Operational characteristics of continuous renal replacement modalities used for critically ill patients with acute kidney injury.

Renal replacement therapy (RRT) is required in a significant percentage of patients developing acute kidney injury (AKI) in an intensive care unit (ICU) setting. One of the foremost objectives of continuous renal replacement therapy (CRRT) is the removal of excess fluid and blood solutes that are retained as a consequence of decreased or absent glomerular filtration. Because prescription of CRRT requires goals to be set with regard to the rate and extent of both solute and fluid removal, a thorough understanding of the mechanisms by which solute and fluid removal occurs during CRRT is necessar

Sustained low-efficiency dialysis.

Sustained low-efficiency dialysis (SLED) is an increasingly popular form of renal replacement therapy for patients with renal failure in the intensive care unit. Advantages of SLED are efficient clearance of small solutes, good hemodynamic tolerability, flexible treatment schedules, and reduced costs. Studies comparing outcomes of SLED with those of other dialysis modalities are being performed.

[Dialysate bacteriological quality in a health district].

BACKGROUND: There is a serious lack of data in literature on the quality of dialysate used in haemodialysis units throughout Spain and there also exist discrepancies between clinical guides on criteria related to dialysate bacteriological quality. AIM: Ascertain bacteriological quality of dialysate used in our area. MATERIALS AND METHODS: Descriptive observational studies were carried out monthly and over a period of one year, at two haemodialysis units (unit A: third level public hospital using Monitral-Hospal monitors and unit B: state subsidised non-profit organisation using AK90-Gambro m

The financing and organization of medical care for patients with end-stage renal disease in Sweden.

The total health care expenditure as a percentage of the gross domestic product in Sweden is 9.2%, and health care is funded by global budgets almost entirely through general taxation. The prevalence rate of end-stage renal disease (ESRD) in Sweden is 756 per million. Fifty-two percent of ESRD patients have a functioning transplant. Almost all ESRD treatment facilities are public. Compared with other Dialysis Outcomes and Practice Patterns Study (DOPPS) countries, the salaries for both nephrologists and professional dialysis unit staff are low. Sweden's high cost per ESRD patient, relative to

[Dialysis treatment of infants with end stage failure].

BACKGROUND: End stage renal failure in infants is rare, and was until recently regarded as untreatable. Advancements in dialysis techniques and other renal replacement therapy, have now made lifesaving treatment possible. MATERIAL AND METHODS: Three infants who developed end stage renal failure shortly after birth and were subsequently treated with long-term dialysis (as a bridge to transplantation) are presented and their results are compared with those from other dialysis centres. RESULTS AND INTERPRETATION: All three patients were successfully dialysed until transplantation, two with peri

Plasma lipoprotein abnormalities in hemodialysis patients--clinical implications and therapeutic guidelines.

Patients with advanced stages of chronic kidney disease (CKD) have an increased risk of death from cardiovascular disease (CVD). Dyslipidemias are associated with atherosclerotic vascular changes and the risk of occurrence of acute myocardial infarction in hemodialysis patients. However, management of dyslipidemia in hemodialysis patients does not appear to be actively carried out in routine practice. Presumably, there are three reasons for this reluctance to lipid-lowering in hemodialysis patients. First, there are epidemiological data showing the inverse relationship between cholesterol and

[The treatment of hyperhomocysteinemia in patients on dialysis: folic acid or the high-flow polysulphonic membrane?].

AIM: The aim of the study was to determine the effects of high-flow and low-flow hemodialysis (HD), with simultaneous treatment with folic acid and vitamin B12, on total homocysteine (tHcy) concentration in plasma of dialyzed patients. METHODS: The planned clinical observation included 46 patients of both sexes, aged 21-82, treated with bicarbonate dialysis for a mean of 4.7 years. The patients were divided into group A, subsequently dialyzed by use of high-flow polysulphonic membrane (AN 69ST, Nephral 300), and group B that continued to be dialyzed by use of low-flow diacetate membrane (Diac

Hip fractures in end-stage renal disease patients: incidence, risk factors, and prevention.

Hip fractures are an important problem for end-stage renal disease (ESRD) patients treated with dialysis. The incidence of hip fractures in the dialysis population is approximately four times that of the general population. Even more concerning is the associated 1 year mortality, which is twice that of other dialysis patients. Management of this problem is complicated by the heterogeneous nature of renal osteodystrophy and the inadequate methods of diagnosis currently available. Fall prevention has been shown to reduce the incidence of hip fracture and associated decline in functional ability

Quality of life and psychological issues in peritoneal dialysis patients.

Both peritoneal dialysis (PD) and hemodialysis (HD) patients have diminished quality of life (QOL) scores compared to healthy patients. QOL tends to decline over time, with the perception of the quality of physical health deteriorating more than mental health. However, many patients continue to feel hopeless, anxious, and worry about finances, loss of sexual function, family burden, and loss of independence. Depression is the most widely acknowledged psychosocial factor seen in patients with chronic kidney disease. Major depression occurs in 25% of patients facing impending dialysis. Once on P

Once-weekly intravenous paricalcitol in the treatment of secondary hyperparathyroidism in hemodialysis patients.

BACKGROUND: Paricalcitol, a vitamin D analog, is commonly administered three times weekly to control secondary hyperparathyroidism in hemodialysis patients. Less frequent dosing would be more convenient, require less nursing time, and be an option in other dialysis modalities. No studies have examined the efficacy of once-weekly dosing of paricalcitol. METHODS: Chronic hemodialysis patients receiving a stable dose of paricalcitol three times weekly with intact PTH (iPTH) 100-500 ng/l were monitored during a two-week baseline, then were converted to a single mid-week paricalcitol dose equal to

Dialysis choice in the context of an early referral policy: there is room for self care.

BACKGROUND: Predialysis care is vital for the patient and is crucial for dialysis choice: empowered, early referred patients tend to prefer out-of-hospital and self-care treatment; despite these claims, early referral remains too often a program more than a reality. Aim of the study was to evaluate the pattern and reasons for RRT choice in patients treated in a long-standing outpatient network, presently following 850 chronic patients (about 80% diabetics), working with an early referral policy and offering a wide set of dialysis options (home hemo and PD; self care and limited care hemodialys

Prognostic value of C-reactive protein for heart disease in dialysis patients.

C-reactive protein (CRP) is considered to be the prototype marker of inflammation. In the general population, there are ample clinical and epidemiological data that indicate its usefulness both in predicting the prognosis for various forms of cardiovascular disease, and in monitoring response to treatment. There is also evolving evidence that CRP may be directly involved in the pathological disease process itself. In end-stage renal disease (ESRD) patients, cardiovascular disease remains the leading cause of morbidity and mortality, and is accounted for by a clustering of both traditional and

Long-term use of vitamin E-coated polysulfone membrane reduces oxidative stress markers in haemodialysis patients.

BACKGROUND: Asymmetric dimethylarginine (ADMA) is an endogenous inhibitor of nitric oxide synthase and an independent predictor of overall mortality and cardiovascular outcome in haemodialysis (HD) patients. In the present study, we compared the effects of a vitamin E-coated polysulfone membrane (PSE) and a non-vitamin E-coated polysulfone membrane (PS) on oxidative stress markers such as ADMA. METHODS: Thirty-one HD patients were enrolled to this investigation. They were allocated into two groups: in the PSE group (n = 16), PSE was used for 6 months, followed by PS for an additional 12 month

Custom-made raster method for fistula and graft.

Unfamiliarity with fistula and graft characteristics can lead to failed punctures, haematomas and sometimes access occlusion. The Custom-made Raster Method provides detailed shunt visualisation and angiographic images by using photo-editing software. Access veins of an individual shunt and an adapted raster are projected on a digital picture of the arm. During angiography the shunt arm is fixated and a digital picture is taken from a fixed vertical angle and distance. Reference points are marked on the shunt arm, which serves as a fixation to draw a raster with coordination points. In this way

[Choosing between peritoneal dialysis and haemodialysis: a critical appraisal of the literature].

In France, incidence and prevalence of end-stage renal disease (ESRD) are increasing, requiring a more rational use of available replacement therapies. To help practitioners make their choice between treatment modalities of peritoneal dialysis (PD) and haemodialysis (HD), critical appraisal of relevant literature has been conducted. Although few absolute or relative contraindications for PD and HD exist, arguments in favour of one versus the other dialysis technique can be made. When patients receive adequate and complete information, their preference is neutral between PD and HD. To date, no

Effect of cool temperature dialysate on the quality and patients' perception of haemodialysis.

BACKGROUND: The effects of cool dialysate on the urea reduction ratio (URR) in high efficiency haemodialysis have not been completely studied. After reviewing the literature, it appeared that patients' perceptions of cool dialysis have not been studied. Since patients' perception have an impact on patient satisfaction, this motivated the authors to research this area of practice. METHODS: This study was designed to determine whether a high URR and haemodynamic stability could be achieved by using cool dialysate in two groups of patients. The first group of five patients were known to have hyp

The effect of convection on the nutritional status of haemodialysis patients.

Malnutrition, defined as insufficient protein-calorie intake, is highly prevalent in haemodialysis (HD) patients. It is commonly associated with decreased body weight, depleted energy stores (fat tissue) and loss of somatic proteins. It has been suggested that there may be at least two fundamentally different types of malnutrition in HD patients. The first is related to low protein and energy intake. This type of malnutrition may be amenable to adequate nutritional and dialysis support. In contrast, the second type of malnutrition is associated with inflammation and atherosclerotic cardiovascu

Arteriovenous fistula use and heart disease in long-term elderly hemodialysis patients: analysis of United States Renal Data System Dialysis Morbidity and Mortality Wave II.

BACKGROUND: There are some concerns that arteriovenous fistula (AVF) use or other dialysis specific factors may exacerbate cardiovascular disease in long-term hemodialysis (HD) patients. METHODS: We performed a historical cohort study of the United States Renal Data System Dialysis Morbidity and Mortality Wave II study, limited to 993 patients who started HD in 1996 with valid information on vascular access and who were primarily eligible for Medicare at the start of the study. We assessed the association between hemodialysis vascular access and heart disease, defined as Medicare Claims for h

Outcome of peritoneal dialysis in cirrhotic patients with chronic renal failure.

BACKGROUND: Results of peritoneal dialysis (PD) in cirrhotic patients with renal failure are debated. The aim of the present study is to assess the outcome of PD patients with liver cirrhosis. METHODS: A retrospective study based on clinical records was performed in 21 cirrhotic and 41 PD patients without liver disease followed up at our PD unit. RESULTS: Five-year patient and technique survival were similar in the two groups. Seven cirrhotic patients (33%) died of liver failure (5 patients), liver cancer (1 patient), and peritonitis (1 patient). Ten controls (24%) died of cardiovascular com
