Successful neonatal outcome after postmortem caesarean section at Sumbawanga Regional Referral Hospital, Rukwa, Tanzania: case report.

BACKGROUND: Cardiac arrest in pregnancy can occur for a variety of reasons. During maternal cardiac arrest or shortly thereafter, the fetus may remain viable. Postmortem caesarean section can potentially save the fetus with or without long-term neurological sequelae; in the absence of timely intervention, however, fetal death is inevitable. Given the limited documentation in developing countries and the possibility of unfavourable outcomes, this report presents one of the few documented cases from resource-limited Sub-Saharan Africa describing a live birth with a sustained favourable neurologi

Comparison of a Modified Regimen of Prophylactic Phenylephrine Boluses Versus Variable Rate Infusion During Elective Cesarean Delivery Under Spinal Anesthesia: A Noninferiority Randomized Double-Blind Study.

BACKGROUND: Prophylactic phenylephrine boluses have been found to be as effective as variable rate infusions during elective cesarean delivery but require a higher number of physician interventions to maintain blood pressure near baseline values. Therefore, there is a need to find a feasible regimen of bolus administration that is equally efficacious to the infusion regimen while at the same time requires a comparable number of physician interventions and is thus non-inferior to the infusion regimen. METHODS: Healthy pregnant women with term, uncomplicated, singleton pregnancies undergoing el

A Prospective Study on the Efficacy of Amniotic Membrane in Promoting Wound Healing and Pain Reduction after Cesarean Delivery.

Cesarean delivery is one of the most common obstetric procedures worldwide. The amniotic membrane, the inner layer of the placenta, surrounds the embryo and forms a fluid-filled sac. This research aimed to assess how the immediate application of amniotic membrane after delivery affects wound healing and pain. The researchers used a quasi-experimental design and conducted the study at Tbarak Hospital, Cairo, Egypt. The researchers used a purposive sample of 70 women undergoing cesarean delivery equally divided into a study group (n = 35) and a control group (n = 35). Three tools were used for d

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Intraoperative Hemorrhage during Implantable Collamer Lens Surgery: A Case Report and Management Strategy.

INTRODUCTION: The aim of the study was to report the intraoperative signs, management, and postoperative outcomes of iris hemorrhage during implantable collamer lens (ICL) surgery. CASE PRESENTATION: A 32-year-old Asian woman experienced iris bleeding via a superior incision during distal haptics delivery in ICL surgery for her right eye. The inferior iris was displaced against the ocular wall, resulting in traction-induced vertical elongation and elliptical distortion of the pupil, which indicated excessive iris traction and ultimately led to focal rupture of delicate iris vessels with intra

Femoral artery Doppler as a novel predictor of spinal hypotension in elective cesarean delivery cases: a prospective observational study.

BACKGROUND: Spinal anesthesia is the preferred anesthetic for elective cesarean deliveries, though spinal hypotension requires prophylactic intervention to avoid serious complications. Several parameters have been studied to predict spinal hypotension; however, no reliable predictor has been identified to date. Spinal anesthesia induces femoral artery vasodilatation that induces flow velocity changes. Therefore, our study aimed to evaluate whether changes in femoral artery Doppler indices could serve as predictors of spinal hypotension. METHODS: This prospective observational study included p

Predictors of Anemia Intolerance for Real-Time Transfusion Decision-Making During Resuscitation of Trauma Subjects: A Machine Learning Approach Using Heart Rate Variability.

OBJECTIVES: RBC transfusion in anemic patients with sustainable tolerance may cause harm, emphasizing the need for reliable metrics that quantify adequacy (oxygen delivery ≥ demand) and sustainability (oxygen delivery remains adequate without transfusion) of compensatory physiology. Our objective was to identify personalized predictors of anemia intolerance (inadequate and unsustainable physiologic compensation) that predict the likelihood of transfusion benefit. We studied adult trauma subjects at arrival to the emergency department, employing machine learning to evaluate ability of heart rat

Association of Prepregnancy Obesity versus Excessive Gestational Weight Gain with Adverse Neonatal Outcomes in the United States.

Prepregnancy obesity (PPO) and excessive gestational weight gain (eGWG) during pregnancy, both are associated with adverse neonatal outcomes. The objective of this study was to compare the independent associations of PPO and eGWG with adverse neonatal outcomes. This was a retrospective cohort study of singleton, live births in the United States in 2018 using National Vital Statistics System data. These were divided into four groups: 1) normal prepregnancy BMI and normal GWG, 2) normal prepregnancy BMI and eGWG, 3) prepregnancy BMI >30 kg/m2 (PPO) and normal GWG and 4) PPO and eGWG. Challenges in managing cervical cancer in pregnancy: Three case reports.

Cervical cancer in pregnancy is a rare but increasingly recognised condition, affecting approximately 0.1-12.0 per 10,000 pregnancies annually. Symptoms such as vaginal bleeding, discharge, dyspareunia, and pelvic pain may overlap with normal pregnancy-related changes, often leading to delayed diagnosis. Speculum and bimanual examination is essential in pregnant women presenting with vaginal bleeding. Urgent colposcopy referral (for example, in the UK through the two-week wait pathway) is essential. Management strategies depend on cancer stage, gestational age, and patient preferences. Options
