A low-resource tracking workflow improves documentation and localizes the latent tuberculosis infection care gap in college student health.

OBJECTIVE: To assess a low-resource tracking workflow for improved documentation of latent tuberculosis infection (LTBI) treatment status and describe identified follow-up gaps in a Student Health clinic. PARTICIPANTS: Students with confirmed LTBI before (2018-2023; n = 82) and after (2024-2025; n = 147) implementation. METHODS: Retrospective before-after chart review. In 2024, the clinic added a tracking spreadsheet, nurse-initiated follow-up offering tuberculosis preventive treatment (TPT), 1-month dispensing, and standardized documentation. Outcomes were summarized descriptively. RESULTS

KLRG1+CD28⁻CD57+ Ag85A-specific CD8+ T cells mark a severity-associated terminal-differentiation imprint in pulmonary tuberculosis.

BACKGROUND: Pulmonary tuberculosis (PTB) is characterized by prolonged host-pathogen interaction, chronic granulomatous inflammation, and sustained exposure to Mycobacterium tuberculosis (Mtb) antigens. How Mtb antigen-specific CD8+ T cell differentiation states relate to clinical disease activity and severity remains incompletely defined. This study investigated the clinical relevance of a terminal differentiation-associated phenotype within Ag85A-specific CD8+ T cells across clinically defined states of Mtb infection and disease. METHODS: A total of 124 participants were enrolled, including

Comparative serum cytokine and chemokine profiles across tuberculosis, latent TB, HIV, and COVID-19 infections in Namibian adults.

BACKGROUND: The devastating pandemic of coronavirus disease 2019 (COVID-19) highlighted the importance of early detection and global collaboration in combating infectious diseases. Namibia is one of the countries that bears a high burden of tuberculosis (TB) and human immunodeficiency virus (HIV) infection. The overlap of these diseases underscores the need to better understand host immune responses, which play a critical role in disease pathogenesis and may differ across infections. This study aimed to profile selected cytokines and chemokines among Namibian individuals with TB, latent TB inf

Infectious, Environmental, and Chronic Disease Burden among 1,844 Newly Arrived Humanitarian Migrants in Oklahoma, 2019-2025.

Global displacement has brought increasingly diverse migrant populations into health systems with limited experience managing large-scale humanitarian arrivals, yet contemporary data describing infectious, environmental, and noncommunicable disease burden across heterogeneous newcomer groups remain scarce. A cross-sectional analysis of 1,844 consecutively screened newly arrived individuals who completed the U.S. domestic medical examination at the University of Oklahoma Center for Travel and Immigrant Health between October 2019 and May 2025 was conducted. Screening was performed in accordance

Global Survey of Latent Tuberculosis Screening and Treatment Practices in Liver Transplantation.

We surveyed transplant and infectious disease (ID) practitioners to better understand approaches to managing latent tuberculosis infection (LTBI) in liver transplantation (LT), an area in which high-quality evidence is lacking. The survey was distributed to clinicians via international ID society listservs, and one respondent per transplant center was requested. There were 86 respondents, including 78 (91%) ID physicians, from North America (68%), Europe (12%), South America (11%), Asia (7%), Africa (1%), and Australia (1%). Most centers (92%) screen all LT candidates for LTBI, and 95% use tub

Targeting Latent Tuberculosis Using Hypoxia-Activated Prodrugs of Bedaquiline Analogues.

The increasing emergence of antimicrobial resistance (AMR) is reducing the effectiveness of existing antibiotics against bacterial infections, including tuberculosis (TB), a deadly disease caused by Mycobacterium tuberculosis (M.tb). Approximately 90% of infected individuals develop latent tuberculosis infection, for which current treatment options are limited. Latent tuberculosis infection is characterised by the formation of hypoxic granulomas, which restrict oxygen and drug penetration, highlighting the need for therapies effective under hypoxic conditions. In this study, a hypoxia‑activate

Diagnostic accuracy of Mycobacterium tuberculosis antigen-induced interferon-γ-inducible protein 10 for latent tuberculosis infection among high-risk groups: a systematic review and meta-analysis.

BACKGROUND: Latent tuberculosis infection (LTBI) affects nearly a quarter of the world's population, yet current diagnostic tools are suboptimal. Interferon-γ-inducible protein 10 (IP-10) has been proposed as a candidate biomarker, but its diagnostic value for LTBI among high-risk groups is unclear. To address this, we performed a systematic review and meta-analysis to assess the accuracy of Mycobacterium tuberculosis (Mtb) antigen-induced IP-10 for detecting LTBI among high-risk groups. METHODS: PubMed, Web of Science, Embase, Scopus, and the Cochrane Library were searched up to April 2026.

Tuberculosis Risk in the Returning Traveler: Assessing Tuberculosis Test Conversion Among Routinely Tested Patients at an Asian American Health Center.

BACKGROUND: In the United States, testing for latent tuberculosis (TB) infection is recommended for persons from higher-TB-prevalence countries, but guidelines do not address repeat testing for persons with international travel. METHODS: Using 2010-2022 data from a Californian federally qualified health center serving an Asian American population, we measured the incidence of TB test conversion (negative to positive result) and proportion of test converters receiving treatment among routinely tested patients (ie, patients with no history of TB infection, at least one prior negative result, an

[The role of therapeutic education in the care of tuberculosis patients].

Tuberculosis is a major cause of death worldwide. It affects mainly low- and middle-income countries, but also persists in vulnerable populations in high-income countries. How well tuberculosis treatments work depends on strict adherence, which is compromised by the length of the protocols, the adverse effects of the drugs, and unfavourable psychosocial factors. Therapeutic education aims to strengthen the self-management skills, autonomy, and quality of life of patients with tuberculosis. It applies to both latent tuberculosis infection and active tuberculosis and improves understanding of th

Neutrophil CD64 index for diagnosing and tracking treatment response across the mycobacterial infection spectrum: a retrospective two-cohort study.

BACKGROUND: The neutrophil CD64 (nCD64) index has emerged as a promising biomarker for infection detection, but its evaluation within the mycobacterial infection spectrum remains limited. Here, we evaluated the diagnostic and treatment response performance of the nCD64 index in the active tuberculosis (ATB)-including pulmonary tuberculosis (PTB) and extrapulmonary tuberculosis (ePTB), and non-tuberculous mycobacterial (NTM) disease, and determined whether it adds value beyond standard inflammatory indicators. METHODS: We measured the nCD64 index in a cross-sectional cohort comprising 248 part

Māori notified with tuberculosis in one public health region of Aotearoa New Zealand: a document review.

AIM: Among Aotearoa New Zealand-born people with tuberculosis (TB), half are Māori. TB is preventable, and Aotearoa New Zealand has signed up to the global strategy to end the TB epidemic. TB among Māori has endured since the late 1800s and occurs in a complex political, social and wellbeing context. We aimed to review current public health practices used for contacting and following up Māori with TB and their contacts. METHODS: We undertook a retrospective case review of Māori notified with pulmonary TB (2018- 2023) in the Waikato Region. Data included TB notification, treatment start and en

Optimizing diagnosis of Mycobacterium tuberculosis latent infection: ELISpot as a technique of choice for samples with high negative control values in QuantiFERON assay.

A quarter of the world's population is considered to be affected by latent tuberculosis infection (LTBI) by the World Health Organization, which recommends that individuals with LTBI receive anti-tuberculosis treatment, aiming to avoid progression to active tuberculosis and transmission. Moreover, patients undergoing immunosuppressive therapy, such as anti-TNFα antibodies, constitute a growing high-risk group for which LTBI screening prior to treatment initiation is strongly advised. Improving LTBI diagnosis and patient management relies on the accuracy of broadly deployed laboratory tests, su

Identification of Plasma Immune markers Associated with Latent Tuberculosis Conversion in HIV-Infected Individuals who received isoniazid preventive therapy.

Tuberculosis remains a major risk for people living with HIV, even those on antiretroviral therapy, due to increased susceptibility to latent TB infection. Identifying immune signatures that distinguish progression from resistance is critical for developing predictive biomarkers and targeted prevention strategies. In this study, 80 HIV-infected adults on antiretroviral therapy who received isoniazid preventive therapy (IPT) were followed for six months and categorized as Progressors, Sustainers, Converters, or Resisters based on serial QuantiFERON-TB results. Plasma levels of cytokines, chemok

Nativity as a Clinical Variable in Gastric Cancer Prevention: An Immigrant Health Perspective.

Marked differences in gastric cancer risk among immigrant populations have long been recognized, yet migration-related information remains underutilized in routine clinical care. Nativity is more than a demographic characteristic; it serves as a pragmatic surrogate for cumulative life-course exposures, including early-life environmental conditions, Helicobacter pylori acquisition, dietary patterns, and socioeconomic influences that shape gastric cancer risk long after migration. Although birthplace and exposure history are routinely incorporated into prevention strategies for conditions such a

A Multi-Scale Computational Framework for Anti-Persister Tuberculosis Drug Discovery.

Latent tuberculosis (TB), caused by dormant, non-replicating Mycobacterium tuberculosis (Mtb), afflicts an estimated 1.7 billion individuals globally and remains a principal barrier to TB eradication. Dormant bacilli are phenotypically tolerant to conventional antibiotics and survive through continued dependence on oxidative phosphorylation (OXPHOS) and the electron transport chain (ETC), making bioenergetic enzymes, including the F1Fo-ATP synthase, cytochrome bcc:aa3 supercomplex, Type II NADH dehydrogenase (NDH-2) and cytochrome bd oxidase, compelling drug targets.
