Alloplastic temporomandibular joint replacement using patient-specific prostheses: a multicentre analysis of functional and aesthetic outcomes.

Total alloplastic temporomandibular joint (TMJ) replacement with patient-specific prostheses is an established option for end-stage TMJ disease, but multicentre data integrating functional, pain, dietary, aesthetic and patient-reported outcomes are scarce. This retrospective multicentre cohort study assessed functional, pain, dietary, aesthetic and satisfaction outcomes after unilateral or bilateral patient-specific TMJ replacement in three tertiary units. Patients with ≥12 months' follow-up completed a postoperative questionnaire including the modified Helkimo index, diet score, numerical rat

Ventral Spinal Cord Displacement After Single-Level Anterior Cervical Corpectomy: A Case Report.

Spinal cord herniation is a rare condition characterized by protrusion of the spinal cord through a dural defect, typically associated with progressive neurological deficits. However, postoperative alterations at the ventral dural-cord interface may present as cord displacement without a demonstrable dural breach. We report the case of a 44-year-old woman who presented with progressive gait imbalance, neck pain, and right upper limb radiculopathy of four weeks' duration. Imaging suggested compressive cervical pathology, and the patient underwent anterior cervical corpectomy and stabilization.

The comparative impact of body mass index on outcomes following minimally invasive versus open surgery for grade 1 spondylolisthesis: 2-year follow-up from the Quality Outcomes Database.

OBJECTIVE: Both minimally invasive (MI) and open surgical approaches can be effective for the treatment of degenerative lumbar spondylolisthesis (DLS). However, comparative effectiveness might be influenced by body mass index (BMI). The authors aimed to determine if there is a differential impact of open versus MI approaches on patient outcomes based on BMI. METHODS: The authors performed a retrospective analysis of the prospective Quality Outcomes Database, identifying patients surgically treated for grade 1 DLS between July 2014 and June 2016. BMI was dichotomized into 2 groups: nonobese (B

Intervention to optimise body mass index in adolescents and address the triple burden of malnutrition-the Ntshembo (Hope) trial in rural and urban South Africa study: a study protocol for a randomised controlled trial.

INTRODUCTION: South Africa faces a complex health burden with burgeoning non-communicable diseases against a background of prevalent infection. The triple burden of malnutrition, comprising undernutrition alongside overweight/obesity and micronutrient deficiencies, is widespread and imposes risks for non-communicable diseases along the life course, especially among adolescent girls. We hypothesise that, by optimising nutrition and body mass index (BMI) of at-risk adolescent girls, we can realise a triple return on investment: improved nutritional status, reduced metabolic risk, and moderated p

AQP4+ NMOSD, MDA5+ Clinically Amyopathic Dermatomyositis and sHLH in a Pediatric Patient: A Case Report and Literature Review.

BACKGROUND: Neuromyelitis optica spectrum disorder (NMOSD) is a rare central nervous system (CNS) autoimmune disorder. It is associated with other autoimmune disorders, which included dermatomyositis in rare conditions. Type I interferon (IFN) has been considered a common link between the two diseases. Furthermore, secondary hemophagocytic lymphohistiocytosis (sHLH) results from overactivation of the immune system and is also associated with autoimmune disorders. This study aims to report a pediatric case of NMOSD overlapping with dermatomyositis, who also developed sHLH, and to summarize all

Does surgical treatment of grade 2 spondylolisthesis have sustained improvement in postoperative outcomes regardless of age? A Spine CORe™ analysis of QOD data.

OBJECTIVE: Surgical intervention for grade 2 lumbar spondylolisthesis is routinely performed, but outcomes in older patients, in whom the disease is most prevalent, remain poorly understood. The aim of this study was to compare patient-reported surgical outcomes between age groups (< 65 years of age vs ≥ 65 years of age) with 5 years of follow-up. The authors hypothesized that patients would have sustained improvement in outcomes in response to surgical treatment for grade 2 spondylolisthesis regardless of age. METHODS: The multicenter prospectively collected Quality Outcomes Database by the

Toward Personalized Withdrawal of TNF-α Inhibitors in Non-Systemic Juvenile Idiopathic Arthritis: Predictors of Biologic-Free Remission and Flare.

Background: Tumor necrosis factor-α (TNFα) inhibitors have significantly improved outcomes in children with non-systemic juvenile idiopathic arthritis (JIA), achieving long-term clinical remission for many patients. However, the optimal strategy for TNF-α inhibitor withdrawal remains unknown, whether through abrupt discontinuation, gradual dose reduction, or interval extension. Objective: We aim to identify patient-, disease-, and treatment-related predictors of successful TNF-α inhibitor withdrawal in children with non-systemic JIA. Methods: In this prospective, randomized, open-label, single

Comparative Short-Term Clinical Outcomes of Hybrid Hyaluronic Acid and Platelet-Rich Plasma Injections in Knee Degenerative Conditions: An Exploratory Real-World Retrospective Study.

Background and Objectives: Knee osteoarthritis (KOA) and other degenerative chondropathies are major causes of pain and disability. When core conservative treatments are insufficient, intra-articular hyaluronic acid (HA) and platelet-rich plasma (PRP) are commonly used as adjunctive options, although evidence remains difficult to interpret because of heterogeneity in patients, products, preparation protocols, and treatment schedules. This exploratory retrospective study described short-term clinical outcomes after two standardized intra-articular protocols, hybrid HA and autologous PRP, in a r

Liver Resection after Disease Control with Abemaciclib for Solitary Liver Metastasis from Breast Cancer: A Case Report.

INTRODUCTION: Liver metastases from breast cancer are associated with a poorer prognosis than bone or soft tissue metastases. In hormone receptor (HR)-positive, human epidermal growth factor receptor 2 (HER2)-negative recurrent or metastatic breast cancer, endocrine therapy combined with cyclin-dependent kinase 4/6 (CDK4/6) inhibitors has been established as the standard of care. Abemaciclib has demonstrated favorable efficacy in phase III clinical trials and is considered an important option for long-term disease control. With recent advances in systemic therapy, an increasing number of patie

How Is Lumbar Fusion Associated With Compensatory Hip Motion After THA?

BACKGROUND: Although patients with THA may increase their use of hip flexion to compensate for loss of spinal motion after lumbar fusion, no studies have directly examined this relationship. Furthermore, the association between the number of fused levels on postoperative compensatory hip motion is not clear, making it difficult to anticipate these changes prior to lumbar fusion. QUESTIONS/PURPOSES: (1) Is lumbar fusion after THA associated with resting spinopelvic alignment or compensatory changes at the hip? (2) Is the number of fused spinal segments associated with an increase in hip compen

Choroidal thickness as a biomarker of inflammatory activity in patients with polymyalgia rheumatica receiving anti-IL-6 treatment.

OBJECTIVE: In corticosteroid-dependent polymyalgia rheumatica (PMR), anti-interleukin-6 (IL-6) therapies are frequently used; however, they may limit the reliability of acute-phase reactants (APRs) as biomarkers of disease activity. Choroidal thickness (CT) may serve as an alternative inflammatory biomarker in these patients. So, the aim of this study is to evaluate changes in CT in patients with PMR initiating anti-IL-6 therapy over 6 months follow-up period, and to assess its correlation and level of agreement with other inflammatory parameters. METHODS: It is a prospective, observational,

Liposuction versus conservative therapy for patients with lipoedema in Germany: a multicentre, randomised controlled clinical trial.

BACKGROUND: Lipoedema is a painful, chronic disease of the subcutaneous adipose tissue, predominantly affecting women. Conservative complex decongestive therapy is the standard of care but offers limited symptom relief. Liposuction appears to be a promising approach but lacks evidence of effectiveness and safety. The aim of this trial was to show superiority of liposuction compared with conservative therapy regarding relevant pain reduction with reasonable safety. METHODS: In this multicentre, randomised controlled trial at 11 German centres, eligible women with stage I, II, or III lipoedema

[Clinical efficacy of anterior cervical mortise-tenon corpectomy and fusion for the treatment of ossification of the posterior longitudinal ligament].

OBJECTIVES: For the segmental ossification of cervical posterior longitudinal ligament (OPLL) accompanied by spinal cord compression, anterior cervical corpectomy and fusion (ACCF) can achieve direct decompression through removal of most of the involved vertebral body. However, reconstruction-related complications such as titanium mesh cage subsidence, loss of cervical lordosis, and poor bone fusion are common after surgery. This study aims to investigate the clinical efficacy of a novel anterior cervical mortise-tenon corpectomy and fusion (ACMTF) technique, designed based on principles of bi

[Three-Dimensional Surgical Correction of Scoliotic Curves Monitored Using the EOS Imaging].

PURPOSE OF THE STUDY: Structural scoliosis is a three-dimensional deformity that adversely affects the frontal, sagittal, and axial planes. Surgical correction of scoliotic deformities in children requires effective correction of all three planes, with restoration of a balanced position of the spine and the entire trunk. Using modern surgical instruments and targeted intraoperative maneuvers, we achieve this comprehensive 3D correction. The EOS Imaging System enables effective preoperative planning and postoperative verification of outcomes based on software processing of the acquired X-ray im

Atraumatic Splenic Rupture: A Case Report.

Atraumatic splenic rupture (ASR) is a rare but potentially fatal emergency, accounting for less than 0.5% of all splenic ruptures. Its nonspecific presentation often complicates diagnosis and management. True idiopathic ASR, occurring without an underlying splenic disease, is extremely rare. We report the case of a 61-year-old woman with diffuse large B-cell lymphoma in complete metabolic remission who developed sudden, severe epigastric and left upper quadrant pain 7 days after receiving high-dose methotrexate for central nervous system prophylaxis. There was no evidence of splenic involvemen

A Rare Case of Fluoxetine-Induced Vortex Keratopathy.

Vortex keratopathy is a distinctive corneal epithelial disorder marked by whorl-shaped deposits, most often linked to prolonged exposure to cationic amphiphilic medications. While classic drug associations are well established, the involvement of selective serotonin reuptake inhibitors remains sparsely documented. This report describes an unusual presentation of vortex keratopathy temporally associated with long-term fluoxetine 40 mg therapy for two years. A 25-year-old woman receiving treatment for depressive disorder presented with gradually progressive bilateral visual disturbance. Comprehe
