary hypertension (PH) and hemodynamics, and transthoracic echocardiography (TTE). The study aims to examine the correlation between PCP and diastolic and systolic function, as well as PH, in patients with recent-onset dilated cardiomyopathy (RODCM).
