A comparison of the University of New Brunswick Test of Prosthetic Function and the Assessment of Capacity for Myoelectric Control.

BACKGROUND: Valid outcome measures are required for gathering evidence on when, how and using which prosthetic components to start prosthetic fitting to children with congenital upper limb deficiencies and those following acquired upper limb amputation. We have been using the University of New Brunswick Test of Prosthetic Function (UNB, which measures skill and spontaneity of prosthetic use) in our country since 1996, and the Assessment of Capacity for Myoelectric Control (ACMC, which was developed for persons using a myoelectric prosthesis) since 2008. AIM: We wanted to explore whether the U

"If I can do it I will do it, if I can't, I can't": a study of adaptive self-regulatory strategies following lower limb amputation.

PURPOSE: To explore the goal-related strategies employed by people following lower limb amputation using a framework based on the dual-process model of adaptive self-regulation. METHODS: Semi-structured interviews were conducted with 30 individuals with a lower limb amputation. RESULTS: Theoretical thematic analysis identified four broad assimilative/goal pursuit strategies; internal resource use, planning, technology use and help use. The most common strategies were maintaining a specific leisure activity (n=20), seeking instrumental help (n=15) and determination (n=15). Three broad categor

Adaptation to prostheses among patients with major lower-limb amputations and its association with sociodemographic and clinical data.

CONTEXT AND OBJECTIVE: Lower-limb amputation compromises patients' independence and autonomy, and therefore they should be referred for rehabilitation in order to adapt to prostheses and regain autonomy. The aim here was to assess adaptation to prostheses among patients with major lower-limb amputations and its association with sociodemographic and clinical data. DESIGN AND SETTING: This was a cross-sectional study in the city of Aracaju, Brazil. METHODS: The patients were identified by primary healthcare teams. The inclusion criterion was that these should be patients who underwent major lo

Prosthetics socket that incorporates an air splint system focusing on dynamic interface pressure.

BACKGROUND: The interface pressure between the residual limb and prosthetic socket has a significant effect on an amputee's satisfaction and comfort. This paper presents the design and performance of a new prosthetic socket that uses an air splint system. METHODS: The air splint prosthetic socket system was implemented by combining the air splint with a pressure sensor that the transhumeral user controls through the use of a microcontroller. The modular construction of the system developed allows the FSR pressure sensors that are placed inside the air splint socket to determine the required s

The use of an adjustable electrode housing unit to compare electrode alignment and contact variation with myoelectric prosthesis functionality: A pilot study.

BACKGROUND: Usable myoelectric control relies on secure and intimate contact at all times between the electrode, the socket and the residual limb. At present, there is little post-fitting socket adjustment available to prosthetists with respect to electrode contact security or alignment. Failure to provide secure electrode contact could result in the development of motion artefacts, poor prehensor response and subsequent prosthesis non-usage. OBJECTIVES: To establish the effect of alteration to electrode contract security and alignment on prosthesis functionality using a bespoke electrode hou

Development of an air pneumatic suspension system for transtibial prostheses.

The suspension system and socket fitting of artificial limbs have major roles and vital effects on the comfort, mobility, and satisfaction of amputees. This paper introduces a new pneumatic suspension system that overcomes the drawbacks of current suspension systems in donning and doffing, change in volume during daily activities, and pressure distribution in the socket-stump interface. An air pneumatic suspension system (APSS) for total-contact sockets was designed and developed. Pistoning and pressure distribution in the socket-stump interface were tested for the new APSS. More than 95% of t

Comparison of transhumeral socket designs utilizing patient assessment and in vivo skeletal and socket motion tracking: a case study.

PURPOSE: This case study compares the impact of two prosthetic socket designs, a "traditional" transhumeral (TH) socket design and a Compression Released Stabilized (CRS) socket. METHODS: A CRS socket was compared to the existing socket of two persons with transhumeral amputation. Comparisons included assessments of patient comfort and satisfaction with fit, as well as dynamic kinematic assessment using a novel high-speed, high-resolution, bi-plane video radiography system (XROMM, for X-ray Reconstruction of Moving Morphology). RESULTS: Subjects were more satisfied with the comfort of the tr

Horizontal alveolar ridge augmentation using autologous press fit bone cylinders and micro-lag-screw fixation: technical note and initial experience.

INTRODUCTION: The use of autologous block bone grafts for horizontal alveolar ridge augmentation in dental implantology is a common surgical procedure. Typically, bone grafts are individually moulded. OBJECTIVE: The aim of this paper is to introduce an innovative procedure in lateral bone augmentation, where the recipient side is adjusted to the graft, not vice versa as in common procedures. Our initial clinical experience of twenty-five consecutive cases is presented. MATERIALS AND METHODS: Adjusted trephine drills were used to harvest partly cylindrical grafts from the retromolar region of

Accuracy of an adjustable patient-specific guide for acetabular alignment in hip replacement surgery (Optihip).

Implant malalignment in hip arthroplasty increases the risk of revision surgery due to problems such as hip instability, wear, and impingement. Traditional instrumentation lacks accuracy and does not individualize the goal. Computer-assisted surgery (CAS) and patient-specific solutions improve accuracy but add considerably to the cost, amongst other drawbacks. We developed an adjustable mechanical device, called Optihip, which is set to a patient-specific goal preoperatively and is independent of pelvis position intraoperatively. The purpose of the present study was to evaluate Optihip's accur

Methods for characterization of mechanical and electrical prosthetic vacuum pumps.

Despite increasingly widespread adoption of vacuum-assisted suspension systems in prosthetic clinical practices, there remain gaps in the body of scientific knowledge guiding clinicians' choices of existing products. In this study, we identified important pump-performance metrics and developed techniques to objectively characterize the evacuation performance of prosthetic vacuum pumps. The sensitivity of the proposed techniques was assessed by characterizing the evacuation performance of two electrical (Harmony e-Pulse [Ottobock; Duderstadt, Germany] and LimbLogic VS [Ohio Willow Wood; Mt. Prosthetic fitting in a patient with a transtibial amputation due to a congenital vascular malformation of the right leg.

BACKGROUND: The problems of prescribing a prosthesis for a young girl with severe congenital vascular malformation deformity leading to a transtibial amputation. CASE DESCRIPTION AND METHODS: Due to the high risk of recurrent bleeding and limitations regarding full weight bearing of the stump, a normal socket fitting process was not possible. Using a multidisciplinary approach, a prosthesis was designed to enable full weight bearing in a flexed knee position with ischial tuberosity support to prevent full weight bearing on the tibial part of the stump. FINDINGS AND OUTCOMES: After training a

Effects of heel lifting on transtibial amputee gait before and after treadmill walking: a case study.

BACKGROUND: Prosthetic alignment is usually unchanged once optimized. However, a previous study indicated that long-distance walking significantly altered gait patterns, suggesting some alignment adjustments after walking are required. This study investigated the effects of alignment changes (by inserting a heel lift) on gait of a transtibial amputee before and after treadmill walking. CASE DESCRIPTION AND METHODS: The subject walked, without heel lifts, on a treadmill until perception of fatigue. Gait changes upon heel lifting at the prosthetic side were studied before and after the treadmil

The SmartHand transradial prosthesis.

BACKGROUND: Prosthetic components and control interfaces for upper limb amputees have barely changed in the past 40 years. Many transradial prostheses have been developed in the past, nonetheless most of them would be inappropriate if/when a large bandwidth human-machine interface for control and perception would be available, due to either their limited (or inexistent) sensorization or limited dexterity. SmartHand tackles this issue as is meant to be clinically experimented in amputees employing different neuro-interfaces, in order to investigate their effectiveness. This paper presents the d
