Adrenal Infections: An Underrecognized Cause of Primary Adrenal Insufficiency.

INTRODUCTION: Adrenal infections are an underrecognized but clinically significant cause of primary adrenal insufficiency (PAI). OBJECTIVES: This review proposes a diagnostic algorithm for the evaluation of suspected cases. RESULTS: The most common causative pathogens are Mycobacterium tuberculosis and Histoplasma capsulatum; however, bacterial infections and viral pathogens such as cytomegalovirus (CMV) and SARS-CoV-2 (COVID-19) have also been reported. These infections, particularly in tuberculosis and histoplasmosis, can induce granulomatous inflammation, tissue necrosis, and destruction

Tuberculous Longitudinally Extensive Transverse Myelitis: A Case Report and Literature Review.

Longitudinally extensive transverse myelitis (LETM) is a rare but severe manifestation of central nervous system (CNS) tuberculosis (TB). Because of its nonspecific clinical presentation and broad differential diagnosis, early recognition remains challenging. We report the case of a 27-year-old man with concurrent pulmonary and meningeal TB who developed rapidly progressive quadriparesis. Magnetic resonance imaging demonstrated longitudinally extensive T2-hyperintense lesions extending from C2 to T12, accompanied by diffuse meningeal enhancement, consistent with TB LETM. The patient was treate

Data-driven subtypes of mild cognitive impairment and their modifiable determinants: latent profile analysis of the BIOCIS cohort.

BACKGROUND: "Dry mouth" syndrome (chronic hyposalivation) can be caused by a number of pathophysiological conditions such as acute and chronic stress exposure, abnormal body weight (both too high and too low ones), eating disorders (such as anorexia nervosa), metabolic syndrome(s), Sjögren's and Sicca syndromes, drugs and head/neck radiotherapy application. In turn, the chronic hyposalivation as a suboptimal health condition significantly reduces quality of life, may indicate a systemic dehydration, provokes and contributes to a number of pathologies such as a strongly compromised protection o

Prior pulmonary tuberculosis deteriorated all-cause mortality in AECOPD patients after discharge.

Chronic obstructive pulmonary disease (COPD) patients who have a history of pulmonary tuberculosis (PTB) often present with worse dyspnea and more acute exacerbations (AEs). However, the impact of prior PTB on prognosis in AECOPD patients remains to be further investigated. The diagnosis of patients with prior PTB was based on a combination of imaging and prior tuberculosis history. This study compared the clinical characteristics and prognosis of the Chinese AECOPD inpatients with or without prior PTB, and discussed the role of glucocorticoids in the prognosis of AECOPD patients with prior PT

Corticosteroids as adjunctive therapy in patients with acute/subacute paracoccidioidomycosis presenting a severe paradoxical inflammatory reaction: Two case reports and literature review.

Paradoxical inflammatory reactions, similar to Immune Reconstitution Inflammatory Syndrome (IRIS) in HIV, have been occasionally reported in leprosy, tuberculosis, certain immunosuppressive conditions, and Paracoccidioidomycosis (PCM), among others. This report presents the clinical data and outcomes of two PCM cases in which the patients developed a severe inflammatory reaction following antifungal therapy, with subsequent improvement after adjunctive corticosteroid use. Both patients were adults with acute/subacute PCM who experienced clinical worsening after starting liposomal amphotericin

Beyond corticosteroids: A systematic review and meta-analysis of novel therapies for hypersensitivity pneumonitis.

INTRODUCTION: Hypersensitivity pneumonitis (HP) is a complex immune-mediated interstitial lung disease triggered by inhaled antigens. While corticosteroids remain the mainstay of therapy, their benefit is limited in fibrotic HP. Several novel agents-including pirfenidone, nintedanib, rituximab, azathioprine, mycophenolate mofetil (MMF), leflunomide, and roflumilast-have been investigated as alternatives. METHODS: This meta-analysis followed PRISMA guidelines and was registered on PROSPERO (CRD42023428070). Eligible studies included clinical and non-clinical trials assessing both traditional a

Eosinophilic Ascites: An Underrecognized Entity in Peritoneal Disorders.

BACKGROUND: Eosinophilic ascites (EA) is a rare, under-recognized condition characterized by eosinophil-rich peritoneal fluid, most frequently associated with eosinophilic gastroenteritis (EGE). Its true prevalence is unclear, often misdiagnosed as malignancy, tuberculosis, or cirrhosis. EA typically presents between ages 30 and 50, with slight male predominance. Its clinical importance lies in a favorable response to corticosteroids and the potential for severe complications such as intestinal obstruction if untreated. OBJECTIVE: This review consolidates current knowledge on EA, focusing on

A Rare Presentation of Tuberculosis: Dilated Cardiomyopathy Secondary to Myocarditis with a Dual-secretory Adrenal Pheochromocytoma.

We describe an unusual case involving a 25-year-old male who presented with breathlessness, cough, chest pain, abdominal pain, and significant weight loss. Detailed evaluation revealed pulmonary tuberculosis, an adrenal mass with biochemical evidence of a dual-secretory pheochromocytoma and severe dilated cardiomyopathy with profound left ventricular dysfunction. The coexistence of these conditions is extremely rare and highlights several clinical challenges - both in diagnosis and management. This case report emphasizes the importance of considering uncommon etiologies when confronted with mu

[Idiopathic granulomatous mastitis - a rare inflammatory condition].

Idiopathic granulomatous mastitis (IGM) is a rare inflammatory condition of the breast with unknown etiology, primarily affecting women of childbearing age. IGM commonly presents with culture-negative abscesses, often leading to significant scarring, hyperpigmentation, and deformation of the affected breast. Breast cancer and tuberculosis are important differential diagnoses. IGM is confirmed by the presence of non-caseating granulomas and microabscesses on histopathological examination. The literature mainly consists of case studies, and there is no evidence-based treatment. A regional multid

A diagnostic puzzle: when pheochromocytoma meets tuberculosis!

The association between tuberculosis and pheochromocytoma is extremely rare, although their clinical manifestations may overlap. This report presents a case of pheochromocytoma associated with pulmonary tuberculosis. Such an uncommon coexistence highlights the importance of considering infectious etiologies when evaluating adrenal pathology, particularly in regions where tuberculosis is endemic, underlines the critical role of radiological imaging in identifying dual pathology and ensuring appropriate management.

Community-acquired pneumonia mimicking tuberculosis in a patient with Down syndrome treated with adjunctive injectable corticosteroids.

Community-acquired pneumonia (CAP) is a frequent cause of morbidity in individuals with Down syndrome (DS) due to their inherent immunological vulnerabilities and anatomical predispositions. We present the case of a young woman in late adolescence with DS who developed severe CAP with radiological findings mimicking pulmonary tuberculosis. Despite initial diagnostic uncertainty, the patient improved remarkably following initiation of antibiotics combined with injectable corticosteroids. This case highlights both the diagnostic challenge of distinguishing CAP from tuberculosis in high-risk popu

Risk of Tuberculosis with the Use of Inhaled Corticosteroids in Delhi/NCR.

BACKGROUND: Tuberculosis (TB) is a global health concern caused by Mycobacterium tuberculosis, primarily affecting the lungs. In addition to TB, chronic respiratory conditions like Chronic Obstructive Pulmonary Disease (COPD) and asthma are becoming more prevalent globally. Inhaled corticosteroids (ICS) are commonly used for COPD and bronchial asthma management, but some recent studies suggest a potential association between ICS usage and an increased risk of TB, raising concerns that they may lower lung immunity and enhance tuberculosis infection. AIMS AND OBJECTIVES: This research study was

Comparison of Allergic Rhinitis Treatments on Utilities and Quality of Life: A MASK-air Study.

BACKGROUND: Allergic rhinitis displays a relevant impact on quality of life. Medications used in the treatment of rhinitis have been assessed on their impact on rhinoconjunctivitis-related quality of life, but not on generic health-related quality of life metrics, such as utilities or EQ-5D visual analogue scale (VAS) levels. This study aimed to compare different medication classes and individual medications on utilities and EQ-5D VAS levels using data from a mobile app. METHODS: We conducted an observational study using direct patient data from the MASK-air mobile application, collected betw

Application of FeNO in the diagnostic and therapeutic algorithm of pediatric asthma: From chronic cough and wheeze to personalized management.

Bronchial asthma is the most common chronic respiratory disease in children. Treatment approaches should aim to achieve disease control, reduce limiting symptoms, and improve quality of life. The routine management of patients with asthma relies on the assessment of symptoms (including persistent cough and wheezing) and spirometry results However, these strategies do not consider the level of airway inflammation, which is a fundamental pathognomonic feature of the disease. The use of biomarkers is increasing in an effort to better understand individual asthma pathways (endotyping), offering th

EUS-B FNA of Enlarged Left Adrenal Gland in Lung Cancer-Case Series.

Combined endobronchial and endoscopic ultrasound via bronchoscope (EBUS EUS-B) is an established method for diagnosis, staging and tissue acquisition in lung cancer. With incorporation of EUS-B fine needle aspirate (EUS-B FNA) of enlarged left adrenal gland (LAG), combined EBUS EUS-B can aid in excluding metastatic spread or differentiating between regional (M1a) versus distant (M1b) single organ or multi-organ metastatic disease (M1c). In comparing, EUS FNA of LAG via conventional gastro-echoendoscope with EUS-B, conventional EUS can successfully visualise LAG more frequently. However, once v

Non-resolving Pulmonary Consolidation Mimicking Tuberculosis: A Diagnostic Pitfall Revealing Lung Adenocarcinoma.

Pulmonary tuberculosis (TB) remains highly prevalent in endemic regions; however, current international and national guidelines emphasize microbiological confirmation prior to initiation of anti-tubercular therapy (ATT) whenever feasible. Empirical treatment without microbiological confirmation represents a deviation from recommended standards and may contribute to diagnostic delay. We report a 52-year-old male presenting with chronic cough with mucoid expectoration and progressive dyspnea without fever, hemoptysis, or weight loss. Despite negative sputum acid-fast bacilli (AFB) smear and cart
