Impact of postoperative hyperamylasemia and post-pancreatectomy acute pancreatitis following pancreaticoduodenectomy in a single-center cohort.

BACKGROUND: The clinical significance of postoperative hyperamylasemia (POH) and post-pancreatectomy acute pancreatitis (PPAP) following pancreaticoduodenectomy (PD) is understudied. Hence, we aimed to determine the incidence, risk factors, and short-term clinical impact of post-PD POH and PPAP. METHODS: A retrospective, single-center, observational study was conducted with consecutive adult patients who underwent surgery between January 2012 and September 2025. POH and PPAP were defined and graded using the 2022 International Study Group of Pancreatic Surgery (ISGPS) consensus criteria. RES

Transmural migration of an internal pancreaticojejunostomy stent causing delayed intra-abdominal sepsis: a case report.

BACKGROUND: Internal pancreatic duct stents are commonly used during pancreaticojejunostomy (PJ) reconstruction following pancreaticoduodenectomy to facilitate drainage and potentially reduce postoperative pancreatic fistula (POPF). These stents are typically expected to migrate distally and pass without consequence. However, rare complications related to stent migration may occur and can present with atypical clinical and biochemical findings, creating diagnostic challenges. CASE DESCRIPTION: A 63-year-old woman underwent pancreaticoduodenectomy for pancreatic adenocarcinoma with placement o

Right Coronary Artery Thrombosis Causing ST-Elevation Myocardial Infarction After a Coronary Artery Disease Reporting and Data System Score of 0, With Disseminated Intravascular Coagulation During Idiopathic Acute Pancreatitis: A Case Report.

Acute pancreatitis may cause electrocardiographic abnormalities and troponin elevation that mimic acute myocardial infarction. True coronary thrombosis is less common. The development of disseminated intravascular coagulation shortly after percutaneous coronary intervention creates a difficult balance between bleeding control and prevention of early stent thrombosis. A 68-year-old man with no previous pancreatitis presented with acute interstitial pancreatitis. Evaluation did not identify biliary disease, alcohol use, hypertriglyceridemia, hypercalcemia, medication exposure, autoimmune pancrea

Ultrathin endoscope-assisted pancreatobiliary interventions: from direct transpapillary access to created access routes.

BACKGROUND: ERCP is the standard endoscopic approach for pancreatobiliary diseases, but it relies mainly on fluoroscopic assessment and cannot provide direct intraductal visualization. Ultrathin upper gastrointestinal endoscopes, originally developed for transnasal or pediatric endoscopy, have therefore been used as forward-viewing platforms for direct observation and therapy in the bile duct, pancreatic duct, gallbladder, and peripancreatic cavities. This review summarizes ultrathin endoscope-assisted interventions through both natural and created access routes, excluding dedicated mother-bab

Improving predictive accuracy for surgical indication of the current management guideline for intraductal papillary mucinous neoplasm of the pancreas via cutoff adjustments.

OBJECTIVE: To improve the predictive accuracy of international guidelines for malignant pancreatic intraductal papillary mucinous neoplasm (IPMN) via cutoff adjustments. MATERIALS AND METHODS: We retrospectively analyzed surgically confirmed IPMN patients at a single center between 2011 and 2023. Preoperative MRI were reviewed based on international guidelines. We calculated sensitivity and specificity of high-risk stigmata (HRS). Receiver operating characteristic analyses for enhancing mural nodule (EMN) size and main pancreatic duct (MPD) diameter identified cutoffs achieving specificities

Chyle Leak Following Pancreatic Resection: A Potential Contributor to Delayed Postoperative Recovery.

Chyle leaks following pancreatic resection are often considered less clinically significant than postoperative pancreatic fistulas (POPFs), despite their potential to delay recovery. We evaluated postoperative outcomes associated with chyle leak and explored differences in morbidity relative to POPF. A retrospective review of consecutive pancreatic resections performed at a single institution between 2017 and 2022 was conducted. Among 369 patients, 24 (6.5%) developed chyle leaks, including 16 (4.3%) isolated chyle leaks and 8 (2.2%) combined chyle and pancreatic leaks. Isolated chyle leaks we

Enucleation vs Pancreatoduodenectomy for Benign and Low-Grade Pancreatic Head Tumors.

IMPORTANCE: Pancreatic enucleation may offer a parenchyma-sparing alternative to pancreatoduodenectomy for benign and low-grade malignant pancreatic head tumors, but its perioperative risk-benefit profile relative to pancreatoduodenectomy remains incompletely defined. OBJECTIVE: To compare short-term outcomes and long-term quality of life after enucleation vs pancreatoduodenectomy for benign and low-grade malignant tumors of the pancreatic head and uncinate process. Mimicking Polycystic Liver Disease: A Case Report of Innumerable Cystic Hepatic Metastases From a Pancreatic Neuroendocrine Tumour and Diagnostic Role of Endoscopic Ultrasound.

Pancreatic neuroendocrine tumours (PNETs) are malignant processes and are identified as being the rarest of all pancreatic cancers. Here we present the case of a 62-year-old male with a background of chronic lymphocytic leukaemia (CLL) who was investigated over 18 months for progressive hepatic cystic lesions with no known aetiology. Imaging revealed main pancreatic duct dilatation without an identifiable intrinsic or extrinsic cause, coupled with enlarged mesenteric lymph nodes. Endoscopic ultrasound identified multiple cystic liver lesions of different sizes, some reflecting the presence of

Recurrent rupture of a pancreatic mucinous cystic neoplasm with suspected main pancreatic duct communication, presenting with a mediastinal pancreatic pseudocyst, pancreatic pleural effusion, and pericardial effusion.

Spontaneous rupture of pancreatic mucinous cystic neoplasm (MCN) is rare, particularly outside pregnancy, and mediastinal extension has not been reported. We report a 34-year-old woman with a pancreatic tail MCN demonstrating continuity with the main pancreatic duct on computed tomography (CT), suggesting ductal communication. The neoplasm ruptured dorsally, and cyst fluid extended through the aortic hiatus, resulting in a mediastinal pancreatic pseudocyst, pancreatic pleural effusion, and pericardial effusion. Pleural fluid amylase was 1541 IU/L. Endoscopic ultrasonography showed a cyst-in-cy

Repair of Main Pancreatic Duct Injury during Surgery for Congenital Biliary Dilatation in a Child: A Case Report.

INTRODUCTION: In surgery for congenital biliary dilatation, it is essential to minimize residual intrapancreatic bile duct tissue to prevent future complications, such as intrapancreatic stones and carcinogenesis. However, dissection of the intrapancreatic bile duct is technically demanding and may result in injury to the main pancreatic duct. As a result, very few cases have been reported regarding the management of this complication in pediatric patients. We present a pediatric case of intraoperative main pancreatic duct injury and describe its surgical management. CASE PRESENTATION: A 3-ye

Dorsal Pancreatic Agenesis Associated With New-Onset Diabetes Mellitus and Acute Pancreatitis in an 18-Year-Old Man: A Case Report.

Dorsal pancreatic agenesis (DPA) is a rare congenital disorder resulting from failure of development of the dorsal pancreatic bud. Complete DPA implies absence of the pancreatic neck, body, and tail and can be associated with both diabetes mellitus and pancreatitis. The patient was an 18-year-old man admitted with a history of epigastric pain, nausea, vomiting, polyuria, polydipsia, and unintentional weight loss of 4 kg over 8 weeks. The lab investigations included elevated serum glucose (412 mg/dL), urine ketones 3+, venous pH 7.31, bicarbonate 17.2 mEq/L, anion gap 14.8 mEq/L, lipase 486 U/L

Spontaneous Bacterial Peritonitis as a Rare Complication of a Pancreatic Duct Leak in a Patient Without Cirrhosis: A Case Report.

Spontaneous bacterial peritonitis (SBP) is classically associated with liver cirrhosis and portal hypertension. Its occurrence in non-cirrhotic patients is rare and poorly characterized. Pancreatic ascites, resulting from disruption of the pancreatic duct (PD) or leakage from a pseudocyst, is an unusual cause of ascites in chronic pancreatitis. The enzyme-rich, exudative nature of this fluid can itself cause chemical peritonitis with neutrophilia, mimicking infection. We report the case of a 50-year-old man with alcohol use disorder and chronic calcific pancreatitis who presented with recurren

Discordant pathology following pancreatoduodenectomy for suspected intraductal papillary mucinous neoplasm: reassessment of cyst fluid genetic analysis.

A molecular analysis of pancreatic cyst fluid is revolutionizing the clinical management of pancreatic cysts. Yet, there remains need for ongoing correlation of molecular testing with surgical pathology. An ~70-year old male was found to have focal main pancreatic duct dilation up to 7 mm at the pancreatic head/neck on imaging without any discrete mass lesions. Duct fluid analysis found elevated carcinoembryonic antigen and decreased glucose level suggestive of intraductal papillary mucinous neoplasm (IPMN). Yet, molecular testing revealed no mutations. In the absence of a mass lesion, he was

Lipomatous pseudohypertrophy of pancreas with chronic calcific pancreatitis presenting with nutritional dermatosis and type 3c diabetes.

A woman in her early 50s presented with a 4-month history of erythema and scaling of the limbs, weight loss, anasarca and chronic foul-smelling diarrhoea, suggestive of nutritional dermatosis with steatorrhoea due to malabsorption. She was non-obese and had no prior history of diabetes or exocrine deficiency. During admission, she experienced abdominal pain and hyperglycaemia consistent with diabetic ketoacidosis. CT of the abdomen revealed a diffusely enlarged, entirely fatty pancreas with preserved shape and a dilated main pancreatic duct containing intraductal calculi, indicating lipomatous

Duplicated ventral pancreatic ducts arising separately from the duodenal wall: anatomical evidence suggestive of persistence of the left ventral pancreatic bud.

PURPOSE: Human embryological studies have documented the transient presence of paired ventral pancreatic buds during early development. The persistence of the left ventral bud into adulthood could theoretically result in duplicated ventral pancreatic ducts in adulthood. Here, we report a case of duplicated ventral ducts incidentally observed in a human pancreas retrieved from an organ donor. ANATOMIC VARIATION: The pancreas was obtained from a 46-year-old male donor after brain death for the purpose of islet transplantation. During the islet isolation procedure, we unexpectedly identified two

Risk Analysis of Pancreatitis With Pancreatolithiasis in Relation to Dyslipidemia as a Cofactor.

Background Pancreatic duct stones are a frequent structural complication of chronic pancreatitis and may contribute to recurrent attacks and progressive pancreatic damage. Dyslipidemia is associated with metabolic dysfunction and may influence the clinical course of pancreatic disease. However, the relationship between dyslipidemia and clinical outcomes among patients with chronic pancreatitis complicated by pancreatic duct stones remains incompletely defined. This study evaluated the association between dyslipidemia and clinical characteristics and short-term outcomes in these patients. Metho

Serological Screening of Coxiella burnetii Phase II IgG Antibodies for the First Trimester of Ante-natal Women Using ELISA and Immunofluorescent Assay (IFA).

OBJECTIVES: Both liver fibrosis and osteoporosis share inflammatory pathways, with liver fibrosis potentially contributing to decreased bone mineral density (BMD). The rising prevalence of non-alcoholic fatty liver disease (NAFLD) and associated liver fibrosis, especially in older populations, may increase the risk of osteoporosis, but evidence remains inconclusive. This study aims to investigate the relationship between liver fibrosis and osteoporosis in individuals over 50 years old. METHODS: This cross-sectional study used data from the Bushehr Elderly Health Program (BEHP), a cohort of 2,

OBJECTIVE: Endoscopic retrograde cholangiopancreatography (ERCP) is a widely utilized endoscopic technique for the diagnosis and therapy of biliary and pancreatic diseases. It usually leads to different complications including post-ERCP pancreatitis (PEP). This study aimed to evaluate the correlation between miR-30c-5p rs928508 polymorphism and the risk of PEP and to analyze the effect of it on serum miR-30c-5p level. MATERIALS AND METHODS: Patients with PEP were collected as a case group, while patients without complications were gathered as a control group. miR-30c-5p levels were measured u
