Exploring socioeconomic disparities in outcomes and follow-up after endovascular treatment of abdominal aortic aneurysms among Medicare beneficiaries.

OBJECTIVE: Socioeconomic disparities are known to contribute to adverse outcomes after surgery; however, the role of individual wealth and neighborhood environment on both follow-up and outcomes following endovascular aneurysm repair (EVAR) are not well-understood. METHODS: We included all fee-for-service Medicare beneficiaries aged ≥66 years who underwent infrarenal EVAR with a bifurcated endograft for intact abdominal aortic aneurysm (AAA) from 2011 to 2019. Patients were divided into cohorts using dual enrollment in Medicare/Medicaid (vs Medicare only) as a measure of individual wealth and

The relationship between catheter-related bloodstream infection and multi-drug resistant bacteria: a five-year retrospective study.

BACKGROUND: Catheter-related bloodstream infection (CRBSI) is a severe hospital-acquired infection primarily associated with using central venous catheters (CVCs). With the rise of antibiotic resistance and the emergence of multidrug-resistant organisms (MDROs), there is a growing concern about the relationship between MDROs and CRBSI. OBJECTIVE: To explore the association between MDROs and CRBSI. METHODS: A retrospective study was conducted on hospitalized patients with implanted CVCs over 54 months (January 2019 to June 2023). RESULTS: We included 50,718 patients with indwelling central v

Quality of anticoagulation and outcomes after mechanical aortic valve replacement in patients with atrial fibrillation: a nationwide cohort study.

AIMS: Mechanical aortic valve replacement (AVR) remains the primary treatment for younger patients with severe aortic valve disease. However, limited information is available regarding the quality of the required lifelong vitamin K antagonist (VKA) therapy, atrial fibrillation (AF), and their relationship with adverse events after AVR. This study assessed the quality of VKA therapy prior to bleeding and ischaemic events following mechanical AVR in patients with AF. METHODS AND RESULTS: The registry-based Finnish AntiCoagulation in Atrial Fibrillation study combining data from several Finnish

The incremental role of late gadolinium enhancement in risk stratifying high risk patients with hypertrophic cardiomyopathy.

BACKGROUND: Current risk stratification models for hypertrophic cardiomyopathy (HCM) rely on a combination of clinical and imaging factors. Recent studies have highlighted the potential role of late gadolinium enhancement (LGE) in enhancing sudden cardiac death (SCD) risk stratification. This study evaluates whether LGE can recalibrate risk stratification models and influence decisions regarding implantable cardioverter-defibrillator (ICD) implantation. We aim to assess the prognostic value of LGE in predicting SCD and its interaction with other established risk factors in patients with HCM.

Algorithm-based remote monitoring of heart failure risk data in people with cardiac implantable electronic devices: a systematic review and cost-effectiveness analysis.

BACKGROUND: Heart failure is a clinical syndrome caused by any structural or functional cardiac disorder that impairs the heart's ability to function efficiently and pump blood around the body. Function can also be monitored using cardiac implantable electronic devices, some of which may also deliver a therapeutic benefit (e.g. pacemakers), while others only monitor metrics over time. Implantable devices can include algorithms that aim to predict the occurrence of a heart failure event. They are intended to be used alongside clinical judgement and make treatment decisions. OBJECTIVES: To dete

Role of implantable loop recorders in optimizing care for patients during and after left ventricular assist device support: a single centre experience.

BACKGROUND: Left ventricular assist devices (LVADs) are increasingly used in heart failure patients. Arrhythmias are common in those patients, including patients with LVADs, and the interplay between electrical activity and LVAD-related hemodynamic changes is critical for managing complications. While preexisting cardiac implantable electronic devices (CIEDs) can aid in monitoring, they are not always present, and the role of de novo implantable cardioverter-defibrillators (ICDs) in LVAD patients remains debated. Implantable loop recorders (ILRs) may offer a solution by enabling continuous mon

A Large, Real-World Cohort Analysis of Arrhythmia Detection and Therapeutic Interventions in Patients With Insertable Cardiac Monitors and Long-Term Monitoring.

BACKGROUND: Insertable cardiac monitors (ICMs) provide long-term continuous monitoring for arrhythmia diagnosis and management for various clinical indications. However, little data exists on comprehensive real-world arrhythmia diagnostic yield and therapy rates in patients indicated for ICMs with validated artificial intelligence (AI) algorithms enabling large-scale, automated adjudication of ICM-detected episodes. We report the largest real-world analysis of arrhythmia detection as well as medical and procedural therapies in patients with ICMs implanted for guideline-approved indications wit

Escalation From Intra-Aortic Balloon Pump in Cardiogenic Shock Patients: A Multistate Analysis of the Altshock-2 Registry.

Cardiogenic shock is a deadly disorder and temporary mechanical circulatory support devices (tMCS) have been introduced to improve survival. However, criteria and strategies for timely placement and escalation are lacking. A major limitation of previous research is the focus on a single intervention or device; in clinical practice, these patients are transitioned through multiple configurations of tMCS. Accordingly, we have applied the generalized framework of a multistate model to enhance our understanding of the disease process. A multistate survival analysis was conducted on the Altshock-2

Trends in mechanical circulatory support utilization, left ventricular assist device implantation and transplant during cardiogenic shock hospitalizations, after the new heart allocation policy.

BACKGROUND: In October 2018, a new heart allocation policy was implemented to risk stratify patients listed for transplant, prioritizing patients supported with temporary mechanical circulatory support (MCS). The policy changes may have had an impact on the management of cardiogenic shock (CS). We sought to determine the changes in use of temporary MCS, durable left ventricular assist device (LVAD) and transplant in patients hospitalized before and after the new policy. METHODS: A retrospective analysis was conducted using the National Inpatient Sample (NIS) between 2017 and 2020. Hospitaliza

Impact of Lead-shielding on the Utilization of Magnetic Resonance Imaging Among Patients with Spinal Cord Stimulation Systems: A Study of Retrospective Claims in the US.

BACKGROUND: Spinal cord stimulation (SCS) is an effective approach to managing chronic, intractable pain. However, even when the designs of SCS devices are compatible with magnetic resonance imaging (MRI), migration and the fracture or wire breakage of epidural leads can result in increased lead impedance and a requirement that the devices be explanted before safe imaging can take place. Selected SCS systems are designed with other technical features. Specifically, lead shielding (lead wires are encased in a matrix-like tantalum shield) serves to dissipate radiofrequency energy along the lead
