Parvimonas micra and central nervous system infections: mechanisms of transformation from oral commensal to intracranial pathogen and clinical evidence.

Parvimonas micra (P. micra), an anaerobic oral coccus long dismissed as a culture contaminant, is increasingly recognized as a causative agent of pyogenic central nervous system (CNS) infections, yet its neuropathogenesis remains poorly defined. This narrative review synthesizes 41 peer-reviewed studies identified via a structured comprehensive literature search of PubMed, Embase and Web of Science through June 2026. Epidemiologically, pooling available case reports and small series (n=64 confirmed CNS cases), P. micra is among the most frequently isolated Gram-positive anaerobic cocci in repo

Clinical and metagenomic characterization of HHV-6 versus HHV-7 encephalitis in immunocompetent children.

BACKGROUND: Human herpesvirus 6 (HHV-6) and human herpesvirus 7 (HHV-7) are betaherpesviruses that can cause encephalitis/encephalopathy in immunocompetent children. However, early clinical manifestations, routine laboratory tests, and neuroimaging findings often lack specificity, making etiological characterization challenging. This study aimed to characterize the clinical features and metagenomic findings of HHV-6 and HHV-7 encephalitis in immunocompetent children. METHODS: This single-center retrospective cohort study included children (<18 years) with suspected central nervous system infe

Evidence of autoimmune etiology in children with small fiber neuropathy.

OBJECTIVE: Small fiber neuropathy (SFN) is becoming increasingly recognized as a cause of autonomic dysfunction, sensory disturbances, and neuropathic pain. While SFN was previously considered predominantly idiopathic, cases of autoimmune SFN are now well described in adults but remain underexplored in children. METHODS: In this retrospective chart review, we identified pediatric patients diagnosed with SFN of suspected autoimmune etiology. SFN was diagnosed based on epidermal nerve fiber density (ENFD) and/or sweat gland nerve fiber density (SGNFD) values from skin biopsy and/or quantitative

Precision Medicine in Epilepsy: Diagnostic Yield by Next-Generation Sequencing and Genotype-Phenotype Findings in a Romanian Cohort.

Epilepsy ranks among the most prevalent neurological disorders globally, with a substantial proportion of cases, particularly in pediatric populations, attributed to genetic etiologies. This study provides the first molecular characterization of suspected genetic epilepsy in a pediatric cohort from the Moldova region of Romania. Following the exclusion of 124 patients with secondary acquired or structural causes from an initial cohort of 159, we enrolled 35 individuals with a high suspicion of primary genetic epilepsy, who underwent targeted gene panel testing or whole-exome sequencing (WES).

Multiple Sclerosis During Pregnancy and the Postpartum Period: Diagnostic and Multidisciplinary Challenges in Two Cases.

BACKGROUND: Pregnancy may complicate the recognition and evaluation of new neurological symptoms suggestive of a central nervous system demyelinating disease. Diagnostic uncertainty may arise when clinical progression, neuroimaging findings, and supportive investigations are incomplete or inconclusive. CASE PRESENTATION: We report two pregnant women with progressive focal neurological symptoms and radiological findings suggestive of a demyelinating disease. Case 1 involved a 27-year-old woman with a history of autoimmune spondyloarthritis who developed dizziness, gait instability, asymmetric

Probable vestibular paroxysmia mimicking benign paroxysmal positional vertigo of the superior semicircular canal in the presence of a neurovascular conflict: a case report and review of the literature.

BACKGROUND: Positional vertigo is a frequent complaint in otology and vestibular physiotherapy, arising from functional or structural causes affecting the peripheral or central vestibular systems. Although benign paroxysmal positional vertigo (BPPV) is the most common form, other etiologies such as vestibular paroxysmia (VP), central positional vertigo, or vertebrobasilar insufficiency should be considered. Recognizing the specific features and differential diagnosis of these conditions is crucial for accurate assessment. CASE PRESENTATION: We describe the case of a 52-year-old French woman p

[Differential diagnosis between Bipolar Disorder and behavioral variant Frontotemporal Dementia: the role of longitudinal neuropsychological assessment. A Case Report.].

INTRODUCTION: Bipolar Disorder (BD) and the behavioral variant of Frontotemporal Dementia (bvFTD) can present overlapping clinical features in their early stages, characterized by disinhibition, euphoria, and executive function deficits. Differential diagnosis is particularly challenging when behavioral symptoms are accompanied by evidence of cortical atrophy on neuroimaging. CASE PRESENTATION: We describe the case of a 48-year-old woman diagnosed with Type I BD since age 25, referred for suspected bvFTD due to a progressive decline in memory functions and sleep disturbances. At the time of e

Association between obstructive sleep apnea and liver fibrosis in patients with suspected nonalcoholic fatty liver disease.

STUDY OBJECTIVES: The association between obstructive sleep apnea (OSA) and liver fibrosis has been primarily evaluated using noninvasive tools or in selected populations (bariatric surgery). The aim of the study was to determine whether OSA is associated with liver fibrosis in patients with suspected nonalcoholic fatty liver disease (NAFLD) referred for liver biopsy (LB). METHODS: Patients who underwent percutaneous LB for suspected NAFLD were prospectively included and investigated by in-lab polysomnography. Liver lesions were evaluated using the NASH-CRN scoring system and advanced fibrosi

Pathomechanistic role of central venous pressure in idiopathic intracranial hypertension.

OBJECTIVE: The presence of dural venous stenosis in idiopathic intracranial hypertension (IIH) is well recognized. However, the complex interplay between intracranial and extracranial factors contributing to pathological intracranial venous congestion remains poorly understood. In this study, the authors explored venous sinus characteristics in cohorts with elevated central venous pressure (eCVP; ≥ 8 mm Hg) and normal CVP (nCVP; < 8 mm Hg). METHODS: The authors retrospectively reviewed medical records for adult patients who underwent invasive venography with manometry for a suspected or confi

Exploring the Role of the Head Rotation Sit-Up Test in Identifying Epley-Responsive, Non-Classical Presentations of BPPV.

Background and Objectives: The Dix-Hallpike test (DHT) is the standard diagnostic maneuver for posterior semicircular canal benign paroxysmal positional vertigo (BPPV). However, some patients present with positional symptoms compatible with BPPV yet show no observable nystagmus on the DHT. We introduced the Head Rotation Sit-up Test (HRST) as a symptom-based maneuver and hypothesized that it would identify DHT-negative patients who nonetheless benefit from canalith repositioning. This study aimed to explore the potential role of the HRST. It was introduced as a complementary, symptom-based man

Tissue oxygenation dynamics during transition from seizure to spreading depolarization in rat brain.

OBJECTIVE: Spreading depolarization (SD) is a phenomenon underlying various neurological conditions, including epilepsy. Researchers have suspected that local tissue oxygenation breakdown induces spontaneous SD. In this study, we investigated the relationship between spontaneous epileptic seizures and SD, with a focus on the role of local tissue oxygenation during the transition from seizure to seizure-associated SD. METHODS: We applied a long pulse voltametric method to characterize local tissue oxygenation and extracellular space (ECS) volume change in the hippocampus (HC) of freely moving
