Treatment of Disturbed Sleep in Progressive Supranuclear Palsy: a randomized, remote, double-blinded, 6-week cross-over design study protocol comparing zolpidem, suvorexant, and placebo.

BACKGROUND: Prior research identified profound sleep disruption in progressive supranuclear palsy (PSP). The hypothalamus and brainstem, areas that help regulate sleep/wake patterns, are among the earliest affected brain regions in PSP disease progression. Comparing polysomnography and quantitative-neuropathology metrics, we identified relative sparing of wake-promoting nuclei in PSP compared to Alzheimer's disease, though PSP had more disrupted sleep. It led to the hypothesis that PSP patients have hyperinsomnia (or hyposomnia, little sleep) due to degeneration of sleep nuclei with a preserva

Incidence, Predictors and Prognosis of Pacemaker Implantation after Transcatheter Aortic Valve Implantation versus Surgical Aortic Valve Replacement: A Prospective Cohort Study.

BACKGROUND: Cardiac conduction disturbances remain a frequent complication after aortic valve replacement, particularly following transcatheter aortic valve implantation. These disturbances may necessitate permanent pacemaker implantation and can influence post procedural outcomes, yet comparative prospective data integrating anatomical predictors remain limited. AIM: To compare the incidence and prognosis of cardiac conduction disturbances and permanent pacemaker implantation after transcatheter versus surgical aortic valve replacement, and to identify independent clinical and radiological p

Intravascular Lithotripsy Is Associated With Superior Clinical Outcomes Compared to Atherectomy: A Large-Scale, Propensity-Matched Analysis.

BACKGROUND: Coronary artery disease (CAD) with severe calcification remains a growing challenge in percutaneous coronary intervention (PCI). Atherectomy (ATH) has long been used for plaque modification, whereas intravascular lithotripsy (IVL) has emerged as a promising alternative. Large-scale comparative data on long-term outcomes are limited. AIMS: To compare 1-year clinical outcomes, 30-day procedural complications, and temporal trends between IVL and ATH use. METHODS: We conducted a retrospective cohort study using the TriNetX Research Network, analyzing de-identified data from 47 health

Do Arterial Allografts Outperform Venous in Cryopreserved Bypass for Limb Salvage? A Systematic Review and Meta-Analysis of Infrainguinal Revascularization Outcomes.

BACKGROUND: Cryopreserved allografts remain a viable option for infrainguinal revascularization in limb salvage when autologous veins are unavailable or prosthetic material is undesirable. Uncertainty persists regarding comparative outcomes by allograft type (arterial vs venous) and clinical indication (infectious vs noninfectious settings). This systematic review and meta-analysis summarizes outcomes and key evidence gaps. METHODS: A systematic search of studies reporting infrainguinal reconstructions with cryopreserved arterial or venous allografts was conducted according to Preferred Repor

Acute Stent Thrombosis During Glycoprotein IIb/IIIa Inhibitor Infusion.

Acute stent thrombosis (ST) represents an infrequent but critical complication occurring within 24 h post-percutaneous coronary intervention (PCI). It is most associated with inadequate antiplatelet therapy, stent underexpansion, or malapposition. We report the case of a patient with acute coronary syndrome treated with PCI under optical coherence tomography guidance, who developed recurrent chest pain and ST-segment elevation 7 h after the initial procedure. Urgent coronary angiography revealed acute ST within the newly implanted stent, despite intravascular imaging-guided PCI and optimal ant

Mechanical Versus Bioprosthetic Conduits for the Bentall Procedure: A 10-Year Propensity Score-matched Analysis of Early and Long-Term Outcomes.

OBJECTIVES: The optimal conduit choice for the Bentall procedure-mechanical (MC) or biological (BC)-is confounded by significant baseline selection bias. We aimed to evaluate the impact of conduit choice on early and long-term outcomes after mitigating this bias in a contemporary surgical cohort. METHODS: We retrospectively reviewed 442 consecutive patients undergoing elective Bentall procedures between 2015 and 2025 (365 MC vs 77 BC). Propensity score matching (1:1 nearest-neighbour, calliper 0.1) was performed based on 10 baseline covariates (including age, EuroSCORE II, and comorbidities)

Diagnosis of postoperative atrial fibrillation using smartwatch monitoring after cardiac surgery: protocol for the FAWATCH single-centre, open-label, randomised controlled trial.

INTRODUCTION: Postoperative atrial fibrillation (POAF) remains the most frequent arrhythmic complication after cardiac surgery, with most episodes occurring after discharge from the intensive care unit (ICU) when continuous rhythm monitoring is no longer routinely available. Early detection of POAF is clinically relevant and influences long-term cardiovascular outcomes. Wearable devices combining photoplethysmography technology with single-lead ECG capability offer a potential solution for continuous rhythm monitoring in conventional surgical wards. However, their effectiveness and feasibility

Activated partial thromboplastin time is a potential risk factor for prolonged chest drain placement following surgery for primary spontaneous pneumothorax: a case-control study.

OBJECTIVES: Primary spontaneous pneumothorax (PSP) is a condition that primarily affects young patients and has a high recurrence rate. While surgery is the treatment option associated with the lowest recurrence rate for PSP, some patients experience long-term chest drain placement due to prolonged air leak. Our study aimed to elucidate the relationship between coagulation abnormalities and prolonged postoperative air leak in PSP. METHODS: Patients who underwent surgery for PSP were retrospectively reviewed. Patients were divided into the exploratory and the validation cohorts. From the explo

Long Term Target Vessel Outcomes in Patients Treated with Fenestrated Endovascular Aortic Repair.

OBJECTIVE: Fenestrated endovascular aortic repair (FEVAR) has been proposed as an alternative to open surgical repair (OSR) in complex aortic cases. Target vessel outcomes have shown good results, with re-interventions constituting an important FEVAR pitfall. This single centre, retrospective, observational cohort study presented the target vessel outcomes in patients treated with FEVAR in a high volume aortic centre. METHODS: Consecutive patients managed with custom made FEVAR from 2010 to 2024 were included. Vessels targeted through scallops or intentionally unstented fenestrations were exc

Reflux and stenosis after minimally invasive proximal gastrectomy with double-flap technique: risk factors and impact of robotic surgery.

BACKGROUND: Proximal gastrectomy (PG) with double-flap technique (DFT) is increasingly performed in East Asia as a function-preserving alternative to total gastrectomy. However, reflux esophagitis and anastomotic stenosis remain major concerns, and risk factors after minimally invasive approaches, including robotic surgery, have not been well defined. METHODS: We retrospectively analyzed 399 patients who underwent laparoscopic or robotic PG-DFT for esophagogastric junction (EGJ) or gastric cancers (GC) between 2009 and 2023. The primary endpoint was reflux esophagitis assessed by endoscopy at
