Skin cleansers and leave-on product interventions for preventing incontinence-associated dermatitis in adults.

BACKGROUND: Incontinence-associated dermatitis (IAD) is a common skin problem in adults with urinary incontinence, faecal incontinence, or both. Prevention involves skin care interventions such as skin cleansing and the application of skin protectants/barriers (leave-on products). OBJECTIVES: To assess the effects of skin care cleansers, leave-on products, and procedures for preventing incontinence-associated dermatitis in adults. SEARCH METHODS: On 29 April 2024, we searched the Cochrane Incontinence Specialised Register - which includes searches of CENTRAL, MEDLINE, MEDLINE In-Process, MED

Constipation in adults: undertaking a holistic nursing assessment.

Constipation results in infrequent or hard stools and pain during bowel movements and is a widespread issue across all age groups, but particularly among hospitalised older adults. Undertaking a holistic assessment of constipation in adults is essential to enable the nurse to identify any underlying causes, prevent complications and ensure effective management. Early recognition of constipation reduces the risk of faecal impaction, discomfort and associated conditions, which can affect patients' well-being and quality of life. This article describes the pathophysiology of constipation, the dia

Early palliative care collaboration for paediatric patients with inherited jaw conditions-a traffic light system.

Paediatric maxillofacial surgeons infrequently encounter primary life-limiting conditions with escalating symptoms, contributing to the underutilization of palliative care services. This paper highlights two rare life-limiting oral and maxillofacial conditions - infantile osteopetrosis and osteosclerotic metaphyseal dysplasia - and proposes a collaborative framework integrating palliative care. The cases illustrate the multidisciplinary approach needed for complex symptom management, including end-of-life care in one case. The collaboration involved anaesthesia, surgery, pharmacy, dietetics, en

Digital cognitive behavioural self-management programme for fatigue, pain, and faecal incontinence in inflammatory bowel disease (IBD-BOOST): a multicentre, parallel, randomised controlled trial.

BACKGROUND: Fatigue, pain, and faecal urgency or incontinence are common, debilitating symptoms in inflammatory bowel disease (IBD). We developed IBD-BOOST, a digital, interactive, facilitator-supported, self-management intervention, and aimed to assess its effects compared with care as usual in relieving these symptoms and improving quality of life. METHODS: This multicentre, parallel, randomised controlled trial was conducted online in the UK, with allocation concealment maintained. Participants aged 18 years or older with IBD who rated the impact of fatigue, pain, and faecal urgency or inc

Shifting strategies to address systemic outflow tract obstruction in single ventricle anatomy with transposed great arteries.

OBJECTIVE: Double inlet left ventricle and tricuspid atresia both with transposed great arteries are at risk of systemic outflow tract obstruction. This study aimed to document evolving management strategies for these substrates and compare historical versus contemporary surgical approaches on long-term Fontan outcomes. METHODS: A retrospective study of patients with double inlet left ventricle-transposed great arteries/tricuspid atresia-transposed great arteries who underwent single ventricle palliation in Australia and New Zealand from 1965 to 2023 was conducted using the Fontan Registry. Process evaluation findings contradict RCT results of the IBD-BOOST digital self-management intervention for fatigue, pain and faecal urgency in inflammatory bowel disease: A mixed methods study of patient perspectives.

PURPOSE: This parallel process evaluation examined the implementation of a randomised controlled trial (RCT) of IBD-BOOST-a digital, interactive, facilitator-supported self-management intervention targeting fatigue, pain and urgency/faecal incontinence in individuals with inflammatory bowel disease (IBD). The RCT, involving 780 participants, compared IBD-BOOST with usual care but found no significant differences in quality of life (QoL) or symptom relief at six months post-randomisation. METHODS: A concurrent mixed methods design was employed. Qualitative data were gathered through semi-struc

Perinatal palliative care in an infant with exencephaly: Supporting life beyond 3 years of age.

Neural tube defects are the second most common type of fetal malformation in humans. Encephalocele, a type of neural tube defect, can be defined as a sack-like protrusion of brain tissue herniating through an opening hole in the skull. Its prognosis depends on the location, size, the amount of brain tissue involved, and the presence of other intracranial anomalies. Exencephaly, another type of neural tube defect, is characterized by an incomplete covering of the skull, resulting in an exposed fetal brain. It is described in the literature as incompatible with extrauterine life and, in most cas

[Managing pruritus in palliative care].

Pruritus is a symptom frequently encountered in the elderly people or those with advanced illnesses. It is therefore a common complaint in palliative care. Approximately 35 % of palliative care patients report experiencing itchiness, although this incidence varies depending on the underlying condition. This symptom significantly affects quality of life, notably by disrupting sleep, increasing fatigue, and contributing to the development of skin lesions, which can sometimes become superinfected. The etiologies are numerous, and identifying an appropriate treatment can be challenging. This artic

The Lancet Child & Adolescent Health Commission on the future of neonatology.

Neonatal mortality remains unacceptably high throughout the world. Survival of sick infants in their first month of life has improved over the past six decades. However, many comorbidities persist, with lifelong implications for health. The current ecosystem for research and development of drugs and medical devices to treat neonatal disorders is hindering further improvements to neonatal outcomes, especially infants born preterm or needing critical care. Innovation is lagging, and this is a public health problem characterised by multifactorial challenges in leadership, collaboration, regulatio

Optimizing outcomes for patients undergoing superior cavopulmonary connection procedure: what we know.

The Superior cavopulmonary connection (SCPC) is typically the second stage of the single ventricle palliation. There are modifiable factors that have a complex interplay in determining outcomes such as mortality or hospital length of stay. There is no clear consensus on the timing of SCPC as this involves a critical balance between the timing of operation and the length of the high-risk interstage period. Younger age has not been associated with reduced transplant-free survival but has been associated with longer hospital length of stay. Another critical decision is for children with viral res

Gene replacement therapy for Piga GPI-anchor deficiency in the developing nervous system.

Glycosylphosphatidylinositol (GPI) anchors are a class of post-translational modifications observed on over 150 proteins. Pathogenic variants in the GPI biosynthesis enzyme, PIGA, in humans are associated with several brain anomalies such as hypomyelination, cerebellar hypoplasia, ataxic gait, and can lead to premature mortality. We previously genetically deleted Piga from the embryonic mouse brain which led to early postnatal death and significant structural brain malformations similar to those observed in humans with PIGA variants. The current treatment options for PIGA patients only manage

Discordance in Ratings of Symptoms Experienced by Nursing Home Residents With Cognitive Impairment.

CONTEXT: Evaluation of palliative care programs in nursing homes often includes symptom assessments. For residents with cognitive impairment, proxy symptom reports, such as those from family members or nursing home staff, may be used as outcome measures. OBJECTIVES: This analysis compares differences between family and staff proxy reports of symptoms of nursing home residents with moderate to severe cognitive impairment. METHODS: Data were collected as part of a clinical trial, conducted in 16 nursing homes (NHs) in Indiana and Maryland (U.S.). We calculated the difference in total modified

Unforeseen Changes after Discontinuing Artificial Nutrition in a Patient with Giant Encephalocele.

BACKGROUND: Infants born with occipital encephalocele carry a high morbidity and mortality risk. Palliative care plays a central role in their management, which is difficult due to variability in outcomes and prognosis. CASE DESCRIPTION: An infant with an inoperable giant occipital who had remained admitted to the hospital since birth experienced a progressive decline in mental status and quality of life. Her family made the decision to stop enteral nutrition at 18 months of age. In contrast to the expectation of progressive fading of consciousness, the patient became increasingly alert and a

Safety of enteral nutrition practices in neonates with umbilical arterial catheters in situ: current evidence to guide clinical practice.

Umbilical arterial catheters (UAC) in neonates are used for blood pressure monitoring, blood sampling, administration of fluids, nutrition, and medications. As UAC applications evolve, enteral nutrition practices vary in neonates in the presence of a UAC. The theoretical concern for mesenteric ischemia when a UAC is in place led to early nil per os approaches, delaying the initiation of enteral nutrition. More contemporary practices have favored introducing enteral feeding in neonates with UACs. However, there remains a paucity of data to guide clinical practice approaches regarding enteral fe

Child death review: understanding variations in practice using normalisation process theory.

BACKGROUND: In England, Child Death Review (CDR) is a statutory process designed to identify full reasons for death, support parents, improve care and save lives. At CDR meetings, professionals review care and identify learning. Although parents do not attend CDR meetings, they should be invited to contribute questions and feedback and be informed of outcomes. There is a lack of evidence to support implementation, particularly for families after an expected child death. OBJECTIVE: To examine how parent involvement in CDR is operationalised currently, following expected child death, to support

Women's psychological experiences of preterm labour and birth which results in an intrapartum stillbirth or a neonatal death: an empty systematic review.

INTRODUCTION: Nearly three quarters of stillbirths and neonatal deaths occur in infants born prematurely. The mothers of these children may be at increased risk of developing mental health difficulties as a result of their premature labour and/or subsequent loss. METHODS: This systematic review was conducted to understand the psychological experiences of mothers who gave birth prematurely to a baby who subsequently dies as a result of an intrapartum stillbirth or a neonatal death. Ten databases were searched. Any studies which included women who had suffered a perinatal bereavement as a resul

Outcomes of Patients With Arrhythmia During Single-Ventricle Palliation.

OBJECTIVES: This study evaluates the short- and long-term outcomes of patients with single-ventricle morphology with arrhythmias prior to Fontan surgery, assessing the association between early arrhythmias and subsequent clinical outcomes. BACKGROUND: Staged surgical palliation for single-ventricle physiology involves complex operations with a high incidence of arrhythmias. The optimal duration of antiarrhythmic medication use and its impact on surgical outcomes and long-term prognosis remain unclear. METHODS: A single-center retrospective analysis was conducted on patients treated between A

Prognostication: A Young Patient With AA Amyloidosis.

Young patients and patients with multiple comorbidities often present a challenge of prognostic uncertainty. We present a case of a patient with end-stage renal failure secondary to AA amyloidosis from ongoing IV heroin use. This case presented difficulties with ethics, analgesia, prognostication, and, therefore, advanced care planning and communication challenges. These were overcome with multi-disciplinary input and careful communication with the patient and family members. The learning points include: the need for early and personalised advanced care planning, better provision of end-of-lif

Decision-Making and Downstream Outcomes of the Gabapentinoid-Diuretic Prescribing Cascade.

IMPORTANCE: Prescribing cascades are an underrecognized driver of polypharmacy among older adults (aged ≥65 years). The clinical decision-making processes underlying cascades and their downstream consequences are poorly understood. OBJECTIVE: To explore clinical reasoning leading to prescribing cascades and downstream outcomes (eg, falls, electrolyte abnormalities) via the exemplar gabapentinoid (gabapentin and pregabalin)-loop diuretic (LD) cascade.
