Maxillary sinus myoepithelioma: a case report.

INTRODUCTION: Pleomorphic adenoma (PA) in the sinonasal tract is very rare, with few cases documented in the literature. Myoepithelioma is a recognized variant of PA characterized by predominant myoepithelial differentiation and the absence of ductal structures. CASE PRESENTATION: A 67-year-old man presented with a right-sided sinonasal mass. Cross-sectional imaging revealed a lesion originating from the right maxillary sinus and extending into the ipsilateral nasal cavity and nasopharynx. An initial biopsy favored a diagnosis of inverted papilloma, and the patient subsequently underwent endo

Coordinate attention and deformable convolution-enhanced YOLO for bilateral maxillary sinus segmentation, a paranasal sinus structure, in CT images.

Accurate delineation of the maxillary sinuses in CT images is a relevant step supporting preoperative planning and the evaluation of sinonasal disease within otolaryngology. Manual contouring of these paranasal sinus structures is difficult and error-prone, since the surrounding anatomy is complex, bony structures are thin, and inter-patient variability is large. This paper proposes an instance segmentation framework that extends the YOLO11n-Seg architecture through the integration of Coordinate Attention (CoordAtt) modules and Deformable Convolutional Networks v2 (DCNv2), to improve spatial f

Microsurgical two-wall orbital decompression for treatment of visual impairment in patients with thyroid-associated ophthalmopathy.

BackgroundDysthyroid optic neuropathy (DON) is a vision-threatening complication of thyroid-associated ophthalmopathy that may require urgent surgical intervention. We describe a microsurgical two-wall orbital decompression technique designed to achieve effective orbital and optic nerve decompression in extremely severe cases as an additional option.MethodsA frontotemporal extradural approach is used to remove the lateral orbital wall and orbital roof, with optional decompression of the optic canal. Neuronavigation guides bone removal and helps avoid violation of the frontal sinus. The surgica

Infected Frontal Sinus Mucocele Mimicking a Cutaneous Abscess.

Frontal sinus mucoceles are benign expansile lesions that may progressively erode the surrounding bone and extend toward the orbit or intracranial compartment. When expansion occurs anteriorly, however, they may present as an apparently superficial forehead swelling and be mistaken for a cutaneous abscess. A 77-year-old woman with chronic sinusitis developed recurrent left paramedian frontal swelling over several months, with only transient improvement following antibiotic treatment. The lesion was initially interpreted as a cutaneous abscess and incised in an outpatient setting. Clear, nonpur

Treatment of recurrent barotraumatic and infectious frontal sinusitis by navigated balloon dilation of the nasofrontal duct.

INTRODUCTION: Recurrent barotraumatic or isolated infectious frontal sinusitis is linked to anatomical variants of the nasofrontal duct (NFD). Anterior ethmoidectomy, the standard treatment, is incomplete in 50-80% of cases, incurring a 19% rate of stenosis. Balloon dilation of the NFD could be an alternative. OBJECTIVES: The primary study objective was to evaluate the safety of the device. The secondary objective was to analyze its long-term efficacy. MATERIALS AND METHODS: This was a retrospective single-center study including patients treated by NFD enlargement using a navigated dilation

Trans-septal interfrontal septectomy for chronic unilateral frontal sinusitis.

Recurrent or treatment-resistant chronic unilateral frontal sinusitis resulting from drainage pathway obstruction presents a major surgical challenge. Several techniques have been proposed, including frontal endoscopic sinusotomy (Draf), or inter-frontal septectomy using an external approach. This article presents a novel trans-septal approach for interfrontal septectomy, ensuring contralateral drainage in chronic or recurrent unilateral frontal sinusitis, preserving the frontal sinus drainage pathway.

Disseminated Aspergillus flavus infection presenting as spondylodiscitis: a fatal tuberculosis mimicker in an immunocompetent host - case report and literature review.

INTRODUCTION: Disseminated invasive aspergillosis is a rare but life-threatening fungal infection that predominantly affects immunocompromised individuals. Vertebral involvement is an uncommon manifestation, and diagnosis in immunocompetent patients is particularly challenging because of its nonspecific clinical and radiological features, often resulting in delayed diagnosis and inappropriate therapy. We report a fatal case of disseminated Aspergillus flavus aspergillosis presenting as spondylodiscitis in an immunocompetent host, highlighting the diagnostic challenges and therapeutic implicati

Two-Plate Reconstruction of Lateral-Posterior Wall Defects for Functional Recovery of the Maxillary Sinus.

INTRODUCTION: Mucociliary clearance (MCC) is essential for the normal physiological function of the maxillary sinus. Maxillary sinus lateral-posterior wall defects, often caused by benign tumor excision or cyst enucleation in the posterior maxilla, can severely impair MCC. Therefore, reconstruction of these defects is necessary to restore sinus function and maintain its original volume. This study introduces a technique for maxillary sinus lateral-posterior wall reconstruction using 2 plates and evaluates its efficacy in maintaining normal sinus function and volume. METHODS: This retrospectiv

THREE-DIMENSIONAL CT-BASED MORPHOMETRIC ASSESSMENT OF THE MAXILLARY SINUS.

INTRODUCTION: The maxillary sinus demonstrates marked anatomical variability, which has important implications for sinus ventilation, surgical access, and implant planning. While computed tomography (CT) remains the standard modality for anatomical assessment, conventional two-dimensional measurements may inadequately represent complex three-dimensional structures. AIM: The aim of our study was to perform a three-dimensional CT-based morphometric analysis of the maxillary sinus using 3D Slicer software and classical statistical methods, focusing on volumetric characteristics, wall thickness,

The Application of Autonomous Dental Implant Robots in Avoiding Maxillary Sinus and Making Full Use of Sinus Septa for Fixed Restoration Supported by Implants: A Case Report.

Implant rehabilitation in the atrophic posterior maxilla presents significant challenges, primarily due to pneumatized maxillary sinuses and the presence of septa. Although sinus floor elevation is a standard approach, utilizing native basal bone through tilted implant placement offers a less invasive alternative. However, avoiding the sinus cavity while anchoring implants in restricted bone volumes, such as sinus septa, is technically demanding with freehand methods. This case report describes the novel application of an autonomous dental implant robot to address these anatomical complexities

Sinus membrane perforation and oroantral communications in lateral sinus lift: a narrative review and proposal of a clinical decision tree.

INTRODUCTION: Sinus membrane perforation is the most frequent intraoperative complication of lateral window sinus floor elevation, with reported incidences ranging from 7 % to 56 %. Pre-existing oroantral communications (OACs) represent a frequently associated clinical situation encountered during implant rehabilitation of the posterior edentulous maxilla. Despite an extensive literature, no clear consensus guides the clinician in selecting a management technique. The aim of this work is to propose an educational clinical decision tree, based on a narrative review of the literature, to guide t

Bone-Preserving Management of Maxillary Cemento-Ossifying Fibroma With Orbital Floor Thinning: A Case Report.

Cemento-ossifying fibroma (COF) is a benign fibro-osseous neoplasm that most often affects the mandible and occurs predominantly in women during the third and fourth decades of life. Maxillary involvement is less common and may remain minimally symptomatic while expanding into the maxillary sinus. We present a 26-year-old woman with a six-month history of progressive right midface enlargement without pain, nasal obstruction, diplopia, visual symptoms, or dental mobility. Computed tomography demonstrated a well-circumscribed expansile right maxillary lesion measuring 44.9 × 42.5 × 31.9 mm, occu

Crestal Sinus Lift With Osseodensification in Severely Atrophic Maxilla: Case Series With Long-Term Follow-up.

In the posterior maxilla, bone volume, height, and density are often insufficient for implant placement and rehabilitation, which can affect implant long-term stability and success. These anatomical limitations may dictate the need for sinus grafting procedures. Osseodensification is an implant site instrumentation method that enhances bone density and can also be used for transcrestal maxillary sinus augmentation. This technique utilizes bone plasticity through specially designed burs that compact and densify bone along the osteotomy walls while simultaneously propelling bone particles latera
