Agentic Automation of BT-RADS Scoring Using an End-to-End System for Standardized Glioma Follow-up Assessment.

The Brain Tumor Reporting and Data System (BT-RADS) standardizes posttreatment MRI response assessment in patients with diffuse gliomas but requires complex integration of imaging trends, medication effects, and radiation timing. An end-to-end multiagent large language model and convolutional neural network system for automated BTRADS classification was developed and evaluated. Five hundred nine consecutive posttreatment glioma MRI examinations (March 2017-July 2021) from a single center were retrospectively evaluated. An extractor agent identified clinical variables from notes using gpt-oss (

Clinical evaluation of deep learning-based acquisition and reconstruction techniques for accelerated 2D shoulder magnetic resonance imaging acceleration at 3 T.

PURPOSE: GE has recently developed Sonic DL MRI acceleration technology. We wished to (1) quantify scanner time reduction compared to the AIR Recon DL already in clinical use at our institution and (2) preliminarily evaluate any change in diagnostic findings between these methods. METHODS: A total of 30 outpatients undergoing routine shoulder MRI for shoulder pain were recruited. Each underwent routine AIR Recon DL and Sonic DL exams containing the same sequences. Three subspecialized musculoskeletal radiologists assessed image quality, graded pathology, and reported diagnostic confidence in

Clinical CT Data Infrastructure Provides Partial Support for an Operational Ontology for Radiology.

Medical imaging data are increasingly used beyond patient care to support research, quality assessment, and artificial intelligence development. These applications require imaging examinations to be represented in ways that enable consistent aggregation across clinical operations to ensure reliable interpretation. In prior work, we introduced an ontology framework, the Operational Ontology for Radiology (OOR), designed to represent the radiology imaging workflow for computed tomography (CT) examinations through entities and relationships. This investigation evaluates the extent to which existi

Case Report: Klebsiella pneumoniae-associated pyogenic spondylitis and occult intrahepatic cholangiocarcinoma during calculous cholangitis.

BACKGROUND: Klebsiella pneumoniae is a common Gram-negative pathogen in biliary tract infections and may cause deep-seated infection following bacteremia, although vertebral involvement is uncommon. Severe biliary infection and stone disease may also complicate the recognition of coexisting hepatic malignancy. CASE PRESENTATION: A 51-year-old woman presented with calculous acute obstructive suppurative cholangitis complicated by septic shock and multiorgan dysfunction. Blood and bile cultures yielded Klebsiella pneumoniae, and her condition improved after urgent biliary drainage and antimicro

Denosumab Versus Zoledronic Acid for Muscle Strength and Physical Performance in Postmenopausal Osteoporosis: A 3-Year Prospective Study.

BACKGROUND: Osteoporosis and sarcopenia frequently coexist in older adults, yet whether denosumab provides muscle-related benefits beyond bone remains uncertain. We compared denosumab and zoledronic acid across functional, imaging, biologic and study-defined sarcopenia outcomes based on an Asian Working Group for Sarcopenia (AWGS) 2025-informed framework and examined whether early osteoprotegerin (OPG)/soluble receptor activator of nuclear factor-kappa B ligand (sRANKL) responses tracked with later benefit. METHODS: This prospective observational cohort study included 186 postmenopausal women

Invasive fungal diseases in immunocompetent adults: emerging patterns and clinical challenges.

BACKGROUND: Invasive fungal diseases (IFDs) have traditionally been regarded as complications of classical immunosuppression. However, IFDs are increasingly recognized in adults who are apparently immunocompetent or non-neutropenic, particularly in the setting of severe viral pneumonia, critical illness, diabetes, structural lung disease, surgery, trauma or burns, injection or inhalational drug use, invasive devices, broad-spectrum antibiotics, and previously unrecognized immune dysregulation. OBJECTIVE: This narrative review summarizes emerging patterns and clinical challenges of IFD in appa

Pancreatic hamartoma presenting with sleep-related behavioral abnormalities due to neuroglycopenia: a case report.

Pancreatic hamartoma (PH) is an exceedingly rare benign non-neoplastic lesion of the pancreas. Owing to its nonspecific clinical and radiological features, it is prone to misdiagnosis. Among its various subtypes, functionally active pancreatic hamartomas with distinct endocrine activity have only been sporadically reported in the literature as individual cases. We report the case of a 58-year-old female patient whose initial presentation was characterized by paroxysmal behavioral abnormalities during nocturnal sleep, manifesting as sudden agitation, arm-flinging, and purposeless stereotyped mo

Contrast-Enhanced NeuroPOCUS in Neurocritical Care: Overcoming Technical Barriers to Bedside Transcranial Ultrasound.

Neuro-point-of-care ultrasound (NeuroPOCUS) is an emerging bedside modality that extends the neurological examination through real-time assessment of cerebral structure and hemodynamics in neurocritical care. Despite increasing integration into clinical pathways and endorsement of transcranial ultrasound-based neuromonitoring by international guidelines, implementation is often hampered by inadequate acoustic windows and poor image and Doppler signal quality, frequently leading to nondiagnostic examinations. Ultrasound contrast agents (UCAs), traditionally used in specialist neurosonology, may

Minute Pulmonary Meningothelial-Like Nodules in Pediatric Patients.

BACKGROUND: Minute pulmonary meningothelial-like nodules (MPMN) are benign lung lesions-called "incidental" but of unknown etiology and significance. Usually identified during histologic examination for other diseases, they have to date been identified virtually only in adults, where they are more common in females and overweight/obese individuals. We aimed to confirm MPMN's existence in pediatric patients and gain insight into their early stages. METHODS: Archival reports were examined for patients <20 with MPMN. Record review and histologic examination were conducted. RESULTS: We identifie

Gray-Scale vs. Color-Doppler Ultrasound in Pediatric Fatty Liver: A Diagnostic Disparity.

INTRODUCTION: Fatty liver disease is a growing cause of chronic liver disorders in children, largely attributed to the increasing prevalence of obesity. Accurate, non-invasive diagnosis is important to prevent progression to fibrosis or cirrhosis. While liver biopsy is the gold standard, ultrasonography remains the most widely used diagnostic tool. This study compared the diagnostic efficacy of Gray-scale and color-Doppler ultrasound, alongside biochemical markers, in overweight and obese children. METHODS: In this cross-sectional study, 67 children aged 2-15 years with body mass index (BMI)

Neurogenic thoracic outlet syndrome: a review of diagnostic and therapeutic strategies.

The diagnosis and treatment of neurogenic thoracic outlet syndrome (nTOS) remain challenging. Although consensus-based clinical frameworks exist, they lack robust externa validation. This study conducts a review of nTOS publications in the PubMed, Embase, and Web of Science databases, with a search cutoff date of December 2025. Current evidence suggests that the incidence of nTOS may be underestimated. Diagnosis should be based on a comprehensive evaluation, including medical history, systematic physical examination, ancillary examinations, diagnostic muscle block, and exclusion of other possi

Relative Brain Perfusion Differences and Cognitive Improvement During Multimodal Management in a Patient With Progressive Cognitive Symptoms: A Case Report.

Progressive cognitive symptoms in older adults may reflect multiple interacting neurologic, vascular, sleep-related, medical, and lifestyle factors, making both etiologic attribution and assessment of treatment response challenging. We report the case of a 72-year-old man with approximately five years of progressive cognitive symptoms, including short-term memory impairment, slowed processing, distractibility, reduced initiative, and executive difficulties, in the setting of a remote cardiac arrest, cardiovascular and cerebrovascular risk, subsequent head trauma, and suspected sleep-disordered
