Silently Gradually Progressing Paraparesis Pott's Spine Lumbar Vertebra with Bilateral Psoas Abscess.

INTRODUCTION: Extrapulmonary tuberculosis (TB) can involve the spine, presenting as Pott's disease. Patients typically develop back pain, and complications, such as psoas abscess and paraplegia may occur if diagnosis is delayed. A high index of suspicion and gadolinium-enhanced magnetic resonance imaging (MRI) are essential for early detection. Timely treatment helps prevent neurological deficits. We report the case of a 55-year-old afebrile man with progressive lower-limb weakness, unexplained weight loss, and an atraumatic vertebral fracture. MRI revealed classic cold abscesses consistent wit

Clinical assessment and biomarkers for the detection of bacterial community-acquired pneumonia in children and adults: a protocol for systematic review of diagnostic accuracy.

BACKGROUND: Community-acquired pneumonia (CAP) is defined as an infection of the pulmonary parenchyma acquired in the community and outside the hospital setting. CAP is generally diagnosed based on clinical parameters and chest radiography, with biomarkers increasingly used to support diagnostic decisions. Our review aims to synthesise evidence on the diagnosis of bacterial CAP using clinical assessments and biomarkers and to guide evidence-based national antibiotic guidelines for bacterial CAP in India. Our primary objective is to determine the diagnostic accuracy of clinical criteria (sympto

Delayed Intrathoracic Textile Foreign Body Following Penetrating Shrapnel Injury Presenting with Expectoration of Fabric-Like Material and Chronic Middle-Lobe Destruction: A Case Report.

BACKGROUND: Retained intrathoracic and tracheobronchial foreign bodies are uncommon but important causes of chronic respiratory symptoms. Diagnosis may be delayed because presentations are nonspecific and retained textile material is often radiolucent. Such material can lead to chronic infection, pleural adhesions, and progressive lung destruction. CASE PRESENTATION: A 31-year-old Syrian male military serviceman and active smoker (20 pack-years) presented with a long-standing intermittent cough that had worsened over one week and become productive of white sputum. He also reported a single ep

Rib-Origin Ewing Sarcoma With Cranial Nerve Involvement From Orbital Metastasis: A Rare Case Report.

Ewing sarcoma (ES) is an aggressive tumor of the bone in young adults, commonly originating from the pelvis and long bones. It is characterized by chromosomal translocation t(11;22) (q24;q12), resulting in the EWSR1-FLI1 fusion gene. It presents with metastasis commonly to the lungs and other soft tissues. While the metastasis to lungs is common, orbital involvement with cranial involvement is exceptionally rare. The case highlights the need for the early detection and aggressive treatment in order to reduce mortality in patients ES. A 23-year-old South-Asian male presented with a 2 month hist

An ensemble model approach for identifying pulmonary tuberculosis on chest X-ray.

Tuberculosis (TB) is a global infectious disease, and chest x-ray (CXR) examination is a primary screening method. However, manual image interpretation can suffer from low diagnostic efficiency, poor consistency, and limitations imposed by the experience of the interpreters, necessitating the development of intelligent identification tools. This study aims to construct and validate a deep learning ensemble model-based CXR image recognition system for TB, achieving accurate and intelligent screening. The Shenzhen TB CXR public dataset was used as the training dataset, randomly divided into trai

Comparative efficacy and complications of Transthoracic Ultrasound guided Pleural biopsy versus Medical thoracoscopic Pleural biopsy in Undiagnosed Exudative Pleural Effusion: Experience from a Tertiary Care Institute of Eastern India.

BACKGROUND: Approximately 20% of exudative pleural effusions remain undiagnosed even after diagnostic thoracentesis. In such situations, pleural biopsy is essential for establishing a diagnosis. Pleural biopsy can be obtained either through invasive procedures, such as medical or surgical thoracoscopy, or through semi-invasive techniques, such as closed pleural biopsy, with or without image guidance. This study compared the efficacy and complications of transthoracic ultrasound-guided versus medical thoracoscopic pleural biopsy for undiagnosed exudative pleural effusion. METHODOLOGY: This pro

A 43-Year-Old Man With Isolated Cutaneous Lymphomatoid Granulomatosis Presenting as a Chronic Necrotizing Ulcer of the Upper Arm.

BACKGROUND We report a 43-year-old man with ulcerative lesions of the right arm without pulmonary symptoms that was diagnosed as cutaneous lymphomatoid granulomatosis, a rare Epstein Barr virus-associated angiocentric and angiodestructive lymphoproliferative disorder that most commonly involves the lungs. Cutaneous involvement usually occurs in association with pulmonary disease, while isolated cutaneous disease is rare and most often presents with papules, nodules, or plaques. Necrotizing ulcers are uncommon and can closely mimic other dermatologic conditions. CASE REPORT A 43-year-old man pr

Concomitant effect of high altitude and thrombophilia on multi-organ thrombosis.

We report the case of a 33-year-old male mountaineer, presenting with chest pain, severe headache, dyspnea, and lower extremity edema, on his first expedition to Paraw mountain located in Kermanshah Province. His condition involved a complex thrombotic presentation, including both venous and arterial events, such as a myocardial infarction due to stenosis of the left main coronary artery, which we believe to be a coincidental pathology. His neurological symptom includes headache, vomiting, and slight drowsiness associated with chest pain, which were evaluated alongside confirmatory testing for

Expert consensus on imaging diagnosis of human infection with avian influenza.

Avian influenza viruses (AIVs) can infect humans through respiratory transmission, direct contact, and other routes, with the lungs being the most commonly affected site. In recent years, sporadic cases and localized outbreaks of human infection with avian influenza (HIAI) have emerged in multiple countries worldwide. Beyond the well-established subtypes, novel avian influenza strains-such as H7N4, H9N2, H10N8, and H10N3-continue to emerge, underscoring the ongoing public health threat posed by human avian influenza. In this context, imaging plays a crucial role in the early diagnosis, differe

Radiological Anatomy of the Pelvis and Hind Limb of the Southern Giant Pouched Rat (Cricetomys ansorgei).

The southern giant pouched rat (Cricetomys ansorgei) is a large rodent utilised in recognition of landmines, pulmonary tuberculosis and Brucella infection. Additionally, they are kept as pet animals. The aim of this study was to describe the radiological anatomy of the pelvis and hind limb and to provide measurements of the hind limb bones in southern giant pouched rats. Radiological examinations were performed in 10 live and five dead adult southern giant pouched rats. The pelvis had a relatively short pelvic symphysis and the ischial tuberosities were less prominent. The obturator foramina a

Concomitant Pulmonary and Tonsillar Tuberculosis.

We report the case of a 54-year-old man presenting with concomitant pulmonary and tonsillar tuberculosis. The coexistence of these two localizations poses a diagnostic challenge because of the nonspecific clinical manifestations and the potential confusion with malignant diseases. The diagnosis of pulmonary tuberculosis was confirmed by positive sputum GeneXpert MTB/RIF, while histopathological examination of the tonsillar lesion revealed non-caseating granulomatous inflammation, with no evidence of malignancy. The patient received the standard anti-tuberculosis therapy for six months, with co

Primary tuberculous osteomyelitis of the maxilla mimicking a refractory post-endodontic infection: A diagnostic challenge.

Primary extrapulmonary tuberculosis involving the maxillofacial region is rare and often poses a diagnostic challenge because of its atypical presentation and the absence of pulmonary involvement. We report a case of a 38-year-old woman who developed rapidly progressive osteomyelitis of the maxilla following elective root canal treatment. The lesion was unresponsive to empirical antibiotic therapy and standard dental intervention. Cone-beam computed tomography revealed extensive osteolytic destruction, and histopathological examination demonstrated granulomatous inflammation with Langhans gian

Case study of elevated alpha-fetoprotein secondary to drug-induced hepatitis mimicking germ cell tumor: a diagnostic pitfall.

INTRODUCTION AND IMPORTANCE: Alpha-fetoprotein (AFP) is a tumor marker commonly associated with hepatocellular carcinoma and germ cell tumors. However, non-malignant conditions, such as drug-induced liver injury (DILI), can also cause significant AFP elevation, leading to potential diagnostic confusion. We present a case highlighting this important diagnostic pitfall. CASE PRESENTATION: A 30-year-old female presented with respiratory symptoms and right-sided pleural effusion. In a high tuberculosis prevalence setting, she was empirically started on anti-tubercular therapy (ATT) for presumed p

Concurrent therapy-related acute myeloid leukemia and lymph node tuberculosis following treatment for lung squamous cell carcinoma: a case report and literature review.

Concurrent development of therapy-related acute myeloid leukemia (t-AML) and lymph node tuberculosis (LNTB) following comprehensive anti-tumor therapy for locally advanced lung squamous cell carcinoma (LSCC) is extremely rare in clinical practice. This is not a new biological concept but represents a rarely documented clinical scenario in the setting of neoadjuvant chemoimmunotherapy, surgery, and anti-PD-1 maintenance therapy. Herein, we systematically summarize the clinical features, pathogenesis and individualized therapeutic strategies of these two concurrent rare complications based on a
