What do we mean by preconception health and preconception care in research and policy? A systematic review.

BACKGROUND: Preconception health (PCH) is a globally accepted strategy to reduce preventable adverse pregnancy outcomes and ultimately improve the health of the unborn child. Optimal PCH can be achieved through preconception care (PCC), which encompasses the behavioural, biomedical, and social interventions women and couples undertake and/or receive before conception. However, there is a lack of clarity on various aspects of PCH and PCC, such as what constitutes preconception risk factors, what the optimal interventions are, when the preconception period is, and who the overall target populati

Development of a melt-cast microneedle patch for sustained drug release of islatravir.

Low adherence to daily oral drug regimens is a significant obstacle in effectively managing chronic therapies like HIV treatment, pre-exposure prophylaxis (PrEP), and contraception. While long-acting injectables eliminate the need for daily compliance, they often require administration by healthcare professionals. To facilitate self-administration of long-acting treatments, we developed a microneedle patch (MNP) for the sustained release of islatravir or levonorgestrel, used for HIV treatment/PrEP or contraception, respectively. The MNPs were produced using a melt-casting technique to quickly

Downplaying reality: is the UN population fund forgetting population growth and family planning?

BACKGROUND: The global population is projected by the UN Population Division to continue growing by over 2 billion, from 8.1 in 2024 to 10.3 in 2084, aggravating climate change and biodiversity loss, raising risks of poverty and starvation, and impairing reproductive health in many developing countries, especially in Africa. OBJECTIVE: Evaluation of the population policy of a major international actor in reproductive health, the United Nations Population Fund (UNFPA), and presentation of alternative and complementary policy addressing population growth, high fertility rates and unmet need for

Translation, Adaptation, and Validation of the French Version of the Low Literacy Decisional Conflict Scale in Adolescents.

UNLABELLED: Purpose. We aimed 1) to translate and adapt the English version of Low Literacy Decisional Conflict Scale (LL-DCS) into French and 2) to assess its psychometric properties in adolescents. Methods. The original LL-DCS was translated into French by 2 independent translators, with discrepancies resolved by a multidisciplinary team. The reconciled version was back-translated into English by 2 other translators to ensure accuracy. We tested the prefinal version among 14 adolescents, who evaluated the clarity of the items. Second, we conducted psychometric testing of the final version am

Leveraging connections between family planning and nutrition to improve the health of women.

The global burden of malnutrition and unmet need for family planning (FP) among women of reproductive age remains high. Leveraging the epidemiological and programmatic links between FP and nutrition could help improve outcomes in both domains for women, with wider benefits to health and well-being. Here, we draw on findings from our recently concluded Family Planning and Nutrition Project, which synthesised epidemiological and programmatic linkages between FP and nutrition, alongside other related literature, to argue that limited research connecting FP and nutrition is constraining concrete i

Natural history of focal nodular hyperplasia on gadoxetic acid-enhanced MRI: a multicenter cohort study.

PURPOSE: To investigate the natural history and longitudinal imaging evolution of focal nodular hyperplasia (FNH) using gadoxetic acid-enhanced MRI (EOB-MRI) in a multicenter cohort. METHODS: This retrospective multicenter cohort study enrolled patients with FNH who underwent both baseline and follow-up EOB-MRI. Lesion evolution was assessed using three complementary metrics: absolute diameter change (threshold ≥ 0.5 cm), relative percentage change (threshold ≥ 10%), and annual diameter change (cm/year). Hepatobiliary phase (HBP) phenotype transitions from baseline to final follow-up were ass

When Prevention is an Emergency: The Imperative of Contraception Initiation in the Emergency Department.

BACKGROUND: Millions of U.S. women live in contraceptive deserts, with limited or no access to comprehensive reproductive health services. Barriers are intensified by Medicaid restrictions, provider shortages, and growing reproductive health inequities. The emergency department (ED), as the healthcare safety net, is uniquely positioned to expand contraceptive access. OBJECTIVES: To highlight the public health need for ED-initiated contraception, describe feasible approaches for implementation, and address common challenges to integrating contraception into emergency care. DISCUSSION: Women o

On-site IUD implementation in a student-run free clinic.

Uninsured populations face disproportionately high rates of unplanned pregnancy due to financial and structural barriers to effective contraception. While long-acting reversible contraception (LARC), such as intrauterine devices (IUDs), offers high efficacy for pregnancy prevention, implementation of IUDs within independent student-run free clinics (SRFCs) can be limited by a lack of hospital affiliation or government funding. This community case study details a sustainable model for integrating IUD services into an independent SRFC in Northern Illinois. By partnering with the humanitarian org

Clinical Effects of the U.S. Food and Drug Administration's Newest Restrictions on the Etonogestrel Implant.

In 2026, the U.S. Food and Drug Administration introduced a new Risk Evaluation and Management Strategy (REMS) for the etonogestrel contraceptive implant. Although the intent was to maintain safe practices for device insertion and removal, the large body of long-standing safety data should raise questions about the need for these new requirements. In the absence of safety concerns regarding device insertion and removal, a REMS requirement on the etonogestrel contraceptive implant may lead to decreased reproductive health care access and provision.

Interests and preferences regarding family planning self-care interventions: cross-sectional surveys with Kenyan and Nigerian women.

Self-care interventions for family planning encourage women to play a central role in their own health care. This study assesses women's interests and preferences related to mobile access to information and products amenable to self-administration, and examines factors associated with preferred place of access. We added a module to the female questionnaire of the cross-sectional Performance Monitoring for Action survey in Kenya (n = 9,489) and in two Nigerian states (Kano (n = 1,144) and Lagos (n = 1,426)) between November 2021 and January 2022. We analysed data on interests and preferences re

Contraceptive Needs of the Transmasculine Patient.

Patient preference plays a big role in deciding which contraceptive option is right for them. Testosterone, while not a contraceptive, can induce amenorrhea, but it does not reliably inhibit ovulation, so pregnancy is still possible if transmasculine patients are exposed to semen. No contraceptive option is considered superior from a dysphoria standpoint and shared decision-making should be used to determine the right choice for the patient. Estrogen-containing contraceptives tend to be less frequently utilized given perceived dysphoria from taking estrogen. Outside of other underlying conditi

Socioeconomic, Demographic, and Environmental Determinants of Hemoglobin Levels Among Women: A Multilevel Analysis in South and Southeast Asia.

Anemia, mainly caused by hemoglobin (Hb) deficiency, is a serious health issue in countries that are developing, such as Bangladesh, India, Nepal, the Maldives, and Myanmar. This study examines the factors influencing Hb levels in women aged 15-49 across these nations using data from the Demographic Health Surveys (2011-2016). Analyzing 716,273 women from 71 subnational regions, we employed multilevel linear mixed-effect modeling to identify key determinants. The study revealed that its mean Hb level was 117.5 g/L with a standard deviation (SD) of 16.5 g/L overall, with its highest mean in Nep

Progress in family planning in Sierra Leone: a mixed-methods case study.

INTRODUCTION: Expanding access to voluntary family planning is a global health priority, yet progress has been uneven across low-income and middle-income countries. Despite being a fragile state with historically high fertility and limited health resources, Sierra Leone has achieved one of the fastest increases in modern contraceptive prevalence in sub-Saharan Africa. This study explores the drivers of this progress through a mixed-methods case study. METHODS: We analysed national-level data from urban and rural settings, and engaged diverse stakeholders including family planning professional

Women's health across the reproductive lifespan in women with germline TP53 pathogenic variants: a clinical perspective.

Li-Fraumeni syndrome, caused by germline TP53 pathogenic sequence variants (PSVs), is among the most penetrant hereditary cancer syndromes. With broader genetic testing and improved survivorship, more women with TP53 PSVs now seek reproductive and hormonal guidance throughout their lifespan. Yet, no evidence-based framework currently exists to support women's health care in this population. In this review, we searched the available literature and found an absence of data regarding contraception, fertility preservation, pregnancy, and menopause, resulting in inconsistent counseling and both und

Family planning service quality and contraceptive discontinuation among short-acting contraceptive users in Indonesia: a survival analysis of the 2017 Indonesia demographic health survey.

BACKGROUND: Contraceptive discontinuation rates in Indonesia are higher among short-acting contraceptive users. The quality of family planning services was suspected to be the cause. The Indonesian Demographic Health Survey (IDHS) assesses family planning, discontinuation rates, and factors related to family planning. This study aimed to analyze short-acting contraceptive discontinuation and the quality of family planning services using The IDHS 2017 data. METHODS: This research analyzes data from the 2017 IDHS of women aged 15-49 years who had been using contraception in the last five years
