Haemodiafiltration, haemofiltration and haemodialysis for adults with kidney failure.

RATIONALE: Convective extracorporeal dialysis modalities, haemofiltration (HF), haemodiafiltration (HDF), and acetate-free biofiltration (AFB), remove excess body fluid across the dialysis membrane through an interplay of positive and negative pressures and accumulated middle- and larger-sized solutes more efficiently than haemodialysis (HD). This increased removal of larger solutes, combined with the use of ultrapure dialysis fluid in convective dialysis, is hypothesised to reduce the frequency and severity of symptoms during HD and improve long-term clinical outcomes. This is an update of a

Perspectives on potential pharmacist prescribing in an outpatient dialysis center: qualitative interviews with patients and clinicians.

INTRODUCTION: Pharmacist prescribing is expanding across care settings, supported by pharmacists' expertise in pharmacology, therapeutics, disease management, and medication optimization. In settings where pharmacists can prescribe, patients and providers report positive outcomes. However, limited research has examined pharmacist prescribing in dialysis centers. AIM: This study explored patient and clinician perspectives on potential pharmacist prescribing in the outpatient hemodialysis unit at Toronto General Hospital, University Health Network (TGH-UHN) in Toronto, Canada. METHOD: Semi-str

The long-term dialysis due to diabetic nephropathy may be associated with in-hospital death in patients with acute myocardial infarction.

The clinical outcomes of acute myocardial infarction (AMI) in dialysis patients are poor, and the outcomes may be worse in the long-term dialysis patients due to diabetic nephropathy (DMN). However, there are limited reports on the in-hospital outcomes of dialysis patients with AMI considering the underlying disease and duration of dialysis. This retrospective study aimed to compare the in-hospital outcomes in dialysis patients who underwent percutaneous coronary intervention (PCI) for AMI between with and without long-term dialysis due to DMN. We included 239 dialysis patients with AMI and di

Inclusion of patient care technicians in hemodialysis patient care conferences: a pilot implementation study.

BACKGROUND: Including patient care technicians (PCTs), who spend the most chairside time with patients receiving hemodialysis, in monthly interdisciplinary patient care conferences (PCCs) may provide the dialysis care team with valuable patient information and improve PCTs' sense of belonging on the team. We assessed the implementation of including PCTs in PCCs ("In-Tech"). METHODS: In this pre-post pilot study, In-Tech was implemented at both clinics for 6 months (March-August 2025) by dialysis care team members (nephrologists, nurses, social workers, dietitians, PCTs) at two independent dia

Continuous Glucose Monitoring and Mortality Risk Among U.S. Veterans Receiving Dialysis With Diabetes.

OBJECTIVE: Patients receiving dialysis who have diabetes have a high burden of dysglycemia. Whereas traditional glycemic markers have limited accuracy and convenience in dialysis, continuous glucose monitoring (CGM) provides automated, less invasive, and more comprehensive glycemic data than do conventional measures. However, it remains unclear whether CGM is associated with improved clinical outcomes in patients undergoing dialysis. RESEARCH DESIGN AND METHODS: We examined the association between CGM use versus non-CGM use and survival among U.S. veterans receiving dialysis who also have dia

Membrane-to-Patient Optimization: Individualized Dialyzer Selection for Extracorporeal Dialysis.

Extracorporeal dialysis for uremic toxin removal and fluid regulation relies on specialized dialyzers whose membranes differ markedly in polymer chemistry, pore architecture, adsorption capacity, surface bioactivity, and convective performance. These structural and material distinctions result in wide variation in the clearance of chemically diverse uremic solutes. Despite the expanding range of dialyzer options, membrane selection in clinical practice remains largely non-individualized. In this review, we propose a phenotype-based model for dialyzer membrane selection. We outline how distinct

Peritoneal dialysis results in similar outcomes to extracorporeal dialysis in acute kidney injury: A systematic review and meta-analysis.

BackgroundThe choice of kidney replacement therapy (KRT) modality for treatment of dialysis requiring acute kidney injury (AKI-D) is underappreciated as a modifiable factor that can impact outcomes. Peritoneal dialysis (PD) and extracorporeal dialysis are both established modalities for treatment of kidney failure; their comparative effectiveness in AKI remains controversial. We conducted a systematic review and meta-analysis to compare clinical outcomes between PD and extracorporeal dialysis in patients with AKI.MethodsWe systematically searched Medline, Embase, and Cochrane Central Register

Effect of Hemodialysis With Citrate on Vascular Calcification.

INTRODUCTION: Vascular calcification, specifically medial calcification, is prevalent in patients undergoing hemodialysis (HD) and leads to poor outcomes. Citrate is present in some HD solutions and could potentially inhibit vascular calcification. This was examined by comparing the progression of medial arterial calcification, as measured using mammography, in female patients with end-stage renal disease (ESRD) undergoing HD with and without citrate. METHODS: In a cross-over study, 28 female chronic in-center HD patients with preexisting breast arterial calcification (BAC) underwent HD using

Comparison of Kirpa Kit™ manual hemodialysis device to standard continuous kidney replacement therapy in vitro.

BACKGROUND: Access to kidney replacement therapy remains limited in many low- and middle-income countries. The Kirpa Kit™ is a low-cost, electricity-free manual hemodialysis device designed for resource-limited settings. Although it has previously shown feasibility in vitro, in vivo, and in first-in-human use, it has never been directly compared with an automated extracorporeal dialysis platform under matched conditions. METHODS: In vitro experiments were performed in triplicate comparing the Kirpa Kit™ and the Prismaflex™. A 5-L bag of diluted packed red blood cells enriched with urea was us

Use of Telemedicine and Surrogate Health Outcomes in In-Center Hemodialysis.

BACKGROUND: In March 2020, responding to the COVID-19 pandemic, US public health emergency waivers enabled kidney care providers (nephrologists and advanced practice providers) to substitute face-to-face in-center hemodialysis visits with telemedicine encounters. METHODS: We merged an electronic health records database from dialysis patients at clinics owned by DaVita Inc. with Medicare claims-based records. We identified dialysis patients in the first six months of the pandemic. Kidney care providers were divided into those who used telemedicine frequently (above median) versus less frequent

A randomized cross-over trial of pre-and post-filter hemodiafiltration modalities in children on maintenance dialysis -- effects on solute removal and blood pressure.

INTRODUCTION: Children on post-filter hemodiafiltration (post-HDF) have reduced cardiovascular damage and improved growth compared to conventional hemodialysis. High convection volumes in post-HDF are associated with improved outcomes. Even higher convection volumes can be reached with pre-filter HDF (pre-HDF). However, data is limited. Here, we compare the effect of pre- vs post-HDF on solute removal, inflammation, nutrition markers and blood pressure in a randomized, multicenter cross-over trial. METHODS: After a four-week wash-out period in post-HDF, 24 children on maintenance dialysis wer

Exploring Haemodialysis Nurses' Perceptions on Kidney Replacement Therapy Modality Education: A Framework Analysis.

BACKGROUND: Many people with kidney failure start and remain on in-centre haemodialysis treatment despite evidence of improved outcomes with home dialysis. To make an informed modality decision patients must receive frequent, high-quality modality education. This education is inconsistent in the in-centre haemodialysis setting, where patients spend the most time with nurses while receiving haemodialysis treatments. OBJECTIVES: The aim of this study was to examine in-centre haemodialysis nurses' perceptions around modality education for patients receiving in-centre haemodialysis using the COM-

Candida auris Containment Responses in Health Care Facilities that Provide Hemodialysis Services - New Jersey, North Carolina, South Carolina, and Tennessee, 2020-2023.

Candida auris, a frequently multidrug-resistant fungal pathogen, poses an urgent public health threat due to its potential to spread within and between health care facilities. Facilities that offer dialysis services might face particular challenges in preventing and containing C. auris and other multidrug-resistant pathogens, given the frequent use of invasive treatments in an immune-compromised patient population. During 2020-2023, in five separate facilities providing dialysis care across four states (New Jersey, North Carolina, South Carolina, and Tennessee), six patients infected or coloni

The Chain of Cannulation: A Qualitative Comparison of the Cannulation Process and European Guidelines.

BACKGROUND: The cannulation of an arteriovenous fistula can be performed using several different techniques and surrounding routines, which are presently not well-defined. The recently described chain of cannulation includes planning cannulation, pre-cannulation, during cannulation, evaluating cannulation and post-cannulation, and was based on the Swedish context, in which the buttonhole cannulation technique is the most common. OBJECTIVES: To explore whether the chain of cannulation is in agreement with nursing guidelines from other dialysis contexts, in terms of cannulation techniques and t

Innovation of a Neonatal Peritoneal Dialysis Catheter to Expand Dialysis Capabilities for Critically Ill Neonates in Low-Resource Settings.

INTRODUCTION: The lack of peritoneal dialysis (PD) catheters designed explicitly for neonates creates significant challenges in the provision of neonatal PD. High resource settings can circumvent this limitation by resorting to alternative extracorporeal dialysis methods. However, in low-resource settings, PD remains the preferred dialysis modality, and the use of off-label catheters for PD results in complications such as omental wrapping and occlusion. This study introduces a novel catheter design featuring a multi-diameter side port configuration and a helical geometry. METHODS: We employe

Comment on: Association Between Complex Indices of Blood Cell Types and Lipid Levels With All-Cause, Cardiovascular Mortality in Hemodialysis Patients: A Multicenter Retrospective Study.

We reviewed the study by Chen et al. on the association between blood cell indices, lipid levels, and mortality in hemodialysis patients. While the study provides valuable insights, several limitations affect its generalizability and validity. The data were limited to four centers within a single country, and key confounders-such as lifestyle, psychological factors, medication use, and nutritional status-were not addressed. Additionally, the findings are not applicable to patients on other dialysis modalities. We recommend future multicenter studies that incorporate broader populations, accoun

Advanced Strategies for Managing Pleuroperitoneal Communication in Peritoneal Dialysis Patients: Report of Two Cases.

BACKGROUND Pleuroperitoneal communication (PPC) is a rare yet significant complication in patients undergoing peritoneal dialysis (PD), characterized by an abnormal connection between the peritoneal and pleural cavities. This condition can lead to pleural effusion, respiratory complications, and, in severe cases, necessitate the discontinuation of PD in favor of hemodialysis, threatening residual renal function. While the incidence of PPC is low (1-6%), it presents considerable diagnostic and therapeutic challenges, often requiring a multidisciplinary approach. CASE REPORT We report 2 cases of

The epidemiology of encapsulating peritoneal sclerosis in an era of biocompatible neutral peritoneal dialysis solution in Japan.

BACKGROUND: Encapsulating peritoneal sclerosis (EPS) is one of the most serious complications of peritoneal dialysis (PD). Although the incidence and severity of EPS are said to have decreased with the widespread adoption of biocompatible neutral PD solutions, the overall prevalence in Japan remains unclear. METHODS: We extracted patients with EPS from 2012 to 2022 using the Web-based Analysis of Dialysis Data Archives (WADDA) system of the Japanese Society for Dialysis Therapy. EPS was defined as present in patients with a history of PD treatment presenting with symptoms of ileus due to inte
