Two Cases of Plasmablastic Myeloma Mimicking Plasmablastic Lymphoma With In-Depth Review of Literature.

BACKGROUND: Plasmablastic myeloma (PBM) is a rare, aggressive subtype of multiple myeloma (MM) with poor prognosis. On the other hand, plasmablastic lymphoma (PBL) is an aggressive B-cell lymphoma with a plasmacytic phenotype. Importantly, PBM is difficult to distinguish from PBL, because clinical features of both diseases closely overlap. We report two cases of PBM accompanied by apparent extramedullary lesions. CASE: Case 1: A 38-year-old female complained of fatigue. She presented with pancytopenia, splenomegaly, a soft tissue lesion over the chest wall, and multiple osteolytic lesions. In

Primary pancreatic hydatid cyst-a rare case report with review of literature.

Hydatid disease is caused by the larval stage of Echinococcus granulosus. It most commonly affects the liver and lungs. Pancreatic hydatid cyst is very rare with incidence of 0.14%-2%. Presenting symptoms vary depending on the location and size ranging from mild non-specific symptoms to less commonly encountered serious pancreato-biliary complications. Due to non-specific symptoms, overlapping imaging features in the early stages and low index of suspicion, the preoperative diagnosis remains a challenge. We present the case of 23-year-old Asian male who presented with complaints of nausea, abd

Lymphoepithelial cyst of the pancreatic tail: a rare entity with surgical and histopathological insights.

Lymphoepithelial cysts (LECs) of the pancreas are rare, benign lesions often discovered incidentally during imaging for unrelated conditions. Their radiological features frequently overlap with those of other more common pancreatic cystic lesions, presenting a diagnostic challenge. In this report from a general surgical department on the Australia, we detail the assessment and management of a 54-year-old male with an enlarging pancreatic tail lesion initially thought to be a mucinous cystic neoplasm. Laparoscopic distal pancreatectomy and splenectomy was performed, and subsequent histopatholog

Challenges in the Management of Pediatric Pancreatic Hydatid Disease: A Systematic Review of the Literature and Our Clinical Experience.

Pancreatic hydatid disease (PHD) in children is rare compared to liver and lung involvement. The rarity and overlap with other cystic lesions often lead to challenges in the preoperative diagnosis and intraoperative surprises. Clear guidelines on the appropriate management of PHD are lacking. This review is intended to address these gaps in the literature. A systematic literature search was performed in PubMed, Scopus, Cochrane Central Registry of Controlled Trials (The Cochrane Library), and ScienceDirect databases for pediatric PHD. Observational studies such as case reports, case series, an

A rare case of dermoid cyst in the pancreas.

Pancreatic dermoid cyst is an extremely rare benign neoplasm of the pancreas. Pre-operative diagnosis is often difficult due to its rarity and overlapping features with other pancreatic cystic neoplasms. We report a case of a 55-year-old male with an incidental finding of a lobulated complex cystic lesion in the tail of the pancreas on imaging and the challenges to obtain a definitive diagnosis. Due to suspicious features on imaging and elevated CA 19-9 tumour marker, surgical resection was recommended at the Pancreatic Multidisciplinary Team Meeting. The patient subsequently underwent a dista

Secondary pancreatic tumors in endoscopic ultrasound-guided fine-needle biopsy: Clinicopathologic characteristics and morphological diagnostic challenges.

Pancreatic metastases are rare and often difficult to diagnose, especially in limited tissue samples. However, accurate diagnosis is crucial for guiding appropriate clinical management. This study evaluates the clinical, radiologic, and pathologic features of pancreatic metastases, with a focus on morphological patterns that mimic common primary pancreatic tumors in endoscopic ultrasound-guided fine-needle biopsy (EUS-FNB) specimens. Among 1054 pancreatic neoplasms diagnosed over a nine-year period, 62 cases (5.9 %) were identified as metastases. The interval from initial diagnosis to pancreat

Clinicopathologic Characteristics of Immune Checkpoint Inhibitor-related Pancreatitis.

The advent of immune checkpoint inhibitors (ICIs), although associated with adverse events, has heralded a new era in cancer therapy. ICI-related pancreatitis is a rare adverse effect of ICIs. The nonspecific clinical manifestations have posed diagnostic challenges, and the detailed histologic features remain largely unknown. This study aims to characterize the clinical and histopathologic features of ICI-related pancreatitis to increase awareness and improve diagnostic accuracy. We retrospectively identified 5 specimens from 4 patients from our database and consultation practice. We reviewed

Diagnostic Pearls and Pitfalls in the Evaluation of Small Biopsies From the Bile Duct and Ampulla.

CONTEXT.—: Histopathologic evaluation of bile duct and ampullary biopsies can be challenging. Biopsies from these sites are often tiny, scant, and/or fragmented. When assessing these biopsies, there is significant overlap between reactive atypia and malignancy, in situ precursor lesions can be misinterpreted as malignancy, and nonprimary tumors can mimic primary disease. OBJECTIVE.—: To provide diagnostic pearls and pitfalls in the evaluation of small biopsies from the biliary tract. DATA SOURCES.—: Literature review of published studies and the author's own observations. CONCLUSIONS.—: Bec

Beyond the Common: A Pictorial Review of Rare and Atypical Periampullary Lesions.

The periampullary region, defined as the area within 2 cm of the major papilla of the duodenum, is associated with various benign and malignant conditions. Although periampullary cancers are rare, they are the third leading cause of cancer-related deaths in the Western world. Tumors in this area often mimic benign lesions due to overlapping clinical presentations. Diagnostic imaging techniques, including computed tomography, magnetic resonance imaging, ultrasonography, and MR cholangiopancreatography, are commonly used in clinical practice and typically confirmed by pathological analysis. Benign focal small bowel lesions : a review of the features on multiphasic multidetector computed tomography.

The detection of small bowel lesions and their discrimination from normal bowel tissue are the most elementary and important factors in the computed tomography (CT) diagnosis of focal small bowel lesions. The detection and characterization of small bowel lesions have recently improved with advances in CT technology. Post-contrast multiphasic multidetector CT (MDCT) aids in the assessment of the vascular features of focal small bowel lesions. Understanding the typical multiphasic MDCT features of focal small bowel lesions is valuable because CT features overlap, and the severity and associated

Large metastatic lymph nodes misdiagnosed as a pancreatic tumor: a case report.

Due to the anatomical proximity between pancreatic tumors and the group 8 lymph nodes of the stomach, misdiagnosis can easily occur. The patient's preoperative examination: computed tomography (CT) scan examination indicated an abdominal mass, 3.0T pancreatic magnetic resonance imaging (MRI) with contrast enhancement showed a pancreatic head lesion, possibly a neuroendocrine tumor. Gastroscopy revealed highly suspicious for cancer. Pancreaticoduodenectomy was planned. However, intraoperative exploration revealed the tumor to be localized within the Group 8 lymph nodes rather than the pancreas.

Diagnostic Challenges during Inflammation and Cancer: Current Biomarkers and Future Perspectives in Navigating through the Minefield of Reactive versus Dysplastic and Cancerous Lesions in the Digestive System.

In the setting of pronounced inflammation, changes in the epithelium may overlap with neoplasia, often rendering it impossible to establish a diagnosis with certainty in daily clinical practice. Here, we discuss the underlying molecular mechanisms driving tissue response during persistent inflammatory signaling along with the potential association with cancer in the gastrointestinal tract, pancreas, extrahepatic bile ducts, and liver. We highlight the histopathological challenges encountered in the diagnosis of chronic inflammation in routine practice and pinpoint tissue-based biomarkers that

Qualitative and quantitative imaging features of solid pancreas tumours in portal venous phase CT: are they useful in determining tumour type and grade?

PURPOSE: Solid pancreatic lesions might have overlapping findings in portal venous phase computed tomography (CT). In this study, we aimed to investigate the quantitative and qualitative imaging features of solid pancreas lesions based on subtype and grade. MATERIAL AND METHODS: The study group consisted of 159 patients with solid pancreatic tumours detected after exclusion criteria. According to the pathology results, the patients were divided into 3 groups as PDAC (pancreatic ductal adenocarcinoma, n = 137), PNET (pancreatic neuroendocrine tumour, n = 15), and SC (sarcomatoid carcinoma, n =

Impact of different peak tube voltage settings on adrenal adenomas attenuation at unenhanced CT.

OBJECTIVES: To assess the influence of peak tube voltage peak setting on adrenal adenomas (AA) attenuation on unenhanced abdominal CT. MATERIALS AND METHODS: IRB-approved retrospective observational cohort study. We included 89 patients with imaging-defined AAs with shortest diameter > 6 mm who underwent two or more unenhanced abdominal CTs using at least two different peak tube voltage settings. Two readers independently measured adenoma attenuation on different CT acquisitions by drawing a round ROI on 3 mm thick axial MPR reconstructions encompassing at least 2/3 of the lesion's surface.
