From Exposure to Disease: Predicting Tuberculosis Progression.

Mycobacterium tuberculosis, the causative agent of tuberculosis (TB), infects nearly one-fourth of global population, with 5%-10% progressing to active TB, one of the deadliest infectious diseases worldwide. Despite advancements in identifying infected individuals, actual tools lack precision in identifying those at highest risk of active TB. Close contacts of TB cases are at particularly high risk of progression. Multinational networks, such RePORT (Regional Prospective Observational Research for TB) International-a consortium of eight country-based networks-have advanced the field through co

Incidence of latent TB infection among hospital contacts in an intermediate-burden country.

BACKGROUND: This study aimed to investigate the incidence of latent tuberculosis infection (LTBI) and conversion rate among hospital inpatients contacts using an interferon-gamma release assay (IGRA)-based contact tracing strategy. METHODS: We conducted a retrospective study at a 2,700-bed tertiary hospital in Seoul, South Korea. Data from January 2019 to December 2024 were reviewed. Contacts were recommended to undergo IGRA testing at the time of exposure (baseline) and again at ≥ 6 weeks later after exposure. RESULTS: A total of 109 index cases required contact tracing, yielding 902 contac

Prevalence and predictors of tuberculosis among children in Zambezi District, North-Western Province, Zambia.

BACKGROUND: Tuberculosis (TB) remains a leading cause of morbidity and mortality among children in low- and middle-income countries, particularly in sub-Saharan Africa. Paediatric TB is often underdiagnosed due to nonspecific symptoms, limited access to diagnostic tools, and its tendency to mimic other common childhood illnesses. In Zambia, where TB and HIV are co-endemic, understanding the burden of childhood TB at the sub-national level is critical for tailoring effective interventions. This study aimed to determine the prevalence and identify key predictors of TB among children aged 0–14 ye

What Is the Significance of an Accelerated BCG Reaction in Children Living in Low Tuberculosis Incidence Countries?

AIM: In high tuberculosis (TB) incidence countries, accelerated Bacille Calmette-Guérin (BCG) reactions are considered markers of prior Mycobacterium tuberculosis exposure and may prompt investigation. Their significance in low-incidence countries is unclear. We aimed to describe the frequency, clinical characteristics and outcomes of accelerated BCG reactions in children vaccinated in Australia. METHODS: We conducted a retrospective case series at a quaternary hospital in Sydney, NSW. All children receiving intradermal Danish strain 1331 BCG between December 2023 and December 2024 were inclu

Early clearance of Mycobacterium tuberculosis among Indonesian household contacts is not associated with circulating beta-glucan present at the time of exposure.

A significant proportion of individuals heavily exposed to infectious tuberculosis patients do not acquire Mycobacterium tuberculosis (Mtb) infection, as detected by an interferon gamma release assay (IGRA). Trained immunity may contribute to this host resistance to Mtb infection, also termed early clearance. From a prospective tuberculosis household study in Indonesia we selected 80 heavily exposed IGRA-negative household contacts, of whom 40 converted their baseline-negative IGRA to positive after three months (IGRA converters) and 40 remained IGRA-negative (early clearers). From all individ

Comparative agreement between tuberculin skin test and QuantiFERON-TB Gold In-Tube across different levels of exposure in a college-based tuberculosis outbreak.

BACKGROUND: Optimal intervention thresholds for students in college TB outbreaks remain debated. Tuberculosis preventive treatment (TPT) is recommended for tuberculin skin test (TST) induration greater than 15 mm. The possibility of TPT recommendation for individuals with TST medium or weak positivity in the context of recent exposure, particularly with close contact with TB, is not entirely clear. METHODS: The study used a cross-sectional design and included a 1-year follow-up of participants after preventive treatment. TST and QuantiFERON-TB Gold In-Tube test (QFT) were used among close con

Tuberculosis in households with infectious cases in Kampala city: Harnessing health data science for new insights on an ancient disease with persistent, unresolved problems.

Tuberculosis (TB) is prevalent in Uganda and overlaps with a high rate of HIV/TB coinfection. While nearly all hospital-based TB cases in Kampala, the capital of Uganda, show clear TB symptoms, 30% or more of undiagnosed TB cases found through active screening are asymptomatic. Additionally, the host risk factors for TB in Kampala cannot be distinguished from environmental risk factors. These TB-specific challenges are just part of the complexity, especially in areas with high HIV/AIDS burden. Data science techniques, especially Artificial Intelligence (AI) and Machine Learning (ML) algorithms

A Novel Therapy With a One-Month Ultrashort Regimen to Halt Progression From Latent Infection to Active Tuberculosis Among Close Contacts (The TB‑YOUTH Study): Protocol for a Cluster Randomized Controlled Trial.

BACKGROUND: Close contacts of individuals with active pulmonary tuberculosis (TB) face an elevated risk of TB acquisition, necessitating systematic screening for latent TB infection and subsequent TB preventive treatment (TPT). Major TPT regimens require ≥3 months of drug exposure and demonstrate suboptimal safety profiles, significantly compromising treatment completion rates. Therefore, the development of shorter, safer chemoprophylaxis strategies represents a critical need in global TB control. Among close contacts, school-aged children and adolescents constitute the most vulnerable demogra

Inclusion of young adolescents in policy development for new tuberculosis vaccines.

The infant tuberculosis vaccine, BCG, prevents severe tuberculosis disease, but protection is rarely durable beyond childhood. New tuberculosis vaccines are being developed for the prevention of infectious pulmonary tuberculosis in older adolescents and adults, but younger adolescents have been historically excluded from clinical trials of new tuberculosis vaccines. Reasons to include young adolescents (aged 9-14 years) in tuberculosis vaccine policy development include the opportunity to vaccinate before the age-related increase in risk of tuberculosis disease, as well as increased rates of H

Risk of tuberculosis infection according to exposure time and other factors of contacts and their index cases.

BACKGROUND: The time of exposure to an index case of tuberculosis (TB) increases the risk of tuberculosis infection (TI) for contacts. Our aim was to determine the impact of exposure time on the TI risk for contacts of pulmonary TB index cases. METHODS: We conducted a cross-sectional prevalence study of TI among pulmonary TB contacts in Catalonia notified 1 January 2019-30 June 2021. Contacts with TI or TB were identified by means of the tuberculin skin test and/or interferon gamma release assay. The association of exposure time with TI in contacts was analysed using adjusted OR (aOR) and the

Community health education through WhatsApp and mobile applications for TB prevention.

This study examines whether tuberculosis (TB) prevention education delivered through WhatsApp groups and a mobile application is associated with higher completion of TB screening. Many eligible community members do not complete screening, and evidence on staged digital education in community rollout settings remains limited. A prospective stepped-wedge rollout across community clusters compared outcomes after exposure with outcomes in the pre-rollout period within the same cluster, using a fixed follow-up window ending 90 days after first contact. TB screening uptake was higher after exposure,

Addressing stigma and discrimination through ethical community-based TB policies.

Tuberculosis (TB) control is not only an issue for clinical intervention but also considered as one that requires ethics and community-responsive practices to reduce stigma and discrimination. In this research by using mixed methods, we will analyse how stigma affects patient's engagement with tuberculosis services and assesses whether policies that are applied in communities uphold principles of dignity, autonomy, and confidentiality. Quantitative survey data from 120 TB patients included four domains of stigma: self-stigma, perceived stigma, enacted discrimination and structural discriminati

Competency gaps in pediatric tuberculosis care and priorities for continuing education: Evidence from a national survey in China.

BACKGROUND: Early recognition of pediatric tuberculosis (TB) depends on frontline healthcare workers' ability to identify suspected cases. In China, pediatric TB is often under-recognized, and little is known about how physicians from different specialties and healthcare settings understand the disease. Understanding these gaps is important in improving continuing medical education (CME) and strengthening pediatric TB services. METHODS: We conducted a national, multicenter, cross-sectional survey from 2021 to 2022 using a 20-item questionnaire based on the WHO Module 5 to assess knowledge and

Screening and tuberculosis preventive treatment among household contacts of drug-resistant tuberculosis patients: a scoping review.

BACKGROUND: Drug-resistant tuberculosis (DR-TB) is associated with low treatment success rates and increased mortality. Household contacts (HHCs) face an elevated risk of infection and disease progression due to prolonged exposure. Consequently, systematic screening and tuberculosis preventive treatment (TPT) are critical to interrupt transmission within households. However, screening practices and TPT regimens vary substantially across countries and settings worldwide, and the available evidence remains fragmented. This study aims to synthesize evidence on screening strategies and TPT regimen

Comparison of VPM1002 with BCG in the prevention of tuberculosis in newborn infants: a multicentre, double-blind, randomised, phase 3, non-inferiority trial.

BACKGROUND: Although BCG provides protection against severe forms of tuberculosis in children, its efficacy against pulmonary tuberculosis in adolescents and adults is highly variable, and it offers limited and inconsistent protection against infection and transmission. VPM1002 is a recombinant BCG vaccine that showed manageable toxicity and immunogenicity in phase 1 trials in adults and in phase 2 trials in South African newborns. We compared VPM1002 with BCG for the prevention of Mycobacterium tuberculosis infection in infants. METHODS: This double-blind, randomised, active-controlled, phas

[Guidelines for off-label use of anti-tuberculosis drugs].

Tuberculosis (TB) remains a major global public health problem. According to the World Health Organization (WHO) Global Tuberculosis Report 2025, an estimated 10.7 million people developed TB and approximately 1.23 million died from the disease in 2024. China continues to face a high burden of both TB and multidrug-resistant/rifampicin-resistant tuberculosis (MDR/RR-TB). The introduction and expanded use of new and repurposed anti-tuberculosis drugs, including bedaquiline and linezolid, together with accumulating evidence supporting all-oral shorter regimens, have resulted in evidence-based us

Teacher-student relationship moderates the association between tuberculosis close-contact exposure and somatic symptoms among adolescents in Hangzhou, China.

BACKGROUND: Although tuberculosis (TB) control programs have primarily focused on infection prevention, diagnosis, and treatment outcomes, increasing attention has been directed toward the psychosocial consequences of TB-related experiences among affected populations. In school settings, adolescents identified as TB close contacts may experience concerns about infection risk, repeated health monitoring, activity restrictions, and disease-related stigma, which may represent important psychosocial stressors beyond the biological risk of infection. However, evidence regarding whether TB close-con
