Incident heart failure in patients with screening-detected atrial fibrillation: a post hoc analysis of the STROKESTOP and STROKESTOP II studies.

AIMS: Atrial fibrillation is associated with increased risks of stroke, heart failure, and death. While stroke prevention is a key focus, heart failure is more frequent and a major contributor to mortality in atrial fibrillation. This study aimed to assess the incidence and timing of heart failure in screening-detected atrial fibrillation. METHODS AND RESULTS: This post hoc analysis included participants without prior heart failure from the STROKESTOP and STROKESTOP II randomized screening studies, in which individuals aged 75-76 years were invited to ECG-based atrial fibrillation screening.

Gefitinib response after acquisition of C797S following first-line osimertinib in EGFR L858R/E709K-mutated non-small cell lung cancer: A case report.

Osimertinib is the standard first-line therapy for epidermal growth factor receptor (EGFR)-mutated non-small cell lung cancer, but acquired resistance remains a major clinical issue. Here, we report a case of lung adenocarcinoma harboring compound EGFR L858R and E709K mutations that acquired C797S after progression on first-line osimertinib. Gefitinib achieved a durable partial response lasting approximately 15 months, despite E709K generally conferring reduced sensitivity to first-generation EGFR-tyrosine kinase inhibitors. This case highlights the importance of repeat molecular profiling and

A Dynamic Modeling Framework for Real-Time Monitoring of Porcine Lung Tissue Decellularization.

Decellularized extracellular matrix (dECM) scaffolds are critical for tissue engineering and regenerative medicine, yet standardized methods for real-time monitoring of decellularization remain lacking. Current protocols typically use fixed treatment durations to match endpoint control quality attributes (CQAs), such as final dsDNA content in the scaffold, without a mechanistic understanding of DNA removal kinetics. This knowledge gap leads to either incomplete decellularization (CQA rejections) or unnecessary processing time, compromising dECM scaffold quality and process efficiency. In this

Subtyping limited stage small cell lung cancer (LS SCLC) reveals inflammatory subtypes with improved chemoradiotherapy outcomes.

BACKGROUND: Concurrent chemoradiotherapy (CRT) remains the primary treatment for limited stage (LS) small cell lung cancer (SCLC). Although most patients respond, the majority relapse and die from the disease, and treatment-related toxicity is common. Molecular subtyping has been extensively investigated in SCLC, but its clinical relevance remains uncertain, and most studies have focused on systemic treatment of extensive-stage SCLC. We analysed baseline tumor samples from a subset of participants (n = 77) in a randomized trial (n = 170) comparing high-dose (60 Gy/40 fractions) with standard-d

Toxicity of tungsten and molybdenum disulphide particles after acute and subacute exposure in vitro.

Tungsten (W) and molybdenum disulphide (MoS₂) particles are increasingly used industrially, but may pose potential health risks, particularly following inhalation in occupational settings. Existing data is inconsistent but there is evidence that these particles are genotoxic. Therefore, there is a need to further increase our understanding on their toxicity and underlying mechanisms. To this end, we tested W and MoS2 particles in two human cell models, in vitro bronchial epithelial cells (BEAS-2B) and monocyte-derived macrophages (dTHP-1) after acute exposure (24 h), but we also investigated e

Mitral Annular Calcification and Its Severity in Relation to Left Atrial Reservoir Strain and a Non-Invasive Left Atrial Stiffness Index.

PURPOSE: Mitral annular calcification (MAC) is associated with atrial fibrillation and diastolic dysfunction, but its relationship with left atrial (LA) mechanics is poorly defined. The ratio of E/e' to LA reservoir strain (LASr) is a non-invasive surrogate of, not a direct measure of, LA stiffness. We assessed MAC and its severity against LASr and this LA stiffness index (LASI). METHODS: In this single-center cross-sectional study with prospective recruitment, 112 adults in sinus rhythm with ejection fraction ≥50% (58 with MAC, 54 age- and sex-similar controls) underwent two-dimensional spec

Frailty and nutritional status as prognostic factors in older adults with heart failure: a public health perspective.

BACKGROUND: Heart failure is a common cardiovascular disease among older adults. Combined with frailty, it may aggravate the patient's condition and affect the prognosis. Nutritional status is important in older patients, but its relationship to frailty associated with heart failure is not fully understood. METHODS: This single-center retrospective study initially screened 236 older patients with heart failure who were admitted to Yuncheng Central Hospital in Yuncheng, Shanxi Province, China, between March 2020 and February 2023. After 36 patients were excluded according to the predefined eli

Tolerance-aware robust ranking of ventricular assist device rotor designs using strict feasibility and percentile-based criteria.

Manufacturing variability can alter the hydraulic and hemocompatibility performance of a ventricular assist device (VAD) rotor even when the nominal design is computationally optimal. This study presents a surrogate-assisted robust-design workflow for a rotary VAD using a CFD-derived design-of-experiments database of 4809 rotor geometries defined by six input variables and seven performance outputs. Output-specific surrogate models were selected from linear regression, quadratic response-surface models, support-vector regression, random forests, gradient boosting, and multilayer perceptrons us

Safety of withdrawal of pharmacological treatment after recovery from HER2 therapy-related cardiac dysfunction: the HER-SAFE trial.

BACKGROUND: Heart failure therapy (HFT) is routinely continued in breast cancer survivors after recovery from anti-HER2 cancer therapy-related cardiac dysfunction (CTRCD), despite limited evidence. HER-SAFE is the first randomised trial to assess HFT withdrawal in this population. METHODS: In this open-label, multicentre, randomised controlled trial, breast cancer survivors with recovered anti-HER2 CTRCD (asymptomatic, left ventricular ejection fraction [LVEF] ≥50%, normalised biomarkers) were randomised to HFT withdrawal or continuation. Antecedent CTRCD was mild (relative global longitudina

Myocardial Histopathology-Based Heart Failure Phenotyping.

BACKGROUND: Myocardial hypertrophy and interstitial fibrosis are the structural hallmarks evaluated on histopathology in heart failure (HF). Direct study of myocardial tissue has reemerged as a strategy to dissect the biology of heart failure with preserved ejection fraction (HFpEF), but whether these features, assessed together, define distinct subgroups within HF phenotypes remains unknown. OBJECTIVES: The authors assessed whether phenogroups defined by fibrosis and hypertrophy (histogroups) display distinct clinical profiles and outcomes in HFpEF vs heart failure with reduced ejection frac

The role of sex and ethnicity in the variability of estimated glomerular filtration rate (eGFR) in people with type 1 diabetes.

AIMS: To examine sex-based differences in estimated glomerular filtration rate (eGFR) trajectories for people with type 1 diabetes (T1D) over 10 years and to test the hypothesis that sex and ethnicity interaction predicts eGFR decline. METHODS: A cohort of 1495 individuals, 48% men (81% White, 12% African-Caribbean and 7% Other) and baseline eGFR ≥45 mL/min/1.73 m2 was analysed. Group-based trajectory modelling (GBTM) was used to identify eGFR sex-specific trajectories, and linear mixed models were used to evaluate the role of sex and ethnicity interaction in predicting eGFR heterogeneity. Diet and Risk of Cardiovascular Disease.

Diet ranks among the most modifiable determinants of cardiovascular disease risk, yet the evidence base is vast, conflicting, and burdened by observational confounding. This narrative review synthesizes evidence on controversial dietary components and established heart-healthy dietary patterns. Moderate egg intake is acceptable within a high-quality dietary pattern; preparation method meaningfully modifies risk. Habitual moderate coffee consumption is neutral to modestly cardioprotective; unfiltered preparations elevate low-density lipoprotein cholesterol (LDL-C) through diterpene retention. N

Geometric morphometric analysis of cardiac shape changes associated with myxomatous mitral valve disease in Cavalier King Charles Spaniels.

Myxomatous mitral valve disease (MMVD) is a progressive cardiac disorder in Cavalier King Charles Spaniels (CKCS), characterized not only by cardiac enlargement but also by complex morphological remodeling of the cardiac silhouette. This study aimed to evaluate stage-dependent configurational changes in heart shape across the American College of Veterinary Internal Medicine (ACVIM) stages (A-D) using two-dimensional geometric morphometric analysis of thoracic radiographs. A total of 54 CKCS dogs (Stage A: n = 9, Stage B1: n = 11, Stage B2: n = 10, Stage C: n = 12, Stage D: n = 12) that underwe

Influence of including or excluding endocardial borders on left ventricular wall thickness measurements in cats.

BACKGROUND: Consensus is lacking on which endocardial borders to include when measuring left ventricular wall thickness (LVWT) in cats. HYPOTHESIS/OBJECTIVES: The number of endocardial borders included can influence whether an individual cat is classified as having hypertrophic cardiomyopathy (HCM) or not. ANIMALS: Fifty-two cats that previously were diagnosed as either healthy, equivocal for HCM, or having an HCM phenotype. METHODS: Single-center retrospective cross-sectional study. Three observers measured LVWT using 4 different LVWT measurement protocols: leading edge-to-leading edge (L-

Selective organ deposition of nanoparticles directed by metal-organic network coatings.

Despite significant advances in nanoengineering, developing a universal nanoparticle engineering strategy that enables tailored nanoparticle organ deposition post intravenous administration remains challenging. Here, we report a simple and modular strategy mediated by metal-organic network coatings composed of polyethylene glycol (PEG), proteins, phenolic ligands, and metal ions. The versatile binding affinity of phenolic ligands facilitates the formation of coatings on diverse nanoparticle platforms, including quantum dots, lipid, polymeric, inorganic, and metal-organic nanoparticles.
