Ovarian endometrioid adenofibroma in a perimenopausal woman with intermittent vaginal bleeding: a case report.

Ovarian endometrioid adenofibroma (EA) is an exceptionally rare benign epithelial tumor of the ovary with nonspecific clinical and imaging manifestations, and its diagnosis relies mainly on pathological examination. This case describes a 55-year-old perimenopausal woman with a 1-year history of intermittent vaginal bleeding and a progressively enlarging right adnexal mass. Imaging revealed a solid mass in the right adnexal region with minimal blood flow signal. Serum tumor markers were within normal ranges, and the sex hormone profile showed no evidence of estrogen excess. Serial ultrasonograp

A case of parasitic fibroid in unscarred abdomen mimicking pedunculated uterine fibroid: a case report.

INTRODUCTION AND IMPORTANCE: Parasitic leiomyomas are a type of extra-uterine fibroid that develop outside the uterus and thus receive their blood supply from surrounding structures. While most cases are iatrogenic, spontaneous occurrences are rare and often pose diagnostic challenges due to atypical locations. CASE PRESENTATION: We report the case of a 28-year-old woman, para 2, with no prior abdominal or pelvic surgery, who presented with a 5-year history of intermittent lower abdominal pain. Ultrasound suggested a pedunculated mass arising from the uterine fundus, mimicking an ovarian endo

Pregnancy after the Manchester-Fothergill procedure: a case report.

INTRODUCTION AND IMPORTANCE: Cervical elongation is commonly associated with uterine prolapse. The Manchester-Fothergill procedure offers a fertility-preserving surgical option for its correction. However, conception following this procedure is exceedingly rare, with an estimated incidence of 1 in 10 000. CASE PRESENTATION: We present the case of a 35-year-old multiparous woman (G3P2L2) who conceived 18 months after undergoing the Manchester-Fothergill procedure for cervical elongation. Her obstetric history included one vaginal delivery and one cesarean section performed for cervical elongat

Spontaneous Ovarian Hyperstimulation Syndrome in a Naturally Conceived Singleton Pregnancy With Familial Predisposition: A Case Report.

Spontaneous ovarian hyperstimulation syndrome (sOHSS) is an uncommon condition that can occur in naturally conceived pregnancies and may present a significant diagnostic challenge because its clinical and radiological features can resemble ovarian malignancy. We describe the case of a 30-year-old primigravida who presented in early pregnancy with abdominal pain and was found to have bilateral multicystic ovarian enlargement, ascites, and a markedly elevated cancer antigen-125 (CA-125) despite a viable singleton pregnancy conceived without fertility treatment. A detailed history revealed a stro

Pelvic splenosis and endometriosis in adolescent with hereditary spherocytosis.

The patient is a 16-year-old with a history of hereditary spherocytosis. At age 12, she underwent a laparoscopic partial converted to total splenectomy for intraoperative bleeding. The spleen was placed in a retrieval bag and removed via morcellation. She first reported abdominal pain at age 8, with dysmenorrhea after menarche at age 13. Her dysmenorrhea was managed with a progesterone IUD. Due to worsening pain, she underwent MRI which showed multiple lobulated structures throughout the pelvis and ventral abdomen, suggestive of endometriomas versus splenules. Laparoscopy revealed dozens of bl

Myo-inositol to prevent gestational diabetes: "What do conflicting trials tell clinicians?".

BACKGROUND: A recent meta-analysis reported that myo-inositol supplementation was associated with a reduced risk of gestational diabetes mellitus. However, the primary outcome showed substantial between-study heterogeneity, complicating the interpretation and clinical applicability of the evidence. OBJECTIVE: To discuss why randomized trials may have produced different results and identify the information needed to guide future research and clinical interpretation. STUDY DESIGN: We critically appraised the clinical and methodological characteristics of the randomized controlled trials includ

Hormonal and Metabolic Features of Complete Androgen Insensitivity Syndrome with and without Gonadectomy.

CONTEXT: Complete Androgen Insensitivity Syndrome (CAIS) is a variation of sex characteristics caused by androgen receptor gene mutations. As clinical practice increasingly supports deferring gonadectomy when appropriate, understanding hormonal and metabolic profiles is critical to inform longitudinal health. OBJECTIVE: To characterize hormonal, metabolic, and anthropometric features in CAIS, including those with intact gonads. DESIGN: Cross-sectional analysis of participants enrolled in an ongoing natural history study. (NCT04708431). SETTING: Tertiary referral center. PARTICIPANTS: Twent

Malignant transformation of abdominal wall endometriosis: a case report and literature review.

BACKGROUND: Abdominal wall endometriosis (AWE) is a relatively rare subtype of endometriosis that has the potential for malignant transformation. It typically presents with nonspecific clinical symptoms, such as abdominal pain and palpable masses. Diagnosis primarily relies on multimodal imaging combined with histopathological examination. The main treatment strategies include carboplatin- and paclitaxel-based combination chemotherapy, resection of the abdominal wall mass, and hysterectomy with bilateral salpingo-oophorectomy. The uniqueness of this article lies in the patient experimentally r

Determinants of vulnerability and onset of depression during pregnancy: Evidence from the PRESeNT cohort.

BACKGROUND: Antenatal depression is a major public health concern, yet the factors distinguishing clinically manifest symptoms from underlying vulnerability remain poorly understood. This study examined a range of dimensions to identify determinants of antenatal depression and subclinical risk. METHODS: Two hundred thirty-one pregnant women participating in the PRESeNT cohort study have been stratified as controls (n = 71), at risk of depression during pregnancy (n = 107), or with antenatal depressive symptoms (n = 53). Assessments included psychological measures (depressive symptoms, general

Systematic review and network meta-analysis of performance of the Sapporo criteria, the Revised Sapporo criteria, and the 2023 ACR/EULAR APS classification criteria for patients with antiphospholipid syndrome: A protocol.

BACKGROUND: Antiphospholipid syndrome is an autoimmune disorder characterized by recurrent thrombotic events and/or adverse pregnancy outcomes. Although three classification criteria-the 1999 Sapporo criteria, the Revised Sapporo criteria, and the 2023 ACR/EULAR antiphospholipid syndrome classification-have been developed primarily for research purposes to standardize patient selection, clinicians sometimes apply these criteria to aid in diagnostic decision-making. However, direct head-to-head studies comparing these three criteria remain scarce, and a comprehensive systematic review with netw

Narrative review: The role of maternal nutrition in recurrent miscarriage: From molecular pathways to clinical care gaps.

Recurrent miscarriage (RM), defined as three or more consecutive pregnancy losses before 24 weeks' gestation, represents a profound clinical and psychological challenge, affecting approximately 1-2% of couples. Despite standard investigation, up to 50% of cases remain unexplained, shifting focus towards modifiable environmental factors. Maternal nutrition, particularly micronutrient status, is a critical yet underexplored component influencing the biological pathways essential for early pregnancy success. This narrative review synthesises evidence on the role of nutrition in RM, focusing on mo

Incidental Appendiceal Adenocarcinoma Discovered After Ileocecectomy for Recurrent Small Bowel Obstruction: A Case Report.

We report a case of moderately differentiated appendiceal adenocarcinoma discovered on histopathologic examination following ileocecectomy for recurrent SBO in a woman in her early 50s with a history of stage III rectal adenocarcinoma in remission. Notably, serial surveillance imaging over two years had demonstrated a radiographically normal appendix prior to the obstructive presentation. Immunohistochemical and histopathologic features confirmed a metachronous primary malignancy rather than metastatic recurrence. This case exemplifies the diagnostic challenge posed by occult appendiceal neopl

Plasma metabolomic profiles associated with breastfeeding history and subsequent ovarian cancer risk.

BACKGROUND: Breastfeeding reduces ovarian cancer risk, but the underlying biological mechanisms remain unclear. METHODS: We examined the association between a previously reported metabolite-based breastfeeding score with incident epithelial ovarian cancer risk in case-control studies nested within the Nurses' Health Studies (NHS/NHSII, n = 436) and Prostate, Lung, Colorectal, and Ovarian Cancer Screening Trial (PLCO, n = 300) using conditional logistic regression to calculate odds ratios (ORs) and 95% confidence intervals (CIs) for each study and conducted a meta-analysis. We investigated 606

Pregnancy-Associated Acquired Hemophilia A (PA-AHA) Treated Successfully With the Factor-Mimetic Bispecific Antibody Emicizumab: A Case Report and Literature Review.

Acquired hemophilia A (AHA) is a rare bleeding disorder characterized by spontaneous bleeding in individuals with no previous personal or family history of bleeding, caused by the development of autoantibodies against coagulation Factor VIII (FVIII). These autoantibodies may be associated with drugs, autoimmune disorders, malignancies, and pregnancy. Pregnancy-associated AHA (PA-AHA) is a very rare condition, accounting for approximately 1%-5% of all AHA cases. Emicizumab, a bispecific FVIII-mimetic monoclonal antibody, was originally approved for Congenital Hemophilia A (with or without FVIII

From Local Evidence to Regional Leadership: Post-Preeclampsia Care in Thailand.

Hypertensive disorders of pregnancy, particularly preeclampsia, identify women at increased risk of persistent hypertension and later cardiovascular disease, yet postpartum services often remain organized around short-term obstetric recovery. Thailand is well positioned to address this discontinuity because prospective local studies have demonstrated elevated blood pressure, hypertension, and selected cardiovascular and renal risk markers at 6 weeks and 6-12 months after preeclampsia or gestational hypertension. The principal policy gap is therefore not the absence of warning evidence, but the

Successful interval laparoscopic appendectomy for walled-off perforated appendicitis during pregnancy: a case report.

INTRODUCTION: Acute appendicitis is the most common non-obstetric surgical emergency during pregnancy, with an incidence of approximately 0.067% (1 in 1493 pregnancies). Although prompt laparoscopic appendectomy is the standard treatment, optimal management of walled-off perforated appendicitis (WOPA) during pregnancy is unclear. CASE PRESENTATION: A 28-year-old woman presented with a 1-week history of abdominal pain and fever at 9 weeks of gestation. Ultrasonography demonstrated an ill-defined right lower quadrant mass, and contrast-enhanced computed tomography revealed WOPA with a localized
