ΔNp63α‑mediated regulation of tumor‑associated genes and pathways in hepatocellular carcinoma progression.

An understanding of the neural control of lower oesophageal sphincter (LOS) relaxation is clinically relevant because transient LOS relaxations (TLOSRs) are a mechanism of acid reflux into the oesophagus. Preganglionic motor neurones innervating the LOS are localized in the dorsal motor nucleus of the vagus (DMV). Based on a single study in cats, it is now widely accepted that these neurones are functionally organized into two separate populations, such that stimulation of the caudal and rostral DMV evokes LOS relaxation and contraction, respectively. Our goal was to map the functional LOS res

Challenges in respiratory medicine: the need for integrated tuberculosis and respiratory care in low-resource settings.

BACKGROUND: Pulmonary tuberculosis (PTB) and chronic respiratory diseases (CRDs) are intricately linked. People with PTB and CRDs experience similar symptoms, including breathlessness, cough and chest pain. They may have similar risk factors for disease, including smoking and occupational exposures. PTB is also a direct cause of lung damage in the form of post-TB lung disease. However, despite the overlap in risk factors, symptoms and sequelae, public health and clinical care pathways for TB and CRDs remain almost entirely separate in many low- and middle-income countries (LMICs). Those with r

Social inequalities as effect modifiers of active case-finding strategies for tuberculosis in Brazil: an ecological study.

BACKGROUND: Social inequalities play a crucial role in the incidence of TB, making it plausible that they act as effect modifiers on the impact of active case-finding (ACF) strategies in the detection of the disease. We estimated the association between ACF strategies and TB detection rates and evaluated their effect modification due to social inequalities in Brazilian municipalities. METHODS: We included 5033 municipalities that reported at least one new TB case. We defined the TB detection rate as the outcome variable. Our exposure variables were the proportion of primary care team (PCT) re

The Role of Youths in Within-Household Tuberculosis Transmission: A Household Contact Cohort Study.

BACKGROUND: Youth aged 15-24 years are significantly impacted by tuberculosis globally. Their expanding social networks heighten exposure risks, potentially amplifying tuberculosis transmission, but their role in within-household transmission remains poorly understood. METHODS: A household contact cohort study in Lima, Peru (2009-2012) enrolled individuals (>15 years) with incident tuberculosis (index patients) and their household contacts (HHCs), following them for 12 months. We evaluated whether the age of index patients and HHCs modified the risk of tuberculosis infection among HHCs. Study

Reducing Household Tuberculosis Transmission: A Pilot Cluster-Randomized Controlled Trial.

BACKGROUND: The duration of infectiousness following pulmonary tuberculosis treatment initiation remains uncertain. We aimed to assess whether a bundled intervention designed to decrease respiratory exposure was feasible and would reduce new tuberculosis infections in household contacts (HHCs). METHODS: We conducted a pilot cluster-randomized controlled trial with a hybrid type 1 effectiveness-implementation design in Santiago, Chile. Random allocation was performed, and 2 healthcare districts were assigned to the intervention (n = 180 HHCs) and 1 to standard of care (n = 149 HHCs). Eligible

Asymptomatic Tuberculosis in Children With Household Exposure to Mycobacterium tuberculosis.

BACKGROUND: Asymptomatic tuberculosis (TB) is common in adult community prevalence surveys. Evidence to inform our understanding of asymptomatic TB in children is lacking, because diagnostic algorithms typically include only symptomatic children. METHODS: Children aged 2-60 months, without human immunodeficiency virus (HIV), with household exposure to an adult TB patient in the last year, were enrolled in South Africa. Symptom screening, chest radiography (CXR) and induced sputum were performed. Confirmed TB (requiring positive Xpert Ultra and/or MGIT culture) and unconfirmed TB (requiring 2

Determinants of tuberculosis infection and disease in paediatric patients in the Czech Republic after tuberculosis exposure.

BACKGROUND: Mycobacterium tuberculosis is a transmissible pathogen most commonly acquired through contact with an infectious individual. Early initiation of tuberculosis (TB) preventive treatment (TPT) in high-risk populations can substantially reduce the risk of TB infection (TBI) and/or progression to active disease. Our objectives were to identify risk factors for children and adolescents developing TBI and TB after exposure and to evaluate the diagnostic utility of chest computed tomography (CT). METHODS: A 6.5-year retrospective cohort study was conducted at a single centre in the Czech

Performance evaluation of the AFIAS IGRA-TB (IFN-gamma) test versus QuantiFERON®-TB gold plus (QFT®-Plus) ELISA in the Barrio Obrero hospital network, Asunción-Paraguay, 2025.

BACKGROUND: Tuberculosis infection affects an estimated one-quarter of the global population and represents a key target for TB elimination strategies. While interferon-gamma release assays (IGRAs) such as QuantiFERON-TB Gold Plus (QFT-Plus) are WHO-endorsed, they remain logistically complex in low-resource settings. This study evaluates the performance of the AFIAS IGRA-TB, a qualitative fluorescence immunoassay (FIA), compared to QFT-Plus in Paraguay. METHODS: A cross-sectional diagnostic accuracy study was conducted from January to May 2025 among 210 individuals aged 18-59 years within the

Implementing a Program of Comprehensive Tuberculosis Preventive Treatment: Safety, Effectiveness, and Acceptability of Moxifloxacin or Bedaquiline Use for Contacts Exposed to Drug-Resistant Strains.

BACKGROUND: Rates of drug-resistant tuberculosis (DR-TB) are increasing worldwide. Tuberculosis preventive treatment (TPT) for contacts of people with active TB is essential to halt infection progression and transmission. While newer TPT regimens for exposure to drug-susceptible and rifampin (R)-resistant strains (MDR-TB/RR-TB) are expanding, optimal treatment for contacts exposed to fluoroquinolone-resistant strains of TB (pre-XDR- or XDR-TB) remains unclear. In 2019-2020, Vladimir City, Russia, introduced moxifloxacin (Mfx)- and bedaquiline (Bdq)-based TPT regimens to prevent disease develop

Protocol for a biomarker discovery study to identify correlates of risk for future tuberculosis disease progression in South African children (INTREPID).

INTRODUCTION: Young children and children living with HIV are at high risk of progressing to tuberculosis (TB) disease following Mycobacterium tuberculosis (Mtb) exposure and infection, and also of developing severe forms of disease and TB-related mortality. Identifying children who have very early (sub-clinical) TB disease, prior to progression to clinically apparent TB, would mean that TB preventive treatment (TPT) could be more efficiently targeted to this group. Identifying biomarker changes on drug therapy in children with Mtb infection or very early disease could pave the way for the dev

Characterizing social behavior relevant for tuberculosis transmission in four low- and middle-income countries.

BACKGROUND: Tuberculosis (TB) is caused by Mycobacterium tuberculosis (Mtb), a bacterium which is transmitted through the air. Close, sustained contact can lead to transmission of Mtb, but evidence also shows that transmission occurs also in community settings through more transient contact. However, social patterns that influence Mtb transmission, and locations that are most central to spread, are likely different by setting. METHODS: We use data from the GlobalMix study, which characterized social behavior across four countries, to evaluate differences in age-sex patterning and locations of

Environmental health recommendations for Multidrug-Resistant Tuberculosis in low- and middle-income countries: a systematic review.

BACKGROUND: The impact of environmental health risk factors on the optimization of health in MDR-TB infected individuals, as well as on the prevention of transmission to household contacts, is not well documented. Establishing the recommendations of environmental health risk factors for individuals living at home with MDR-TB will have important policy implications in LMICs, such as South Africa. AIMS: The study objectives were to identify current environmental health recommendations to optimize the health of individuals infected with MDR-TB, while living at home during the completion of their

Detection of Anti-Mycobacterium leprae Antibodies in Leprosy Contacts Using a Label-Free Approach with the Heat-Transfer Method.

Early detection of Mycobacterium leprae infection is essential to interrupt leprosy transmission and guide prophylactic interventions. Current diagnostic strategies rely largely on clinical signs, and laboratory tools lack sensitivity to detect asymptomatic individuals, who can play a critical role in disease transmission and evolve to disease as well. In this study, we report the development of a heat-transfer method (HTM)-based biosensor employing a newly designed multiepitope protein receptor, capable of detecting anti-M. leprae antibodies in human serum, with sensitivity for leprosy contac
