High Rates of Defect Closure After Resection of Large Nonpedunculated Colorectal Lesions Using a Through-The-Scope Clip With Anchor Prongs.

BACKGROUND: Prophylactic complete closure of mucosal defects after resection of gastrointestinal lesions is key to reducing delayed bleeding, but complete closure for large defects can be challenging with conventional through-the-scope clips (TTSC). The introduction of a TTSC with anchor prongs offers ability to approximate margins of larger defects. OBJECTIVE: The study objective was to evaluate prophylactic complete closure after polypectomy, endoscopic mucosal resection (EMR), or endoscopic submucosal dissection (ESD) in large (≥ 20 mm) nonpedunculated colorectal lesions (LNPCLs). METHODS

A first-in-human phase I clinical trial investigating the safety and efficacy of repeat dosing MSCTRAIL cell therapy in addition to chemotherapy and immunotherapy in patients with advanced lung cancer.

BACKGROUND AND AIMS: Despite recent progress, advanced non-small cell lung cancer (NSCLC) has poor survival outcomes, necessitating the development of novel therapies. TNF-related apoptosis-inducing ligand (TRAIL) selectively induces cancer cell death and can be delivered to tumors by mesenchymal stromal cells (MSCs) due to the cells' migratory properties. This first-in-human phase I trial assessed safety and dose of umbilical cord-derived MSCs expressing TRAIL (UC-MSCTRAIL) alongside standard NSCLC therapy. METHODS: Participants performance status 0-1 with treatment-naïve, inoperable stage I

Perioperative Antimicrobial Prophylaxis for Invasive Dental Procedures: A Systematic Review and Random-Effects Meta-Analysis of Randomized and Placebo-Controlled Studies.

Prophylactic antimicrobials are frequently administered before invasive dental procedures (including implant placement and extractions) to reduce early infectious complications and procedure-related bacteremia; however, clinical benefit remains debated and must be balanced against ecological disruption and antimicrobial resistance. Evidence from experimental work indicates that even a single prophylactic dose of amoxicillin may transiently perturb the oral microbiome and select resistant strains. A Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA)-guided systematic re

Factors influencing uptake of risk-reducing mastectomy among unaffected Israeli BRCA1/BRCA2 pathogenic variant carriers.

Women harboring pathogenic variant (PV) in the BRCA1 or BRCA2 genes (= BRCA) have an elevated lifetime risk for breast cancer (BC). One of the main options for active BC risk reduction is bilateral risk-reducing mastectomy (RRM). Understanding the factors influencing that decision is important for genetic-counselling and risk mitigation strategy planning. A structured questionnaire was circulated to BRCA carriers, members of the Good Genes NGO in Israel. Data on RRM uptake and timing, factors previously reported to be associated with decision to undergo RRM (e.g., psychosocial, family history,

Impact of prevention of postoperative pulmonary complications with penehyclidine inhalation on long-term outcomes in high-risk patients: follow-up of a randomized trial.

PURPOSE: Postoperative pulmonary complications are common and associated with worse long-term outcomes. In our previous trial, prophylactic penehyclidine inhalation reduced pulmonary complications in high-risk patients. Herein, we tested the hypothesis that penehyclidine inhalation might also improve long-term survival after surgery. METHODS: This was a long-term follow-up of a randomized trial. We enrolled 864 patients aged more than 50 years who were scheduled for major upper-abdominal or non-cardiac thoracic surgery and judged to be at high risk of pulmonary complications. Participants wer

Surrogate Decision-Making by Family Caregivers for Hyperthermic Intraperitoneal Chemotherapy in Gastric Cancer: Qualitative Study in a High-Volume Chinese Center.

BACKGROUND: Hyperthermic intraperitoneal chemotherapy (HIPEC) has been integrated into the management of gastric cancer (GC) as a combined approach for addressing peritoneal metastasis, serving both prophylactic and therapeutic roles following GC surgery. The pivotal decision regarding HIPEC administration typically arises intraoperatively, creating a complex clinical scenario where family caregivers must act as surrogate decision-makers under substantial time constraints. This decision-making process proves particularly challenging due to limited understanding of the procedure's risk-benefit

The impact of breast cancer polygenic risk score disclosure on decisional conflict around risk-reducing mastectomy in women with pathogenic BRCA1/2 variants.

PURPOSE: Women with BRCA1/2 pathogenic variants (PVs) face high breast cancer risks and complex preventive decisions, including whether to undergo risk-reducing mastectomy, which can generate decisional conflict. Polygenic risk scores (PRSs) offer individualized risk estimates that may support informed preventive decisions. We examined whether PRS disclosure reduces decisional conflict about risk-reducing mastectomy in women with BRCA1/2 PVs and whether PRS magnitude interacts with individual characteristics. METHODS: Unaffected women with BRCA1/2 PVs (N = 354) were assessed before PRS disclo
