BACKGROUND: Tuberculous otitis media is a chronic Mycobacterium tuberculosis infection of the middle ear tissues. Diseases with varied and insidious clinical features can make diagnosis difficult and delay treatment.
CASE PRESENTATION: Here, we document a case of tuberculous otitis media in a 46-year-old ethnic Han woman that manifested as nonspecific chronic otitis media. A mastoidectomy and tympanoplasty were performed for the initial diagnosis of cholesteatoma. The histopathology of the tissue specimen revealed granuloma formation with necrosis. Staining for acid-fast bacilli and the polym

Infections in renal transplant recipients (RTRs) carry great risk of morbidity-mortality, and the risk of graft loss. Concomitant infections, although common in immunosuppressed patients, should be highly suspected. The present case involves a RTR with opportunistic pulmonary co-infections caused by Mycobacterium tuberculosis (MTb), Mycobacterium smegmatis (MSm), and Aspergillus spp. While MSm can be a colonizing microorganism, we demonstrate that it can also be pathogenic.

INTRODUCTION: The risk of tuberculosis (TB) during pregnancy is significantly increased, and untimely Mycobacterium tuberculosis (M. tuberculosis) treatment during pregnancy increases the risk of perinatal complications and poor fetal prognosis. Hormonal use in vitro fertilization and embryo transfer (IVF-ET) promotes the growth of M. tuberculosis, leading to more severe disease and adverse pregnancy outcomes. However, its clinical characteristics and possible mechanisms of TB infection in pregnant women who conceive by IVF-ET remain unclear. Therefore, we report three cases of young pregnant

Atypical forms of tuberculosis (TB) are gaining recognition, challenging traditional diagnostic criteria classically linked to chronic cough, lethargy, anorexia, evening pyrexia, and weight loss in immunocompromised patients. We present the case of a 23-year-old immunocompetent man from rural central India who developed unilateral pulmonary miliary TB with concurrent abdominal TB lymphadenitis. The patient presented with a two-month history of chronic cough and diffuse abdominal pain. Initial chest radiograph revealed unilateral, right-sided miliary opacities, while abdominal ultrasonography i

Peritoneal tuberculosis (PTB) is a diagnostic challenge due to its nonspecific presentation, low sensitivity of ascitic fluid microbiologic tests, and possible resemblance to peritoneal carcinomatosis. We describe a case of 51-year-old woman with long-term tumor necrosis factor alpha (TNF-α) inhibitor exposure presented with fever and abdominal distension after returning from the Philippines. Initial tests, including microbiological studies, were inconclusive, despite imaging findings suggestive of carcinomatosis with elevated CA-125. Diagnostic laparoscopy revealed miliary white nodules; howe

An 8-year-old girl presented with left leukocoria, prompting an evaluation that revealed bilateral intraocular masses, including a right upper nasal choroidal lesion and a large left heterogeneous mass with exudative detachment. Imaging and systemic assessment uncovered multiple intracranial lesions, miliary lung lesion, a facial lesion, and a right tibial lesion. A strongly positive tuberculin test and a history of close contact with a tuberculosis (TB) patient led to the diagnosis of disseminated TB. Treatment with anti-tuberculosis therapy and systemic steroids initially resulted in improve

Crohn's disease can lead to complications such as fistula formation, with ileosigmoid fistulas being common. This case report presents a 54-year-old male with Crohn's disease, who had abdominal pain, diarrhea, and weight loss. He had a history of miliary tuberculosis, which complicated the diagnostic process. Imaging revealed fistulas and thickening in the terminal ileum, while surgery included ileocecectomy, fistulectomy, and repair of affected areas. The patient recovered without complications. This case highlights the diagnostic challenges when Crohn's disease coexists with infectious disea

Mycobacterium (M.) tuberculosis mostly spreads from active tuberculosis (TB) patients to human contacts, although human-to-animal and animal-to-human transmission has been described. Here, we present a rare case of rifampicin-resistant tuberculosis (RR-TB) transmission from a companion dog to its owner, highlighting the zoonotic potential of the pathogen. Namely, a 37-year-old Croatian man was diagnosed with RR-TB, with whole-genome sequencing analysis revealing a close genetic link to the strain isolated from his dog, which had died of miliary TB six years earlier. This case emphasizes the co

In 2023, tuberculosis (TB) caused 1.25 million deaths among 10.8 million cases, remaining the deadliest infectious disease worldwide. In Germany, incidence declined for decades but rose slightly in 2022-2023, driven mainly by migration from high-prevalence countries (e.g., Syria, Afghanistan, Ukraine). While most cases are pulmonary TB (PTB), 22.8% are extrapulmonary TB (EPTB), which can affect any organ and is often hard to diagnose due to nonspecific symptoms. Diagnosis uses microscopy, PCR, and culture; treatment typically lasts at least 6 months, extended for CNS, bone, or miliary TB. Trea

Miliary tuberculosis (TB) presents significant diagnostic challenges in resource-limited, high-burden regions when microbiological confirmation is unattainable. We describe a 65-year-old post-menopausal Nepalese woman with prolonged fever, anemia, and constitutional symptoms. Initial chest radiography was inconclusive, but contrast-enhanced computed tomography (CECT) revealed diffuse miliary nodules, calcified lymphadenopathy, and pleural effusion. Microbiological confirmation was precluded by the inability to expectorate and refusal of bronchoscopy. Empirical anti-TB therapy (isoniazid, rifam

BACKGROUND: Choroidal granuloma is one of the common manifestations of ocular tuberculosis. Tubercles indicate hematogenous dissemination of the disease. Tubercular granulomas respond to Anti-Tubercular Treatment (ATT) and systemic corticosteroids. However, in some patients with large granulomas involving macula, adjunct treatment with intravitreal anti-VEGF may be required for prompt management of the granuloma.
FINDINGS: We report a case of bilateral Tubercular granuloma in a patient with miliary Tuberculosis (TB). The patient was an immunocompetent young female with miliary tuberculosis. M
