Clinically Recognised Alcohol Use Disorder and Risk of Dementia Subtypes in 1.3 Million Women: A Prospective Cohort Study.

BACKGROUND: Alcohol use disorder (AUD) is associated with increased dementia risk, but its long-term relationship with dementia, variation by age and subtype, and how this risk compares with that associated with alcohol consumption remain poorly characterised. We investigated these questions in a large prospective cohort of UK women. METHODS: We analysed Million Women Study participants recruited aged 50-64 years and followed for more than 20 years through linked health records. Women with hospital-recorded AUD were matched to up to five controls. Cox models estimated associations between tim

Mentorship as metadata: clinical annotation and the limits of real-world data in oculoplastic surgery.

In oculoplastic surgery, real-world databases capture diagnosis and procedure codes but rarely the clinical variables that define surgical meaning: laterality, levator function, margin-reflex distance, inflammatory activity, operative intent, or the reason for revision. We propose that clinical supervision be recorded as documented clinical interpretive annotation: an explicit, written layer that defines the limits of routinely collected data before they are analyzed. "Mentorship as metadata" names the educational relationship that produces this annotation and is used throughout as a labelled

Long-term exposure to ultrafine particles and mortality from neurodegenerative diseases: a population-based cohort study.

BACKGROUND: Transportation and industrial activities emit large quantities of air pollutants, including unregulated ultrafine particles (UFP, <0.1 um), which may adversely affect brain health. METHODS: We analyzed data from 2.1 million adults in the Canadian Census Health and Environment Cohorts (CanCHECs) living in Montreal and Toronto (2001-2019). Vital status and primary cause of death were obtained from the Canadian Vital Statistics- Death database. We assigned high-resolution estimates of UFP number concentrations and UFP size to participants' residential postal codes using three-year mo

A sound approach: scoping review of adult lower urinary tract foreign body recognition and management.

INTRODUCTION: Insertion of foreign bodies into the lower urinary tract by adults is an uncommon but clinically important presentation encountered by urologists and emergency providers across a range of motivations. Retained or migrated objects can lead to serious complications including infection, calculus formation, and urethral trauma, often necessitating careful and complex management. METHODS: We conducted a scoping review following the PRISMA extension for Scoping Reviews. Because only published, de-identified case material was used, institutional review board approval was not required.

[Evaluating the performance of generative AI in assisting the differential diagnosis of weight loss].

OBJECTIVES: To systematically evaluate the performance of generative artificial intelligence (GenAI) models, DeepSeek-V3 and the Qwen3 series, in the differential diagnosis of weight loss. METHODS: A search was conducted in the PubMed database for all case reports published in the American Journal of Case Reports between January 1, 2012 and June 2, 2025, containing the term "weight loss" in the title or abstract. Two senior general practitioners independently reviewed each case to determine whether it met predefined diagnostic criteria for weight loss (emaciation). Cases that did not meet the

Validating Physiologically-Based Pharmacokinetic Models Using the Continuous Ranked Probability Score: Beyond Being Correct on Average.

Physiologically-based pharmacokinetic (PBPK) models have become increasingly popular for model-informed drug development (MIDD) over the past decade. While several guidelines for model evaluation exist, these are by design often of a general and non-specific nature. It is clear what steps should be carried out but not necessarily how. For instance, a validation step needs to be performed to check if the predictions of a model indeed match with an external dataset. However, how to quantify this is yet unspecified. In this paper, we propose a more thorough validation approach based on the Contin

Radiomics for early detection of pancreatic cancer: a systematic review and meta-analysis.

BACKGROUND: Pancreatic ductal adenocarcinoma (PDAC) is one of the lethal malignancies, in which accurate and faster detection is required in high-risk population to improve prognosis and decrease cancer-associated mortality. Currently, radiomics has emerged as a promising computational approach to address this challenge, reporting increased accuracy in differentiating PDAC from benign lesions. Our study aimed to evaluate radiomics-based models derived from computed tomography, magnetic resonance imaging, positron emission tomography, or ultrasound for the detection of PDAC in patients under su

Utilization Patterns of Hospital Biobank Samples: Time to First Use and Material-Specific Demand.

Objectives: Biobanks collect biological material prospectively for future, yet unspecified research needs. The relationship between collection strategy and actual utilization remains poorly documented, yet understanding usage patterns is critical for resource allocation and planning. We aimed to analyze the time until first use and the utilization rates stratified by collection type and sample material. Methods: We conducted a retrospective analysis of metadata for all submissions (patient sample set per timepoint) deposited in 2019 (n = 13,666 from 56 collections) at the MedUni Wien Biobank.

Patterns in Curated Celebrity Mortality Data: An Exploratory Analysis of Cardiovascular Disease Representation and Dataset Bias.

BACKGROUND: Publicly reported celebrity deaths receive substantial media attention and may influence how individuals perceive disease burden. However, datasets constructed from publicly available sources are often curated and may not reflect underlying epidemiological distributions. Understanding how dataset construction influences observed mortality patterns is therefore important. OBJECTIVE: To present a purposively curated and openly documented dataset of publicly reported celebrity deaths as a worked methodological illustration of how dataset construction, selection, and reporting practic

BACKGROUND: Hospital morbidity data are used for hospital reimbursement in many countries and are also critical for epidemiological profiling and health planning, both of which depend on data quality. Exploring the clinical coders' perspective is crucial for implementing targeted interventions in this field. OBJECTIVE: To identify the main factors influencing the quality of health registries, the coding process and data quality, from the perspective of clinical coders in Portugal. METHODS: We conducted a nationwide online survey of clinical coders, informed by prior focus groups and a litera

Applying the Moving Epidemic Method to Emergency Department Syndromic Surveillance to identify the Start of Influenza Seasons in New South Wales, Australia.

BACKGROUND: The Moving Epidemic Method (MEM) is a widely used threshold-based approach for identifying the start of an influenza season but has rarely been used with emergency department (ED) syndromic surveillance (EDSyS). This retrospective study aimed to evaluate MEM applied to rates of ED presentation for conditions compatible with influenza to calculate thresholds for identifying the start of the influenza season (pre-epidemic thresholds) in New South Wales, Australia, 2012 through 2019. METHODS: MEM was tested on weekly rates of ED presentations for three Syndromic Groups based on combi

KRAS Mutation and Venous Thromboembolism Risk in Colorectal Cancer: A Danish Population-Based Cohort Study.

INTRODUCTION: Kirsten rat sarcoma viral proto-oncogene (KRAS) mutations may promote a prothrombotic state through tumor-related activation of coagulation pathways. However, evidence for an association with venous thromboembolism (VTE) in colorectal cancer (CRC) patients remains inconclusive. We examined the association between KRAS mutation status and VTE risk. METHODS: We conducted a population-based cohort study using Danish nationwide registries. We included patients diagnosed with stage I-IV CRC who underwent somatic KRAS mutation testing between 2015 and 2023. We computed 1-year risks of

Association of radiotherapy with survival across metastatic patterns in metastatic small cell lung cancer in the immunotherapy era: a SEER analysis.

BACKGROUND: Radiotherapy (RT) combined with chemoimmunotherapy may have synergistic effects and improve treatment efficacy and survival in extensive-stage small cell lung cancer (ES-SCLC). However, its association with survival appears to be inconsistent across different metastatic patterns. METHODS: Patients with metastatic SCLC diagnosed between 2020 and 2023 were identified from the Surveillance, Epidemiology, and End Results database. Propensity score matching (PSM), Kaplan-Meier analysis, and Cox regression were used to evaluate the association between RT and cancer-specific survival (CS

Long-term risk of incident dementia following documented COVID-19 among older adults with type 2 diabetes: a propensity score-matched cohort study.

BACKGROUND: Type 2 diabetes mellitus (T2DM) is a well-established risk factor for dementia, and COVID-19 has been associated with persistent neurological and cognitive sequelae. However, whether this association extends beyond the early post-infection period among older adults with T2DM-a population with compounded vascular and metabolic vulnerability-remains unclear. METHODS: Using the TriNetX Global Collaborative Network, we conducted a retrospective cohort study of patients aged ≥65 years with T2DM, comparing those with documented COVID-19 to a matched unexposed cohort (1:1 propensity scor
