INTRODUCTION: Lesions of ampulla of Vater are uncommon and often discovered incidentally. Most adenomatous lesions can be treated by endoscopic papillectomy which can obviate the need for surgery. Data regarding effectiveness for endoscopic papillectomy especially for larger lesions is lacking. We aimed to assess the impact of polyp size on recurrence on subsequent ERCP procedures following initial papillectomy.
METHODS: After approval of the study from local IRB, patients with diagnosis of ampullary lesion who were treated with endoscopic papillectomy at our quaternary care referral center w

BACKGROUND: Stachybotrys chartarum is a damp building mould and a potent toxin producer that has been related to serious cases of respiratory health problems. However, the direct link between exposure and health symptoms has not been established.
OBJECTIVE: To examine the mechanism by which exposure to spores of satratoxin producing and non-producing S. chartarum strains induce inflammatory responses in murine lungs.
METHODS: BALB/c mice were intranasally exposed for 3 weeks to spores of a satratoxin-producing and a non-producing S. chartarum strain. Inflammatory cell infiltration was charac

We report successful en bloc underwater endoscopic mucosal resection (UEMR) of a lesion involving the minor duodenal papilla. A woman in her 60s presented with a 10-mm depressed lesion in the descending duodenum. White-light imaging and Magnifying endoscopy suggested a tumor involving a minor papilla. Although side-viewing endoscopes are conventionally used for the minor papilla, pre-procedural assessment indicated inadequate maneuverability for this specific lesion (ED-580T; Fujifilm Corporation, Tokyo, Japan). Consequently, we strategically used a forward-viewing dual-channel endoscope (GIF-

BACKGROUND: Accurate histologic classification of ampullary lesions is essential for guiding therapeutic decisions; however, conventional biopsy is limited by sampling error and false-negative results. We evaluated the clinical utility of a generative artificial intelligence (AI)-based computer-aided diagnosis (CAD) system that integrates real and synthetic endoscopic images to improve diagnostic performance.
METHODS: In this retrospective multicenter study conducted across seven hospitals, duodenoscopic images were classified as Normal, Adenoma, or Cancer. A generative AI-based CAD system us

Background and Clinical Significance: Ganglioneuromas are rare benign tumors of neural crest origin, with gastrointestinal involvement being uncommon and ampullary localization exceptionally rare. Pancreatic neuroendocrine tumors (NETs) are also uncommon neoplasms with variable biological behavior. The coexistence of these two entities is unusual, particularly in the absence of an identifiable hereditary syndrome. Case Presentation: A 38-year-old man presented with hematemesis and multiple episodes of melena over 18 h and was found to have significant anemia with borderline hemodynamic stabili

BACKGROUND: Pancreaticoduodenectomy (PD) is a key surgical treatment for pancreatic and periampullary malignancies, but morbidity remains a challenge. Lymph node dissection improves outcomes by removing cancerous tissue and preventing metastasis. This study aims to assess survival rates, recurrence, and other postoperative outcomes.
METHODS: A retrospective cohort study was conducted on 390 patients who underwent PD with for pancreatic and periampullary adenocarcinoma between 2019 and 2023 at a single tertiary center. Non-adenocarcinoma histologies were excluded to ensure a homogeneous cohort

BACKGROUND: Ampulla of Vater (AoV) carcinoma is a rare malignancy arising at the junction of intestinal and pancreatobiliary epithelium. Its heterogeneous clinical behavior and histological diversity have hindered therapeutic advances, and the cellular basis of this heterogeneity remains unclear. We aimed to construct a single-cell transcriptomic atlas of AoV carcinoma, with a focus on identifying epithelial subtypes and their interactions with the tumor microenvironment (TME).
METHODS: We performed single-cell RNA sequencing on eight primary AoV tumors and four matched normal tissues. Compre

Ampullary adenocarcinoma (AMPAC) is a rare and heterogeneous malignancy with markedly variable clinical outcomes, underscoring the urgent need for molecularly informed classification and personalized therapeutic strategies. Current AMPAC classification relies primarily on morphological and immunohistochemical criteria, which lack prognostic accuracy and fail to capture the underlying molecular and tumor microenvironment heterogeneity. In this study, we integrated bulk RNA sequencing, single-cell RNA sequencing (scRNA-seq), and clinicopathological data to elucidate the molecular architecture of

INTRODUCTION: Pancreaticoduodenectomy (PD) is the preferred surgical approach for resectable periampullary carcinomas. Non-pancreatic periampullary carcinomas (NPPC) demonstrate distinct biological behaviours and are generally less aggressive than pancreatic cancer. Despite over three decades of advancements, the role of laparoscopic PD (LPD) remains debated. Few studies have directly compared LPD and open PD (OPD) while accounting for tumour invasiveness, and the data specifically evaluating LPD vs OPD for NPPC are limited. Randomised controlled trials are necessary to determine whether thera

Lemmel syndrome is a rare and misdiagnosed cause of obstructive jaundice due to a periampullary duodenal diverticulum (PAD) in the absence of choledocholithiasis or a tumor. PADs are extraluminal outpouchings of duodenal mucosa that develop within a radius of 2-3 cm from the ampulla of Vater. They are largely asymptomatic, but rarely they can cause acute abdominal pain and obstructive jaundice due to the extrinsic compression of the common bile duct or pancreatic duct. PADs are mostly asymptomatic, but complications can occur, such as hemorrhage, perforation, diverticulitis, pancreatitis, chol

PURPOSE: Whether congenital gastrointestinal atresia (atresia) specifically affects the meconium microbiota because of an altered intrauterine intestinal environment remains unclear. Therefore, we aimed to characterize the meconium microbiota of neonates with congenital anomalies, specifically atresia.
METHODS: Meconium samples were collected from healthy term neonates (control), neonates with congenital malformations other than atresia (other anomalies), and those with atresia who were admitted to the growing care unit or obstetrics ward. Alpha-diversity (Shannon index and observed features)

INTRODUCTION: Most present-day survival-models of patients with cancer do not account for tumor-host interactions. We hypothesized that host phenotypes based on systemic inflammation and body composition are prognostic of overall survival (OS) in patients with pancreatic ductal adenocarcinoma (PDAC).
METHODS: We performed a post-hoc analysis of the nationwide PORSCH-trial, including all patients undergoing pancreatoduodenectomy for PDAC. Primary outcome was OS. Body composition analysis was performed using automatic segmentation (Mosamatic™) of preoperative abdominal computed tomography scans

BACKGROUND: Benchmarking of surgical outcomes across institutions requires adjustment for established risk factors. Although pathological diagnosis is recognized as a determinant of postoperative morbidity after pancreatoduodenectomy, its independent impact on outcomes has not been systematically quantified at a national level. This study aimed to investigate the impact of pathological diagnosis on postoperative outcomes after pancreatoduodenectomy and to assess inter-hospital variation in case-mix.
METHODS: Data were obtained from the nationwide Dutch Pancreatic Cancer Audit. Postoperative c

BACKGROUND: Key prognostic factors in periampullary tumors include tumor size, histopathological subtype, degree of differentiation, lymph node metastasis, perineural invasion, and lymphovascular invasion. Therefore, evaluating molecular markers alongside traditional histopathological parameters may improve the accuracy of prognostic classification.
AIM: We seek to examine how HER2/neu status relates to clinicopathological features like T-stage, N-stage, and anatomical site to better define its potential as a clinical tool for managing periampullary tumors.
METHODS: This study was conducted

Hodgkin lymphoma (HL) has a wide spectrum of presentation. Most cases affect lymph nodes (nodal), while extranodal involvement is rare. Whereas the gastrointestinal tract is enriched with lymphoid tissues, the ampulla of Vater is not rich in lymphoid tissue. Involvement of the ampulla of Vater with HL has rarely been reported in adults and has never been reported in children. Its presentation mimics other causes of obstructive jaundice, and immunodeficiency increases the risk of lymphoma. There have been reports of cutaneous T-cell lymphomas and one case of HL following the use of dupilumab, a

As population aging accelerates, patients aged ≥ 75 years represent a growing proportion of those diagnosed with ampullary tumors. Endoscopic papillectomy (EP) is an established minimally invasive treatment for ampullary adenoma; however, treatment selection in older patients requires careful consideration of oncologic benefit, procedural risk, geriatric vulnerability, and life expectancy. This review outlines key geriatric assessment tools and evaluates the safety, prognosis, and treatment selection of endoscopic and surgical approaches for ampullary tumors in older patients. With appropriate

BACKGROUND AND AIMS: Endoscopic papillectomy (EP) is an established treatment for ampullary tumors. This study aimed to analyze recurrence patterns and risk factors in a large cohort with long-term follow-up.
METHODS: Patients who underwent EP for ampullary tumors at a large tertiary center from January 2010 to June 2025 were retrospectively reviewed. Cumulative recurrence rates were estimated using Kaplan-Meier analysis, and Gaussian kernel density estimation was employed to explore dynamic recurrence patterns. Cox proportional hazards model was used to identify risk factors for recurrence.

A 77-year-old woman undergoing surveillance for an intraductal papillary mucinous neoplasm (IPMN) was found to have new enlargement of the major papilla on esophagogastroduodenoscopy. Contrast-enhanced computed tomography and endoscopic ultrasound (EUS) revealed an ampullary mass with multiple hepatic lesions. Because no tumor exposure was evident on the duodenal mucosal surface, endoscopic ultrasound-guided fine-needle biopsy (EUS-FNB) was performed. Histopathological and immunohistochemical findings, including a high Ki-67 labeling index, confirmed the diagnosis of neuroendocrine carcinoma (
