CAR-T cell-induced cytokine release syndrome is rapidly alleviated by Tripterygium glycosides: a case series and laboratory examination study.

BACKGROUND: Cytokine release syndrome (CRS) is a life-threatening complication of chimeric antigen receptor T cell (CAR-T) therapy. Macrophages/monocytes are mediators of CRS. Tripterygium glycosides is an immunomodulator, which could inhibit macrophages/monocytes in animal models. METHODS: Two patients with relapsed and refractory hematological malignancies developed CRS after receiving CAR-T therapy. They received short-term Tripterygium glycosides orally. RESULTS: Both patients showed rapid mitigation of fever with evidently decrease in elevated inflammatory cytokines within 72 hours. The

Generalized Megaesophagus Concurrent With Toxocariasis in a Cat.

Feline megaesophagus is a rare disorder characterized by esophageal hypomotility and dilation, resulting in regurgitation, weight loss, and occasionally death. This report describes the clinical and paraclinical findings of generalized megaesophagus with a possible contribution of toxocariasis in a kitten. A 2-month-old female cat was referred with chief complaints of lethargy, hyporexia, dyspnea, and vomiting/regurgitation. Physical examination of the kitten revealed bradycardia, bradypnea, hypothermia, dehydration, and lung crackles. The most notable laboratory results were leukopenia with s

Comparative Analysis of Dental Emergency Visits and Dental Readiness Classifications on a Major U.S. Army Installation.

INTRODUCTION: Optimal oral health is essential to service members' well-being, deployability, and operational performance. Dental emergencies (DEs) result in lost duty time and degrade combat effectiveness, particularly during large-scale combat operations (LSCO), when evacuation and definitive dental care may be limited. Dental Readiness Classifications (DRC) are used to determine service member deployability; however, their effectiveness in predicting and preventing DEs has not been well-characterized using contemporary data. This study evaluated the nature of DEs and its relationship with D

Improving reliability of permanent tooth staging for dental age estimation using the crown-root ratio method.

OBJECTIVE: Root staging for dental age estimation is subjective when no standard reference is available. This study developed a quantitative approach for Moorrees root stages (R1/4, R1/2, R3/4 and Rc) using crown-root ratio (CRR) measurements and stage-specific bounds. MATERIALS AND METHODS: The development phase included 269 Malaysian subjects (145 males, 124 females) aged 16-25 years. A total of 1,502 mature permanent mandibular left teeth (I1-M3) were measured on panoramic radiographs using ImageJ software to determine mean CRR values for mature roots (Rc), the fraction stages and stage-sp

Mandibular condylar cortication and third molar apical closure on cone beam computed tomography for the assessment of legal adulthood: a pilot retrospective study.

INTRODUCTION: Accurate estimation of legal adulthood in undocumented individuals remains a major challenge in forensic and medico-legal contexts. This study evaluates mandibular condylar cortication and third molar apical closure assessed by cone beam computed tomography (CBCT) as potential indicators of age of majority. A retrospective observational pilot study was conducted on 142 individuals aged 13-25 years from six geographic regions (Spain, Uruguay, Peru, Mexico, Venezuela and Colombia). Two radiological variables were analyzed: the presence of cortical bone in the mandibular condyle and

ECG parameters to detect cardiac involvement in Fabry disease.

BACKGROUND: Cardiac involvement of the disease is a leading cause of death and disability in Fabry disease characterized by pathological accumulation of globotriaosylceramide (Gb3) and lyso-globotriaosylceramide (lyso-Gb3) in multiple organs. In this study, we sought to investigate the electrocardiographic (ECG) changes in different clinical stages and evaluate the value of these parameters in assessing cardiac involvement. METHODS: 62 patients with Fabry disease and 45 healthy controls were recruited in this study. ECG assessment, echocardiographic assessment and cardiac magnetic resonance (

Slice-Wise Quality Assessment of High b Value Breast DWI via Deep Learning-Based Artifact Detection.

BACKGROUND: Diffusion-weighted imaging (DWI) can support lesion detection and characterization in breast MRI. Therefore, DWI is increasingly being incorporated into breast MRI protocols to address some of the shortcomings of routine clinical breast MRI. However, especially high b value DWI can be prone to intensity artifacts that can affect diagnostic image assessment. PURPOSE: To detect hyper- and hypointense artifacts on high b value DWI (b = 1500 s/mm2) using deep learning, employing either a binary classification (artifact presence) or a multiclass classification (artifact intensity) appr

False-positive cervical lymph nodes on FAPI PET/CT after neoadjuvant chemotherapy for breast cancer: a case report.

A 49-year-old woman with cT3N1M0 stage IIIA HER2-positive invasive breast cancer is described in this retrospective case report. After six cycles of TCHP neoadjuvant chemotherapy, breast MRI confirmed that the primary lesion and axillary lymph nodes achieved radiological complete response. However, newly enlarged lymph nodes in levels II and III of the left neck were detected by neck ultrasound and contrast-enhanced CT. Subsequent 18F-FAPI-04 PET/CT revealed significantly elevated tracer uptake in these cervical lymph nodes, with a maximum standardized uptake value (SUVmax) of 18.2, which was

Early-stage HER2-positive breast cancer in neurofibromatosis type 1: a case report with literature review.

Neurofibromatosis type 1 (NF1) is an autosomal dominant disorder associated with an increased risk of malignancies, particularly breast cancer at a younger age. We report the case of a 48-year-old woman with clinically diagnosed NF1 who presented with a left breast mass detected during routine screening. Physical examination revealed numerous café-au-lait macules and cutaneous neurofibromas distributed over the trunk and extremities. Imaging findings were classified as BI-RADS 4B-4C. Following intraoperative confirmation of malignancy, total mastectomy and sentinel lymph node biopsy were perfo

Incidental Appendiceal Adenocarcinoma Discovered After Ileocecectomy for Recurrent Small Bowel Obstruction: A Case Report.

We report a case of moderately differentiated appendiceal adenocarcinoma discovered on histopathologic examination following ileocecectomy for recurrent SBO in a woman in her early 50s with a history of stage III rectal adenocarcinoma in remission. Notably, serial surveillance imaging over two years had demonstrated a radiographically normal appendix prior to the obstructive presentation. Immunohistochemical and histopathologic features confirmed a metachronous primary malignancy rather than metastatic recurrence. This case exemplifies the diagnostic challenge posed by occult appendiceal neopl

The risk of not imaging: a three-scenario framework for justification.

A recurring argument in radiological protection holds that the radiation risk of an imaging examination must be weighed against the clinical risk of not performing it. The argument is applied inconsistently, and evidence on that clinical risk remains sparse compared with the extensive literature on radiation risk. This asymmetry masks an important distinction: the clinical risk of not imaging is not uniformly high. We propose a three-scenario framework for medical imaging, illustrated using computed tomography. In Scenario 1, the harm of withholding or delaying imaging is well documented and s

Giant Cell Tumor of the Lateral Malleolus Treated by Tumor Excision and Reconstruction using A 180° Rotated Proximal Fibular Autograft - A Rare Case Report.

INTRODUCTION: Giant cell tumor (GCT) of bone was first described by Sir Astley Cooper in 1818. It is one of the most frequently encountered benign bone tumors in orthopedic practice and is characterized by a significant risk of local recurrence following surgical treatment. GCT involving the distal fibula, particularly the lateral malleolus, is extremely rare, accounting for <1% of all cases. Various treatment modalities have been described for lateral malleolus GCT, including tumor excision, ankle arthrodesis, reconstruction with ipsilateral proximal fibular graft, and autograft or allograft

Proton-density fat-saturated non-contrast MR imaging is comparable to post-contrast MR imaging for revealing wrist synovitis.

OBJECTIVES: Standard MR imaging (MRI) for rheumatoid arthritis (RA) patients comprises a post-contrast T1-fat-saturated (T1FS + C) sequence to detect wrist synovitis and tenosynovitis. This study investigates whether non-contrast sequences can provide similar information to post-contrast imaging. MATERIALS AND METHODS: Seventy-seven RA patients (F/M:  64/13; age: 61 ± 12 years) underwent MRI examination of the most symptomatic wrist on a 3.0-T system. Axial T1FS + C as well as proton-density imaging with fat-saturation (PDFS) and T2-weighted imaging with fat-saturation (T2FS) sequences were p
