A Review of Machine Learning Approaches for the Personalization of Amplification in Hearing Aids.

This paper provides a review of various machine learning approaches that have appeared in the literature aimed at individualizing or personalizing the amplification settings of hearing aids. After stating the limitations associated with the current one-size-fits-all settings of hearing aid prescriptions, a spectrum of studies in engineering and hearing science are discussed. These studies involve making adjustments to prescriptive values in order to enable preferred and individualized settings for a hearing aid user in an audio environment of interest to that user. This review gathers, in one

Towards ASSR-based hearing assessment using natural sounds.

Objective. The auditory steady-state response (ASSR) allows estimation of hearing thresholds. The ASSR can be estimated from electroencephalography (EEG) recordings from electrodes positioned on both the scalp and within the ear (ear-EEG). Ear-EEG can potentially be integrated into hearing aids, which would enable automatic fitting of the hearing device in daily life. The conventional stimuli for ASSR-based hearing assessment, such as pure tones and chirps, are monotonous and tiresome, making them inconvenient for repeated use in everyday situations. In this study we investigate the use of nat

Remote or in-clinic? The effect of service delivery mode on hearing aid output: study protocol for a double-blinded, randomised trial in adults with mild to moderate sensorineural hearing loss.

BACKGROUND: Teleaudiology can potentially improve access to hearing healthcare services. Remote hearing aid fittings offer a new mode of service delivery that removes barriers of geography and access to an audiologist. Real-ear measurements (REMs) are the gold standard for hearing aid output verification but require in-clinic appointments. This study will investigate whether remote hearing aid fittings can provide clinically equivalent outcomes when compared to current, in-clinic, best practice guidelines. RESEARCH DESIGN: A repeated measure, double-blinded crossover design will be used. Part

[Cochlear implantation in patients with autoimmune hearing loss].

BACKGROUND: Autoimmune inner ear disease (AIED) manifests with recurrent fluctuating sensorineural hearing loss and vestibular symptoms. Treatment includes steroids and a variety of immunosuppressants. Despite adequate treatment, sensorineural hearing loss can be progressive to the point of deafness. In these patients, a cochlear implant (CI) is indicated. We present the case of a 25-year-old male who underwent cochlear implantation in the left ear. After implantation we noticed brisk variations in impedances which were related to application of the previously prescribed tumor necrosis alpha (

In-situ Audiometry Compared to Conventional Audiometry for Hearing Aid Fitting.

The use of in-situ audiometry for hearing aid fitting is appealing due to its reduced resource and equipment requirements compared to standard approaches employing conventional audiometry alongside real-ear measures. However, its validity has been a subject of debate, as previous studies noted differences between hearing thresholds measured using conventional and in-situ audiometry. The differences were particularly notable for open-fit hearing aids, attributed to low-frequency leakage caused by the vent. Here, in-situ audiometry was investigated for six receiver-in-canal hearing aids from dif

Comparing Self-Fitting Strategies for Over-the-Counter Hearing Aids: A Crossover Clinical Trial.

IMPORTANCE: Fewer than 20% of US adults with hearing loss use hearing aids due to barriers like high cost. Over-the-counter (OTC) hearing aids offer a potential solution, incorporating self-fitting strategies via smartphone apps. Self-fitting strategies have been validated for Food and Drug Administration (FDA)-approved OTC hearing aids compared with prescription-based approaches. However, no direct comparative analysis exists between in situ audiometry and self-adjustment strategies using self-fitting OTC (OTC-SF) hearing aids. OBJECTIVE: To compare self-adjustment and in situ audiometry sel

Comparison between the International Outcome Inventory for Hearing Aids Questionnaire and Real-Ear Measurement.

INTRODUCTION: The international outcome inventory for hearing aids (IOI-HA) is a questionnaire widely used to assess the subjective benefits of hearing aids. This study aimed to evaluate the relationship between IOI-HA outcomes and target mismatch in real-ear measurement (REM). METHODS: Thirty-four ears of 25 patients who had worn hearing aids were evaluated using the Korean version of the IOI-HA and REM after fitting for 2 months. Real-ear insertion gain (REIG) was measured at three different levels of input intensity - 50, 65, and 80 dB sound pressure level (SPL) - and a frequency range of

Evaluation of a semi-supervised self-adjustment fine-tuning procedure for hearing aids for asymmetrical hearing loss.

OBJECTIVE: This study investigated a previously evaluated self-adjustment procedure with respect to its applicability for asymmetrical hearing loss (AHL). Self-adjusted settings were evaluated for speech recognition in noise and sound preference. DESIGN: Participants were given the possibility to adjust the left and right hearing aid separately using a two-dimensional user interface. Two different adjustment sequences were tested. Realistic everyday sound scenes in a laboratory environment were presented. The difference between the ears regarding their speech recognition in noise was tested w

Uncovering the effects of pandemic conditions on hearing aid experiences: a dual perspective from audiologists and hearing aid users.

Purpose: This study examines the effects of the pandemic on hearing aid experiences from the perspectives of audiologists and patients, as well as communication challenges faced by both users and non-users of hearing aids. Materials and Methods: The study consisted of two phases: a longitudinal study examining the services provided by audiologists during the peak of the COVID-19 pandemic and after the first vaccination dose. The second phase involved a survey of 120 elderly individuals with hearing loss, divided into hearing aid users and non-users for assessing communication challenges. Data

Development and Evaluation of a Loudness Validation Method With Natural Signals for Hearing Aid Fitting.

Loudness is a fundamental dimension of auditory perception. When hearing impairment results in a loudness deficit, hearing aids are typically prescribed to compensate for this. However, the relationship between an individual's specific hearing impairment and the hearing aid fitting strategy used to address it is usually not straightforward. Various iterations of fine-tuning and troubleshooting by the hearing care professional are required, based largely on experience and the introspective feedback from the hearing aid user. We present the development of a new method for validating an individua

Consideration of Sufficient and Insufficient Benefits in Hearing Aid Fitting.

BACKGROUND: During hearing aid (HA) fitting, individuals may experience better speech discrimination at normal speech levels and worse discrimination at loud speech levels than without an HA. Therefore, we investigated factors that worsen speech discrimination when the speech sound level increases. METHODS: Speech discrimination was measured in patients aged >20 years who had average hearing thresholds <90 dB on pure-tone audiometry. An insufficient benefit was defined as speech discrimination being better at 65 dB sound pressure level (SPL) with HAs than without HAs and worse at 80 dB SPL.

Relationships Between Speech, Spatial and Qualities of Hearing Short Form SSQ12 Item Scores and their Use in Guiding Rehabilitation for Cochlear Implant Recipients.

Cochlear implantation successfully improves hearing in most adult recipients. However, in rare cases, post-implant rehabilitation is required to maximize benefit. The primary aim of this investigation was to test if self-reports by cochlear implant users indicate the need for post-implant rehabilitation. Listening performance was assessed with the Speech, Spatial and Qualities short-form SSQ12, which was self-administered via a web-based survey. Subjects included over 2000 adult bilateral or unilateral cochlear implant users with at least one year of experience. A novel application of regressi

An evolutionary perspective to intramedullary nail fit - comparing present-day data with a historical specimen collection.

Manufacturers aim to design implants fitting for the broadest possible population segment. Due to the scarcity of available morphological data of intact long bones, anatomical collections of historical bone specimens may represent valuable additional sources. Previous work on femoral morphology measurements suggests that historical specimens are widely consistent with data from present-day populations. This study aimed to investigate whether this also applies to the anatomical fitting of a clinically used femoral nail. Nail fit was computer-graphically quantified through virtual implantation i
