Establishment of an Animal Model of Type 2 Cardiorenal Syndrome.

Type 2 cardiorenal syndrome (CRS) is a complex clinical syndrome characterized by chronic kidney injury resulting from chronic cardiac dysfunction. Its underlying pathophysiological mechanisms remain incompletely understood, highlighting the urgent need for disease models that better mimic human pathology and accurately simulate the progression of cardiac-induced renal injury. Existing models possess significant limitations and often fail to adequately replicate the onset and progression observed in clinical Type 2 CRS patients. We therefore hypothesized that a double-hit approach combining le

Identification of CCSER1 missense variants and diplotypes as molecular markers for growth and lambing-related traits in Junken meat sheep.

To validate the potential role of CCSER1, a candidate gene for litter size and yearling weight identified by selection signature analysis, in lambing performance and growth development of Junken meat sheep, CCSER1 polymorphisms were genotyped using the Sequenom MassARRAY® SNP assay. Association analysis was conducted to assess the associations of CCSER1 polymorphisms and SNP2-SNP3 diplotypes with growth and lambing-related traits, and reverse transcription quantitative PCR (RT-qPCR) was used to determine the expression profile of CCSER1 in the ewes' digestive, developmental and reproductive ti

Cerebral Oxygenation Changes During Sequential Postural Transitions From Supine to Walking in Healthy Adults: An Exploratory Near-Infrared Spectroscopy Study.

Objective Early mobilization (EM) in the intensive care unit (ICU) reduces complications from prolonged bed rest and improves functional outcomes. However, there are still no objective criteria for cerebral hemodynamics during postural changes, especially walking. Methods This exploratory observational study used near-infrared spectroscopy (NIRS) to measure prefrontal cerebral oxygenation changes in 14 healthy adults during sequential postural transitions from supine rest to sitting, standing, and walking. The following parameters were assessed: oxygenated hemoglobin (O₂Hb), deoxygenated hemog

Aspirin Omission at the Time of Primary Percutaneous Coronary Intervention in STEMI.

BACKGROUND: The safety of omitting up-front treatment with aspirin during primary percutaneous coronary intervention (PCI) in patients with ST-segment elevation myocardial infarction (STEMI) remains unclear. METHODS: We conducted a multicenter, open-label, randomized trial in Japan involving patients with STEMI who were undergoing primary PCI. Patients were randomly assigned in a 1:1 ratio before PCI to receive low-dose prasugrel monotherapy or dual antiplatelet therapy (DAPT) with aspirin and low-dose prasugrel for 12 months. The primary outcome was a composite of death from any cause, strok

Milvexian with Antiplatelet Therapy after Acute Coronary Syndrome Event.

BACKGROUND: Patients are at increased risk for recurrent ischemic events after an acute coronary syndrome event. Milvexian, an oral factor XIa inhibitor, may reduce the risk of major adverse clinical events with minimal bleeding risk. METHODS: In this phase 3, randomized, placebo-controlled trial, we evaluated the efficacy and safety of milvexian when added to standard antiplatelet therapy within 7 days after an acute coronary syndrome event. Patients were assigned in a 1:1 ratio to receive oral milvexian (25 mg twice daily) or matched placebo. The primary efficacy outcome was a composite of

Targeted Use of Computed Tomographic Coronary Angiography in Acute Chest Pain.

BACKGROUND: Many patients with suspected acute coronary syndrome in whom myocardial infarction has been ruled out in the emergency department remain at risk for cardiovascular events. Whether further investigation reduces this risk is unknown. METHODS: In a multicenter, randomized, controlled trial, patients who presented to an emergency department in 14 hospitals across the United Kingdom were enrolled if myocardial infarction had been ruled out and high-sensitivity cardiac troponin testing indicated an intermediate risk of a cardiovascular event (maximum high-sensitivity cardiac troponin I

Patients with angina and non-obstructive coronary artery disease undergoing comprehensive invasive coronary function testing: long-term clinical outcomes.

BACKGROUND AND AIMS: Comprehensive evaluation by invasive coronary function test (CFT) of patients with angina and non-obstructive coronary artery disease is recommended in current guidelines. The prognosis of patients with CFT-proven coronary vasomotor dysfunction (CVDys) is less well-known. METHODS: Patients that provided consent for the previously published NL-CFT registry who underwent clinically indicated invasive CFT between February 1st, 2019 and June 1st, 2024 were included. Patients with CVDys defined as an abnormal spasm provocation test and/or microvascular functional assessment, w

Comparison Between the Acurate Neo and the Acurate Neo 2 Transcatheter Heart Valve System: Impact of Midframe Under-Expansion.

BACKGROUND: Transcatheter aortic valve implantation (TAVI) with the Acurate Neo has been linked to a significant incidence of paravalvular leakage (PVL), addressed by the Acurate Neo 2 incorporating an active sealing skirt. Recent findings suggest that midframe under-expansion is associated with worse outcomes. AIMS: We investigated the hemodynamic and clinical outcomes after TAVI with Acurate Neo 2 compared to Acurate Neo and assessed the impact of midframe under-expansion. METHODS: Patients who received Acurate Neo (n = 124) or Acurate Neo 2 (n = 133) between February 2018 and December 202
