Early Mother-Newborn Skin-to-Skin Contact at Term Birth and Early Neonatal Thermoregulation Under Routine Clinical Practice.

Background and Objectives: Early mother-newborn skin-to-skin contact (SSC) after birth is widely recommended to support neonatal physiological stabilization, including thermoregulation. Under routine clinical practice, however, SSC may be brief or interrupted, and its effectiveness in maintaining neonatal body temperature under such conditions is less well described. This study aimed to evaluate early neonatal temperature changes under routine post-birth care practices that included brief SSC followed by separation for incubation care. Materials and Methods: This retrospective cohort study inc

Perinatal mental health issues in consultations regarding medication use during breastfeeding: Insights from the Japan Drug Information Institute in Pregnancy (JDIIP).

PURPOSE: We comprehensively identified the challenges associated with psychotropic-medication use during breastfeeding in Japan and explored strategies to enhance breastfeeding support. METHODS: We analyzed breastfeeding-consultation records of cases involving postpartum mothers received by the Japan Drug Information Institute in Pregnancy (JDIIP) between February 2012 and March 2022, and corresponding questionnaire responses regarding the use of psychotropic medications. The "Perinatal Mental Health Consensus Guide" was used as a benchmark to examine temporal trends. To assess changes in con

Impact of quality improvement initiative on incidence of hypothermia in neonates in the delivery room.

Normothermia during the first hour of life predicts admission to neonatal intensive care unit (NICU) and improves the outcome for both sick and stable babies across all gestational ages. Immediate skin-to-skin contact (iSTS) offers a simple, effective approach to prevent hypothermia while enhancing feeding, survival, and bonding. The aim of this study is to implement a multidisciplinary quality improvement initiative addressing the barriers to optimal thermal care in the delivery room setting and establish sustainable protocols for preventing neonatal hypothermia across all gestational ages. Post-spinal position and its impact on hemodynamic, block height, and comfort in caesarean delivery: A randomized assessor-blinded trial.

BACKGROUND: Maternal hypotension is common during spinal anesthesia for cesarean delivery and is influenced by patient position. Positioning also affects sensory block height, which determines visceral analgesia. This study compared immediate supine, sitting, and semi-sitting positions to identify the optimal balance between cardiovascular stability and surgical anesthesia. METHODS: In this randomized controlled trial, 216 ASA II term parturients scheduled for cesarean delivery under spinal anesthesia were assigned to three groups (n = 72). Group A received immediate supine positioning, Group

Society for Maternal-Fetal Medicine Consult Series #76: Cancer in pregnancy.

Approximately one in 1000 pregnancies is complicated by the diagnosis of cancer each year, and the incidence of cancer among reproductive-age individuals is increasing. Management of a pregnant person with cancer can be complex and warrants a multidisciplinary approach to care. Recent data have demonstrated reassuring outcomes for pregnant persons and their offspring after treatment of many types of cancer in pregnancy. Treatment of cancer in pregnancy must be individualized based on the specific type and stage of cancer, gestational age at diagnosis, and the patient's desire to continue the p

Management of neonatal atlantoaxial rotatory subluxation (AARS) using a rigid spinal immobiliser.

We present a very rare case of neonatal atlantoaxial rotatory subluxation (AARS) and its management.A term female neonate born via vacuum-assisted delivery presented with distress immediately after birth. On examination, she had right-sided torticollis and an audible cervical spine 'click'. Imaging confirmed AARS. Spinal precautions were initiated, and she was transferred to a surgical tertiary children's hospital. A rigid cervical collar was applied initially with limited success for immobilisation. Hence, after a multidisciplinary team meeting, it was changed to a rigid spinal immobiliser. M

Safety of Vojta's reflex locomotion in term pregnancy: a randomized controlled pilot study of uterine activity and labor onset.

The Vojta's method (VRL) is a neurophysiological rehabilitation method used to support and induce reflex responses of locomotor and vegetative systems. It uses involuntary motor reaction of the body during pressure stimulation of so-called trigger zones. Pregnancy is currently considered a contraindication for the use of VRL due to the potential risk of inducing regular uterine activity and, consequently, triggering labor. The aim of the study is to evaluate changes in uterine activity during Vojta's reflex locomotion and to determine whether the method is associated with the induction of labo

Maternal Mental Health After Stillbirth: Associations With Autopsy Decisions, Cause of Death Knowledge, and Psychosocial Risk Factors.

PURPOSE: Stillbirth is a traumatic event with potentially lifelong effects on maternal mental health, but there are few clinical guidelines or resources for supporting families. Additionally, the effects of parental autopsy decision-making and knowledge of cause of death effect on maternal mental health is unclear. METHODS: We evaluated associations between autopsy decision, cause of death determination, and maternal mental health in data from the multi-center, five region Stillbirth Collaborative Research Network (2006-2008). Secondarily, we examined psychosocial risk factors associated with

Successful neonatal outcome after postmortem caesarean section at Sumbawanga Regional Referral Hospital, Rukwa, Tanzania: case report.

BACKGROUND: Cardiac arrest in pregnancy can occur for a variety of reasons. During maternal cardiac arrest or shortly thereafter, the fetus may remain viable. Postmortem caesarean section can potentially save the fetus with or without long-term neurological sequelae; in the absence of timely intervention, however, fetal death is inevitable. Given the limited documentation in developing countries and the possibility of unfavourable outcomes, this report presents one of the few documented cases from resource-limited Sub-Saharan Africa describing a live birth with a sustained favourable neurologi

Comparison of a Modified Regimen of Prophylactic Phenylephrine Boluses Versus Variable Rate Infusion During Elective Cesarean Delivery Under Spinal Anesthesia: A Noninferiority Randomized Double-Blind Study.

BACKGROUND: Prophylactic phenylephrine boluses have been found to be as effective as variable rate infusions during elective cesarean delivery but require a higher number of physician interventions to maintain blood pressure near baseline values. Therefore, there is a need to find a feasible regimen of bolus administration that is equally efficacious to the infusion regimen while at the same time requires a comparable number of physician interventions and is thus non-inferior to the infusion regimen. METHODS: Healthy pregnant women with term, uncomplicated, singleton pregnancies undergoing el

A Prospective Study on the Efficacy of Amniotic Membrane in Promoting Wound Healing and Pain Reduction after Cesarean Delivery.

Cesarean delivery is one of the most common obstetric procedures worldwide. The amniotic membrane, the inner layer of the placenta, surrounds the embryo and forms a fluid-filled sac. This research aimed to assess how the immediate application of amniotic membrane after delivery affects wound healing and pain. The researchers used a quasi-experimental design and conducted the study at Tbarak Hospital, Cairo, Egypt. The researchers used a purposive sample of 70 women undergoing cesarean delivery equally divided into a study group (n = 35) and a control group (n = 35). Three tools were used for d

Effect of Kangaroo Mother Care during the first 72 hours of life on early growth and breastfeeding in normal birthweight newborns: protocol for a randomised controlled trial.

INTRODUCTION: Skin-to-skin contact (SSC) during the first hour of birth is recommended for healthy newborn infants and their mothers and improves early stabilisation and breastfeeding outcomes. Kangaroo Mother Care (KMC), involving prolonged SSC and exclusive breastfeeding, provides an optimal physiological transition from intrauterine to extrauterine life for preterm and low birthweight infants, improving survival by 32% and conferring multiple clinical and neurodevelopmental benefits. Extending the duration of SSC contact beyond the first hour may similarly provide underexplored benefits to

Plasma fractionation in Latin America: a path towards sovereignty and sustainability in the supply of plasma-derived medicines.

PURPOSE: Increased reactive oxygen species (ROS) released by NADPH oxidase and inflammation are associated with arterial hypertension and eye diseases associated with high blood pressure, including glaucoma, retinopathies (e.g., age-related macular degeneration), and choroidopathies affecting ocular function; however, the mechanisms underlying these adverse outcomes remain undefined. The present study was designed to highlight the importance of oxidative stress in severe hypertension-related eye damage. METHODS: Male Wistar rats (n = 7, unless otherwise specified for specific experiments) wer

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Maternal Phenotype, Infant Size and Breast Milk Composition in Women Living With HIV.

The impact of maternal factors on the size of HIV-exposed-uninfected (HEU) infants and breast milk composition is poorly understood. Anthropometry, bio-electrical impedance, haemoglobin and HIV viral load data of women living with HIV (WLWH) and without HIV (WLWOH) were compared and related to their infants' anthropometric Z-scores and breast milk macronutrients 6 weeks and 6 months postnatally. At both time points, WLWH (6-week: n = 83; 6-month: n = 63) had lower reactance (measure of body cell mass) (6-week: p = 0.016; 6-month: p < 0.001), phase angle (PhA) (measure of cell health) (6-week:

Effectiveness of a new breastfeeding counselling intervention on breastfeeding prevalence, infant growth velocity and postpartum weight loss in overweight women: a randomized controlled trial.

BACKGROUND: Maternal overweight is a risk factor for child obesity. Breastfeeding may decrease this risk, but breastfeeding prevalence is low in overweight or obese mothers. METHODS: We conducted a randomized trial in 90 overweight/obese pregnant women in Bogotá-Colombia during 2019, to evaluate the effects of a new exclusive breastfeeding (EBF) counselling intervention for overweight/obese mothers, based on Carl Rogers' client-centered theory. The Intervention included individualized breastfeeding counseling, empowerment sessions, and a set of problem-solving strategies based on Carl Rogers'
