Proximal Splenorenal Shunt in Non-cirrhotic Portal Hypertension: Our Experience.

Background Non-cirrhotic portal hypertension (NCPH) is characterized by an elevated portal pressure without liver disease and without obvious venous thrombosis. It is characterized by splenomegaly and gastroesophageal varices with intact liver histology. Extrahepatic portal vein obstruction (EHPVO) and non-cirrhotic portal fibrosis (NCPF) come under the umbrella of NCPH. EHPVO is further characterized by portal cavernoma, cholangiopathy, and growth retardation. Surgical shunts remain a definitive option for patients requiring frequent endoscopies. This study was done to evaluate the surgical o

Mucosal Small Cell Neuroendocrine Carcinoma of the Gallbladder With Extraregional Lymph Node Metastasis: A Case Report.

Gallbladder small cell neuroendocrine carcinoma (SCNEC) is a rare and highly aggressive malignancy, and an optimal treatment strategy has not been established. We report a case of mucosal gallbladder SCNEC with extraregional lymph node metastasis that responded well to chemotherapy. A 69-year-old woman presented with severe epigastric pain and inflammatory findings. Ultrasonography, endoscopic ultrasonography, and computed tomography revealed a 40 mm gallbladder mass and enlarged lymph nodes in the hepatoduodenal ligament. Because inflammation and pain could not be controlled by conservative t

An Atypical Presentation of Richter's Transformation in Chronic Lymphocytic Leukemia: Stroke-Like Onset With Multiorgan Involvement Including the Heart and Seminal Vesicles.

Chronic lymphocytic leukemia (CLL) is usually an indolent lymphoproliferative disorder. However, a small proportion of patients may develop Richter's transformation (RT), in which CLL transforms into an aggressive lymphoma. This transformation is often associated with rapid clinical progression and extranodal disease. Here, we describe a 72-year-old man diagnosed with CLL during evaluation for persistent cough and leukocytosis. Peripheral blood smear demonstrated smudge cells, and the diagnosis was confirmed by peripheral blood flow cytometry. As the patient was asymptomatic, he was managed wi

Adenopathy and Extensive Skin Patch Overlying a Plasmacytoma (AESOP) Syndrome Associated With a Solitary Bone Plasmacytoma of the Left Scapula: A Rare Paraneoplastic Presentation Managed With High-Precision Radiotherapy.

Adenopathy and extensive skin patch overlying a plasmacytoma (AESOP) syndrome is a rare paraneoplastic manifestation characterized by three cardinal components: the underlying pathology (plasma cell dyscrasia), the cutaneous lesion (vascular cutaneous lesion), and the skeletal/nodal manifestation (solitary plasmacytoma/regional lymphadenopathy). Scapular involvement is exceptionally uncommon and may represent an early indicator of progression within the polyneuropathy, organomegaly, endocrinopathy, monoclonal plasma cell disorder, and skin changes (POEMS) spectrum. A 47-year-old male with esse

[Livedoid vasculopathy: A case report].

Livedoid vasculopathy (LV) is a painful and thrombotic obliterative vascular disorder characterized by spontaneous thrombus formation within skin capillaries or small veins, ultimately leading to localized hypoxia and the development of skin ulcers. LV is a rare condition predominantly affecting young women, with an incidence rate of approximately 1/100 000. Clinically, it manifests as recurrent reticular green spots and painful ulcers on the distal lower extremities, which subsequently heal, leaving behind white and atrophic scars, termed "white atrophy". The skin manifestations of the diseas

Systolic blood pressure modifies the association between triglyceride-glucose-waist-to-height ratio and cardiovascular disease: a nationwide cohort study.

BACKGROUND AND AIM: Triglyceride-glucose-waist-to-height ratio (TyG-WHtR) index and blood pressure are independent predictors of cardiovascular disease risk. However, the interaction between these factors in Cardiovascular disease (CVD) risk stratification remains underexplored. This cohort study explored the association between TyG-WHtR and CVD risk stratified by systolic blood pressure (SBP) to disentangle their complex interaction effects in middle-aged and older Chinese populations. METHODS AND RESULT: A total of 6542 participants without CVD at baseline were recruited from the China Heal

Long-term stability after non-surgical furcation retreatment with adjunctive collagen hydrogel.

BACKGROUND: Management of inflammation at previously surgically treated periodontal sites represents a clinical challenge. Although surgical retreatment may be indicated in selected cases, a conservative non-surgical approach may restore periodontal stability while avoiding the additional morbidity associated with repeated surgery. This case report describes the long-term management of a previously surgically treated mandibular molar furcation using site-specific non-surgical retreatment combined with adjunctive application of a collagen-based hydrogel. METHODS: A 45-year-old non-smoking male

An examination of chronotype associations with symptoms of depression, PTSD, and sleep disturbances in US military veterans.

PURPOSE: Chronotype is associated with depression and PTSD, which are common mental health conditions among military veterans. Few studies have examined chronotype in this population, which is known to also have more severe sleep difficulties than non-veteran community-residing adults. METHODS: In the current study, self-reported sleep and mental health symptoms were examined by chronotype using data collected from US military veterans (N = 144) participating in a multi-site post-deployment health repository. Analysis of covariance was used to assess symptom measure differences by chronotype

A Cluster-Randomized Clinical Trial of Silver Diamine Fluoride vs Atraumatic Restoration in Older Adults.

Untreated dental caries, a chronic disease linked to systemic health, is common among older adults due to a lack of dental insurance and affordable dental care. Minimally invasive interventions such as silver diamine fluoride (SDF) and atraumatic restorative treatments (ARTs) have been recommended by the World Health Organization to control caries where evidence is lacking in older adults. This cluster-randomized clinical trial tested the noninferiority of SDF vs ART in caries arrest (primary) and prevention (secondary) at 52 wk. Older adults ≥62 y living in 33 subsidized housing facilities we

Randomized, single-centre, double-blinded repeated dose investigation of safety and tolerability of ocular administrations of a novel silica eye drop product in healthy volunteers.

PURPOSE: Managing ocular diseases often requires frequent eye drop administration, which can challenge patient compliance. A long-acting eye drop technology using an amorphous synthetic silica composite was developed to address this. Our study aimed to assess the safety and tolerability of the Silica Eye Drop platform in healthy volunteers over 15 days. METHODS: Twelve healthy volunteers participated in a randomized, double-blinded, placebo-controlled trial of the Silica Eye Drop Product, containing no active substance. Participants received one drop in one eye daily for 13 days, with the oth

Cobblestone-like Gastric Mucosal Changes on Endoscopy in Dogs with a History of Prolonged Proton Pump Inhibitor Therapy.

This study describes the clinicopathological features of seven canine cases showing a diffuse cobblestone-like gastric mucosal pattern on endoscopy. Cases were retrospectively retrieved from endoscopic databases (2017-2025). Clinical data, treatment history, endoscopic findings, and histology were reviewed. Endoscopically, all dogs exhibited thickened, irregular, and poorly distensible gastric folds. Histopathologic examination showed mild-to-moderate foveolar hyperplasia, variable cystic dilation of the fundic glands, mild chronic lymphoplasmacytic inflammation, and interstitial fibrosis. Par

Comparative Evaluation of Oral Health Status in Patients Undergoing Orthodontic Treatment With Clear Aligner Therapy Versus Conventional Fixed Appliance Therapy: An Umbrella Review.

Orthodontic treatment serves an essential function in correcting malocclusion and improving both function and esthetics, but different appliance systems can influence oral health outcomes due to variations in plaque accumulation, gingival response, and enamel demineralization. Clear aligner therapy (CAT) has gained recognition as a substitute for traditional fixed appliance therapy (CFAT), asserting advantages such as improved oral hygiene and fewer periodontal issues. This overview assesses and integrates results from current systematic reviews and meta-analyses that compare the oral health c

Prepubertal Growth Trajectory and Pubertal Onset.

IMPORTANCE: Adiposity has been reported to be a major contributor to earlier pubertal timing, but most pediatric studies have relied on body mass index (BMI) from single or short-term measurements. Studies tracking growth repeatedly from birth to puberty are needed. OBJECTIVE: To explore the association of growth trajectory and cumulative exposure to different levels of adiposity (CEA) with pubertal onset across the first decade of life and identify sensitive periods for possible weight intervention. DESIGN, SETTING, AND PARTICIPANTS: This population-based cohort study included data from 2 b

Effects of a 12-week Tai Chi program on postural stability and gait biomechanics in prefrail older adults: a randomized controlled study.

BACKGROUND: Prefrailty is a reversible transitional stage of frailty and is associated with impaired postural stability and increased fall risk. Tai Chi may improve balance-related performance in older adults, but its effects on postural stability and gait biomechanics in prefrail populations remain insufficiently characterized. METHODS: In this single-blind randomized controlled trial, 68 prefrail older adults defined by the Fried Frailty Phenotype were allocated to a Tai Chi plus health education group or a health education control group. Sixty-four participants completed the study (mean ag
