Ability of computed tomography and endoscopic ultrasound to predict pathological tumor node metastasis staging of resected ampullary carcinomas (ACs): an analysis of the Fédération Française de Cancérologie Digestive AC prospective cohort study.

BACKGROUND AND AIMS: Pretherapeutic evaluation of ampullary carcinomas (ACs) is important to choose the optimal therapeutic strategy. We aimed to assess the ability of EUS and CT to predict the pathologic tumor node metastasis stage of resected AC. METHODS: We analyzed data collected in the Fédération Française de Cancérologie Digestive AC cohort, a French multicentric prospective cohort of patients with resected AC. Our main outcome was the diagnostic performance of EUS to predict pathologic tumor (pT) and pathologic node (pN) and CT to predict pN. RESULTS: Among the 389 patients included i

Histopathologic Subtype as a Determinant of Prognosis and Chemotherapy Benefit in Resected Ampullary Adenocarcinoma.

BACKGROUND: Ampullary adenocarcinoma (AAC) is a rare malignancy with heterogeneous biological behavior. Although surgical resection is the only potentially curative option, the prognostic role of histopathologic features and adjuvant chemotherapy (AC) remains unclear. METHODS: Consecutive patients undergoing curative-intent pancreatoduodenectomy for AAC at the University of Colorado Hospital between January 2013 and May 2025 were retrospectively reviewed. Clinicopathologic variables associated with overall (OS) and recurrence-free survival (RFS) were analyzed using multivariable Cox regressio

Compartment-specific CD8 infiltration and disease-specific survival in resected ampullary adenocarcinoma.

Ampullary adenocarcinoma is rare and its immune biomarker landscape is incompletely characterized. This paper evaluated mismatch repair (MMR), programmed death-ligand 1 (PD-L1) and compartment-specific CD3/CD8 tumor-infiltrating lymphocytes (TILs) in resected tumors and explored associations with disease-specific survival (DSS). Ninety-nine patients underwent pancreaticoduodenectomy at Vancouver General Hospital between 1984-2013. Duplicate 0.6-mm tissue microarrays were assessed for MMR proteins, PD-L1 combined positive score (CPS), and stromal and intra-epithelial CD3 and CD8 counts. DSS was

Beyond Rigler's Triad: Gallstone Ileus in the Absence of a Bilioenteric Fistula.

Gallstone ileus is an uncommon cause of mechanical intestinal obstruction, classically associated with bilioenteric fistula formation that enables gallstone migration into the gastrointestinal tract. Its incidence increases among elderly patients and is associated with significant morbidity due to delayed diagnosis. Obstruction typically occurs at the terminal ileum, and diagnosis traditionally relies on Rigler's triad: intestinal obstruction, pneumobilia, and an ectopic gallstone. Gallstone ileus without a bilioenteric fistula is an exceptionally rare presentation that challenges conventional

Interpreting MALDI imaging data for rare types of ampullary cancer using machine learning.

Rare tumor diseases are difficult to diagnose and there is a lack of routine diagnostic procedures. Approaches must be found that allow comprehensive identification and evaluation of prognostic relevant target proteins or transcripts. Analyzing rare ampullary cancer, respectively, to their prognosis and predictive factors by machine learning (ML) based matrix-assisted laser desorption/ionization (MALDI) time-of-flight (TOF) imaging is a first step towards providing new solutions for diagnostics of those cancer samples. In this study, we investigated a cohort of ampullary adenocarcinomas, inclu

Transpapillary pancreatic duct stenting as a salvage therapy for refractory pancreatic fistula after distal pancreatectomy: a case series supporting the role of duodenal papilla obstruction.

BACKGROUND & AIMS: Endoscopic retrograde pancreatography (ERP) with pancreatic duct stenting is a recognized intervention for pancreatic fistulas. However, its application as a salvage therapy for refractory postoperative pancreatic fistula (POPF) after distal pancreatectomy (DP) remains less defined, particularly when guided by the hypothesis that duodenal papilla obstruction perpetuates the fistula. This study evaluated the efficacy of ERP with stenting for refractory POPF after DP and investigated the role of papilla morphology. METHODS: Five patients with refractory Grade B POPF after DP

Microlithiasis as a Predictor of Disease Severity in Gallstone-induced Acute Pancreatitis.

INTRODUCTION: Gallstones, particularly microlithiasis, constitute the predominant etiology of acute pancreatitis. A minority of people with gallstones experience pancreatitis. The close closeness of the common bile duct (CBD) and the main pancreatic duct, which converge at the hepatopancreatic ampulla of Vater, renders the main pancreatic duct susceptible to obstruction by gallstone impaction. This impaction obstructs the normal flow of pancreatic enzymes, resulting in leakage through the glandular parenchyma and autodigestion of cells, so inducing pancreatitis. A smaller gallstone size (<5 mm

Hemosuccus Pancreaticus from Large Inferior Pancreaticoduodenal Artery Pseudoaneurysm Complicating Chronic Pancreatitis.

Hemosuccus pancreaticus is an uncommon cause of upper gastrointestinal bleeding characterized by hemorrhage entering the pancreatic duct and draining into the duodenum. Although pseudoaneurysms may arise from several peripancreatic vessels in the setting of chronic pancreatitis, involvement of the inferior pancreaticoduodenal artery (IPDA) is particularly uncommon and carries a high risk of rupture. A man in his late 30s with a known history of chronic calcific pancreatitis presented with melena, severe anemia, and hemodynamic instability. Side-viewing duodenoscopy revealed intermittent bleedi

Endovascular management of hemosuccus pancreaticus secondary to splenic artery pseudoaneurysm: a case report.

BACKGROUND: Hemosuccus pancreaticus (HP) is a rare cause of gastrointestinal bleeding, characterized by the pathological passage of blood from the pancreatic duct into the duodenum via the ampulla of Vater. HP presents considerable diagnostic and therapeutic challenges due to its nonspecific clinical presentation and the intermittent nature of bleeding. CASE DESCRIPTION: A 62-year-old male with no history of pancreatitis, abdominal trauma, or surgery presented with recurrent abdominal pain and hematochezia over 3 months, acutely worsening in 72 hours. Initial evaluation revealed severe anemia

Dual Ampullary Orifices: A Forgotten Embryologic Anomaly in Modern ERCP.

Bile duct and pancreatic duct join as a single orifice at ampulla of Vater. Anomalous rotation or recanalization of hepatopancreatic ducts during embryogenesis can lead to anatomical variations of ampulla of Vater. One such anatomical variation is double major papilla with dual ampullary orifices for bile and pancreatic duct, the incidence of which is exceedingly rare. Identification and early recognition of such variations before initiating cannulation can prevent inadvertent pancreatic duct cannulation. We report a case of 78-year-old man with chronic calcific pancreatitis with choledocholit

Visualizing Periampullary Tumors with Intraductal Ultrasound utilizing an Intracardiac Echocardiography Catheter: A Feasibility Study.

OBJECTIVE: Early detection of pancreatic cancer is crucial for survival, but detecting smalllesions remains challenging. Intraductal Ultrasound (IDUS) using intracardiac echocardiography (ICE) catheters for B-mode and Shear-Wave Elastography (SWE) potentially offers improved visualization and characterization of small tumors. This study assesses the feasibility of IDUS using ICE catheters to detect and visualize periampullary tumors in surgically resected specimens. METHODS: In this two-phase ex-vivo feasibility study, 25 pancreatic specimens were included, of which the first 10 were used to

Ampullary adenomas greater than 20 mm in size have higher risk of recurrence compared to smaller lesions.

INTRODUCTION: Lesions of ampulla of Vater are uncommon and often discovered incidentally. Most adenomatous lesions can be treated by endoscopic papillectomy which can obviate the need for surgery. Data regarding effectiveness for endoscopic papillectomy especially for larger lesions is lacking. We aimed to assess the impact of polyp size on recurrence on subsequent ERCP procedures following initial papillectomy. METHODS: After approval of the study from local IRB, patients with diagnosis of ampullary lesion who were treated with endoscopic papillectomy at our quaternary care referral center w

En bloc resection of intestinal-type adenoma extending to the minor duodenal papilla by underwater endoscopic mucosal resection with a forward-viewing endoscope.

We report successful en bloc underwater endoscopic mucosal resection (UEMR) of a lesion involving the minor duodenal papilla. A woman in her 60s presented with a 10-mm depressed lesion in the descending duodenum. White-light imaging and Magnifying endoscopy suggested a tumor involving a minor papilla. Although side-viewing endoscopes are conventionally used for the minor papilla, pre-procedural assessment indicated inadequate maneuverability for this specific lesion (ED-580T; Fujifilm Corporation, Tokyo, Japan). Consequently, we strategically used a forward-viewing dual-channel endoscope (GIF-

Malignant epithelial states drive immune dysfunction in ampulla of Vater carcinoma.

BACKGROUND: Ampulla of Vater (AoV) carcinoma is a rare malignancy arising at the junction of intestinal and pancreatobiliary epithelium. Its heterogeneous clinical behavior and histological diversity have hindered therapeutic advances, and the cellular basis of this heterogeneity remains unclear. We aimed to construct a single-cell transcriptomic atlas of AoV carcinoma, with a focus on identifying epithelial subtypes and their interactions with the tumor microenvironment (TME). METHODS: We performed single-cell RNA sequencing on eight primary AoV tumors and four matched normal tissues. Compre
