Combined reduced-antigen-content diphtheria-tetanus-acellular pertussis and polio vaccine (dTpa-IPV) for booster vaccination of adults.

Many countries recommend diphtheria, tetanus and/or poliomyelitis boosters in adolescents or adults and the need for pertussis booster vaccination beyond childhood is increasingly recognized. A new combined reduced-antigen-content dTpa-IPV vaccine provides booster vaccination against all four diseases in one single injection. The immunogenicity and safety of the dTpa-IPV vaccine was compared to that of licensed dTpa+IPV or Td-IPV vaccines in 806 adolescents >14 years of age and adults with a heterogeneous vaccination history. The dTpa-IPV vaccine was immunogenic and well tolerated. No clinical

[Global eradication of poliomyelitis. On the 80th anniversary of the founding of the National Institute of Health].

Global eradication of poliomyelitis launched and coordinated by the World Health Organization since 1988 is close to being achieved: the main objective is to discontinue the circulation of the wild, neurovirulent, endemic virus of poliomyelitis. Although this objective has been within the reach, there are two other risks, lower but existing and posing possible threat to the eradication, that need to be controlled: 1) wild polioviruses stored in laboratories should be destroyed or reliably contained and 2) emergence and epidemic role of neurovirulent polioviruses potentially derived from attenu

Risks and benefits of immunizing pregnant women: the risk of doing nothing.

The medical, social and legal risks of immunizing pregnant women are obstacles preventing the initiation of programs to immunize women for their protection and for their infant's protection. Recent projects devoted to vaccine development have focused on protecting newborns and infants. But there are many other reasons for developing or utilizing vaccines before or during pregnancy, beyond the protection of the newborn. Besides the usual reasons for utilizing immunizations to protect the mother and the neonate, the threat of bio-terrorism adds a new dimension to the necessity for addressing thi

Prevalence of dental caries in 3- and 5-year-old children living in a small Brazilian City.

Despite the reduction in the prevalence of dental caries, very little is known about the prevalence of the disease, especially in deciduous teeth, in small Brazilian cities (<20,000 inhabitants). The aim of this study was to evaluate the prevalence of dental caries in children aged 3 and 5 years of age in the city of Cambira - State of Parana. All children that participated in the national vaccination campaign against poliomyelitis in 2002 at the two basic health units of the city were examined. The campaign covered 99.5% of the population aged 1 to 5 years. Two previously trained and calibrat

The role of routine polio immunization in the post-certification era.

The role of routine vaccination against poliomyelitis for the post-certification era remains an important area for policy decision-making. Two critical decisions need to be taken: first, to continue or discontinue vaccination with the live attenuated oral poliovirus vaccine (OPV); and second, if OPV is to be discontinued, whether vaccination with inactivated poliovirus vaccine (IPV) is needed. Four potential vaccination scenarios can be constructed: stop all polio vaccination; continue with current vaccination policies (OPV, IPV, or sequential schedule); discontinue OPV, but continue IPV unive

Framework for evaluating the risks of paralytic poliomyelitis after global interruption of wild poliovirus transmission.

With the interruption of wild poliovirus transmission globally, the need for new policies to deal with the post-certification era will rapidly arise. New policies will be required in four areas: detection and notification of circulating polioviruses; biocontainment of wild, vaccine-derived and attenuated strains of poliovirus; vaccine stockpiles and response mechanisms; and routine immunization against polioviruses. A common understanding of the potential risks of paralytic poliomyelitis in the post-certification period is essential to the development of these policies. Since 2000, there has b

Polio control after certification: major issues outstanding.

Now that the global eradication of wild poliovirus is almost within sight, planning for the post-certification era is becoming a priority issue. It is agreed that a stockpile of appropriate polio vaccines will need to be established, and a surveillance and response capacity will need to be maintained, in order to protect the world against any possible future outbreaks attributable either to the persistence of wild poliovirus or vaccine-derived polioviruses (VDPVs) or to the unintentional or intentional release of poliovirus from a laboratory or vaccine store. Although it has been suggested tha

[Adult immunization in France: an update].

French immunisation schedule recommends tetanus and poliomyelitis vaccine every 10 years and influenza every year after the age of 65. Several other vaccinations related to occupational health are implemented. Travellers or immunocompromised people who could be exposed to infections need some other vaccinations, too. Pneumoccocal vaccine coverage is insufficient. Reinforcing diphteria immunization is considered, using a recently licensed combined vaccine. Maintenance of immunity against "childhood infectious diseases" preventable with vaccinations is a new challenge; measles, rubella and pertu

Global public health mandates in a diverse world: the polio eradication initiative and the expanded programme on immunization in sub-Saharan Africa and South Asia.

BACKGROUND: The circulation of wild poliovirus is expected to cease soon due to the success of the global polio eradication initiative. Thereafter, intensified polio eradication efforts such as National Immunisation Days (NIDs) will most likely be discontinued. As a consequence, the expanded programme on immunization (EPI) will no longer enjoy extra inputs from the polio eradication initiative. We investigated whether today's EPIs are ensuring universal and equitable vaccine coverage; and whether the removal of extra inputs associated with the implementation of NIDs is likely to affect EPI cov

Exotic and emerging viral encephalitides.

PURPOSE OF REVIEW: The exotic and emerging viral encephalitides are caused by animal or human viruses and characterised by sudden unexpected outbreaks of neurological disease, usually in tropical and sub-tropical regions, but sometimes spreading to temperate areas. Although a wide range of viruses come within this label, as this review highlights, there are common research questions as to the origin and spread of the viruses, the contribution of viral and host factors to the clinical presentations and outcome, and the possibilities for treatment and vaccination. RECENT FINDINGS: During 2002,

Adults receive several vaccinations related to occupational health. Travellers or immunocompromised people who are exposed to infections need some other vaccinations, too. People older than 65 receive influenza vaccine every year. Tetanus and poliomyelitis immunity should be maintained with a decennial injection following adult immunisation schedule but the application of this vaccine remains rather erratic. Diphtheria valence included in a recently licensed combined vaccine could be done together. Maintenance of immunity against "childhood infectious diseases" preventable with vaccinations is

Introduction of inactivated poliovirus vaccine into oral poliovirus vaccine-using countries.

Given the progress already made towards the goal of global eradication of poliomyelitis LE (polio), the risk of paralytic poliomyelitis is changing in many geographical areas. Vaccination against polio will need to continue because of the threat of wild poliovirus importation. However, an increasing number of polio-free countries are determining that the risk of paralytic poliomyelitis associated with continued routine immunization using oral poliovirus vaccine (OPV) is greater than the risk of importation or laboratory handling of wild poliovirus. Some of these countries have introduced inact

Vaccination priorities.

Selection of immunizations should be based on requirements and on risk of infection. According to the International Health Regulations, many countries require yellow fever vaccination and proof thereof as the International Certificate of vaccination. Additionally selected countries require proof of vaccination against cholera and meningococcal disease. A consultation for travel health advice is always an opportunity to ascertain that routine immunizations have been performed. Recommended immunizations often are more important for traveller's health than the required or routine ones. The most f

Current immunizations for travel.

PURPOSE OF REVIEW: International travelers may be at risk from a variety of potentially severe and life-threatening infections. Some of these diseases are preventable, and vaccination remains a cornerstone of travel medicine. Vaccines that are important for international travel are reviewed, in a succinct update based on the most recent literature. RECENT FINDINGS: Discussed are vaccines for enteric infections (polio, cholera, hepatitis A, and typhoid), as well as those for hepatitis B, Japanese encephalitis, yellow fever, and meningococcal vaccines. The controversial end to the polio eradica

[Update on childhood immunization in Switzerland].

The recommendations for routine childhood immunizations undergo continuous re-evaluation and are periodically amended. As a result of the near complete elimination of wild-type poliomyelitis, the oral poliomyelitis live vaccine (OPV) has been replaced by the inactivated vaccine (IPV) for the primary series in infancy. Recent seroepidemiologic data on the dynamics of decay of maternal measles antibodies in infants may lead to the recommendation of administering the first dose of the measles mumps rubella (MMR) vaccine at twelve months rather than at 15 to 23 months of age. The recently introduc

Study of operational aspects of pulse polio booths during Intensified Pulse Polio Immunisation Campaign in assembly segments of East Delhi.

The study was undertaken with the objective of observing operational aspects of 609 pulse polio booths established during Intensified Pulse Polio Campaign in East Delhi on National Immunisation Days (NID) during 2001-2002 through well designed format. The average number of persons present on duty at polio booths was 3.73 against a recommendation of 4 per booth. All the booths had vaccine carriers but some of them were not following the guidelines laid for maintaining the cold chain. One sixth "(15.4%)" of the booths needed fresh supplies of vaccine carrier. There is need for deployment of moti

Poliovirus vaccines. Progress toward global poliomyelitis eradication and changing routine immunization recommendations in the United States.

Poliomyelitis prevention in the United States has relied virtually exclusively on OPV during the past 30 years. Starting in 1997, a major change in the poliomyelitis vaccination policy occurred, facilitated by substantial progress toward worldwide poliomyelitis eradication. A sequential schedule of IPV followed by OPV became the preferred means to prevent poliomyelitis, although an all-OPV and an all-IPV schedule were considered acceptable alternatives. In 1999, two doses of IPV were recommended to start the primary series, followed by two doses of either poliovirus vaccine. As of January 2000

[Opinions and attitudes on the topic of vaccination of established physicians, pharmacists and their personnel and current vaccination status of these groups].

Object of this investigation was to evaluate attitudes and opinions of general practitioners, internal specialists and pharmacists with regard to vaccination. At the same time we investigated the actual vaccination state of these groups and of the medical staff and pharmacy staff. Physicians and pharmacists answered by using a four-page written questionnaire. The vaccination state was evaluated by a single-page questionnaire. Analyses were made by using the statistical program SPSS. The 161 physicians and 188 pharmacists participating in this study generally agree that the risk of infection in

Trends and determinants of immunisation coverage in India.

The 20th century has witnessed many important events in the control of infectious diseases that mostly affect children. In addition to the eradication of smallpox, the interruption of poliomyelitis transmission in many countries with a distinct possibility of its eradication by the turn of this century are some of the major achievements. Also, the rates of other vaccine preventable diseases such as measles, pertussis and diphtheria have gone down significantly. The discovery and use of vaccines have made it possible to save approximately 8 million deaths, annually. This is in addition to the r

[First confirmed case of laryngeal diphtheria in Djibouti].

The first bacteriologically confirmed case of laryngeal diphtheria in Djibouti was reported in 1998. It involved a three-year-old native-born infant who had been vaccinated during the first year of life with three doses of a combined vaccine against diphtheria, tetanus, poliomyelitis, and pertussis. A rapid clinical improvement was observed under erythromycin treatment. Other cases of laryngeal diphtheria have been observed. It is important to reverse decreasing vaccinal coverage in Djibouti and to warn incoming travelers of the need to be adequate immunized against diphtheria. Enhanced epidem
