RAPID LOW-COST DETECTION OF TYPE 2CALR MUTATION BY ALLELE-SPECIFIC RT-PCR FOR DIAGNOSIS OF MYELOPROLIFERATIVE NEOPLASMS.

Automated insulin delivery (AID) systems have become a standard of care in type 1 diabetes and are increasingly used across all age groups, including during pregnancy. As their utilization expands, clinicians more often encounter patients using AID systems in perioperative and peripartum settings. However, guidance regarding continuation of AID therapy during procedures under anesthesia / sedation or during labor remains limited. This document provides practical, consensus‑based recommendations for the use of AID systems during minor procedures, selected major surgeries, and throughout labor a

Behavioral dysregulation exacerbated by intrathecal baclofen in an adolescent with severe traumatic brain injury.

This is a case of a 19-year-old male with a history of remote severe traumatic brain injury (TBI) with an intrathecal baclofen (ITB) pump for dystonia management. Given concern for lack of efficacy despite increasing doses of ITB, his catheter was evaluated and found to be epidural rather than intrathecal. The baclofen dose was down-titrated and he underwent catheter revision. Post-op, his baclofen dose was up-titrated and complicated by significant behavioral changes including aggressive physical and verbal behaviors resulting in hospitalization. Work-up was negative for infection, new neurol

Insulin Allergy to Detemir Followed by Rapid Onset of Diabetic Ketoacidosis: A Case Report and Literature Review.

The management of diabetes mellitus in an insulin-dependent patient is challenging in the setting of concomitant antibody-mediated-insulin hypersensitivity. We report a case of a 62-year-old woman with pre-existing type 2 diabetes mellitus of 10 years duration who developed type 3 hypersensitivity reaction to insulin analogue detemir, and subsequently, severe diabetic ketoacidosis (DKA). She was C-peptide negative and was diagnosed with insulin-dependent diabetes. Despite increasing dose adjustments, insulin-meal matching, and compliance with insulin, she experienced episodes of unexpected hyp

High-Density Branched PEGylation for Nanoparticle Drug Delivery.

UNLABELLED: Infectious bronchitis virus (IBV) remains a major threat to the poultry industry owing to its high transmissibility and the continuous emergence of antigenically diverse variants that compromise vaccine efficacy. The viral spike protein, which mediates host cell attachment and membrane fusion, represents an attractive target for antiviral intervention. In this study, a structure-based computational approach was employed to identify phytochemical inhibitors targeting the IBV spike protein. A curated library (PCLibVer2) comprising 2,400 phytochemicals derived from poultry-safe botani

Minimizing the Risk of Exercise-Induced Glucose Fluctuations in People Living With Type 1 Diabetes Using Continuous Subcutaneous Insulin Infusion: An Overview of Strategies.

Physical activity (PA) is important for individuals living with type 1 diabetes (T1D) due to its various health benefits. Nonetheless, maintaining adequate glycemic control around PA remains a challenge for many individuals living with T1D because of the difficulty in properly managing circulating insulin levels around PA. Although the most common problem is increased incidence of hypoglycemia during and after most types of PA, hyperglycemia can also occur. Accordingly, a large proportion of people living with T1D are sedentary partly due to the fear of PA-associated hypoglycemia. Continuous s

Association Between Management of Continuous Subcutaneous Basal Insulin Administration and HbA1C.

BACKGROUND: While we expect that patients who adjust their insulin delivery algorithms between clinic visits to have better glucose control compared to those who do not, this effect has not been quantified. METHOD: This is a single-center retrospective cohort study including pediatric and adult patients with type 1 diabetes evaluating insulin pump self-management behaviors. Basal insulin dose information was obtained from the Glooko-Diasend database, and used to quantify the frequency and magnitude of basal insulin daily dose adjustments within the 90-day window preceding HbA1c measurement.
