My A-T pack: a qualitative study of the utility, acceptability, design, and content of a family-designed and owned information pack relevant to the lives of children and young people living with ataxia telangiectasia.

BACKGROUND: Ataxia telangiectasia (A-T) is a rare genetic and progressive condition, primarily affecting the neurological, immunological, and pulmonary systems. In the absence of a cure, people living with A-T require co-ordinated multidisciplinary care to manage their complex needs. This often leads to families working with a range of different professionals and feeling burdened by the amount of information and coordination of care that they manage. With the aim to inform the co-production of a family-owned healthcare pack to promote person-centered care and self-management, this study explor

Early Experience With Percutaneous Pulmonary Blood Flow Restrictors to Allow Deferral of Norwood Surgery.

Background: Deferral of the Stage 1 Norwood Palliation (S1P) for infants with hypoplastic left heart syndrome and high-risk features is sometimes accomplished using a hybrid approach involving external pulmonary artery (PA) banding. Recently, percutaneous restriction of pulmonary blood flow has been accomplished with off-label modification of microvascular plugs to delay surgery. Methods: This single-center, retrospective case series examines outcomes associated with PA flow restrictors (PAFR) in S1P candidates. Primary outcomes included death, extracorporeal membrane oxygenation, PA reinterve

Sternotomy healing by secondary intention in a complex single ventricle patient after stage 1 palliation.

Delayed sternal closure is an important and commonly utilized strategy in neonates following cardiac surgery to optimize postoperative cardiac output. We present the case of an infant with hypoplastic left heart syndrome (HLHS) who underwent stage 1 Norwood/Sano palliation and did not tolerate sternal closure in the immediate post-op period, even when attempted after delay, due to persistent hemodynamic instability. Our approach involved closure of the skin and subcutaneous tissue to allow for sternotomy healing by secondary intention. This technique may be considered in rare instances when de

"Failing Fontans:" an emerging challenge in paediatrics.

As staged palliative surgery for patients with hypoplastic left heart syndrome ages to greater than 50 years in practice, providers are increasingly faced with the challenges of managing patients with "failed Fontan" physiology. In this brief case report, we discuss different ways paediatric palliative care can support this population.

Prevalence and Correlates of Distress Detected by the Distress Thermometer and Problem List in Lung Cancer Patients: A Cross-Sectional Study.

BACKGROUND: The distress thermometer (DT) is an effective tool for identifying distress among cancer patients worldwide. Our study objective is to evaluate the prevalence and severity of distress in lung cancer patients by using DT. METHODS: A total of 153 lung cancer patients were retrospective enrolled at the Lung Cancer Center of Peking Union Medical College Hospital. Participants completed three assessments: the clinical characteristics, the associated problem list (PL) scale and the DT. RESULTS: At a DT cut-off score of ≥ 4, 56.2% of lung cancer patients had significant distress, which

Working Environment in a Virtual Emergency Department: A Cross-Sectional Study.

OBJECTIVE: We measured multidisciplinary staff perceptions of the working environment, attitudes to telemedicine and the experience of working in a virtual emergency department (ED) to permit comparison with studies of physical EDs. METHODS: In September 2024, we conducted an online survey of 63 staff in a South Australian virtual ED. The survey comprised the Working Environment Scale (WES-10) and questions assessing attitudes to telemedicine and open-ended questions on attractors and challenges to work. Descriptive statistics and non-parametric inferential statistics were used to compare res

Development and Testing of an Owner-Reported Outcome Measure of Clinical Signs and Quality of Life in Dogs Treated With Chemotherapy.

Cancer is a leading cause of mortality in older dogs. Despite the prevalence of chemotherapy in canine oncology, a good understanding of owners' observations of side effects and clinical signs in real time is still lacking. Owners' perceptions and reporting of clinical signs play an important role in monitoring a dog's condition during treatment and the use of digital owner-reported outcome measures could prove efficient in tracking chemotherapy side effects in the home environment. This could improve care and draws inspiration from the human use of patient-reported outcome measures in oncolog

Prognostic value of liver biomarker scores for risk stratification in adults with Fontan palliation.

BACKGROUND: The assessment and monitoring of Fontan associated liver disease (FALD) is challenging because of variability in clinical presentation and disease progression. The purpose of this study was to compare the prognostic value of liver biomarker scores to baseline (single) liver biomarker score and other modalities of liver disease assessment. METHOD: Retrospective study of adults with Fontan palliation who had ≥3 consecutive assessments of complete blood count and comprehensive metabolic panel. Model for end-stage liver disease excluding international normalized ratio (MELD-XI), fibro

Mechanical power normalisation methods to predict ICU mortality: a retrospective cohort study.

BACKGROUND: The optimal mechanical ventilation strategy to minimise ventilator-induced lung injury (VILI) remains uncertain. Mechanical power (MP) is a key VILI determinant, but whether and how MP should be normalised to individual patient characteristics is unclear. In this study, we aimed to evaluate whether the discriminatory accuracy of MP for ICU mortality in mechanically ventilated patients improves when normalised to physiologically relevant variables that reflect individual susceptibility to VILI. We also explored whether the relationship between MP, MPratio, and mortality is linear or

Loop diuretics and subsequent use of urinary symptom medications in older adults: evaluation of a possible prescribing cascade.

BACKGROUND: Loop diuretic (LD) use may lead to a prescribing cascade whereby urinary symptoms are ascribed to genitourinary syndromes and treated with urinary symptom medications (USMs). We investigated if LDs lead to increased USM use among older adults and whether this potential prescribing cascade varies across key characteristics. METHODS: This was a prescription sequence symmetry analysis of Veterans Administration data, involving veterans ≥66 years who initiated treatment with LD (2010-2019). USMs were antimuscarinics, beta-3 adrenergic agonists, peripheral alpha-1 blockers, and 5-alpha

Laparoscopic Excision of a Psoas Epidermoid Cyst: A Report of a Rare Case.

Retroperitoneal cysts are rare, and retroperitoneal epidermoid cysts are fewer in number; these cysts often pose challenges in being diagnostically vague. This report presents a case of a 41-year-old man who presented with dysuria and bilateral loin pain, and was treated for bilateral ureteric calculi shown on an abdominal X-ray. The patient re-presented with pain, and further imaging revealed a provisional diagnosis of a hydatid cyst, demonstrated by a large mass within the right psoas muscle. Histopathology after a complete laparoscopic excision confirmed a benign epidermoid cyst (Gonzalez-C

An Expanding Perspective: Extreme Aortic Dilation in a Patient With Single Ventricle Palliation.

Aortic dilation following single ventricle palliation is increasingly recognized, although its long-term clinical implications remain unclear. We present the case of a 38-year-old male born with a bicuspid aortic valve, d-transposition of the great arteries, ventricular septal defect, right ventricular hypoplasia with a straddling right atrioventricular valve. He was palliated with Fontan circulation and developed severe neo-aortic root and arch dilation with native aortic valve stenosis, requiring surgical intervention. His neo-aortic root measured 81 × 65 mm, necessitating a Bentall procedur

Don't ask, don't tell (DADT): under-recognition of breathlessness in clinical care-a national survey.

INTRODUCTION: The recognition of breathlessness in clinical practice appears suboptimal, despite its prevalence and impact. This study aimed to explore the content of clinical conversations about breathlessness and patients' level of self-reported openness when discussing their breathlessness. METHODS: A cross-sectional, online survey of Australian adults (≥18 years) stratified to the 2016 National Census for age, sex, state/territory of residence and rurality. Assessments based on self-report included demographics, breathlessness (modified Medical Research Council (mMRC) breathlessness scale

Fracture and Stenosis of Coronary Stents Implanted in Highly Tortuous Patent Ductus Arteriosus for Palliation of Duct Dependent Pulmonary Circulation.

We present two cases of stent fracture and stenosis after patent ductus arteriosus (PDA) stenting for palliation of congenital heart disease with duct dependent pulmonary circulation, resulting in clinical compromise and re-intervention with further stent placement. As interventionists undertake PDA stenting in increasingly complex and tortuous ductal morphology, awareness of this potentially serious complication will allow early detection and management to improve patient outcomes.

Valeo Stent Use for Ductal-Dependent Systemic Circulations.

Our aim is to describe the experience of ductal stenting using the balloon-expandable Valeo stent in ductal-dependent systemic circulation. Outcomes assessed were procedural success, procedural survival, stent related complications, and re-intervention rates. Hybrid palliation, consisting of ductal stenting and bilateral pulmonary artery band placement, is a widely accepted alternative for stage I palliative surgery. Off-label use of stents is the norm for this procedure. The Valeo stent is one such example with minimal data on its use for ductal stenting. Retrospective data was collected for

Faecal incontinence core outcome set: an international Delphi consensus exercise among patients, health-care professionals, and researchers.

Faecal incontinence is a debilitating anorectal disorder that can severely affect a person's quality of life. The variability in reported outcomes in studies on treatments for faecal incontinence complicates the synthesis of evidence, thereby weakening treatment recommendations. Furthermore, the emphasis on clinical outcomes often neglects outcomes that are crucial to patients' daily lives. Incorporating diverse stakeholder perspectives, we aimed to develop a core outcome set (COS)-a minimum set of outcomes that should be measured in future studies evaluating the efficacy of a treatment in adu
