Characterization of adult T-cell leukemia/lymphoma patients with specific skin lesions in a tertiary dermatological service in Brazil.

INTRODUCTION: Human T-lymphotropic virus type-1 (HTLV-1) is endemic in some countries, including Brazil. HTLV-1 is the etiological agent of adult T-cell leukemia-lymphoma (ATLL), a rare and aggressive CD4+ T-lymphocyte malignancy. ATLL affects 1-5% of virus carriers. Dermatological involvement occurs in 40-70%. Diagnosis is based on clinicopathologic correlation and HTLV-1 serology. There are few therapeutic options so far. METHODS: This is an observational retrospective cohort study with ATLL patients followed in a tertiary hospital in São Paulo, Brazil. Data were collected at diagnosis. Sur

Phylogenetic and Mutational Analysis of the Tax Gene in the Human T-Lymphotropic Virus 1-Associated HAM/TSP in Comparison with Asymptomatic Careers.

BACKGROUND: Human T-lymphotropic virus 1 (HTLV-1) is a member of the Retroviridae family that can cause two groups of diseases: HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP) and adult T-cell leukemia/lymphoma (ATLL). Despite HTLV having seven different subtypes, cosmopolitan subtype A is responsible for most HTLV-1-related disorders and is the most widely dispersed type globally, including in the Middle East, known to be an endemic area for the virus. Therefore, due to the importance of determining the subtypes of this virus, we aimed to explore the subtypes of HTLV-1 in

JunB-HBZ nuclear translocation by TGF-β is a key driver in HTLV-1-mediated leukemogenesis.

The HTLV-1 bZIP factor (HBZ) gene, which is the only viral gene conserved and consistently expressed in all adult T-cell leukemia-lymphoma (ATL) cases, is critical for ATL oncogenesis. Although HBZ protein is found in both the nucleus and the cytoplasm, the dynamics of HBZ protein localization and its contribution to oncogenesis have not been fully elucidated. In this study, we analyzed the subcellular expression pattern of HBZ in primary HTLV-1-infected T cells from asymptomatic carriers and leukemic cells of ATL patients using the Proximity Ligation Assay. Nuclear localization of HBZ protein

Advances in clonality analysis of human T-cell leukemia virus type 1-infected cells: from Southern blotting to high-throughput sequencing.

Human T-cell leukemia virus type 1 (HTLV-1) causes persistent infection and can lead to adult T-cell leukemia-lymphoma (ATL), a hematological malignancy with a poor prognosis. In ATL patients, monoclonal expansion of HTLV-1-infected cells is observed. Each HTLV-1-infected clone harbors a unique proviral integration site. Based on this feature, various methods have been developed to detect and characterize HTLV-1-infected clones. Traditional Southern blotting detects monoclonal proliferation; however, it lacks sensitivity for detecting minor clones. Polymerase chain reaction (PCR)-based approac

Sustained inflammation during human T-lymphotropic virus type 1 infection: a wildfire contributing to disease progression.

Human T-lymphotropic virus type 1 (HTLV-1) is a retrovirus affecting 10-20 million people worldwide. While many carriers remain asymptomatic, HTLV-1 infection can trigger intense inflammatory responses which are defined by the sustained release of pro-inflammatory cytokines and chemokines. Central to this process is the HTLV-1 encoded Tax oncoprotein, a viral regulator that drives uncontrolled inflammation by hijacking multiple cellular signaling pathways, such as the RelA/NF-κB signal transduction pathway. CD4 T-cells are the primary targets of Tax-mediated transformation, undergoing uncontro

A viral clonality evenness score to predict progression to adult T-cell leukaemia in asymptomatic carriers of human T-lymphotropic virus type 1 in Japan: a retrospective longitudinal cohort study.

BACKGROUND: Adult T-cell leukaemia/lymphoma (ATL) is a highly aggressive T-cell malignancy that occurs in approximately 2-7% of individuals with human T-lymphotropic virus type 1 (HTLV-1), after decades of asymptomatic infection. To address the urgent need for predictive biomarkers to identify asymptomatic carriers of HTLV-1 at high risk of progression to ATL, we aimed to evaluate viral clonality sequencing as a potential tool for risk stratification. METHODS: This retrospective longitudinal cohort study involved HTLV-1 carriers enrolled in the Joint Study on Predisposing Factors of ATL Devel

Metabolic dysfunction-associated steatotic liver disease in individuals with type 2 diabetes mellitus and diabetic foot ulcers: prevalence and associated factors.

BACKGROUND: Classical congenital adrenal hyperplasia (CAH) is characterized by a defect in cortisol and aldosterone secretion, adrenal hyperandrogenism, impaired adrenal medullary function and insulin insensitivity. The latter along with the increased tendency towards obesity raises questions whether other cardiovascular risk factors are altered in CAH. OBJECTIVE: To evaluate 24-h ambulatory blood pressure and obesity in patients with salt-wasting 21-hydroxylase deficiency diagnosed in the neonatal period and treated with hydrocortisone and 9alpha-fludrocortisone thereafter. Trends of changes in human T-cell leukemia virus type 1 epidemiology in Japan and globally.

Human T-cell leukemia virus type 1 (HTLV-1) has been identified as a cause of adult T-cell leukemia-lymphoma (ATL) and HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP). Despite several HTLV-1 endemic areas being identified, comprehensive investigations have yet to be conducted in all the regions of the world. This review aims to summarize the current reports on HTLV-1. As vertical transmission is known to be a risk factor for ATL development, prevention strategies have been initiated in Japan, and these efforts may be related to the decrease in the estimated number of HTLV-1

HTLV-1-infected cells drive the differentiation of monocytes into macrophages in vitro.

BACKGROUND: The human T-cell lymphotropic virus type 1 (HTLV-1) is a retrovirus that causes HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP). HAM/TSP is a chronic inflammatory neurodegenerative disease characterized by leukocyte infiltration in the spinal cord. T-lymphocytes are the most important targets of HTLV-1 infection, but monocytes are also infected. Monocytes from HTLV-1-infected individuals exhibit important functional differences compared to cells from uninfected donors. Here, we investigated the effects of cell-cell physical contact and/or secreted factors of HTL

Dysregulation of the tumor suppressor Menin and its target Bach2 in HTLV-1 infection.

BACKGROUND: The tumor suppressor Menin, prone to mutations in both hereditary and sporadic endocrine tumors, along with its direct target Bach2, plays a crucial role in preventing autoimmunity by regulating CD4 + T cell senescence and maintaining cytokine homeostasis. Since human T-cell leukemia virus type 1 (HTLV-1) primarily infects CD4 + T cells, and its dysregulation contributes to both the hematological malignancy of adult T-cell leukemia/lymphoma (ATL) and HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP), we examined the involvement of the Menin-Bach2 pathway in HTLV-1

An HTLV-1 carrier with sporadic late-onset nemaline myopathy accompanied by skin lesions indicating indolent adult T-cell leukemia/lymphoma and Sjögren's syndrome: a case report and literature review.

Sporadic late-onset nemaline myopathy (SLONM) is a rare acquired myopathy. Herein, we report a rare case of co-occurring SLONM, indolent adult T-cell leukemia/lymphoma (ATL), and Sjögren's syndrome (SS) in a 60-year-old woman who was a human T-cell leukemia virus type 1 (HTLV-1) carrier. She presented with sub-acute proximal muscle weakness and chest erythema. Muscle biopsy revealed perivascular inflammation and accumulation of nemaline bodies in the atrophic fibers. Skin biopsy indicated indolent ATL. Analysis of specific antibodies and salivary gland biopsy led to a SS diagnosis. CD4-positiv

HTLV-1 and ATLL: Epidemiology, Oncogenesis, and Opportunities for Community-Informed Research in the United States.

Human T-cell leukemia virus type 1 (HTLV-1), the first oncogenic human retrovirus, causes adult T-cell leukemia/lymphoma (ATLL), an aggressive neoplasm of mature CD4+ T-cells that is incurable in most patients and is associated with a median survival of less than 1 year. HTLV-1 also causes inflammatory disorders, including HTLV-associated myelopathy/tropical spastic paraparesis (HAM/TSP) and uveitis. The estimated lifetime risks of ATLL and HAM/TSP in HTLV-1 carriers are 3-5% and 0.25-1.8%, respectively. Although there is uncertainty about other health effects of HTLV-1, a recent meta-analysis

Neutralizing antibodies and the pathogenesis of HTLV-1: Insights from clinical cohorts.

Recent advances in mRNA-based vaccines have generated renewed interest in strategies to prevent Human T-Lymphotropic Virus type 1 (HTLV-1) infection. To explore this, we evaluated neutralizing antibody (NtAb) responses in 47 HTLV-1-infected individuals from Northwestern Argentina, an endemic region with high intrafamilial transmission and frequent cases of HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP). Using a syncytium-inhibition assay, we quantified NtAb titers across three groups: asymptomatic carriers (n = 17), oligosymptomatic individuals (n = 20), and HAM/TSP patien

IgG Idiotype Diversity Shapes Cytokine Profiles and Autoantibody Targets in HTLV-1 Clinical Outcomes.

Human T-lymphotropic virus type 1 (HTLV-1) infection is associated with a spectrum of clinical outcomes, ranging from lifelong asymptomatic carriage to severe conditions such as HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP) and adult T-cell leukemia/lymphoma (ATLL). Although antibody responses are known to shape immune regulation, the functional relevance of IgG idiotype repertoires in HTLV-1 pathogenesis remains poorly understood. This study investigated the immunomodulatory effects of IgG from individuals with distinct HTLV-1 clinical outcomes. IgG was purified from poo

Evidence of Spinal Cord Damage in Human T Cell Lymphotropic Virus Type 1-Infected Subjects Without Motor Disability.

Spinal cord (SC) narrowing has been documented in patients with human T cell lymphotropic virus type 1 (HTLV-1)-associated myelopathy (HAM). However, it remains uncertain whether individuals who present with lower urinary tract symptoms (LUTSs) without motor disability (probable HAM) exhibit damage to the central nervous system. These patients typically experience nocturia, urgency, incontinence, and, less frequently, an arreflexic bladder, which requires intermittent catheterization for voiding. The aim for the present study was to investigate the effectiveness of magnetic resonance imaging w

HTLV-1-Specific CTL Accumulation in CSF Contributes to Neuroinflammation in HTLV-1-Associated Myelopathy.

BACKGROUND AND OBJECTIVES: Human T-lymphotropic virus type 1 (HTLV-1)-associated myelopathy/tropical spastic paraparesis (HAM/TSP) is a progressive neurologic disorder characterized by chronic inflammation in the CNS. HTLV-1-specific cytotoxic CD8+ T lymphocytes (CTLs) play a crucial role in the pathogenesis of HAM/TSP; however, the dynamics of CTLs in the CSF remain poorly understood. The aim of this study was to investigate the accumulation of HTLV-1-specific CTLs in the CSF of patients with HAM/TSP and to evaluate their association with neuroinflammation and neural damage. METHODS: The fre

Highly homologous simian T-cell leukemia virus type 1 genome in Japanese macaques: a large cohort study.

BACKGROUND: Simian T-cell leukemia virus type 1 (STLV-1) is a retrovirus closely related to human T-cell leukemia virus type 1 (HTLV-1), the causative agent of adult T-cell leukemia (ATL). It has been shown that Japanese macaques (Macaca fuscata, JMs) are one of the main hosts of STLV-1 and that a high percentage of JMs (up to 60%) are infected with STLV-1; however, the molecular epidemiology of STLV-1 in JMs has not been examined. METHODS: In this study, we analyzed full-length STLV-1 genome sequences obtained from 5 independent troops including a total of 68 JMs. RESULTS: The overall nucle

The burden of HIV-1 and HIV-2 epidemics in Ivory Coast.

Simian immunodeficiency viruses (SIV) infecting chimpanzees (SIVcpz) and sooty mangabeys (SIVsm) are, respectively, the biological precursors of human immunodeficiency viruses (HIV) Types 1 and 2. Former French colonies in West Africa are the regions where retroviruses first jumped from primates to humans. Ivory Coast is nowadays a country of over 29 million people, being 2% (580,000) persons living with HIV (PLWH). However, one-quarter remains undiagnosed. Heterosexual transmission is by far the most frequent mechanism of HIV acquisition and women exhibit higher rates of infection than men. D
