Single-Port Robotic Ileostomy Reversal Through the Stoma Site With Ileotransverse Anastomosis: A Technical Video.

This video demonstrates the use of the single-port (SP) robotic system for ileostomy takedown through the existing stoma site in an 82-year-old female with hypertension, hyperlipidemia, and type 2 diabetes. Four months earlier, she underwent exploratory laparotomy, right hemicolectomy, and ileostomy for bowel ischemia. After an uneventful recovery, she was scheduled for ileostomy reversal using the SP robotic platform. The video demonstrates platform setup with execution of the single-port access port. The key procedural shows the benefits of the SP approach, including reduced invasiveness and

Comparison of delayed versus immediate coloanal anastomosis in patients with low rectal cancer: a recent meta-analysis.

BACKGROUND: In sphincter-preserving surgery for low rectal cancer (LRC), Immediate coloanal anastomosis (ICA) combined with prophylactic ileostomy remains the standard approach. However, this procedure is associated with the need for a second operation to reverse the stoma and risks of stoma-related complications. Delayed coloanal anastomosis (DCA) has recently regained attention as an alternative strategy, particularly in the context of evolving principles of minimally invasive surgery and enhanced recovery after surgery (ERAS). This meta-analysis aimed to systematically evaluate and compare

Intestinal perforation in recurrent cervical cancer following bevacizumab and pembrolizumab therapy: A case report.

RATIONALE: Since the advent of immunotherapy in clinical practice, it has profoundly transformed the paradigm of cancer treatment and has been rapidly adopted in clinical settings. Concurrently, the combination of immunotherapy with anti-angiogenic therapy has shown great promise in clinical research. The inevitable joint application brings about a greater number of adverse reactions. These adverse reactions are often perplexing, with the uncertainty of whether they stem from immunotherapy, anti-angiogenic therapy, or both. This is a case report of adverse reactions occurring when immune drugs

Complete Intracorporeal Single-Stapling Technique Anastomosis Using a Novel Detachable Purse-String Instrument in Robotic Surgery With Transanal Total Mesorectal Excision for Low Rectal Cancer.

INTRODUCTION: New techniques for creating anastomoses have been evolving as colorectal surgery shifts toward more minimally invasive robotic platforms. However, robotic rectal cancer surgery still typically involves partial extracorporeal manipulation to secure the anastomotic anvil. We previously performed a simple robotic purse-string suture (RPSS) in complete intracorporeal double-stapling technique (DST) anastomosis and first reported its benefit in the surgical time and the burden on the assistant. Moreover, transanal total mesorectal excision (TaTME) for lower rectal cancer has attracted

Improvement in postharvest preservation of guava using edible coating composed of natural components-a synergistic effect.

The patient with status epilepticus has continuous or rapidly repeating seizures. Generalised convulsive status epilepticus (GCSE) is the most common form of the disorder and is a life-threatening condition that requires prompt medical management. Status epilepticus that does not respond to first-line benzodiazepines (lorazepam or diazepam) or to second-line antiepileptic drugs (phenytoin/fosphenytoin, phenobarbital or valproate) is usually considered refractory and requires more aggressive treatment. The optimal treatment of refractory GCSE has not been defined, but patients should be treated

Transvaginal bowel evisceration and gangrenous small-bowel obstruction after self-amputation of neglected uterovaginal prolapse: a case report and review of literature.

Neglected uterovaginal prolapse (UVP) can lead to life-threatening complications. We report a 45-year-old para 5 woman with a 10-year history of UVP who presented after 2 days of a protruding vaginal mass, bilious vomiting, fever, and abdominal pain following an attempt to cut the prolapsed tissue with a blade. On examination, she was tachycardic, hypotensive, actively bleeding from the prolapse, and showed signs of small-bowel evisceration. Emergency laparotomy identified a 4-cm anterior uterine defect with 60 cm gangrenous ileum herniated ~15 cm from the ileocecal junction; the necrotic segm

Clinical application of the B-type sutured ileostomy in robotic-assisted low anterior resection for rectal cancer: a propensity score matching analysis.

Currently, there is no consensus on the position and method for temporary ileostomy in robotic-assisted low anterior resection for rectal cancer. Herein, this study introduced the B-type sutured ileostomy, a new temporary ileostomy technique, and compared it to the traditional one to assess its efficacy and safety. Between September 2020 and December 2022 in our centre, B-type sutured ileostomy was performed on 124 patients undergoing robotic-assisted low anterior resection for rectal cancer. A retrospective review of a prospectively collected database identified patients who underwent robotic

Patient-centred stoma care support: ileostomy patients.

Stoma patients require continuous support throughout their entire journey with a stoma. Although many Stoma Care Services across the UK offer patient follow-up pathways, there is not one unified pathway. Patients may not be prepared for life with a stoma because, depending on their stoma type, they will have specific needs, and if patients and healthcare professionals are not prepared to manage these stoma-specific needs, complications and hospital readmissions may occur, worsening patients' outcomes and quality of life. Ileostomy patients are known to be more likely to experience complication

Effect of genetic profiling on surgical decisions at hereditary colorectal cancer syndromes.

Hereditary colorectal cancer syndromes, such as Lynch syndrome and familial adenomatous polyposis (FAP), present significant clinical challenges due to the heightened cancer risks associated with these genetic conditions. This review explores genetic profiling impact on surgical decisions for hereditary colorectal cancer (HCRC), assessing options, timing, and outcomes. Genotypes of different HCRCs are discussed, revealing a connection between genetic profiles, disease severity, and outcomes. For Lynch syndrome, mutations in the MLH1, MSH2, MSH6, and PMS2 genes guide the choice of surgery. Subt

Single Anastomosis Duodeno-Ileostomy with Sleeve Gastrectomy/Single Anastomosis Duodenal Switch (SADI-S/SADS) IFSO Position Statement-Update 2023.

The single anastomosis duodeno-ileostomy with sleeve gastrectomy/single anastomosis duodenal switch (SADI-S/SADS) has gained attention as an alternative to the traditional biliopancreatic diversion with duodenal switch (BPD-DS). In 2021, IFSO endorsed SADI-S/SADS as a safe and effective procedure, underscoring the necessity for long-term multidisciplinary care and randomized controlled trials (RCTs). A task force was established to conduct a systematic review of current evidence on SADI-S/SADS to guide clinical practice. A systematic review was conducted across three databases, focusing on stu

Spigelian Hernia in Cirrhotic Patients: When and How to Repair?

BACKGROUND: Spigelian hernia, also known as spontaneous lateral ventral hernia, is defined as a protrusion of abdominal contents through an abdominal wall defect within the transversus aponeurosis. The entity was first reported in 1742 and named after Adrian van der Spieghel. It is recognized as a rare condition, accounting for 1 to 2% of all abdominal wall hernias. Cirrhotic patients, in particular, are more predisposed to hernias of all types. When Spigelian hernia is accompanied by hepatic cirrhosis, the decision to repair gets into a controversial aspect. Herein, we present the emergency m

Short-term outcome of diverting loop ileostomy reversals performed by residents: a retrospective cohort prognostic factor study.

AIM: The reversal of diverting loop ileostomy (DLI) is one of surgical trainees' first procedures. Complications of DLI reversal can cause life-threatening complications and increase patient morbidity. This study compared DLI reversals performed by surgical trainees with those by attending surgeons. METHOD: This retrospective cohort study was performed at a single primary care center on 300 patients undergoing DLI reversal. The primary outcome was morbidity, according to the Clavien-Dindo classification (CDC), with special attention paid to the surgeon's level of training. The secondary endpo
