What makes patient navigation work? Identifying functions and forms and conducting causal loop diagramming to specify components of a successful colorectal cancer patient navigation program.

BACKGROUND: We implemented a centralized colorectal cancer (CRC) screening program with navigation to follow-up colonoscopy for community health center (CHC) patients with positive stool-based test screening results. Navigation increased six-month colonoscopy completion by 24 percentage points compared with usual care. Here, we describe how we applied a functions and forms framework alongside causal loop diagramming (CLD) to understand the effectiveness of our navigation program and explore its potential for implementation in other settings. METHODS: We first identified barriers to colonoscop

Data resource profile: the Scottish Combined Medicines Dataset (SCoMeD).

INTRODUCTION: Prescribing data has been collected electronically in Scotland for many years; however, data are collated in individual, non-overlapping datasets based on the origin of the prescription (e.g., primary or secondary care). The vision was to create a unified view of all prescribing data to provide a longitudinal dataset of medicines use for patients treated by the National Health Services (NHS) Scotland, irrespective of where or how that care was provided. METHODS: The Scottish Combined Medicines Dataset (SCoMeD) is, in essence, a data virtualisation tool collating information from

Case Difficulty and Non-Routine Events by Surgical Team Members in a Large Prospective Observational Study at 5 Veterans Affairs Medical Centers.

BACKGROUND: Excessive workload is associated with degraded operator performance and outcomes. The construct of surgical "team workload" is poorly specified, and anesthesiologist workload has rarely been measured concurrent with that of operating room nurses and surgeons during the same cases. We sought to measure workload, operationalized as individual and team "case difficulty," as well as the occurrence of non-routine events, from all surgical team members. METHODS: This multicenter prospective observational study at 5 Veterans Affairs medical centers involved 1107 non-cardiac surgical proc

Evaluation of Overall Health State, Treatment Burden, and Satisfaction with Insulin Efsitora Alfa (Efsitora) vs. Daily Comparator in Adults with Type 2 Diabetes in the QWINT Clinical Trial Program.

INTRODUCTION: Once-weekly efsitora resulted in similar efficacy and safety compared with daily basal insulins glargine or degludec in the treatment of adults with type 2 diabetes in the QWINT phase 3 development program. To fully assess once-weekly insulin's potential and address common barriers associated with insulin therapy (e.g., clinical inertia, fear of injections, treatment complexity), other aspects of the participants' treatment experiences were investigated using patient-reported outcome (PRO) measurements. The results of these PROs from QWINT-1 to -4 are presented here. METHODS: Si

ECPR eligible refractory out of hospital cardiac arrests - A post-hoc analysis of the EVIDENCE randomised controlled trial and Registry.

BACKGROUND: The use of Extracorporeal cardiopulmonary resuscitation (ECPR) for refractory out-of-hospital cardiac arrest (OHCA) is increasing. The EVIDENCE study of refractory OHCA transportation did not report a survival benefit of expedited transportation to hospital, likely in part owing to a very limited number of patients receiving ECPR. The reasons for this, the total potential numbers of ECPR patients and the effects of different system changes would assist in trial and clinical service planning. METHODS: A pre-specified, post-hoc analysis was performed of the randomised controlled tri

Target Trial Emulation for Regulatory and Clinical Decision Making in Cancer.

Randomized trials provide evidence for regulatory and clinical decision making in oncology. However, trials cannot answer every important clinical question. Evidence gaps often persist in under-represented or small patient populations and for questions about head-to-head comparisons of active treatments, complex treatment strategies (including treatment sequencing and other dynamic treatment strategies), and long-term or rare outcomes. To address these questions, clinicians and researchers increasingly turn to observational (real-world) data. The target trial framework provides a systematic ap

A realist evaluation of community-engaged learning for health professions students in rural Japan: explaining learning processes associated with cultural humility and professional reorientation.

Community-engaged learning (CEL) has gained attention in health professions education, yet there remains limited explanatory understanding of how it generates learning processes in East Asian contexts characterized by hierarchical professional cultures. This study used realist evaluation to examine a two-week rural community diagnosis practicum involving Japanese medical, nursing, and pharmacy students. Data were collected through ethnographic fieldwork, students' written reflective reports, and ten focus group interviews conducted in 2023 and 2024. Analysis identified five key context-mechani

An Analysis of Trauma and Elective Orthopaedic Incomes in a UK Hospital: A Rationale for Implementation of Ambulatory Trauma.

Introduction In the UK, one-third of orthopaedic departments experience financial losses from surgical interventions, with specialist hospitals experiencing higher deficits due to factors like patient age and injury severity. This study investigates financial modelling and potential changes in care delivery to enhance the regional flow of orthopaedic care throughout the UK, as well as strategies to address this economic disparity. Methods The study analyzed the impact of various orthopaedic subspecialties on departmental income and loss during the previous financial year (April 2023 to April 2

Mapping eligibility criteria in oncology target trial emulations using real-world data: a scoping review.

BACKGROUND: Target trial emulations often face challenges in accurately identifying the intended target population within real-world data (RWD) because eligibility criteria cannot always be directly mapped to available variables. This scoping review aimed to systematically characterize the proportion of eligibility criteria from oncology target trials that are successfully mapped to their emulated counterparts and to describe the approaches used for this mapping. METHODS: We searched MEDLINE for peer-reviewed studies published between January 1, 2016, and April 23, 2025, that were designed to

Not Every Country Can Absorb a Shock: Unequal Capacity to Withstand World Health Organization Aid Cuts.

External health aid is contracting sharply, and this moment is often framed as a stress test of health system resilience. This perspective argues that it is equally a stress test of global health ethics and governance: When external actors withdraw from essential services, they are not merely responding to fiscal constraints-they are shaping who loses care. In 2025, the World Health Organization (WHO) projected a 30%-40% decline in external health aid to low- and middle-income countries (LMICs) and urged governments to protect essential services, integrate programs into primary healthcare, and

Feasibility of REBOA (Resuscitative Endovascular Balloon Occlusion of the Aorta) Implementation in HEMS (Helicopter Emergency Medical Service) Units in Castilla-La Mancha, Spain.

Introduction: Currently, REBOA (Resuscitative Endovascular Balloon Occlusion of the Aorta) is an emerging technique for resuscitation in patients presenting severe pathology in hemodynamic shock refractory to conventional treatments. The REBOA technique consists of inserting a balloon through the femoral artery to temporarily occlude the aorta and thus control massive bleeding and improve perfusion of vital organs in critical situations such as hemorrhagic shock. Although it is not a definitive technique, its use buys time before the implementation of a definitive treatment when possible. This

Applying the Target Study Conceptual Model to Measure Racial and Ethnic Disparities in Hypertension Treatment Intensification.

BACKGROUND: This tutorial paper demonstrates the application of a conceptual model to measure racial and ethnic disparities in treatment intensification among adults with hypertension consistent with the Institute of Medicine (IOM) definition. The IOM defines disparity as differences in health care quality that are not due to access-related factors, clinical needs, preferences, and appropriateness of intervention (referred to as allowable covariates). METHODS: We used a conceptual model called the Target Study to estimate annual disparities in the probability of treatment intensification betw

Deprescribing in Older People: A Clinical Practice Guideline Summary.

INTRODUCTION: Older people face higher risks of medicine-related harm due to polypharmacy and the use of potentially inappropriate medicines. Current treatment guidelines rarely specify when to stop medicines, leading to medicines often being continued indefinitely without a clear deprescribing plan. While deprescribing guidelines exist for some medicine classes, limited guidance is a major barrier to deprescribing. These new guidelines address this gap by providing structured recommendations that complement more detailed drug-specific deprescribing guidance, disease-specific therapeutic guide

Lost in Translation? A Scoping Review to Explore the Translation and Cultural Adaptation of Preference-Based Measures of Quality of Life.

BACKGROUND: Preference-based measures (PBMs) of quality of life are an important tool to inform the evaluation of health and social care interventions and services. Traditionally, PBMs have tended to be developed in a single country, therefore, reflecting the linguistic and cultural norms of that country. Given increases in the cultural and linguistic diversity of populations in many countries, it is important that PBMs are culturally and linguistically adapted. Linguistic adaptation ensures the translated words are correct and accurate, whilst cultural adaptation ensures the meaning and relev

Professional identity formation (PIF) - What has NKLM 2.0 to say about it? A workshop report.

OBJECTIVE: Medical education should support the development of a professional identity. However, it is still unclear what exactly is meant by professional identity formation (PIF), how it proceeds, and how it can be specifically supported. Against this background, this workshop report describes an attempt by a group of experts to identify and evaluate learning objectives in NKLM 2.0 that could be relevant to PIF. The aims of this project were, on the one hand, to determine the extent to which PIF has been incorporated into the NKLM and, on the other hand, to analyze how PIF can be understood o

Rural health care workforce retention measures: A rapid scoping review.

PURPOSE: Rural Veterans face persistent barriers to care, often exacerbated by health care workforce shortages. Although recruitment initiatives are important, retaining providers in rural areas is equally critical to sustaining access and continuity of care. This rapid scoping review examined how "rural workforce retention" is defined and measured in the peer-reviewed literature and synthesized recommended reporting elements to improve clarity and comparability. METHODS: Six bibliographic databases were searched (2014-2025) for English-language studies of rural health care in the United Stat
