Genetic testing and counselling of patients with monogenic amyotrophic lateral sclerosis in Northern Finland: a retrospective, single-centre study.

Amyotrophic lateral sclerosis (ALS) is a common neurodegenerative disease. Its aetiology is complex, but a subset is caused by single-gene pathogenic variants (monogenic ALS). The most common ALS-associated genes are SOD1 and C9orf72. We evaluated the implementation of genetic testing and associated genetic counselling for ALS at a Finnish tertiary-level university hospital. We used the electronic patient records of Oulu University Hospital (Oulu, Finland) to identify patients diagnosed with ALS between 2000 and 2023. Medical records were reviewed for neurological diagnosis and completion of g

'A Constant Battle for Services': Parents' Experiences on Healthcare Services for Children With 22q11 Deletion Syndrome.

INTRODUCTION: 22q11 deletion syndrome (22q11DS) is a rare disease with a neurodevelopmental component, and the most common microdeletion in humans, that affects many systems. The diagnosis is most often made in childhood. Parents may experience a high burden of care due to the multiple medical consultations and services needed. AIMS: This study aimed to examine the parental perceptions of healthcare services for families of children with 22q11DS. METHODS: This interview study was a component of a broader mixed-methods research project. The qualitative segment focused on insights from purposi

MSL2 orchestrates oligodendrocyte precursor cell cycle via H3K4me3 to prevent white matter defects in neurodevelopmental disorders.

Pathogenic variants in MSL2, a core component of the histone-modifying MSL complex, are associated with neurodevelopmental disorders (NDDs) and white matter abnormalities; however, their mechanisms of pathogenicity remain unknown. In this study, we identified three patients with NDD carrying MSL2 variants, all of whom exhibited white matter abnormalities. Using conditional knockout mice, we demonstrated that oligodendrocyte lineage-specific deletion of Msl2 disrupts oligodendrocyte precursor cell proliferation, maturation, and myelination in a cell-autonomous manner, leading to autism-like beh

Mainstreaming genetic testing: Evaluation of the Australian Familial Melanoma Clinic.

PURPOSE: Familial melanoma genetic testing (GT) guides management but is rarely offered due to limited clinical genetics capacity. We used the RE-AIM framework to evaluate a new Australian Familial Melanoma Clinic (FMC), which integrated comprehensive dermatology examination and GT. METHODS: To inform the RE-AIM domains, demographic and clinical data were collected prospectively. A cost-minimization approach compared costs per patient associated with an embedded genetic counselor in a specialist department compared with referral to specialist genetics services. Experiential training of dermat

Prevalence and risk factors of keratoconus among school children in Oman: a population-based study.

BACKGROUND: To investigate the prevalence and risk factors of keratoconus (KC) among Omani school children. METHODS: The cross-sectional, population-based, descriptive design study assessed a cohort of school students, aged 9 to 18 years from the Al Buraimi Governorate of Sultanate of Oman. They underwent comprehensive ocular examination, including auto-kerato-refractometry (ARK), retinoscopy, and corneal topography. Firth's penalized (bias-reduced) logistic regression was used to identify independent predictors of KC and keratoconus suspect (KCS) status. RESULTS: A total of 733 students par

Multimodal genomic testing in prenatal diagnosis: incremental diagnostic contribution of chromosomal microarray and exome sequencing, real-world NIPT confirmation rates and pregnancy outcomes in 120 consecutive amniocenteses from northeastern Türkiye - a retrospective cohort study.

BACKGROUND: Most evidence on chromosomal microarray analysis (CMA) and exome sequencing (ES) in prenatal diagnosis comes from high-volume centres. We evaluated the incremental diagnostic contribution of additional testing modalities, non-invasive prenatal testing (NIPT) confirmation rates and pregnancy outcomes at a regional centre in Türkiye. METHODS: Retrospective cohort of 120 consecutive amniocenteses (March 2022-June 2025). The pathway was retrospectively observed, not uniformly implemented: the intended first-tier panel (quantitative fluorescent PCR, G-banded chromosome analysis, CMA) w

Factors Associated With Knowledge of Sickle Cell Disease and Haemoglobin Genotype Awareness Among Pregnant Women in Three Health Centres in Libreville and Owendo, Gabon.

INTRODUCTION: In Gabon, screening for sickle cell disease, a hereditary disorder that causes haemolytic anaemia due to the sickling of red blood cells, is systematic during pregnancy and mandatory before marriage. Several years after the implementation of these measures, this study aimed to identify the factors associated with knowledge of sickle cell disease and awareness of haemoglobin genotype among pregnant women. PATIENTS AND METHODS: This was a multicentre, cross-sectional, descriptive and analytical study conducted from May to December 2024 in three health centres in Greater Libreville

Consensus Guidelines for Diagnosis and Management of Pyruvate Dehydrogenase Complex Deficiency.

Primary pyruvate dehydrogenase complex deficiency (PDCD) comprises a group of monogenic disorders caused by pathogenic variants in genes encoding subunits of, or regulatory components affecting, the pyruvate dehydrogenase complex. The clinical phenotype spans a broad continuum, ranging from early onset congenital lactic acidosis to infantile or childhood onset global developmental delay with epilepsy, through to more attenuated adult-onset neurological presentations. Reports from patients and advocacy groups indicate substantial variability in clinical management across both emergency and outp

Regulatory safeguards needed if preimplantation genetic testing for polygenic risk scores (PGT-P) is permitted in Singapore.

Singapore, a highly affluent island city-state located in Southeast Asia, has increasingly leveraged new assisted reproductive technologies (ART) to overcome its dismal fertility rates in recent years. A new frontier in ART is preimplantation genetic testing (PGT) for polygenic risk scores (PRS) to predict complex multifactorial traits in IVF (in vitro fertilisation) embryos, such as type 2 diabetes, cardiovascular diseases and various other characteristics like height, intelligence quotient (IQ), hair and eye colour. Unlike well-known safety risks with human genome editing, there are negligib

Variants in DENND2B are associated with vulnerability for neurodevelopmental impairment, psychosis and catatonia.

DENND2B is a DENN (differentially expressed in normal and neoplastic cells) domain-containing protein that has important roles in regulating the cell cycle, cell division and ciliogenesis, but to date has not been associated with any human disease. Here, we report on 11 individuals with monoallelic variants in DENND2B with a shared constellation of features and perform in silico and in vivo zebrafish modelling of the DENND2B variants identified in these patients. Features shared among these patients include developmental delay, intellectual disability and psychiatric/behavioural concerns, and

Dental agenesis as a novel phenotypical feature associated with hereditary diffuse gastric cancer in China.

BACKGROUND: Hereditary diffuse gastric cancer (HDGC) is an autosomal dominant gastric cancer associated with germline CDH1 mutations. Carriers of CDH1 mutations have a higher risk of developing gastric cancer at a younger age, highlighting the need for a phenotypic feature for early diagnosis and management. METHODS: We analysed 121 patients with gastric cancer who underwent genetic testing at the National Cancer Center in China. CDH1 mutation status was assessed using next-generation sequencing. Fisher's exact test and Mann-Whitney U test were performed to compare clinicopathological feature

Cancer risk in patients with pulmonary fibrosis and a rare telomere related gene variant.

BACKGROUND: Rare telomere related gene (TRG) variants are the cause of telomere biology disorders (TBD), a broad spectrum of age-related diseases, such as pulmonary fibrosis (PF) a disease manifesting at older age. TBD patients are at increased risk of certain types of squamous cell carcinomas and hematological malignancies. However, most studies on cancer incidence in TBDs involved relatively young cohorts in which patients with pulmonary fibrosis were underrepresented or absent. Therefore, we investigated cancer incidence in patients with pulmonary fibrosis as major manifestation of TBD (TBD

(Not) "just your nature". Diagnostic journeys of adults with neurofibromatosis type 1 in Poland.

BACKGROUND: Neurofibromatosis type 1 (NF1) is a rare autosomal dominant disorder that presents complex physical, emotional, and social challenges. Despite clear diagnostic criteria, NF1 is often diagnosed late due to vague early symptoms, everyday or non-medical interpretations of early signs, and limited access to specialists and genetic testing. This study explores how individuals with NF1 experience the diagnostic process in this context. METHODS: Ninety-three adults with medically confirmed NF1 participated in in-depth interviews conducted between August 1 and 31 December 2020. Data were

Does genetic testing impact stigma in autism: A scoping review.

Public stigma is a society’s negative attitude toward a person or groups of people, self-stigma relates to the internalisation of these negative attitudes, and affiliate stigma is experienced by those associated with a stigmatised group. Both autism and genetic testing results can be stigmatised. This scoping review explored relationships between genetic testing and stigma in autism, to understand any perceived positive or negative effects. Embase Classic, and APA PsycINFO via Ovid databases were searched on May 30th, 2023, and updated January 9th, 2025. Included articles related to experience

India's digital-first approach to sickle cell disease elimination.

The Government of India has launched the National Sickle Cell Anaemia Elimination Mission, a significant public health initiative targeting a genetic disorder that disproportionately affects the nation’s tribal populations. Sickle cell disease causes severe lifelong complications, and this mission aims to eliminate it as a public health problem by 2047. The primary objectives are to provide accessible, high-quality care to all patients and to reduce the disease’s prevalence through targeted screening and awareness. The mission’s comprehensive strategy is structured around three pillars: Health

Implementing mainstream germline genetic testing in breast cancer across Europe.

The implementation of mainstream germline genetic testing in breast cancer patients has both benefits and challenges. Multiple aspects need to be considered for the outline of gene panels and the amount of pre-test genetic counselling. Mainstream genetic testing is mainly performed to stratify patients for targeted treatment. In addition, identification of germline pathogenic variants in cancer risk genes may have surgical implications, consequences for surveillance of other organs at risk of cancer, as well as family implications among relatives at risk. To ensure that patients are well infor
